Analysis of Readmission Patients after Lumbar Microdiscectomy (요추간판 미세 현미경 수술 후 재입원 환자의 분석)

  • Objectives : After lumbar microdiscectomy there are relatively higher incidence of readmission because of various postoperative discomfort. Analyzing these readmitted patients in our private hospital where the patients can be readmitted more easily, we expect to find out which factors are associated with improvement of the surgical outcome. Patients and Methoes : After discharge all significant patient's clinical data were registered to our computerized data bank system and periodic follow-up were performed. The authors analysed 651(97.7%) cases followed up over 2 years(average 3 years) out of 666 patients who underwent a initial lumbar microdiscectomy from May, 1994 to April, 1997. Among them, 63 patients(9.7%) were readmitted. Results : Among the patients readmitted, 29 patients(4.5%) were reoperated because of recurred disc herniation, 17 patients were myofascial pain syndrome, 4 patients were discitis, 3 patients were adhesion and one patient was epidural abscess. Treatment results of these readmitted patients showed that success rate of all readmitted patients was 68.3%, reoperation was 58.6% and myofascial pain syndrome was 82.4%. Conclusion : Treatment results of the patients readmitted because of a continuous lumbago or leg pain after microdiscectomy were acceptable through the careful analysis of the causes of the recurrent symptom. Therefore, the incidence of the chronic failed back syndrome can be reduced by efforting a more active management through readmission.

A Retrospective Analysis of Ventriculoperitoneal Shunt Revision Cases of a Single Institute

  • Objective : Ventriculoperitoneal (VP) shunt complication is a major obstacle in the management of hydrocephalus. To study the differences of VP shunt complications between children and adults, we analyzed shunt revision surgery performed at our hospital during the past 10 years. Methods : Patients who had undergone shunt revision surgery from January 2001 to December 2010 were evaluated retrospectively by chart review about age distribution, etiology of hydrocephalus, and causes of revision. Patients were grouped into below and above 20 years old. Results : Among 528 cases of VP shunt surgery performed in our hospital over 10 years, 146 (27.7%) were revision surgery. Infection and obstruction were the most common causes of revision. Fifty-one patients were operated on within 1 month after original VP shunt surgery. Thirty-six of 46 infection cases were operated before 6 months after the initial VP shunt. Incidence of shunt catheter fracture was higher in younger patients compared to older. Two of 8 fractured catheters in the younger group were due to calcification and degradation of shunt catheters with fibrous adhesion to surrounding tissue. Conclusion : The complications of VP shunts were different between children and adults. The incidence of shunt catheter fracture was higher in younger patients. Degradation of shunt catheter associated with surrounding tissue calcification could be one of the reasons of the difference in facture rates.

  • In this study, we evaluate the braking performance of an urban railway vehicle to verify its basic safety condition. The braking performance evaluation methods, deceleration measurement and braked weight percentage, were compared for trains with different numbers of cars, in order to assess the advantages of each method and their compatibility. With a probabilistic braking model, the effect of the adhesion coefficient distribution was analyzed in accordance with the train composition. A train with many cars has a narrower deceleration distribution width than one with few cars. The braked weight percentage method is expected to be useful in the design of train signal systems, because it allows the braking distance to be calculated for various initial brake velocities. The deceleration distribution model and its results are expected to be useful as a basis for precise train signal design.

The effect of sandblasting duration on the bond durability of dual-cure adhesive cement to CAD/CAM resin restoratives

