Background: The survival of hepatocellular carcinoma (HCC) patients is usually low due to late diagnosis. Cipto Mangunkusumo Hospital as the largest tertiary referral hospital in Indonesia, has recently improved its modalities for advanced HCC management, but there has not been any evaluation on any improvement in HCC patient survival. Materials and Methods: A retrospective analysis on 114 HCC patients in 2013-2014 were conducted and compared with the database for 77 HCC patients in 1998-1999. Clinical characteristics and treatment received were recorded and the survival of both groups was analyzed using the Kaplan-Meier method and compared using the log-rank test. Results: The percentage of HBV positive patients had increased after fifteen years from 32.5% to 67.5%. Only two patients (1.8%) in 2013-2014 were diagnosed with HCC during surveillance program. Proportions of Barcelona Clinic Liver Cancer A, B, C, and D in 2013-2014 were 1.8%, 42%, 28.1%, and 28.1%, respectively. There was an increase in the use of potentially curative treatment, such as surgical resection or combination of loco-regional therapies. The one-year survival rate increased from 24.1% in 1998-1999 to 29.4% in 2013-2014, while the median survival decreased from 146 days to 138 days, but the difference was not statistically significant (p=0.913). Conclusions: There was no improvement in the median survival of HCC patients after fifteen years because most continued to present at late stages. There is an urgent need for a nationwide implementation of a hepatitis screening program and HCC surveillance education.

Objectives: This study investigated the role of information and communication technology and women's empowerment in contraceptive discontinuation in Indonesia. Methods: The study used data from the 2017 Indonesia Demographic and Health Survey and monthly contraceptive calendar data. A Gompertz proportional hazards model was used for analysis. Results: The 12-month contraceptive discontinuation rate was higher among women who had used the Internet in the past year, women who were mobile phone owners, and women who reported having fully participated in household decision-making than among their counterparts. These factors significantly impacted the risk of contraceptive discontinuation in Indonesia, even after controlling for contraceptive method, age, parity, contraceptive intent, education, work status, place of residence, and wealth status. Conclusions: After adjustment for the control variables, a higher risk of contraceptive discontinuation was associated with having used the Internet in the past year, owning a mobile phone, and not participating in household decision-making. Higher contraceptive discontinuation risk was also associated with using contraceptive pills, older age, lower parity, intent of spacing births, more education, current unemployment, and rural residence, and the risk was also significantly higher for those in the lowest household wealth quintile than for those in the fourth household wealth quintile. The association of contraceptive discontinuation with the use of modern information and communication technology and relatively disempowerment in household decision-making may imply that information regarding family planning and contraception should be conveyed via social media as part of setting up an eHealth system. This must include a strong communication strategy to empower and educate women in contraceptive decision-making.
Indonesia has undergone the rapid deforestation largely as a result of practical consequences of human overexploitation of the forest. Between 1950 and 2015, around 43% of the forest area in Indonesia had been lost (68.0 million hectares). The process of deforestation has partly been a response to the rapidly intensifying 'global' and 'domestic' economic demands. Deforestation in Indonesia is also indirectly due to 'materialism-driven' value system and the corresponding weakening of Indonesian ethics. Therefore, given that socio-cultural expressions of modern Indonesian value systems have mostly taken place within a framework of Islam, the aim of the paper is to attempt to find Islamic ethics in general, which can provide the basis of ecological ethics to prevent rapid deforestation in Indonesia. The paper is composed of the followings. First, following the 'Introduction', it outlines the historical process of deforestation in Indonesia and also its corresponding socio-economic contexts. Then it moves on to talk about ecological ethics in general, thereby emphasizing that the phenomenological problem of deforestation needs to be conceived at a philosophical level beyond ecological phenomena. After discussing the ecological ethics, the paper proceeds to examine Islamic ethics as a canonical framework of ecological ethics in Indonesia. In doing so, it attempts to apply the Islamic ethics to the diverse Indonesian society and then considers 'Pancasila' as a potential framework for a pragmatic link between Islam ethics and Indonesian society. Having said that, in conclusion, the paper argues that there is a need for 'concrete' translation of 'Pancasila' into implementation in an Indonesian context, thereby various agents (government, policy-practitioners, concessionaires and also all the Indonesian) may agree in saying 'no' to overexploitation of the forest, to rapid depletion of the forest and to 'unsustainable' development practices.
BACKGROUND/OBJECTIVES: Malnutrition during hospitalization is linked to increased morbidity and mortality, but there are insufficient studies observing clinical factors contributing to weight loss during hospitalization in Indonesia. This study was therefore undertaken to determine the rate of weight loss during hospitalization and the contributing factors. SUBJECTS/METHODS: This was a prospective study involving hospitalized adult patients aged 18-59 yrs, conducted between July and September 2019. Body weight measurement was taken at the time of admission and on the last day of hospitalization. The factors studied were malnutrition at admission (body mass index < 18.5 kg/m2), immobilization, depression (Beck Depression Inventory-II Indonesia), polypharmacy, inflammatory status (neutrophil-lymphocytes ratio; NLR), comorbidity status (Charlson Comorbidity Index; CCI), and length of stay. RESULTS: Totally, 55 patients were included in the final analysis, with a median age of 39 (18-59 yrs) yrs. Of these, 27% had malnutrition at admission, 31% had a CCI score > 2, and 26% had an NLR value of ≥ 9. In all, 62% presented with gastrointestinal symptoms, and depression was documented in one-third of the subjects at admission. Overall, we recorded a mean weight loss of 0.41 kg (P = 0.038) during hospitalization, with significant weight loss observed among patients hospitalized for 7 days or more (P = 0.009). The bivariate analysis revealed that inflammatory status (P = 0.016) was associated with in-hospital weight loss, while the multivariate analysis determined that the contributing factors were length of stay (P < 0.001) and depression (P = 0.019). CONCLUSIONS: We found that inflammatory status of the patient might influence the incidence of weight loss during hospitalization, while depression and length of stay were independent predictors of weight loss during hospitalization.
