Effects of swallowing training of high viscosity bolus on swallow function based on videofluoroscopic swallowing examination in stroke patients with dysphagia (비디오 투시조영 검사를 통한 높은 점도의 음식 삼킴 훈련이 삼킴 장애가 있는 뇌졸중 환자의 삼킴 기능에 미치는 효과)

  • The purpose of this study is to investigate the effect of swallowing training of high viscosity bolus for swallow function of stroke patients with dysphagia. This study subjects, acute stroke 18 patients, which were recruited to receive treatment in inpatient at general hospital, located in Kyunggido. Subjects were randomly allocated in experimental group and control group. Experimental group performed swallowing training of high viscosity bolus, while control group conducted Traditional dysphagia therapy. Both groups received treatment 30 minutes a day five times a week for four weeks. The assessment was conducted FDS(: Functional Dysphagia Scale), PAS(: Penetration Aspiration Scale), ASHA NOMS(: American Speech-language-hearing Association National Outcomes Measurements System Swallowing Scale) to compare swallow function for both group. Both groups showed significant improvements after intervention in all measures(p<.05). Change score between the two groups showed a significant improvement on experimental group than control group in FDS(p<.05). Swallowing training of high viscosity bolus could have a positive impact on swallow function for acute stroke patients with dysphagia.

Determinants of Quality in Outpatient Medical Service (외래 의료서비스 질적 수준의 결정요소)

  • This study was conducted to evaluate the subjective ideas about the determinants of quality in ambulatory care unit among outpatients and medical staff of a university hospital, and to compare the differences of the ideas, between patients themselves and hospital staff. A self-administered questionnaire survey was conducted covering 799 outpatients and 190 hospital staff in March, 1998. The questionnaire included general characteristics and 26 determinants of ambulatory care quality. The following are summaries of the findings: 1. Both of outpatients and hospital staff perceived, "Physician's knowledge" as the most important determinant of medical care quality. 2. In respect of 7 determinants related to physician's knowledge and skill, both outpatients and hospital staff perceived "physician's knowledge and skill" as important determinants. The scores of determinants such as, "Not doing unnecessary examinations", and "Assignment of adequate number of patients and duty schedule for the physician" were significantly different between outpatients and hospital staff. 3. In respect of 4 determinants related to doctor-patient relationship, both outpatients and hospital staff perceived "attention to patient's complaints" as the most important determinant. The scores related to the determinants such as "kindness of physician" and "explanation of treatment outcome" were significantly different between outpatients and hospital staff. 4. Among the amenities related determinants, "Modern facilities and equipments" were perceived as the most important determinant in both group. 5. In respect of 8 determinants related to non-financial accommodation, outpatients perceived, "Waiting hours for treatment" as the most important determinant, and hospital staff perceived, "Kindness of hospital staff". 6. In respect of 4 determinants related to financial accommodation, outpatients perceived, "Fare account of medical cost" as the most important determinant, and hospital staff perceived, "Increasing reimbursements". Further comprehensive research should be made on the evaluation of perceptions of medical care quality, both of outpatient and inpatient care, among patients and hospital staff. So good quality in medical care will be achieved based on clients' needs.

An Analytical Study of Mammalian Bite Wounds Requiring Inpatient Management

  • Background Mammalian bite injuries create a public health problem because of their frequency, potential severity, and increasing number. Some researchers have performed fragmentary analyses of bite wounds caused by certain mammalian species. However, little practical information is available concerning serious mammalian bite wounds that require hospitalization and intensive wound management. Therefore, the purpose of this study was to perform a general review of serious mammalian bite wounds. Methods We performed a retrospective review of the medical charts of 68 patients who were referred to our plastic surgery department for the treatment of bite wounds between January 2003 and October 2012. The cases were analyzed according to the species, patient demographics, environmental factors, injury characteristics, and clinical course. Results Among the 68 cases of mammalian bite injury, 58 (85%) were caused by dogs, 8 by humans, and 2 by cats. Most of those bitten by a human and both of those bitten by cats were male. Only one-third of all the patients were children or adolescents. The most frequent site of injury was the face, with 40 cases, followed by the hand, with 16 cases. Of the 68 patients, 7 were treated with secondary intention healing. Sixty-one patients underwent delayed procedures, including delayed direct closure, skin graft, composite graft, and local flap. Conclusions Based on overall findings from our review of the 68 cases of mammalian bites, we suggest practical guidelines for the management of mammalian bite injuries, which could be useful in the treatment of serious mammalian bite wounds.

