개흉술이 필요했던 다발성 외상환자에 대한 임상적 고찰 (Clinical Analysis of Patients with Multiple Organ Injuries Who Required Open thoracotomy)

  • 배경: 다발성 외상환자는 교통사고, 산업재해, 우발적사고, 폭력 등의 증가로 최근 증가 추세에 있다. 다발성 외상은 심장, 폐, 대혈관 등의 흉부외상을 포함하는 경우가 많으며 이때는 환자의 생명을 위협할 수 있으므로 적절한 진단과 치료가 필요하다. 대부분의 흉부외상은 보존적 치료와 흉관 삽관술과 같은 간단한 외과적 시술 만으로 만족할 만한 치료효과를 보이지만 적절한 시기에 개흉술을 시행함으로써 환자의 생명을 구할 수 있는 경우가 있어 그 적응증 이나 시기의 판단에 있어서 경험있는 흉부외과 의사의 역할이 필요하다. 대상 및 방법: 저자는 다발성 외상 후 흉부개흉술이 필요했던 70례에 대하여 분석하였다. 환자들의 평균 나이는 35.6세 남녀비는 3.4:1이었다. 손상의 기전은 주로 교통사고, 추락사고, 자상이었다. 결과: 흉부손상의 가장 일반적인 유형은 혈흉이며, 두 번째는 횡격막 파열이었다. 환자의 60%에서 골절을 동반 하고 42.9%에서 복부손상을 37.1%에서 두부손상을 동반하고 있었다. 개흉술을 시행한 이유로는 출혈교정 혹은 지혈이 48.6%, 횡격막손상의 복구가 35.7%였으며, 폐 열상 봉합이 25.7% 였으며, 흉부 손상으로 인한 수술외에도 비장적출술(14.3%), 간엽절제술(8.6%) 간 열상 복구(5.7%)를 시행하였다. 수술후 합병증으로는 무기폐(8.6%), 창상감염(8.6%), 폐렴(4.3%)이었다. 수술후 6명의 환자가 사망하였고 수술사망율은 8.6%였다. 사망의 원인으로는 호흡부전증(2례), 급성 신기능부전증(2례), 패혈증(1례), 저혈량성 쇽(1례)이었다.

건설공사 재해의 시간대별 발생빈도 (The Distribution of Injuries by Hours of the Day in Construction)

  • 수년간 건설업은 제조업을 비롯한 다른 산업에 비해 높은 재해율을 보이고 있다. 건설투자의 증가는 2001년과 2002년 각각 7.4%와 12%씩 증가하였으나, 재해발생건수는 2001년(16771건)은 2000년(13500건)에 비해 24% 증가하였고 2002년 상반기(8853건)는 2001년 상반기(6636건)보다 33%나 증가하여 건설투자의 증가율보다 높았다. 더욱이 건설취업자수가 2001년(1581천명)의 경우 전년(1575천명)에 비해 0.4%가 감소되었음을 감안할 때 건설물량의 양적 증가에도 불구하고 안전관리에는 소홀한 면이 많았으며 재해예방을 위한 적극적인 노력이 요구된다.(중략)

  • 최근의 농업은 작업의 기계화 등으로 생산성이 높아진 반면, 재해나 건강 문제 등 부작용을 수반하게되었고 그 부작용은 점점 심각한 수준에 이르고있는 반면, 예방측면의 대책은 제조업에 비하여 상대적으로 미미한 수준에 머무르고 있다. 미국의 경우 농업의 재해율(4.3%)은 전 산업평균 재해율의 약1.4배로(NSC,1998) 광업, 건설업 다음의 위험업종이라고 알려져 있고, 일반재해자는 전체의 약 3%임에도 사망자는 전체의 14%나 차지하고 있어서 재해의 강도는 매우 큰 것으로 나타나고 있다. (중략)

이동식 크레인 사고사망재해 심층분석 연구 (A study on Cause Analyses of Fatal injuries by the Mobile Cranes)

  • In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by mobile cranes. Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, fatality rate per 10,000 persons by the mobiles cranes was 0.57 in 2011. Second, the in-depth analysis of the mobile crane accidents showed that the main factors of the accidents were the overturn of mobile crane, the fracture of connection part between mobile crane boom and rope, the breaking of rope and the misunderstanding over the signal between a mobile crane driver and workers around a mobile crane. Third, the order of high defection of connecting parts was bucket bolt, bracket and extension parts. Fourth, since there were several cases of overturn of mobile crane by itself in the in-depth analysis of the mobile crane, the over slop-proof device was proposed as the main alternative in this study. Fifth, the revision of standards of safety factor that shall be more than 10, of connecting parts of mobile crane was proposed.

