From May 1984 through December 1991, twelve patients underwent valve replacement for infective endocarditis at National Medical Center. There were 7 male and 5 female, ranged in age 16 to 61[mean 34.1] years. Four had native valve endocarditis, six had prosthetic valve endocarditis and two were associated with congenital heart disease. The indication of surgery was medically intractable congestive heart failure in all patients. 5 patients revealed systemic embolization and 4 patients had uncontrollable sepsis. The causative organism was Streptococcus in 4 patients, Staphylococcus in 1 patient and Pseudomonas in 2 patients. Hospital mortality was 33.3%[4/12]. The main cause of death was low cardiac output due to perioperative myocardial damage and cerebral vascular accident. There were 2 late mortality because of recurrent endocarditis. This review showed much higher mortality in prosthetic valve endocarditis[66.7%] than native valve endocarditis[33.3%].
This study was carried out to investigate incubating characteristics, LTSEM observation, nature of a disease of infected silkworms(bombyx mori), LD$\_$50/, optimum infective condition for mass production and infection percentage(%) on conservation periods. On the V8 media, the colony was 0.5∼2.0cm/8 days in diameter and white or slighty colored with a white fluffy to powdery appearance. The conidia was subglobos in shape, zig-zag appearance, 1-celled, hyaline and 3.2$\times$2.4 ㎛ in size on the average. The conidiophore was irregular grouped, hyaline, rounded or flask-shaped. The LD$\_$50/ values of the 2nd and 4th silkworms were each other 6.6142(Log) in natural temperature and humidity (25$\^{C}$, 65%). On optimum infective condition for mass production, all of preservation time(hr.) postinoculation in 1.0$\times$10$\^$8/ conidia/㎖ was over 97% and only 20 hr in 1.0$\times$10$\^$7/ condia/㎖ was over 90%.
Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.
This is a report of a successful management of a patient with infective endocarditis involving native aortic valve, mitral valve, tricuspid valve, and Interventric lar septum. A 16 year-old patient who underwent VSD patch closure, and aortic valvuloplasty at the age of 1 1 years showed Intractable congestive heart failure during antibiotics treatment for infective endocarditis. Operative findings revealed that there were large defect along the previous patch, aortic regurgitation with multiple perforations and vegetations, mitral regurgitation with vegetation, aortic paraannular abscess, interventricular myocardial abscess, and tricuspid regurgitation with perforations and vegetations. We reconstructed the interventricular defect with Dacron patch extending to the aortic valve annulus after radical debridement of all infected or devitalized tissues, and could implant aortic valve by anchoring to the reconstructed Dacron patch. Mitral valve was replaced and tricuspid valve was repaired with patient's own pericardium. The patient was discharged after antibiotics treatment for 6 weeks and in good condition without any sequelae for 12 months.

An endemic entomopathogenic nematode, Heterorhabditis megidis, was evaluated by its control efficacies against housefly, Musca domestica, and flower beetle, Gametis jucunda. In Petri-dish assay, the pathogenicity of H. megidis showed 456.4 infective juveniles/larva (IJs/larva) in median lethality (LC$_{50}$) against the second instar larvae of M. domestica and 238.9 IJs/larva against the second instar larvae of G. jucunda. This was contrasted with those of the other well-known entomopathogenic nematode, Steinernema carpocapsae, which showed 115.9 IJs/larva against M. domestica and 388.6 IJs/larva against G. jucunda. In field experiment, H. megidis were applied per square meter of pork farm with 1,000,000 IJs of H. megidis or apple orchard with 370,000 IJs, which were infested with M. domestica or G. jucunda, respectively. H. megidis showed 56.9% and 57.3% of control efficacies against M. domestica and G. jucunda, respectively. These results suggest a promising control technique in the field using H. megidis against M. domestica and G. jucunda.a.
