Moderate diet-induced weight loss is associated with improved insulin sensitivity in middle-aged healthy obese Korean women

  • BACKGROUND/OBJECTIVE: The goal of the present study was to investigate the effects of moderate caloric restriction on ${\beta}$-cell function and insulin sensitivity in middle-aged obese Korean women. SUBJECTS/METHODS: Fifty-seven obese pre-menopausal Korean women participated in a 12-week calorie restriction program. Data on total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglycerides (TG), and fasting serum levels of glucose, insulin, C-peptide, blood pressure, leptin and anthropometrics were collected. A dietary intake assessment was based on three days of food recording. Additionally, ${\beta}$-cell function [homeostasis model assessment of ${\beta}$-cell (HOMA-${\beta}$), insulinogenic index (ISI), C-peptide:glucose ratio, and area under curve insulin/glucose ($AUC_{ins/glu}$)] and insulin sensitivity [homeostasis model assessment for insulin resistance (HOMA-IR), Quantitative insulin-sensitivity check index (QUICKI) and Matsuda index (MI)] were recorded. RESULTS: When calories were reduced by an average of 422 kcal/day for 12 weeks, BMI (-2.7%), body fat mass (-10.2%), and waist circumference (-5%) all decreased significantly (P < 0.05). After calorie restriction, weight, body fat percentage, hip circumference, BP, TC, HDL-C, LDL-C, plasma glucose at fasting, insulin at fasting and 120 min, $AUC_{glu}$ and the insulin area under the curve all decreased significantly (all P < 0.05), while insulin sensitivity (HOMA-IR, QUICKI and Matsuda index) measured by OGTT improved significantly (P < 0.01). CONCLUSIONS: Moderate weight loss due to caloric restriction with reduction in insulin resistance improves glucose tolerance and insulin sensitivity in middle-aged obese women and thereby may help prevent the development of type 2 diabetes mellitus.

인슐린 치료에 대한 당뇨병 환자의 지각과 경험 - 융복합적 관점 (Patients' perspectives on taking insulin in diabetes - Perspectives of convergence)

  • 본 연구는 당뇨병 환자를 대상으로 인슐린 치료에 대한 문화적 관점과 경험을 탐색하기 위해 19명의 제1형과 2형 당뇨병 환자를 대상으로 포커스 그룹 인터뷰를 실시하였다. 인터뷰 내용을 기록하고 녹취한 후 내용분석을 통해 당뇨병 환자들이 경험하는 인슐린 치료에 대한 사전인식요인과 장애요인 및 촉진요인을 목록화하였으며 이들 3가지 목록에 포함되는 주요어를 도출하였다. 그 결과 인슐린에 대한 막연한 두려움이 사전인식요인의 주요어로 확인되었고 장애요인으로는 인슐린과 관련된 걱정(통증, 낮은 지식과 문해력, 자가 주사에 대한 낮은 자신감, 경제적 부담, 저혈당에 대한 두려움)과 의료진에 대한 양가감정, 의지하게 된 삶, 친구, 친척과 가족에 대한 감정, 불편감, 과거에 대한 후회, 부끄러움이 주요어로 도출되었다. 촉진요인으로는 깨달음과 신체적 회복 및 혈당조절에 대한 자신감이 확인되었다. 본 연구결과를 토대로 국내 당뇨병 환자의 인슐린 치료에 대한 경험과 지각에 대한 의료인의 이해가 필요하며 당뇨병 환자의 이러한 경험은 존중되어야 할 것으로 사료된다.