하악골 골절의 임상통계학적 연구 (ANALYSIS OF 334 CASE REPORTS OF MANDIBULAR FRACTURE)

  • We retrospectively reviewed 334 inpatients who sustained a total of 518 mandibular fractures and who ewer treated in our department between l980-1990. This results were obtained as follows : 1. In respect of incidence, there were the highest frequency in July, and the lowest frequency in May. The number of patients has not been increased year after year due to competition with other department in our hospital. 2. The age frequency was the highest in the 2nd decade(38.9%) and the ratio of man to women was 4.9 : 1. 3. The most frequent cause of mandibular fracture was traffic accident(43.4%), and the next was fall down(24.3%), fist blow(71%), industrial accident(21%) and others in order. In the traffic accident, autobicycle accident was 14.1%. 4. The most common location of mandibular fracture was symphysis(38.8%), condyle(20.7% ), angle(19.9%) and body(15.1%) were next in order of frequency. The classification by location of fracture, the frequency of single fracture was 54.8%. 5. In 334 patients of mandibular fracture, the frequency of associated injuries was facial laceration(58.4%), teeth injuries(37.7%), extremity injuries(13.2%) were next in order of frequency. 6. The patients arrived in hospital immediately within 24 hours after accident wee 61.4% of all. In respect of treatment, open reduction was 68.7% of all. 7. Complications including infection were present 11.1% of patient. Other complications inclued delayed healing malocclusion, malocclusion and neurologic problem.

Incidence and management of mandibular fractures in a low-resource health facility in Ghana

  • Objectives: The mandible and other parts of the maxillofacial region suffer significant morbid injuries following road traffic accidents. Our study gives epidemiological description of mandibular fractures in Ghana and also evaluates the relevance of closed reduction and indirect fixation for managing mandibular fractures in low-resource health facilities in low-income countries like Ghana. Patients and Methods: This is a retrospective study involving 268 patients who reported to the Department of Oral and Maxillofacial Surgery of the Sunyani Regional Hospital with mandibular fractures from January 2010 to December 2019. Patient medical records were assessed for information on age, sex, fracture etiology, anatomic location of fracture, time of day of road traffic accident, and other associated injuries. Results: A total of 268 patients were included in this study (males, 216 [80.6%]; females, 52 [19.4%]). Motor vehicular accident (MVA) was the leading cause of mandibular fractures (202 injuries, 75.4%). Other etiologies included assault (39, 14.6%), gunshot (13, 4.9%), falls (12, 4.5%), and industrial accidents (2, 0.7%). Of the 161 male cases caused by MVA, 121 (75.2%) occurred at night and in the evening while the remaining 40 (24.8%) occurred in the morning and afternoon. Among all managed 222 patients, 212 (79.1%) were treated with closed reduction and indirect fixation technique while 10 (3.7%) were treated with open reduction and direct fixation. Conclusion: Closed reduction with indirect fixation could successfully be used to manage mandibular fractures in low resourced health facilities, especially in low-income countries. The poor lightening system on roads in Ghana is a major contributory factor to motor vehicular accidents.

일부지역 산업재해환자 실태 연구 -대구, 경북지역 일부 종합병원 중심으로- (A Study of Industrial Patients from Selected General Hospitals in the Kyung Pook and Taegu City Areas)

  • The purpose of this study is to research the actual conditions of industrial accident patients and to produce worker satisfaction and a rational and effective counter measure pain. Direct interviews with 179 cases (in and out patients) were carried out during a three month period from April to July 1990, at six hospitals two general hospitals Sun Lin and Sung Mo in Po Hang, and four general hospitals in Taegu Kyung Pook University Hospital, Dong San Medical Center, Young Nam Medical Center and Catholic Hospital. The results of this study are summarized as follows: 1. Among the 179 cases, 51.6 % were male and 48.4 % were female. The two largest age groups were 30~39, 31.8 % and 20~29, 27.4 %. Among the 179 cases, 51.6% were married, the largest family number was 2 to 3, 41.1% and 4 to 5, 25.6%. Educationally, graduation from high school was the largest group, 46.4% among the patients, followed by middle school and primary school. The largest group income level was from 40~69만원, 45.2%. The largest group of patients who worked over 50 hrs. a week was 52.0%. The largest group of patients who worked less than 1 year was 44.7%, of the patients in work places of less than 100 people, 60.3% were injured and in work places of 100~299 people, 20.1% were injured. In manufacturing, the lagest group injured was 55.3%, the next group was transport, stroage, communication. The largest group of production workers injured was 40.2%. 2. The cause of injury in the largest group was facility problems, 33.5%. The next group was unsafe habits, 30.2% a lack of safety knowledge, 17.9% and insufficient supervision, 12.3%. The 30~39 year age group was head the highest number of injuries, 40.4% work places with more than 10 yeras of work, 44.4% work palces with more than 1000 people, 56.3% and mining accidents, 80.0%. Among these groups the highest cause of injury was due to facility problems. 3. The accident pattern showed machinery injuries 28.5% as the largest group, followed by falls & falling objects 17.3%, fire & electric 15.1%, struke by an object 14.5%, followed by overaction and vehicular accidents. The accident pattern showed 46.4 % among workers over the 50 year age group, workers in the 5~10 year group, 50.0 % places employing more than 1000 workers, 35.3 % : construction 73.7%, and construction workers 57.1%, among these fall & falling objects caused the greatest number of injuries. 4. The largest group of injuries was fractures 54.8%, trauma 14.5%, amputation 11.7%, open wound, and burns. The largest number of fractures occurred in people in the 30~39 year age group, 63.2 % over 10 years of work, 55.6% in work places of 300~400 people, 63.6% construction 63.2% and general workers 57.2 %. 5. The largest group of injuries was upper extremity 45.3%, lower extremity 24.0%, trunk 18.5 % and head or neck 12.2%. Of these groups, upper extremity injuries were the highest in those less 20 years old 75.0%, less than 1 years of work 59.5%, in work places of 500~999 people 60.0%, manufacturing 56.6 % and production workers 55.6%. 6. Periods of injury showed 34 people injured in September, to be the largest followed by October, 32 August, 22 people July, 19 people and the lowest December, 2 people. During the week, Friday had the largest group injured, 35 people followed by Saturday, 26 people and the lowest was Wednesday, 17 people, During the day 1400 hours had the largest group injured, 38 people followed by 800 hours, 31 people. 7. On a basis of 5 as the highest mark, the average, according to worker satisfaction showed facility safety 3.55, work environment 3.47, income 3.44, job 3.21 and treatment 2.98. 8. The correlation between general characteristics and injury showed that age was directly correlated to the duration of work(r=.2591) p<0.01, age was directly correlated to industry (r=2311) p<0.01, and the duration was directly correlated to occupation(r =.4372) p<0.001.