Clinical Characteristics of Pediatric Bipolar Disorder by Subtype in a Korean Inpatient Sample

  • Objectives : We compared the clinical presentations of manic and depressive episodes and the treatment response among children and adolescents with bipolar disorder (BD) types I and II and BD not otherwise specified (NOS). Methods : The sample consisted of 66 patients, aged between 6 and 18 years, who were admitted for BD to a 20-bed child and adolescent psychiatric ward in a university hospital located in Seoul, Korea. Results : Patients with BD type I were more likely to have lower intelligence quotients and exhibit violent behaviors during manic episodes than patients with BD type II or BD NOS and to show better treatment responses during manic episodes than patients with BD NOS. Patients with BD NOS were more likely to have an irritable mood rather than a euphoric mood during the manic phase than patients with BD type I or II and to exhibit violent behaviors during the depressive phase and chronic course than patients with BD type II. Conclusion : Pediatric BD patients are heterogeneous with respect to their clinical characteristics. Implications for the usefulness of the current diagnostic subtype categories should be investigated in future studies.

The Statistical Study of 84 Traffic Accident Patients hospitalized in Oriental Medical Hospital (한방병원에 입원한 교통사고 환자 84례에 대한 통계적 고찰)

  • Objectives : The purpose of the study was to find out the characteristic of patients admitted in Oriental Medical Hospital We to traffic accident and to promote the superiority of oriental medical treatment on the sequelae. Methods : 84 inpatients were analyzed statistically according to medical charts, hospitalized from January 1, 2003 to December 31, 2004. Results and Conclusions : By gender, males accounted for 38.1%, while females 61.9%. By age, most incidences occurred in the forties (25%), thirties (22.6%) and twenties (16.6%) in order. With regard to the interval from occurrence of accidents to hospitalization, 26 cases (31%) were found to be hospitalized 41 days after the accidents, followed by 21 cases (25%) in $11{\sim}20$ days and 17 cases (202%) in $1{\sim}2$ days. Most cases tumed out to be minor injuries with 40 patients from cervical sprain (47.6%) and lumbar sprain (37%). The mean duration of hospitalization was $36.58{\pm}48.30$ days Inpatients of paralysis of extremities (sequelae of ICH) numbered the longest stay $(121.00{\pm}75.57\;days)$, cervical sprain that recorded $11.28{\pm}6.52$ days hospitalized. Most of inpatients were treated with acupuncture(100%), moxibustion, cupping therapy (97.6%) and herbal medication(96.4%), chuna Manipulation therapy was chosen selectively with sprain, HIVD.

Sensorineural hearing loss in patients with Kawasaki disease

  • Purpose: Kawasaki disease involves acute febrile systemic vasculitis that can cause a variety of symptoms by affecting various organs. Here, we aimed to evaluate the prevalence, causes, and prognosis of sensorineural hearing loss (SNHL) occurring in children with Kawasaki disease. Methods: Patients who were diagnosed with Kawasaki disease and received inpatient treatment in the Pediatrics Department at one of three university hospitals in Daegu city from February 2012 to September 2012 were enrolled in the study. The clinical features, hematological results, echocardiography results, audiometry results, and aspirin and salicylic acid serum levels of the patients were evaluated. Results: Of the 59 children enrolled in the study, three showed mild bilateral SNHL on audiometry tests conducted after 48 hours of defervescence; these patients demonstrated normal patterns of recovery on follow-up tests 8 weeks later. Aspirin serum levels were significantly higher in the SNHL group after 48 hours of afebrile condition with high dose aspirin intake (P=0.034). However, no significant differences were found in other laboratory tests or for fever duration (P>0.05). Upon echocardiography, coronary artery abnormality was observed in 9 cases, but none of these patients showed hearing loss. Conclusion: The results indicate that SNHL in children with Kawasaki disease might occur during treatment of the acute phase; this SNHL usually involves mild bilateral hearing loss and recovers naturally. However, this study suggests that determination of the causes and clinical implications of hearing loss in Kawasaki disease requires long-term follow-up studies with more cases.

Costs of Initial Cancer Care and its Affecting Factors (암 환자의 발생 초기 의료비와 이에 영향을 미치는 요인)

  • Objectives : The purposes of this study is to estimate the cost of cancer care after its diagnosis and to identify factors that can influence the cost of cancer care. Methods : The study subjects were patients with an initial diagnosis one of four selected tumors and had their first two-years of cancer care at a national cancer center. The data were obtained from medical records and patient surveys. We classified cancer care costs into medical and nonmedical costs, and each cost was analyzed for burden type, medical service, and cancer stage according to cancer types. Factors affecting cancer care costs for the initial phase included demographic variables, socioeconomic status and clinical variables. Results : Cancer care costs for the initial year following diagnosis were higher than the costs for the following successive year after diagnosis. Lung cancer (25,648,000 won) had higher costs than the other three cancer types. Of the total costs, patent burden was more than 50% and medical costs accounted for more than 60%. Inpatient costs accounted for more than 60% of the medical costs for stomach and liver cancer in the initial phase. Care for latestage cancer was more expensive than care for early-stage cancer. Nonmedical costs were estimated to be between 4,500,000 to 6,000,000 won with expenses for the caregiver being the highest. The factors affecting cancer care costs were treatment type and cancer stage. Conclusions : The cancer care costs after diagnosis are substantial and vary by cancer site, cancer stage and treatment type. It is useful for policy makers and researchers to identify tumor-specific medical and nonmedical costs. The effort to reduce cancer costs and early detection for cancer can reduce the burden to society and improve quality of life for the cancer patients.

