Effectiveness and feasibility of concurrent chemoradiotherapy using simultaneous integrated boost-intensity modulated radiotherapy with and without induction chemotherapy for locally advanced pancreatic cancer

  • Purpose: To evaluate the effectiveness and feasibility of chemoradiotherapy (CRT) using simultaneous integrated boost-intensity modulated radiotherapy (SIB-IMRT) in locally advanced pancreatic cancer (LAPC) patients. Materials and Methods: Between January 2011 and May 2015, 47 LAPC patients received CRT using SIB-IMRT. Prior to SIB-IMRT, 37 patients (78.7%) received induction chemotherapy (IC-CRT group) and remaining 10 patients (21.3%) did not received induction chemotherapy (CRT group). During SIB-IMRT, all patients received concomitant chemotherapy, with gemcitabine (n = 37) and capecitabine (n = 10). Results: At the time of analysis, 45 patients had died and 2 patients remained alive and the median follow-up time was 14.2 months (range, 3.3 to 51.4 months). For all patients, the median times of local progression-free survival (LPFS), progression-free survival (PFS), and overall survival (OS) were 18.1, 10.3, and 14.2 months, respectively. The median time of LPFS between IC-CRT and CRT groups was similar (18.1 months vs. 18.3 months, p = 0.711). IC-CRT group had a higher trend in PFS (10.9 months vs. 4.1 months, p = 0.054) and had significantly higher OS (15.4 months vs. 9.5 months, p = 0.007) than CRT group. In multivariate analysis, the use of induction chemotherapy and tumor response were significant factors associated with OS (p < 0.05, each). During SIB-IMRT, toxicity of grade ≥3 was observed in 7 patients (14.9%) in all patients. Conclusions: CRT using SIB-IMRT is feasible and promising in LAPC patients.

  • The purpose of this study was to analyses the main factors of research papers for related with home physical therapy. This study was retrospective descriptive study, the period of data collection was from 1991 to 2011. The data was collected by the journal related in physical therapy, the dissertation of academic degree, National Assembly Library and the web-site for academic information. In the web-site, searched with the keyword 'home physical therapy' and 'after school voucher'. The results were as follows; 1. In the home physical therapy, visiting physical therapy, school physical therapy, there were different based on laws; home physical therapy was based on medical law, visiting physical therapy was based on law for community health and law for long term health insurance, school physical therapy was based on special education law. 2. The summary of research title/thema from 1991 to 2011 was as follows; for the home and visiting physical therapy 'the needs and necessity of home and visiting physical therapy' was 18 papers, 'the contents of service of home and visiting physical therapy' was 18 papers, 'program and skill development' was 16 papers, 'system developing and induction strategy of home and visiting physical therapy 'was 15 papers, 'costs of nome and visiting physical therapy' was 2 papers, 'perception and information of home and visiting physical therapy' was 9 papers, 'Recoding system and administration management' was 9 papers, 'the others 'was 14 papers, for the school physical therapy 'after school voucher system' was 9 papers, the others was 4 papers. The total papers was 114 papers. 3. Finally suggested 'Model of Educational Program for HomeVisiting and School Physical Therapy'.

A ROCK Inhibitor Blocks the Inhibitory Effect of Chondroitin Sulfate Proteoglycan on Morphological Changes of Mesenchymal Stromal/Stem Cells into Neuron-Like Cells

  • Chondroitin sulfate proteoglycan (CSPG) inhibits neurite outgrowth of various neuronal cell types, and CSPG-associated inhibition of neurite outgrowth is mediated by the Rho/ROCK pathway. Mesenchymal stromal/stem cells (MSCs) have the potential to differentiate into neuron-like cells under specific conditions and have been shown to differentiate into neuron-like cells by co-treatment with the ROCK inhibitor Y27632 and the hypoxia condition mimicking agent $CoCl_2$. In this study, we addressed the hypothesis that a ROCK inhibitor might be beneficial to regenerate neurons during stem cell therapy by preventing transplanted MSCs from inhibition by CSPG in damaged tissues. Indeed, dose-dependent inhibition by CSPG pretreatment was observed during morphological changes of Wharton's jelly-derived MSCs (WJ-MSCs) induced by Y27632 alone. The formation of neurite-like structures was significantly inhibited when WJ-MSCs were pre-treated with CSPG before induction under Y27632 plus $CoCl_2$ conditions, and pretreatment with a protein kinase C inhibitor reversed such inhibition. However, CSPG treatment resulted in no significant inhibition of the WJ-MSC morphological changes into neuron-like cells after initiating induction by Y27632 plus $CoCl_2$. No marked changes were detected in expression levels of neuronal markers induced by Y27632 plus $CoCl_2$ upon CSPG treatment. CSPG also blocked the morphological changes of human bone marrow-derived MSCs into neuron-like cells under other neuronal induction condition without the ROCK inhibitor, and Y27632 pre-treatment blocked the inhibitory effect of CSPG. These results suggest that a ROCK inhibitor can be efficiently used in stem cell therapy for neuronal induction by avoiding hindrance from CSPG.

