Remission of Progesterone-induced Diabetes Mellitus after Ovariohysterectomy in an Intact Female Dog

  • A 10-year-old, 8.28 kg, intact female Cocker Spaniel was presented with continuous polyuria and polydipsia. The dog had proestrus bleeding 5 weeks earlier, and hyperglycemia, glucosuria, ketouria, and high level of serum fructosamine in laboratory findings. Based on hyperglycemia, glucosuria, ketouria, and ketosis, the patient was tentatively diagnosed as diabetes mellitus (DM) with ketouria. After diagnosis, 5 to 7 U/body porcine lente insulin was administered during 11 days from initial presentation as the remission of DM for the dog. But, blood glucose was still high level. Because there was no reaction to porcine lente insulin, it was replaced by 4-10 U/body neutral protamine Hagedorn (NPH) during 3 days. But, NPH also did not regulate blood glucose level. Because insulin therapy failed to regulate blood glucose level, the dog was considering insulin-resistant diabetes. The dog was tentatively diagnosed with progesterone-resistant DM on the basis of the history that had revealed proestrus bleeding 5 weeks earlier. Progesterone level was moderate high (43.7 ng/ml; reference range, 15.0-90.0 ng/ml). Ovariohysterectomy (OHE) was performed to remove the cause of the dog's diabetes. After OHE 11 days, blood glucose was gradually declined by insulin treatment. Consequently, blood glucose was well controlled in reference range without insulin treatment after 2 months. This case is a report on progesterone-induced DM treated with OHE and insulin treatment during the diestrus.

The protective effects of Aster yomena (Kitam.) Honda on high-fat diet-induced obese C57BL/6J mice

  • BACKGROUND/OBJECTIVES: Aster yomena (Kitam.) Honda (AY) has remarkable bioactivities, such as antioxidant, anti-inflammation, and anti-cancer activities. On the other hand, the effects of AY against obesity-induced insulin resistance have not been reported. Therefore, this study examined the potential of AY against obesity-associated insulin resistance in high-fat diet (HFD)-fed mice. MATERIALS/METHODS: An obesity model was established by feeding C57BL/6J mice a 60% HFD for 16 weeks. The C57BL6/When ethyl acetate fraction from AY (EFAY) at doses of 100 and 200 mg/kg/day was administered orally to mice fed a HFD for the last 4 weeks. Normal and control groups were administered water orally. The body weight and fasting blood glucose were measured every week. Dietary intake was measured every other day. After dissection, blood and tissues were collected from the mice. RESULTS: The administration of EFAY reduced body and organ weights significantly compared to HFD-fed control mice. The EFAY-administered groups also improved the serum lipid profile by decreasing the triglyceride, total cholesterol, and low-density lipoprotein compared to the control group. In addition, EFAY ameliorated the insulin resistance-related metabolic dysfunctions, including the fasting blood glucose and serum insulin level, compared to the HFD-fed control mice. The EFAY inhibited lipid synthesis and insulin resistance by down-regulation of hepatic fatty acid synthase and up-regulation of the AMP-activated protein kinase pathway. EFAY also reduced lipid peroxidation in the liver, indicating that EFAY protected hepatic injury induced by obesity. CONCLUSIONS: These results suggest that EFAY improved obesity-associated insulin resistance by regulating the lipid and glucose metabolism, suggesting that AY could be used as a functional food to prevent obesity and insulin resistance.

Effects of Insulin Level on Dyslipidemia in Children with Simple Obesity (단순 비만아에서 인슐린이 지질대사 이상에 미치는 영향)

  • Purpose : Obesity is closely related to insulin resistance, compensatory hyperinsulinemia and dyslipidemia in adults. We identified the effect of obesity measured by BMI and insulin resistance on dyslipidemia in children and adolescents. Methods : The fasting serum insulin, glucose, total cholesterol, triglyceride, HDL- and LDL-cholesterol were measured and insulin resistance(HOMA-IR) was calculated in 35 children with simple obesity(age :$10.6{\pm}2.8$ years; male 20, female 15; BMI : $27.1{\pm}5.4kg/m^2$). Results : The hypertriglyceridemia(37%), hyperinsulinemia(54%) and HDL-hypocholesterolemia(5.7%) were observed. HOMA-IR was well expressed by fasting insulin. As BMI increased, there was a statistically significant increase in insulin resistance and insulin level in both sexes. BMI was not related with lipid profile in both sexes. Triglyceride was correlated with only insulin level and insulin resistance index in boys. In girls, there was no correlation between triglyceride, HDL-cholesterol and insulin(insulin resistance). Conclusion : These results suggest that hypertriglyceridemia was dependent on insulin resistance in pre-adult males. Monitoring of insulin resistance and those risk factors known to become a part of insulin resistance syndrome should become part of routine medical care for obese children.