대기업섬유업체(大企業纖維業體)에 있어서의 산업재해감소추이(産業災害減少推移) (A Decreasing Trend of Industrial Injuries at a Large Scale Textile Company)

  • 산업재해의 원인을 규명하는 데에 일익이 될까하여 근로자 2,307명을 고용하고 있는 한 대기업 섬유업체를 대상으로 1981년 1월 1일부터 1982년 12월 31일까지 만 2년간에 걸쳐 발생된 재해를 분석해 본 결과는 다음과 같다. 전담안전부서를 설치한 결과 도수율은 81년도 9.70, 82년도 4.15였고 건수율은 근로자 1,000명당 81년도 27.11, 82년도 12.96이었다. 강도율은 81년도 0.33, 82년도 0.01이었고 평균손실 일수는 81년도 34.64, 82년도 3.34로서 전년에 비하여 매우 감소되었다. 산업재해 발생율은 연평균 2.3%로서 남자 2.7%, 여자 2.0%였다. 근속기간별 재해발생빈도는 근속 1년미만군이 3.6%로 가장 높았고 근속기간이 길어질수록 점차 감소하였다. 월별 산업재해건수율은 6월이 근로자 1,000명당 3.6으로서 제일 높게 나타났다. 계절별 재해건수율은 여름이 8.2로서 가장 높았다. 요일별 재해건수율은 각 요일간에 별차이를 발견할 수 없었으나 일요일이 1.8로 제일 낮았다. 산업재해 건수율을 시간별로 관찰해 보면 오전 $10{\sim}12$시가 14.7로 가장 높았으며 반별로는 오전반이 34.4로 가장 높았다. 상해부위는 손가락이 42.3%로 제일 높았고, 상지부 65.4%, 하지부 14.4%, 기타부위가 20.2%였다. 성별로 보면 남자는 상지 46.8%, 기타 53.2%로 상지와 기타부위가 비슷한 상해율을 보였으나 여자는 상지 80.7%, 기타 19.3%로서 주로 상지에 상해를 입고 있었다. 상해종류는 개방창이 46.1%로 가장 높았고 다음이 좌상 15.4%, 표재성손상 6.7% 순이었다. 성별로 보면 남자는 개방창 40.4%, 좌상 17.0%, 골절 10.6%, 화상 8.5%, 표재성손상 6.5% 순위였고 여자는 개방창 50.8%, 좌상 14.0%, 피부박리 8.8%, 표재성손상과 자상이 각각 7.0%순으로서 남여공히 개방창과 좌상이 높게 나타났다. 가해요인별 산업재해율은 기계에 의한 것이 47.1%로 제일 높았고 그 다음이 수공구 16.3%, 차량 및 운반기 9.6%, 유독 및 고열물 7.7%. 낙하물 6.7% 순이었다. 산업재해 발생원인은 환경요인이 13.5%, 인적요인 86.5%를 나타내었다. 인적요인중 심리적요인이 60.6%로 가장 많았다. 연도별로 산업재해 발생율을 보면 수공구, 차량 및 운반기, 전기 낙하물에 의한 것은 감소 경향을 나타내었으나 기계, 유독 및 고열물에 의한 것은 증가 하였고 추락과 전도는 81년도엔 발생되지 않았으나 82년도에 새로이 발생되었다. 이러한 것은 대상업체가 안전사고 재발방지에 주력한 탓으로 동질 재해 방지에는 어느정도 성공할 수 있었으나 기계안전공학에 대한 지식결여와 작업현장에 존재하는 모든 재해요인을 철저히 파악치 못했기 때문에 기계, 추락, 전도에 의한 안전사고는 예방할 수 없었을 것으로 생각된다.
