Effects of Heat Treatment and Rest-inserted Exercise on Muscle Activity

  • Prolonged immobilization leads to significant weakness and atrophy of the skeletal muscle and can also impair the recovery of muscle strength following injury. Therefore, it is important to minimize the period under immobilization and accelerate the return to normal activity. This study examined the effects of combined heat treatment and rest-inserted exercise on the muscle activity of the lower limb during knee flexion/extension. Twelve healthy subjects were assigned to 4 groups that included: (1) heat treatment + rest-inserted exercise; (2) heat treatment + continuous exercise; (3) no heat treatment + rest-inserted exercise; and (4) no heat treatment + continuous exercise. Heat treatment was applied for 15 mins prior to exercise. Continuous exercise groups performed knee flexion/extension at 0.5 Hz for 300 cycles without rest whereas rest-inserted exercise groups performed the same exercise but with 2 mins rest inserted every 60 cycles of continuous exercise. Changes in the rectus femoris and hamstring muscle activities were assessed at 0 and 2 weeks of treatment by measuring the electromyography signals of isokinetic maximum voluntary contraction. Significant increases in both the rectus femoris and hamstring muscles were observed after only 2 weeks of treatment when both heat treatment and rest-inserted exercise were performed. These results suggest that combination of various treatment techniques, such as heat treatment and rest-inserted exercise, may accelerate the recovery of muscle strength following injury or immobilization.

  • A thyroid cartilage fracture is a rare entity and can be overlooked easily. Such cases are difficult to diagnose, and assessment and treatment guidelines are difficult to determine. CT of the neck region may be useful when acute airway intervention is not required or when more information regarding the neck's anatomy is required for management decisions. We describe a case of a thyroid cartilage fracture with displacement. In the emergency department (ED), neck CT and fiberoptic nasopharyngoscopy were used to assess the status of the patient's (a male) vocal chords immediately. He remained unable to phonate continuously. After an immediate assessment, we decided to use steroid and conservative therapy. The patient had a good recovery and was without symptoms one month after injury. There is no question that early surgical repair of neck injuries affords the best results for airway and voice patency in most cases however, we suspect that surgical repair is not needed in all cases. Early recognition and an accurate therapy plan for a thyroid fracture with displacement are essential. Therefore, the emergency physician's immediate and careful decision based on endoscopy and neck CT is important for the patient's long-term recovery.

  • The tenn 'Brain plasticity' has been identified that our central nervous system is continuously being adapted and modulated according to environmental needs and demands, and has been used to encompass the multifarious mechanisms related to learning, development, and recovery from damage to the nervous system. The purpose of this study was to demonstrate cortical reorganization in a 26-year-old right-handed hemiparetic patient with traumatic primary motor cortex (M1) injury, using functional MRI (fMRI). The unaffected (left) primary sensori-motor cortex centered on the precentral knob was activated during unaffected (right) hand movements. However, the medial area of the injured M1 was activated during affected (left) hand movements. It seems that the motor function of the affected hand in this patient was reorganized into the medial area of the injured precentral knob. These investigations provide a great useful information and clinical evidences with the specialized clinician in stroke physical therapy about patient's prognosis and therapeutic guidelines.

Free Flap Reconstruction in Patients with Traumatic Injury of the Forefoot

  • Many techniques have been developed for reconstruction of the hand; however, less attention has been paid to foot reconstruction techniques. In particular, reconstruction of the forefoot and big toe has been considered a minor procedure despite the importance of these body parts for standing and walking. Most of the weight load on the foot is concentrated on the forefoot and big toe, whereas the other toes have a minor role in weight bearing. Moreover, the forefoot and big toe are important for maintaining balance and supporting the body when changing directions. Recently, attention has been focused on the aesthetic appearance and functional aspects of the body, which are important considerations in the field of reconstructive surgery. In patients for whom flap reconstruction in the forefoot and big toe is planned, clinicians should pay close attention to flap survival as well as functional and cosmetic outcomes of surgery. In particular, it is important to assess the ability of the flap to withstand functional weight bearing and maintain sufficient durability under shearing force. Recovery of protective sensation in the forefoot area can reduce the risk of flap loss and promote rapid rehabilitation and functional recovery. Here, we report our experience with two cases of successful reconstruction of the forefoot and big toe with a sensate anterolateral thigh flap, with a review of the relevant literature.