Background: Online teaching and learning extend the duration of using gadgets such as mobile phones and tablets. A prolonged usage of these gadgets in a static position can lead to musculoskeletal disorders (MSD). Therefore, this study aims to identify the risk factors related to musculoskeletal symptoms while using gadgets during work from home due to the COVID-19 pandemic. Method: A cross-sectional survey with online-based questionnaires was collected from the University of Indonesia, consisting of lecturers, students, and managerial staff. The minimum number of respondents was 1,080 and was defined by stratified random sampling. Furthermore, the dependent variable was musculoskeletal symptoms, while the independent were age, gender, job position, duration, activity when using gadgets, and how to hold them. Result: Most of the respondents had mobile phones but only 16% had tablets. Furthermore, about 56.7% have used a mobile phone for more than 10 years, while about 89.7% have used a tablet for less than 10 years. A multivariate analysis found factors that were significantly associated with MSD symptoms while using a mobile phone, such as age, gender, web browsing activity, work, or college activities. These activities include doing assignments and holding the phone with two hands with two thumbs actively operating. The factors that were significantly associated with MSD symptoms when using tablets were gender, academic position, social media activity, and placing the tablet on a table with two actively working index fingers. Conclusion: Therefore, from the results of this study it is necessary to have WFH and e-learning policies to reduce MSD symptoms and enhance productivity at work.
Increasing demand in developing countries for animal sources of food including red meat is predicted to double by 2050. In Indonesia, there has always been a gap between supply and demand of beef with national beef production only satisfying about 45% of demand. This paper aims to describe the current features and prospects for beef production systems in Indonesia. The first part of the article reviews and analyses Indonesian beef cattle production systems. The second part addresses issues related to the current systems for beef production that could become important for future development of the beef industry in Indonesia. Recommendations to improve breeding and reproduction, to empower smallholder farmers, to improve the capacity of industry-related institutions to enhance technology transfer, and to develop systems for industry development such as integration of palm oil or plantations with beef cattle production are briefly discussed.
As Indonesia is rich in natural resources, nuclear power remains a low priority among energy alternatives. However, Indonesia needs to introduce nuclear power to improve the atmospheric environment and to support sustainable economic growth. This study conducted a two-stage survey of logit-probit and analytic hierarchy process to analyze the perception of Indonesian energy policymakers regarding the introduction of nuclear power, the potential for change, and key decision factors. The analysis confirms that the perception of nuclear power is positive and that the willingness to expand nuclear power can improve if negative conditions, such as underdeveloped technology level, foreign aid and assistance, and safety issues are addressed. In addition, it is confirmed that the policy makers consider political/social and environmental factors to be more important for decision-making. The results of this study can give inplications and be used as a key reference for Indonesia's nuclear power policy
Around the world, there are increasing efforts underway to decarbonize the electricity generation system to mitigate the environmental impacts including climate change. While Indonesia has a huge potential for new and renewable energy, particularly solar photovoltaic, Indonesia has been largely dependent on fossil fuels. As of 2017, the installed capacity for solar photovoltaic in Indonesia was 78.5MW and this was only 0.04% of the theoretical solar potential, which is around 207.9GW($4.8kWh/m^2/day$). With the case of solar photovoltaic, this paper examined the reasons of low adoption of the technology and the challenges of energy transition in Indonesia from the policy and institutional perspectives.
The Dutch arrived in the islands that now compose Indonesia in the 17th century, settling first in Banten (1600) and Ambon (1605) before establishing Batavia (1619) and taking over Malacca (1641) from the Portuguese. Referring to this network of strategic bases throughout the Nusantara region, this paper uses the element of urban cultural heritage to examine these four major trade hubs experienced very different destinies as a result of Dutch decisions. It also explores how shifts in political power after Indonesia's independence influenced Jakarta's dominance in modern-day Indonesia. The paper suggests that Dutch opportunism and strategic visions underpinned the decision to make Jakarta the center of colonial power in the 17th century century. It also concludes that Jakarta's continued importance in political control and policies explains why it retained its position after Indonesia's independence, and these have been supported by recent policy measures. Finally, this paper concludes that, despite its Dutch origins, urban cultural heritage has been embraced by Indonesians.
In recent decades, Indonesia and the Middle Eastern countries have increased and expanded their political, security, economic, and socio-cultural ties. Qatar is one of the Middle Eastern countries which have fortified ties with Indonesia. This article aims to analyze the nature and scope of Indonesia-Qatar relations through the five-dimensional framework developed by George Eberling, which consists of political-diplomatic, economic-trade, military-security, cultural, and petroleum-energy relations (Eberling 2017). Despite being significantly distinct in history, demography, geographic size, and location, Jakarta and Doha have been provided opportunities to expand their cooperation on multifaceted dimensions by way of common economic, geopolitical, and cultural interests. Throughout the paper, the primary drivers of the cooperation are also discussed, alongside its future prospects.