Diurnal Variations in Serum Glucose, Insulin and C-Peptide of Normal Korean Adults (정상(正常) 한국인(韓國人) 혈청(血淸) 포도당(葡萄糖), Insulin 및 C-Peptide의 일중(日中) 변동(變動)에 관(關)한 연구(硏究))

  • It is already well known that many factors are involved in maintaining normal blood glucose level. The amount and components of meal are also thought to be some of the factors which affect the blood glucose and insulin levels. It is reported that as for Koreans sugar takes up over 75% out of 2,098 kcal, the average daily calorie intake per adult. It implies that Koreans take a high-sugar diet compared with Westerners who take $40\sim50%$ of sugar out of their total average daily calorie. For the purpose of studying diurnal variations in serum glucose, insulin and C-peptide of normal Korean adults based on ordinary Korean diet, we selceted 13 normal Korean male adults and divided them into two groups, Group I (7 persons) and Group II (6 persons). We put Group I on 3,100kcal and 75% sugar diet, and Group II on 2,100 kcal and 69% sugar diet per day for over 4 days. Serum glucose, insulin and C-peptide were checked every 30 minutes or every hour throughout 24 hours. Results are as follows: 1. As for serum glucose level, in the prep ran dial fasting state in the morning, $mean{\pm}S.D.$ of Group I was $91.1{\pm}8.2mg%$, while that of Group II is $82.5{\pm}4.4mg%$. Both groups showed peaks of increased glucose level at postprandial 1 hour after each meal. The peak returned to the level shown during the fasting state at postprandial 1 hour after breakfast while the relatively high glucose levels were maintained respectively even for 2 or 3 hours.after lunch and dinner. 2. As for serum insulin level, Group showed $mean{\pm}S.D.$ of $14.7{\pm}3.0{\mu}U/ml$ while Group II shows that of $7.0{\pm}2.6{\mu}U/ml$ in the fasting state. Group I particularly showed the largest peak from preprandial a half or one and half an hour to postprandial one hour of lunch, and made relatively small peaks $(47.7{\pm}10.8{\mu}U/ml)$ at postorandial 1 hour after breakfast and dinner. No such large peak was marked in Group II, though it showed relatively similar patterns of peak after each meal. 3. As for C-peptide, in the fasting state, Group I and Group II showed $3.50{\pm}1.85$ and $1.66{\pm}0.53ng/ml$ of $mean{\pm}S.D.$, respectively. Group II showed peaks parallel to those for insulin level. None out of seven in Group I showed expected increase in C-peptide based insulin secretion at a half or one and half an hour before lunch. On the contrary, C-peptide increased in 5 subjects out of seven in Group I at 11:00 p.m. when insulin did not increase. 4. According to the integrated concentration method for a measurement of 24-hour total. insulin secretion rate, the $mean{\pm}S.D.$ of Group I was $76.4{\pm}15.2$ U and that of Group II was $58.6{\pm}21.1$ U. The above results confirm that Koreans, when given ordinary diet of 2,100 kcal and 69%, sugar, show insulin secretion pattern essentially similar to that of Westerners. On the contrary, when they are put on a high-calorie diet of 3,100 kcal a day, 75% of which is sugar, insulin secretion can be increased before lunch without increase in blood glucose. These results implies that insulin secretion can be affected by some other factors. The observation that an increase in C-peptide after 11 : 00 p.m. independent of insulin level supports an assertion that insulin secretion and C-peptide secretion can be thought as being physiologically dissociable, and these changes of diurnal patterns in the levels of serum insulin and C-peptide are thought to be resulted from the large meal and high-carbohydrate diet.

Immunocytochemical Study on the Translocation Mechanism of Glucose Transporters by Insulin

  • The mechanism of insulin action to increase glucose transport is attributed to glucose transporter translocation from intracellular storage pools to the plasma membrane in insulin-sensitive cells. The present study was designed to visualize the redistribution of the glucose transporter by means of an immunogold labelling method. Our data clearly show that glucose transporter molecules were visible by this method. According to the method this distribution of glucose transporters between cell surface and intracellular pool was different in adipocytes. The glucose transporter molecules were randomly distributed at the cell surface whereas the molecules at LDM were farmed as clusters. By insulin treatment the number of homogeneous random particles increased at the cell surface whereas the cluster forms decreased at the intracellular storage pools. It suggests that the active molecules needed to be evenly distributed far effective function and that the inactive molecules in storage pools gathered and termed clusters until being transferred to the plasma membrane.