The recovery effect of polyacetylene constituents from Acanthopanax senticosus extracts on hepatic injury in dogs

  • The present study was performed to clarify the recovery effect of polyacetylene constituents from Acanthopanax senticosus ethanol extracts on $CCl_4$-induced acute hepatic injury in dogs. Sixteen clinically healthy dogs were divided into the control group (six dogs), and experimental group I (three dogs), II (three dogs) and III (four dogs). Acute hepatic injury was induced by single intraperitoneal injection of 1 ml/kg $CCl_4$ solution ($CCl_4$ : olive oil = 1 : 1) after sterilization. The control group was orally administered with 10 ml of corn oil, and experimental groups I, II and III were orally administered with extracts of A. senticosus at the dose of 5, 10 and 30 mg/kg in 10 ml corn oil for seven consecutive days at 24 h after $CCl_4$ injection. We assessed changes occurring in serum ALT, AST and GGT activities on the pre, day 0, day 1, day 2, day 3, day 5 and day 7, together with the histopathological findings. In experimental group I, the significant changes in serum ALT activities were found on day 5 (p<0.05) and day 7 (p<0.05), compared with those in the control group. However, significant differences were not found in the changes of serum AST and GGT activities. In experimental group II, significant changes were found on day 2 (p<0.05), day 3(p<0.05), day 5 (p<0.05) and day 7 (p<0.05) in serum ALT activities, on day 1 (p<0.05) in serum AST, and on day 2 (p<0.05) and day 3 (p<0.05) in serum GGr activities, respectively. In experimental group III, the changes in serum ALT, AST and GGT activities were not significant. In the histopathological evaluation, experimental group I and II showed much improvement, while experimental group III became worse when compared with the control group. Based on the results of the present study, it was concluded that low dose (below 10 mg/kg) administration of polyacetylene constituents from A. senticosus ethanol extracts was effective for recovery of acute hepatic injury induced by $CCl_4$ in dogs.

설신경 손상 환자의 회복에 대한 후향적 연구 (Recovery of lingual nerve injury: retrospective observational study)

  • Introduction: This study evaluated nerve recovery through retrospective study of patients with lingual nerve damage. Patients and Methods: The patients who visited Seoul National University Dental Hospital for an injury to the lingual nerve from April 1988 to August 2009 were enrolled in this study (n=41). The relevance of various factors including the causes of damage, age, etc. was analyzed by the subjective improvement based upon questionnaires and the clinical records. The evaluation variants were a subjective assessment and neurosensory examination composed of the direction, contact threshold, two-point discrimination, pin prick, thermal discrimination and current perception threshold. Results: The causes of lingual nerve damage were an extraction of the lower third molar (75.6%), local anesthesia (9.7%), incision and drainage (4.88%), trauma (2.44%). The evaluation of subjective prognosis exhibited no difference in sensory improvement depending on the cause, age and gender. Based upon the subjective evaluation, 44.7% of patients showed sensory improvement. The first hospital visit from injury was shorter in the group showing subjective improvement (3.41 months) than those showing no improvement (5.24 months) (P=0.301). Thirty six out of 41 patients were treated with only conservative therapy and 5 patients were treated by surgical intervention. Neurosensory examinations revealed improvement, although not statistically significant, and the degree was higher in the subjectively improved group. The contact threshold discrimination showed the highest correlation with subjective improvement (P=0.069). Most of the sensory recovery was gained within 12 months and the degree of improvement at the tip of the tongue was higher than that of the dorsum (P<0.001). Conclusion: The damaged lingual nerve improved at a rate of 44.7% and mostly within 12 months after the incident. There was no difference between the subjective prognosis and neurosensory examination depending on the cause of damage, age and gender, whereas the contact threshold discrimination was the best variant that reflected the subjective prognosis statistically.