  • PURPOSE. To evaluate the effect of prolonged sandblasting on the bond durability of dual-cure adhesive resin cement to computer-aided design and computer-aided manufacturing (CAD/CAM) restoratives. MATERIALS AND METHODS. Nano-ceramic LAVA Ultimate and hybrid-ceramic VITA Enamic CAD/CAM blocks were used for this study. Each CAD/CAM block was sectioned into slabs of 4-mm thickness for the microtensile test (${\mu}TBS$) test and 2-mm thickness for the surface roughness test. Three groups were created according to the sandblasting protocols; group 1: specimens were sandblasted for 15 seconds, group 2: specimens were sandblasted for 30 seconds, and group 3: specimens were sandblasted for 60 seconds. After sandblasting, all specimens were luted using RelyX Ultimate Clicker. Half the specimens were subjected to ${\mu}TBS$ tests at 24 hours, and the other half were subjected to tests after 5000 thermocycles. Additionally, a total of 96 CAD/CAM block sections were prepared for surface roughness tests and scanning electron microscopy (SEM) evaluations. The Mann-Whitney U test, Kruskal-Wallis one-way analysis of variance, and Dunn's post hoc test were used to compare continuous variables among the groups. RESULTS. At baseline, group 1, group 2, and group 3 exhibited statistically similar ${\mu}TBS$ results for LAVA. However, group 3 had significantly lower ${\mu}TBS$ values than groups 1 and 2 for VITA. After 5000 thermocycles, ${\mu}TBS$ values significantly decreased for each block (P<.05). CONCLUSION. It is important to perform controlled sandblasting because it may affect bond strength results. Sixty seconds of sandblasting disturbs the initial ${\mu}TBS$ values and the stability of adhesion of CAD/CAM restoratives to dual-cure adhesive resin cement for VITA Enamic.

  • A TSL (Thin Spray-on Liner) which consists of polymers has a higher initial strength, faster construction time and higher waterproofing performance than the conventional cementitious shotcrete. Main supporting mechanism of TSL is the adhesion and tensile strength which is distinct from the conventional shotcrete. Even though highly in demand due to its outstanding characteristics, TSL is not yet well-known support material. In this study, to evaluate contact behavior of TSL, numerical analysis was performed with comparing result from laboratory tests. From the analysis, cohesive behavior at the contact surface between TSL and rock can be evaluated by using combination of cohesive and the damage model. In addition, results show that the cohesive stiffness controled slope between force and displacement, the fracture energy controled level of force at the contact.

Preparation of the Blends of Poly(amic acid) and PBO Precursor and Their Properties (Poly(amic acid)와 PBO 전구체의 블렌드 제조 및 특성)

  • The thermal properties, morphology, mechanical properties and gas permeability of the blends of poly (amic acid) (PAA) and poly (o-hydroxyamides) (PHAs) having pendant group was investigated. The 5% weight loss and major weight loss of the b)ends occurred in the ranges of $348{\sim}407^{\circ}C$ and $589{\sim}615^{\circ}C$ upon a heating process. After a thermical annealing, the tensile strength and initial modulus of blends increased $3.7{\sim}52.9%$ and $34.4{\sim}70%$ from the value of pure PAA, respectively. Especially the tensile strength and modulus of the PAA/MP-PHA=9/1 showed the highest values (97.5 MPa and 2.67 GPa, respectively), which were 53 and 70% higher than those of pure PAA. The fine PHA domains were found to be uniformly dispersed. The interfacial adhesion between PAA and PHA was identified to be good. The gas permeabilities of PAA/M-PHA blend increased with M-PHA contents.

Change in Risk of Dropout Due to Bleeding during Bloodletting-Cupping Therapy (습식 부항 시술시 사혈량에 따른 부항 탈락 위험도 탐색)

  • Objectives : To investigate minimum pressure by verifying changes in pressure due to bleeding amount during bloodletting-cupping therapy. Methods : (1) We compared adhesion performance of four different cupping cups of same size: two disposable cupping cups(A, B) and two reusable cupping cups(A, B) each were vacuumed three times and kept in place for 10 minutes. (2) We vacuumed two different sized disposable cupping cups(A), size.1(InnerDiameter 48.8 mm) and size.3(InnerDiameter 39.1 mm), twice each(-200 mmHg) on silicon plate. We injected water and air at regular intervals in cupping cups by using a syringe, and then measured change of pressure in cupping cups and pressure at the time of dropout. Results : (1) Pressure reduction was $4.75{\pm}2.78%$ on average in the order of 'Disposable[A]>reusable[B]>Disposable[B]>reusable[A]', so that pressure retention performance of disposable cups can't be regarded as inferior to that of reusable cups. (2) Pressure of disposable cupping B(size.1) decreased by an average of -40.08 mmHg per 5 ml of water. At -24.8 mmHg, when 22 ml of water has been injected, cup has come off. Pressure of disposable cupping B(size. 3) decreased by an average of -99.4 mmHg per 5 ml of water. At -48.6 mmHg, when 13 ml of water was injected, cupping came off. Conclusions : Considering reduction rate of pressure due to water injection, in case of bleeding more than 15 ml, size.3 cup always comes off, therefore it needs to be re-operated at least once. Meanwhile, size.1 cup does not always come off in the same condition, depending on the initial pressure and therefore, re-operation may be considered.