Production of human insulin analogue using recombinant Escherichia coli

  • For the production of $B^{30}-homoserine$ insulin analog as a novel anti-diabetic drug, the fermentative study was attempted for the maximal gene expression of HTS-fused $B^{30}-homoserine$ insulin precursor in the recombinant Escherichia coli cells. In a batch fermentation, the maximal production of insulin precursor as much as 38.95 mg/L-h, which occupied more than 12.8% of total cell protein. was achieved when the gene expression was induced by 0.5 mM IPTG at the middle logarithmic growth phase. The HTS-fused $B^{30}-homoserine$ insulin precursor was recovered from a batch culture through the processes of cell harvest, collection of insoluble fraction after sonication and purification by nickel affinity column chromatography. The isolated insulin precursor was 14 mg/L with a recovery yield of 35.9% of expressed gene product. The insulin A and B chain mixture was recovered after the insulin precursor was subjected to CNBr cleavage and purified by nickel affinity column chromatography. The isolated insulin chains were then sulfitolyzed with sodium thiosulfat and sodium tetrathionate, and reconstituted to insulin analog with ${\beta}-mercaptoethanol$, followed by purification with CM-Sepharose C-25 column chromatography.

Brazilin Inhibits Activities of Protein Kinase C and Insulin Receptor Serine Kinase in Rat Liver

  • Hypoglycemic action of brazilin was found to be based on the improvement of peripheral glucose utility, and this action might be correlated with the insulin action pathway. In the present study we investigated the effect of brazilin on the insulin receptor autophosphorylation, protein kinase C (PKC), protein phosphatase and insulin receptor serine kinase in order to confirm whether the hypoglycemic mechanism is concerned with insulin action pathway. Brazilin was found to inhibit PKC and insulin receptor serine kinase, which are involved in the regulation of insulin signal pathway. But any significant effect was not shown on insulin receptor tyrosine kinase activity, autophosphorylation and phosphatase activity. These findings suggest that brazilin might enhance insulin receptor function by decreasing serine phosphorylation, which might mediate hypoglycemic effect of brazilin.

인슐린 매개성 Microsomal Epoxide Hydrolase의 발현증가에서 Akt의 역할 (Role of Akt in Insulin-Mediated Increase in Expression of Microsomal Epoxide Hydrolase)

  • The present study examines the effect of dominant-negative Akt on the insulin-mediated microsomal epoxide hydrolase (mEH) induction in rat hepatocytes. We also assessed the role of insulin in the expression of soluble epoxide hydrrolase (sEH). Insulin increased mEH levels and the enzyme activities, whereas sEH protein expression was unaffected by insulin. The specific PI3K inhibitors or p70 S6 kinase inhibitor ameliorated the insulin-mediated increase in mEH protein levels. Infection with adenovirus expressing dominant-negative and kinase-dead mutant of Akt1 effectively inhibited the insulin-mediated increase in mEH expression and mEH activity. These results suggest that mEH and sEH are differentially regulated by insulin and PI3K/Akt/p70S6K are active in the insulin-mediated regulation of mEH expression.

심리적 인슐린저항성 개념분석 (Concept Analysis for Psychological Insulin Resistance in Korean People with Diabetes)

  • Purpose: The purpose of this study was to define the concept for psychological insulin resistance in the Korean population with diabetes. Methods: The Hybrid model was used to perform the concept analysis of psychological insulin resistance. Results from both the theoretical review with 26 studies and a field study including 19 participants with diabetes were included in final process. Results: The preceding factors of psychological insulin resistance were uncontrolled blood glucose and change in daily life. The concept of psychological insulin resistance was found to have three categories with 8 attributes such as emotional factors (negative feeling), cognitive factors (low awareness and knowledge, low confidence for self-injection) and supportive factors (economic burden, dependency life, embarrassing, feeling about supporters, feeling of trust in, vs mistrust of health care providers). The 8 attributes included 30 indicators. Conclusion: The psychological insulin resistance of population with diabetes in Korea was defined as a complex phenomenon associated with insulin therapy that can be affected by emotional factors, cognitive factors, and supportive relational factors. Based on the results, a tool for measuring psychological insulin resistance of Koreans with diabetes and effective programs for enhancing insulin adherence should be developed in future studies.