양혈장근건보탕(養血壯筋健步湯)과 녹용약침(鹿茸藥鍼)이 척수손상 유발 흰쥐의 운동기능 회복 및 신경재생에 미치는 영향 (Effects of Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) and Cervi Cornu Parvum Pharmaco-Acupuncture on the Motor Function Recovery and Nerve Regeneration in Rats Induced Spinal Cord Injury)

  • Objectives : The purpose of this study was to investigate the effects of Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) and Cervi Cornu Parvum pharmaco-acupuncture in Spinal Cord Injury(SCI)-induced rats. Methods : The subjects were divided into 5 groups ; Normal, Control no treatment after SCI, Experimental I taken with Yanghyuljanggeungunbo-tang (Yangxuezhuangjinjianbu-tang) 500 mg/kg $0.5m{\ell}$ daily after inducing SCI. Experimental II taken with Cervi Cornu Parvum pharmaco-acupuncture at Taegye(KI3) and $Yangnungch{\acute{o}}n$(GB34) after inducing SCI and Experimental III taken with Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) 500 mg/kg $0.5m{\ell}$ and Cervi Cornu Parvum pharmaco-acupuncture at KI3 and GB34 to SCI-induced rats. After each operation, the present author observed the motor behavior recovery and nerve regeneration by analysis of the motor behavior tests, EMG, hematological(AST, ALT, WBC), histological and immunological changes. Rats were tested at modified Tarlov test at the 1st, 2nd, 3rd, 4th day, and Motor behavior test at 1st, 3rd, 7th, 14th, 21st day. Results : Results are as follows. 1. All the experimental groups were improved compared with control group in the motor behavior tests including Tarlov test, Basso-Beattle-Bresnahan locomotor rating scale, modified inclined plane test, open field test, grid walk test and narrow beam test. Especially Experimental III was improved significantly among other groups. 2. In EMG test, H wave appeared weak only in Experimental III. And M wave was increased significantly in Experimental III. 3. All the experimental groups were significantly decreased compared with control group in serum AST, serum ALT and serum WBC tests. 4. significantly decreased in Tumor Necrosis Factor-${\alpha}$ test compared with the first day of SCI. 5. Muscle contraction and denaturation of all the experimental groups were inhibited in histological observations of gastrocnemius muscle. Especially, those of experimental III was more effective. 6. NGF and BDNF of spinal cord gray matter in all the experimental groups were increased compared with control group. Especially, those of experimental III was more effective. Conclusions : As above, it can be suggested that Yanghyuljanggeungunbo-tang(Yangxuezhuangjinjianbu-tang) and Cervi Cornu Parvum pharmaco-acupuncture may improve motor behavior, EMG, hematological, histological and immunological findings in Spinal Cord Injury(SCI)-induced rats. Especially, effects will be somewhat better in combination of these two treatments.

교통사고(交通事故)로 인한 편타성(鞭打性) 손상(損傷) 환자(患者) 37례(例)에 대한 임상연구(臨床硏究) (The clinical study on 37 cases of whiplash injury patients which caused by traffic accident)