Changes in the Oral Microflora in Patients with Acute Myeloid Leukemia during the Period of Induction Therapy (항암 화학요법중인 급성 골수성 백혈병 환자의 구강내 세균변화에 관한 연구)

  • To investigate the changes in aerobic and facultative anaerobic oral microflora during remission-induction chemotherapy in patients with acute myeloid leukemia, 10 consecutive patients were studied during a period of 28 days. One day before, during and after the induction therapy, patients were given 10% Betadine solution for mouthrinses after breakfast and kept from eating and drinking. After 3 hours, paraffin-stimulated whole saliva was obtained for 2 minutes and transported to the laboratory. The samples were dispersed and homogenized by use of vortex mixer for 20 seconds. From these samples 10-fold serial dilutions (from 10-1 through 10-3) were prepared. Each dilution of 0.1 ml was plated on duplicate set of one nonselective medium (Blood agar) and four selective media (Sabourauds dextrose agar, Mannitol salt agar, Mac-Conkey agar, SF medium ) using applicator woods. All agar plate were incubated at 37$^{\circ}C$ for 48 hours. The total number of microorganisms was calculated and the percentage distribution of the various microorganisms from each specimen was drawn. 1. The salivary flow rate decreased by 66%, going from 5.38 ml/2min to 1.81 ml/2min over two days during the chemotherapy. 2. The total number of microorganisms in saliva increased by 22%, going from 4.88$\times$105/ml to 6.00$\times$105/ml over two days during the chemotherapy. 3. The salivary flow rate and the total number of microorganisms in saliva were recovered within 28 days after the chemotherapy. 4. The quantitative alteration in oral Enterobacteria, Enterococci, Staphylococci, Cndida during the chemotherapy had no statistical significance. 5. In saliva of the patients with acute myeloid leukemia who ahd intraoral ulcer, Enterobacteria was quantitatively predominent. Our study suggests that chemotherapy-induced transient xerostomia may induce acute oral infection. Consequently, the use of saliva substitute, the removal of intraoral infection source and the consistent oral hygiene care seem to be required to avoid the transmission of potential pathogenes in this group of patients.

Protective Effect of Kaempferol on Cultured Neuroglial Cells Damaged by Induction of Ischemia-like Condition

  • This study was performed to evaluate the cytotoxicity induced by ischemia-like condition (ILC) in cultured neuroglial cells (C6 glioma cells). The protective effect of kaempferol (KAE), flavonoid against the cytotoxicity induced by ILC induction was assessed. In addition, antioxidative effects of KAE were done by colorimetric assays. Cell viability and the antioxidative effects such as DPPH-radical scavenging activity, superoxide dismutase (SOD)-like activity and inhibitory activity of lipid peroxidation (LP) were analyzed. ILC induction decreased cell viability in a dose-dependent manner, and the $XTT_{90}$ value (low cytotoxicity value) and $XTT_{50}$ value (high cytotoxicity value) were determined during ILC induction for 15 and 40 minutes, respectively. The butylated hydroxytoluene (BHT) antioxidant significantly increased cell viability damaged by the ILC-induced cytotoxicity. In the protective effect of KAE on ILC-induced cytotoxicity, KAE protected the ILC-induced cytotoxicity by the significant increase of cell viability, and also it showed DPPH-radical scavenging ability, SOD-like ability and inhibitory ability of LP. From these results, it is suggested that ILC induction showed cytotoxicity in these cultures and the oxidative stress is involved in the ILC-induced cytotoxicity. While, KAE prevented ILC-induced cytotoxicity by antioxidative effects. In conclusion, natural products like KAE may be a putative therapeutic agent for the treatment of disease associated with oxidative stress such as ischemia.

The Effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain (근통증이 유발된 흰쥐에 있어 TENS와 냉적용이 이차성 열 통각과민에 미치는 영향)

  • The aim of this study was to investigate the effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain. Muscle pain was induced in male Sprague-Dowley rats by intra-muscular injection of gastrocnemius with $3\%$ carrageenan. The paw withdrawal latency(PWL) and tail flick test(TFT) to heat were used to detect secodary thermal hyperalgesia induced by the muscle pain. PWL and TFT were quantified before and 4, 10, and 24 h after induction of muscle pain and after application of TENS(100Hz, $100{\mu}s$, sensory intensity) and cold($4^{\circ}C$). TENS and cold significantly reduced the PWL and TFT to heat stimuli when compared with controls receiving no TENS and cold(p<.05). These results suggested that application of TENS and cold attributed to decrease secodary thermal hyperalgesia in rat induced by muscle pain.