Up-Regulation of RANK Expression via ERK1/2 by Insulin Contributes to the Enhancement of Osteoclast Differentiation

  • Despite the importance of the receptor activator of nuclear factor (NF)-kappaB ligand (RANKL)-RANK signaling mechanisms on osteoclast differentiation, little has been studied on how RANK expression is regulated or what regulates its expression during osteoclastogenesis. We show here that insulin signaling increases RANK expression, thus enhancing osteoclast differentiation by RANKL. Insulin stimulation induced RANK gene expression in time- and dose-dependent manners and insulin receptor shRNA completely abolished RANK expression induced by insulin in bone marrow-derived monocyte/macrophage cells (BMMs). Moreover, the addition of insulin in the presence of RANKL promoted RANK expression. The ability of insulin to regulate RANK expression depends on extracellular signal-regulated kinase 1/2 (ERK1/2) since only PD98059, an ERK1/2 inhibitor, specifically inhibited its expression by insulin. However, the RANK expression by RANKL was blocked by all three mitogen-activated protein (MAP) kinases inhibitors. The activation of RANK increased differentiation of BMMs into tartrate-resistant acid phosphatase-positive ($TRAP^+$) osteoclasts as well as the expression of dendritic cell-specific transmembrane protein (DC-STAMP) and d2 isoform of vacuolar ($H^+$) ATPase (v-ATPase) Vo domain (Atp6v0d2), genes critical for osteoclastic cell-cell fusion. Collectively, these results suggest that insulin induces RANK expression via ERK1/2, which contributes to the enhancement of osteoclast differentiation.

  • Insulin administration rectally with the liposome has the hypoglycemic effect in the rabbits. The reducton in blood sugar was maximum at about 60 minutes after administration and continued for 4 hrs. at low level in these experiments. No hypoglycemic effect was observed in control administrated rectally without liposome. Rectal absorption of insulin has been effected by addition of the bile salt, as the protective agent which prevented denaturation and the phastransition of insulin in liposome-encapsulation. As a matter of the fact, a significant hypoglycemic action was obtained when the insulin-liposome was given by rectal administration. The use of this agent to enhance insulin absorption offers the possibility of a new approach to rectal insulin therapy.

한국인 인슈린 유전자의 클로닝 및 분석 (Molecular Cloning And analysis of Korean Insulin Gene)

  • Human insulin gene is consisted of the polymorphic region with the repeating units, the regulatory sequence, the structural gene including the intervening sequence, and 3'-flanking region. The polymerase chain reaction, which amplifies the target DNA between two specific primers, has been performed for the amplification of human insulin gene and simple one-step cloning of it into Escherichia coli. Out of 1727 nuceotides compared, only 4 sites were variable: 5'-regulatory region(G2101$\rightarrow$AGG); IVS I(T2401$\rightarrow$A); Exon II(C2411 deletion); IVS II(A2740 dejection). The variations at the G2101 and T2401 were the same as those found in one American allele. The other two variations were observed only in the specific Korean allele. And, the enzyme digestion patterns among normal, insulin dependent diabetes mellitus, and non-insulin dependent diabetes mellitus were the same. On the other hand, PCR method showed the possibility of the quickaccess for the polymorphic region in terms of the restriction fragment length of polymorphism.

Calcium Modulation of Insulin Secretion in Perfused Pancreata of Obese Zucker Rats

  • Insulin secretory response to various calcium concentrations was investigated in 10- to 12-week old male lean and obese Zucker rats using an in vitro pancreatic perfusion procedure. There was no significant difference in insulin secretion response to low, medium, and high calcium concentrations in the lean rat. However, the obese rat shows a characteristics of hypersecretion of insulin. The obese rat pancreas perfused with the low calcium concentration released as low insulin as the lean rat. When perfused with the medium calcium concentration, th obese rat pancreas released twice as much insulin as the lean rat. eh hypersecretory phenomenon was also seen in the obese rat pancreas perfused with the high calcium concentration during the first phase of erfusion period, but his phenomenon was gradually diminished during he second phase of perfusion period. These results indicate that there may be a selective insulin secretory response to the extracellular calcium in he obese Zucker rat pancreas.