Pathogenic aminoacyl-tRNA synthetases (ARSs) are attractive targets for anti-infective agents because their catalytic active sites are different from those of human ARSs. Mupirocin is a topical antibiotic that specifically inhibits bacterial isoleucyl-tRNA synthetase (IleRS), resulting in a block to protein synthesis. Previous studies on Thermus thermophilus IleRS indicated that mupirocin-resistance of eukaryotic IleRS is primarily due to differences in two amino acids, His581 and Leu583, in the active site. However, without a eukaryotic IleRS structure, the structural basis for mupirocin-resistance of eukaryotic IleRS remains elusive. Herein, we determined the crystal structure of Candida albicans IleRS complexed with Ile-AMP at 2.9 A resolution. The largest difference between eukaryotic and prokaryotic IleRS enzymes is closure of the active site pocket by Phe55 in the HIGH loop; Arg410 in the CP core loop; and the second Lys in the KMSKR loop. The Ile-AMP product is lodged in a closed active site, which may restrict its release and thereby enhance catalytic efficiency. The compact active site also prevents the optimal positioning of the 9-hydroxynonanoic acid of mupirocin and plays a critical role in resistance of eukaryotic IleRS to anti-infective agents.
PCR amplication using the primers for gag, pol and env genes in BLV (bovine leukemia virus) proviral DNA and syncytium assay were carried out for the Korean native goats experimentally infected with bovine leukemia virus to investigate pathogenesis of BLV in the goats, and to establish a model animal for BLV infection. The oligonucleotide primers used in PCR revealed very high specificity. The minimal amount of FLK-BLV cellular chromosomal DNA to detect the integrated BLV proviral DNA was 10 ng. The peripheral blood lymphocytes from the goat infected with BLV were examined at regular intervals by PCR amplification and syncytium assay. Pol or gag genes were detected in none of three infected goats at the 1st week post-infection (p.i.). At the 4th week p.i., one of three goats showed the amplified gag gene. Thereafter detection rates for the genes were increased, indicating that the BLV proviral genes were integrated in all of the lymphocytes from three goats, at the 16th weeks p.i., when it was evident in syncytium assay that the lymphocytes from all of three goats were infested with infective BLV. Investigating the tissues from the necropsied goats at the 8th month p.i., the amplified BLV proviral genes and infective BLV were detected in all of the peripheral lymphocytes from three infected-goats. Among various tissues examined, the amplified BLV proviral genes were observed in spleen and superficial cervical, mandibular and retropharyngeal lymph nodes, and the infective BLV, in superficial cervical and mandibular lymph nodes. It was assumed that the Korean native goat was quite susceptible to BLV infection, indicating that the goat could be a good model animal for BLV.
A 3-year-old intact female Maltese dog (2.5 kg of body weight) with the primary complaint of sudden onset of heart murmur, depression and anorexia was referred to the Veterinary Teaching Hospital of Kangwon National University. The dog was febrile with marked leukocytosis and left apical VI/VI holosystolic murmurs. The electrocardiogram implied the left ventricular enlargement. Diagnostic imaging studies revealed left atrial and ventricular dilation, severe vegetations on mitral valvular cusps with concurrent mitral regurgitation. Based on findings from clinical and diagnostic investigation, the case was diagnosed as vegetative mitral valvular regurgitation caused by infective endocarditis. The dog was successfully treated with broad spectrum antibiotics, diuretics, angiotensin converting enzyme inhibitor and antithrombotics.
Cryopreservation of infective juveniles of entomopathogenic nematode, Steinernema carpocapsae Weiser, was conducted at $-190^{\circ}C$ liquid nitrogen and its, efficacy was analysed on nematode survival and pathogenicity with glycerol pretreatments and storage periods. Infective juveniles were pre-treated before being frozen by incubating the nematodes in 22% glycerol for each of 6, 12, and 24 h, followed by 70% methanol at $0^{\circ}C$ for 10 minutes. Just after glycerol and methanol incubations, subsamp1es of the nematodes were resuspended in 0.85% saline and maintained during 24h for viability determination. Different glycerol incubation periods significantly affected the nematode susceptibility to methanol infiltration. Six hour incubation in glycerol resulted in much less nematode survival than did 12 h or 24 h incubation. About 70% of the infective juveniles frozen at $-190^{\circ}C$ for 5 months, preincubat-ed in glycerol at least for 12h, were able to survive after being resuspended in 30°C saline. They did not also show any change in their pathogenicity during cryopreservation. These results suggest an improved technique for long-term storage of the entomopathogenic nematodes.