Effects of Cupping Therapy on Memory Impairment after Hemorrhage in Rats (뇌출혈 동물 모델에서 부항이 뇌 인지기능 회복에 미치는 효과)

  • Hemorrhage is a devastating type of stroke, accounts for 15-20% of all strokes. This disease can cause cognitive dysfunction with a very high mortality rate. Cupping therapy of Traditional Korean medicine has frequently been used to relieve a variety of diseases or clinical conditions, although not in the memory loss after hemorrhage. This study was designed to evaluate the effects of cupping therapy on learning and memory with Y-maze test, as well as its effects on different molecular changes in hippocampus following the induction of hemorrhage in rats. Cupping, using vacuum cupping machine, was applied at target area for 5 min daily for 7 consecutive days, commencing 1 day after brain impairment. As a result, induction of hemorrhage enhanced memory deficit, suppressed brain-derived neurotrophin factor (BDNF) in the hippocampus. Cupping treatment effectively reversed collagenase-induced cognitive impairment in SD rats which was represented by improvement of spontaneous alterations in Y-maze test. In addition, BDNF expression was enhanced after cupping therapy. The present results suggest that the therapeutic effects of cupping treatment after hemorrhage is involved in expression of BDNF.

Study on the Effect of Phenol Compound-Cold Therapy plus Exercise Therapy on the Muscle Pain Induced by Carrageenan (페놀 화합물 냉치료와 운동요법의 병행적용이 Carrageenan에 의하여 유발된 근육 통증에 미치는 영향에 관한 연구)

  • Purpose : The aim of this study is to examine the effect of the phenol compound-cold therapy plus exercise therapy on the carrageenan(CAR)-induced muscle pain. Method : Mice were injected 0.1ml of 2% CAR into the gastrocmemius(GAS) muscle for the induction of muscle pain. After 4 hours from the injection of CAR, the cold therapy with 1% syringic acid was done to GAS muscle. After 2 hours from cold therapy, the exercise therapy such as muscle stretching, climing- and declining-movements was performed three times interval of 10 minutes in each experimental group. After 4, 10 and 24 hours from CAR-induced muscle pain, the measurements of muscle diameter, paw withdrawal latency(PWL) and, tail flick latency(TFL) were carried out. Results : In this study, the thickness of GAS muscle in CAR-induced muscle pain significantly increased compared with control. While, the thickness of GAS muscle adopted cold syringic acid-therapy with exercise-therapy group was significantly decreased than that of CAR-induced muscle pain. In the measurements of PWL and TFL, cold syringic acid-therapy with exercise-therapy group was remarkably increased than CAR-induced muscle pain group in PWL and TFL. All measurements were showed significantly different between the treated-group and the treated-time. Conclusions : From these results, it is suggested that the cold syringic acid-therapy with exercise-therapy such as muscle stretching, climing- and declining-movement was effective in the prevention of CAR-induced muscle pain by the decrease of muscle thickness and the increase of PWL and TFL.

Are Medical Personnel Safe from Radiation Exposure from Patient Receiving Radioiodine Ablation Therapy? (갑상선 암의 방사성요오드 치료 시 의료진은 방사선 피폭으로부터 안전한가?)

  • Radioiodine ablation therapy has been considered to be a standard treatment for patient with differentiated thyroid cancer after total thyroidectomy. Patients may need to be hospitalized to reduce radiation exposure of other people and relatives from radioactive patients receiving radioiodine therapy. Medical staffs, nursing staffs and technologists sometimes hesitate to contact patients in radioiodine therapy ward. The purpose of this paper is to introduce radiation dosimetry, estimate radiation dose from patients and emphasize the safety of radiation exposure from patients treated with high dose radioiodine in therapy ward. The major component of radiation dose from patient is external exposure. However external radiation dose from these patients treated with typical therapeutic dose of 4 to 8 GBq have a very low risk of cancer induction compared with other various risks occurring in daily life. The typical annual radiation dose without shielding received by patient is estimated to be 5 to 10 mSv, which is comparable with 100 to 200 times effective dose received by chest PA examination. Therefore, when we should keep in mind the general principle of radiation protection, the risks of radiation exposure from patients are low and the medical personnel are considered to be safe from radiation exposure.

Differentiation of Mesenchymal Stem Cell-like Cell from Feeder Free Cultured Human Embryonic Stem Cells using Direct Induction System (Feeder-free에서 배양된 인간배아줄기세포의 직접분화유도 방법을 이용한 간엽줄기세포로의 분화)

  • Mesenchymal stem cells (MSCs) have the multipotent capacity and this potential can be applied for obtaining valuable cell types which can use for cell therapy on various regenerative diseases. However, insufficient availability of cellular source is the major problem in cell therapy field using adult stem cell sources. Recently, human embryonic stem cells (hESCs) have been highlighted to overcome a limitation of adult cellular sources because they retain unlimited proliferation capacity and pluripotency. To use of hESCs in cell therapy, above all, animal pathogen free culture system and purification of a specific target cell population to avoid teratoma formation are required. In this study, we describe the differentiation of a mesenchymal stem cell-like cells population from feeder-free cultured hESCs(hESC-MSCs) using direct induction system. hESC-MSCs revealed characteristics similar to MSCs derived from bone marrow, and undifferentiated cell markers were extremely low in hESC-MSCs in RT-PCR, immunostaining and FACS analyses. Thus, this study proffer a basis of effective generation of specialized human mesenchymal stem cell types which can use for further clinical applications, from xenofree cultured hESCs using direct induction system.