  • Objective : This study was performed to evaluate the effect of oriental medical treatment in whiplash injury patients which caused by traffic accident. Methods : The clinical study was carried out 37 cases of whiplash injury patients which had been admitted in Daejon university Cheon-an oriental hospital from June, 2001 to December, 2001. Results : l. In the distribution of sex distinction was female much more than male in the ratio of 3:2 and thirties was most in age distribution. 2. In situation of traffic accident, The most case was rear-ending(70.27%). 3. Except cervical pain or it's reffered pain, there were whole body symptoms as fallow, insomnia in 11cases(29.73%), night pain in 10 cases(27.03%), general body pain in 8cases(21.62), dizziness in 6cases(16.22%). 4. According to Meridian Muscle theory, a injured parts of patient were devided as follows. There were 22cases(59.46%) which have injury in Bladder, Small Intestine Meridian Muscle, 10cases(27.03%) in Gallblader, Triple Energizer Meridian Muscle, 5cases(13.51%) in Stomach, Large Intestine Meridian Muscle. 5. There were 6cases(16.22%) of the grade of complete recovery, 10cases(27.03%) of the grade of excellent, 12cases(32.43%) of the grade of improvement, 6cases(16.22%) of the grade of disimprovement and 3cases(8.11%) of the grade of poor. So 28cases(75.68%) were efficacious. 6. Generally Herb-meds that have efficacy of Geo-Eohyeol were most used(27cases, 72.97%) in early stage. Secondly Herb-med that have efficacy of Yiqi-sunqi were used(20case, 54,05%) in middle stage and Herb-med of Bo-Qiheol were used(l0cases, 27.03%) in latter term. The most used Herb-med was Hoisu-san, Oyaksungi-san(22cases, 54.96%). 7. In l5cases which have Aqua-Acupuncture treatment, There were 3cases of Complete recovery, 5cases of Excellent, 4cases of Improvement, 2cases of Disimprovement, 1cases of Poor. 8cases(53.33%) were above Excellent grade, so it is more efficatious Than simple treatment without Aqua-Acupuncture(36.37%). 8. The improvement of Bladder, Small Intestine Meridian Muscle was 77.27%, Gallblader, Triple Energizer Meridian Muscle was 70% and Stomach, Large Intestine Meridian Muscle was 80%. Additional Aqua-Acupuncture treatment improved to be effective in Bladder, Small Intestine Meridian Muscle(77.78%), in Gallblader, Triple Energizer Meridian Muscle(75%), in Stomach, Large Intestine Meridian Muscle(100%). Conclusions : In this study, Oriental treatment especially Aqua-Acupuncture was effective in whiplash injury patients which caused by traffic accident.

Clinical outcome of conservative treatment of injured inferior alveolar nerve during dental implant placement

  • Objectives: Infererior alveolar nerve (IAN) damage may be one of the distressing complications occurring during implant placement. Because of nature of closed injury, a large proportion is approached non-invasively. The purpose of this study was to analyze the outcomes of conservative management of the injured nerve during dental implant procedure. Materials and Methods: Sixty-four patients of implant related IAN injury, who were managed by medication or observation from January 1997 to March 2007 at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital, were retrospectively investigated. The objective tests and subjective evaluations were performed to evaluate the degree of damage and duration of sensory disturbance recovery. Tests were performed on the day of the first visit and every two months afterward. Patient's initial symptoms, proximity of the implant to the IAN, time interval between implant surgery and the first visit to our clinic, and treatment after implant surgery were analyzed to determine whether these factors affected the final outcomes. Results: Among the 64 patients, 23 had a chief complaint of sensory disturbance and others with dysesthesia. The mean time until first visit to our hospital after the injury was 10.9 months.One year after nerve injury, the sensation was improved in 9 patients, whereas not improved in 38 patients, even 4 patients experienced deterioration. Better prognosis was observed in the group of patients with early visits and with implants placed or managed not too close to the IAN. Conclusion: Nearly 70% of patients with IAN injury during implant placement showed no improvement in sensation or dysesthesia with the conservative management. Earlier decision for active treatment needs to be considered because of possibility of deterioration of symptoms and unsatisfactory recovery.

Effectiveness of Combined Korean Medicine on Traumatic Spinal Cord Injury: A Case Report

  • Objectives: This study aims to evaluate the clinical effectiveness of a Korean medicine treatment on a traumatic spinal cord injury. Metohds : A 44-year-old male patient who had suffered a traumatic spinal cord injury was treated for two months. The effectiveness of complex Korean medicine treatments was measured using the Numeric Rating Scale (NRS) and the Manual Muscle Test (MMT). Result : We found that the NRS of pain caused by numbness was decreased from 7 to 0-1, and the overall motor measured by the MMT was also improved. The patient's walking state was changed from wheelchair ambulation to walker ambulation. Conclusion : Based on this finding, complex Korean medicine treatments may be effective in treating patients' pain, controlling paresthesia, and aiding the recovery function of activities in daily living. However, because of the limitation of our study, further high-quality research should be conducted.