Lactobacillus rhamnosus GG Reverses Insulin Resistance but Does Not Block Its Onset in Diet-Induced Obese Mice

  • Recently, Lactobacillus rhamnosus GG (LGG) was shown to exert insulin-sensitizing and adiposity-reducing effects in high-fat (HF) diet-fed mice. In the present study, we observed that the effects were correlated with the extent of dysbiosis induced by HF diet feeding before LGG administration. LGG-treated mice were protected from HF diet-induced adiposity and/or insulin resistance when LGG was treated after, not along with, HF diet feeding. Results indicate that, under HF dietary condition, supplemented LGG reverses insulin resistance, but does not block its onset.

  • The solution conformation of the human insulin precursor, preproinsulin, is described in terms of NMR spectral data. NMR experiments were performed on preproinsulin, whose structure was compared with the NMR structure of native human insulin. Despite the presence of the C-peptide and/or the signal peptide, secondary structure analyses indicate that the native structures of the A and B chains are well conserved even in preproinsulin. The observed relative robustness of the native structure in precursor forms permits further protein engineering experiments where the C-peptide or N-terminal signal sequence can be altered for the purpose of increasing expression or purification yields when producing recombinant human insulin.

  • Hydroxybrazilin was examined for its effects on glycogen synthesis in primary cultured rat hepatocytes. At 10-6 M hydroxybrazilin, glycogen synthesis was increased in basal state, but not in insulin stimulated state. However, any signiFicant changes were nor observed at 10-5 M hydroxybrazilin in both states. The glycogen synthesis was rather suppressed at 10-5M hydroxybrazilin. It was also observed that hydroxybrazilin increased insulin sensitivity but not insulin responsiveness at 10-5M concentration. These results suggest that hydroxybrazilin might exert hypoglycemic action through its effects on insulin receptor and post receptor events.

  • $B^{30}$ 위치에 homoserine이 치환된 사람 insulin 유사체 ($B^{30}$ -homoserine) insulin을 생산하기 위해, insulin의 B 사슬 유전자에 A 사슬 유전자를 직접 연결한 insulin 유전자를 설계하였다. 이 유전자는 10개의 oligonucleotide로 나누어 합성하여 T4 DNA ligase로 결합시킨 후, pUC19 plasmi의 polylinker 영역에 삽입하였다. 이 유전자의 발현을 높이기 위해 이 유전자는 다시 tac promoter의 지배를 받는 lacZ 유전자의 Cia I 또는 Hpa I 제한부위에 도입하여 융합시켰다. 이렇게 구축된 운반체 pTBA나 pKBA를 Escherichia coli JM103 균주에 형질도입시킨 후, 이를 4시간 배양한 후 0.05mM이상의 isopropyl-$\beta$-D-thiogalactopyranoside (IPTG)를 배지에 공급해 주고 2시간 더 배양하였을 때 유전자 발현이 잘 유도되어짐을 알 수 있었다. 이 때 생산된 insulin 전구체들은 세포내 불온성인 inclusion body로 축적되어지는 것을 관찰하였으며, 그 생산량은 세포내 전체 단백질량의 30%에 달하였다.

Effects of Metformin and Rosiglitazone in Overweight or Obese Women with Polycystic Ovarian Syndrome (과체중 및 비만 다낭성 난소 증후군 환자에서의 Metformin과 Rosiglitazone의 효용성)

  • Objective: The purpose of this study was to evaluate and compare the effects of metformin and rosiglitazone in overweight or obese women with polycystic ovarian syndrome. Methods: Twenty Six overweight or obese patients with polycystic ovarian syndrome were randomly treated with either metformin (500 mg three times daily, n=13) or rosiglitazone (4 mg once daily, n=13) for 6 months. Hormonal studies were performed before and after treatment. Insulin resistances were calculated by computerized HOMA 2 Calculator v2.2. Results: Testosterone decreased while SHBG increased after 6 months treatment in both metformin and rosiglitazone treatment groups. Fasting glucose decreased after metformin or rosiglitazone treatment. HOMA insulin resistance improved after treatment with either drug. There was no differences in hormonal changes and insulin resistance between 2 treatment groups. Conclusions: This study shows that metformin and rosiglitazone are effective in improving insulin sensitivity and ameliorating hyperandrogenism in overweight/obese polycystic ovarian syndrome women.