Effects of Tape Thickness and Inorganic Fillers on the Adhesion Properties of Double-sided Acrylic Adhesive Tape by Ultraviolet Curing (자외선 경화형 아크릴 양면 점착테이프의 두께 및 무기물 충전제 종류에 따른 접착특성)

  • To manufacture of high-performance semi-structural double-sided adhesive tape, 2-ethylhexyl acrylate (2-EHA) and acrylic acid (AAC) were used, and the syrup was prepared by UV irradiation in this study. The effects of the thickness, various inorganic filler contents, and filler types on the semi-structural properties of acrylic double-sided adhesive tape were investigated. The peel strength increased with increasing thickness and wetting time. In case of the thin thickness (under $250{\mu}m$) with decreasing true density of inorganic filler, the peel strength increased with increasing wetting time. The initial peel strength showed a higher value at a big size of inorganic filler, and the filler's size in adhesive tapes was confirmed by SEM images. The peel strength and dynamic shear strength increased as a proportional relationship with various inorganic fillers and contents, and these inorganic fillers in $0.1{\mu}m$ thickness indicated more effect on the dynamic shear strength of double-sided adhesive tape. From these results the thin acrylic double-sided adhesive tape determined to be use for applications as a high-performance semi-structural.

Analysis of Output Characteristics of Lead-free Ribbon based PV Module Using Conductive Paste (전도성 페이스트를 이용한 무연 리본계 PV 모듈의 출력 특성 분석)

  • Environmentally benign lead-free solder coated ribbon (e. g. SnCu, SnZn, SnBi${\cdots}$) has been intensively studied to interconnect cells without lead mixed ribbon (e. g. SnPb) in the crystalline silicon(c-Si) photovoltaic modules. However, high melting point (> $200^{\circ}C$) of non-lead based solder provokes increased thermo-mechanical stress during its soldering process, which causes early degradation of PV module with it. Hence, we proposed low-temperature conductive paste (CP) based tabbing method for lead-free ribbon. Modules, interconnected by the lead-free solder (SnCu) employing CP approach, exhibits similar output without increased resistivity losses at initial condition, in comparison with traditional high temperature soldering method. Moreover, 400 cycles (2,000 hour) of thermal cycle test reveals that the module integrated by CP approach withstands thermo-mechanical stress. Furthermore, this approach guarantees strong mechanical adhesion (peel strength of ~ 2 N) between cell and lead-free ribbons. Therefore, the CP based tabbing process for lead free ribbons enables to interconnect cells in c-Si PV module, without deteriorating its performance.

Investigation into the Thermal Stability of Fluoropolymer Coating for Heat-Resistant Application (내열성 불소수지 코팅막의 열 안정성에 관한 연구)

  • Fluoropolymer of PTFE and PFA etc. is a heat resistance polymeric material that it is known as that maximum continuous use temperature reaches for 260 $^{\circ}C$. It was observed that these polymers had the enough thermal stability so enough that it was kept by thermal aging of 280 $^{\circ}C$/7 weeks too in this study. However, such thermal stability means that bulk material property is kept such as mechanical strength, melting point and initial pyrolysis temperature etc. If these polymers are evaluate by coating property such as surface contact angle, surface morphology, surface scratch, thing that heat resistance is not enough was confirmed in this study. Thermal aging of flouropolymer coating was achieved by gear aging oven that the exchange rate of air was controlled, and the analysis results were indicating serious damage of surface morphology and adhesive strength on metal substrate.