Case Report of Polyarteritis Nodosa Treated with Oriental Treatments Including Acupotomy (침도침 시술을 가미한 복합한방치료를 시행한 결절성 다발 동맥염의 치험례)

  • Objectives : Polyarteritis nodosa is a progressive disease of connective tissue that is characterized by nodules along arteries; nodules may block the artery and result in inadequate circulation to the particular area. This report is intended to estimate the efficacy using oriental complex treatment on a patient with Polyarteritis nodosa. Materials and Methods : From 19th March, 2012 to 12th May, 2012, One male inpatient diagnosed with polyarteritis nodosa was treated with general oriental medicine therapy : needle-embedding therapy ; acupuncture ; pharmacopuncture ; acupotomy therapy and herbal medication. VAS(visual analogue scale) was used for evaluation of both leg pain. Other subjective symptoms including night sweat, tinnitus, upper heat were evaluated by percentage comparing the symtoms before and after treatment. Results : The patient showed a certain degree of improvement in both leg pain and other subjective symtoms. Conclusions : Oriental treatments such as needle-Embedding therapy, acupuncture and moxibustion therapy, pharmacopuncture therapy, acupotomy therapy and herbal medication can be effective for controlling pain and other accompanied symtoms due to polyarteritis nodosa.

A Study of Improvement on Low Back Pain Caused by Lumbar Disc Herniation Depending on Female Inpatient's Bone Mineral Density(BMD), Body Mass Index(BMI), Age, Pain Duration (여성의 골밀도가 요추 추간판 탈출증으로 인한 요통 호전도에 미치는 영향 및 골밀도와 비만도, 연령, 병력 기간의 관계에 관한 연구)

  • Objectives: This study explored the relationships between Bone Mineral Density (BMD), Body Mass Index(BMI), Age, pain duration outcomes of treatment for Low Back Pain(LBP) in 3 groups(normal,osteopenia,osteoporosis) of female patients. Methods: We evaluated 32 cases of normal, 30 cases of osteopenia and 38 cases of osteoporosis based on BMD, BMI, Age, pain duration among the female inpatients with LBP caused by lumbar disc herniation, admitted at Jaseng Hospital of Oriental Medicine from January 2010 to December 2011. And we measured VNRS(Verbal Numeric Rating Scale) to evaluate the conservative treatment effects. Results: In improvement of female LBP caused by lumbar disc herniation, BMD(T-score), BMI had not proved correlation(P-value>0.05). Female patients' age was negatively correlated with T-score. In pain improvement, BMD normal group showed more reduction of NRS(numeric rating scale) than osteoporosis group, but that had not proved correlation(p-value=0.893). Conclusions: Statically, among the female inpatients with LBP caused by lumbar disc herniation, had no significant differences between the normal group, osteopenia group and osteoporosis group's improvement rate, depending on treating period.

Effects of Therapeutic Exercise on Fatigue and Anxiety of Cachexia Patients(Case Report) (악액질(cachexia) 환자에게 적용한 운동치료가 피로와 불안에 미치는 영향(단일사례 보고))

  • Purpose : Cachexia, is a complex metabolic syndrome associated with underlying illness and characterized by loss of muscle with or without loss of fat mass. Patients with cachxia shows various symptoms including fatigue, anxiety, pain, sleep disorders, and poor appetite. Medications therapy, dietary and exercise therapy, and emotional support are recommended to treat patients with cachexia. However, evidence-based research verifying the role of exercise therapy in patients with cachexia is limited. The purpose of this study was to investigate the effects of therapeutic exercise on fatigue and anxiety in patients with cachexia. Methods : Case report. A 29-year-old woman was diagnosed with cachexia. Following 2 weeks of inpatient and 4 weeks of out-patient treatment. we assessed her weight, as well as pain, fatigue, and anxiety level. As an the patient exercised for an hour 5 times a week for 2 weeks,-and during the outpatient visit, she exercised for an hour twice a week for 4 weeks. Her weight was measured using a weighting scale. Pain was assessed using the visual analog scale, and fatigue and anxiety levels were assessed using questionnaires. Results : Following 6 weeks of treatment, exercise therapy a positively affected the patient's weight, as well as pain, fatigue, and anxiety levels. We observed a weight gain of 4.5 kg, pain reduction of 5.1 points. Fatigue reduction of 43 points, anxiety reduction of total 41 points. Conclusion : Exercise therapy positively affects weight, as well as pain, fatigue, and anxiety levels in patients with cachexia. However, generalization of this observation is inappropriate based on this single case study.
