개인회생 채무자의 경제적.심리적 특성에 관한 연구 (A Study on the Demographic, Economic and Psychological Characteristics of the Debtors in Individual Rehabilitation Procedures)

  • There are few studies on the characteristics of debtors in individual rehabilitation procedures, though research in this field is needed. The scarcity of research means that public policy and perception are being made with inadequate information and, to a certain extent, based on personal prejudices and misconceptions. The purpose of this study was to find out demographic, economic and psychological characteristics of debtors in individual rehabilitation procedures. For this study, an in-depth interview was used. The major findings were as follows : The focus of this study is debtors in individual rehabilitation procedures. They were all under 40 and received relatively high educations. Interestingly, they showed a high tendency to use debt and an inability to manage their money. They had short time horizons and imperfect self-control in consumption and borrowing decisions. Monthly income, expenditures, asset and debt of the debtors, and monthly payment to creditors were reported. There were a lot of reasons for their bankruptcy : business failure, job loss, the cost of raising children, and expenditures for entertainment causing them to file for individual rehabilitation procedures. One of the major conclusions of this study was the discovery that their psychological characteristics were nearly the same. The implications of the study suggest that financial education and counseling must consider psychological characteristics.

  • Problem : The several studies were reported to the effects of individual tooth tray with the parallel tooth, but not reported to the effect of individual tooth tray with severe undercut in impression making. Purpose : The purpose of this study is to examine the effect of individual tooth tray; 1) the accuracy of impression in terms of distance. 2) reproducibility of pocket. Material and methods : In this study, a metal model with 3 teeth including tooth prepared 30-degree undercut and gingival crevice of the depth 5mm, width 0.4mm was cast. Impressions of the metal model were taken in 5 times used 4 kinds of impression materials with individual tooth tray and another impression taking were made with conventional method without individual tooth, used same materials, procedure. The stone models were made. Distances between abutments were measured with a Micromeasurescoup-mm. Result : The following results were obtained ; 1. In the comparisonof inter-abutment distance between parallel tooth without individual tooth tray, 4elastomeric materials have not significant difference to the metal model(p>0.05). 2. In the comparisonof inter-abutment distance with undercut tooth without individual tooth tray, 4elastomeric materials have a significant difference to the metal model(p<0.05), especially polyether's dimensional change is large. 3. If individual tooth tray was used to take impression in abutment teeth with severe undercut, more stable results was achieved easily than conventional method. (p>0.05). 4. It is not true that the pressure of impression material with individual tooth tray increased to gingival sulcus. Conclusion : By the results, I knew the fact that individual tooth tray was useful impression method on the teeth with severe undercut for the dimensional stability. Also, I could not confirm the fact that individual tooth tray would increase the penetrating pressure into cervice.

  • Excising part or all part of a larynx as a cancer operation results in changes that transgress anatomic, physiologic, psychologic, and social priciples. The quality of life of a patient's life after any given cancer surgery usually is regarded as a second-priority consideration after oncologic safety. With laryngeal surgery, excision of malignant disease typically results in change that significantly influence an individual for the duration of his or her life. Nonetheless, with appropriate rehabilitation the surgical side effects can be minimized to allow for an excellent quality of life. Successful conservation surgery for laryngeal cancer requires careful interdependent selection for patients, lesions and procedure. The technical goal is to minimize trauma to uninvolved tissue and to wisely utilized local tissues or tree flap for reconstruction, while insuring for oncologically sound procedure. Rehabilitation should aim to produce a glottal sound source if possible, however voice therapy to promote false vocal fold vibration and arytenoid to epiglottis source of vibration can produce very satisfactory phonatory results.

Equating Two Health-related Quality of Life Instruments Applied to Cancer Survivors Who Underwent Palliative Care

  • Background: Equating is a statistical procedure used to create a common measurement scale across two instruments. Item-level information should be taken into consideration so that scores can communicate interchangeably across the instruments. Objects: To investigate a common measurement scale across two health-related quality of life questionnaires (HRQOL) applied to various cancer survivors who underwent palliative care in healthcare institutions. Methods: A total of 139 cancer survivors who underwent palliative care were recruited from two rehabilitation hospitals and an oriental medicine hospital. Participants consisted of various cancer survivors who presented to the sites for palliative care. They were asked to fill out Korean versions of the World Health Organization Quality of Life (WHOQOL-BREF) and EuroQOL-5 dimension (EQ-5D) questionnaires following the palliative care. For the item level comparison, the Rasch rating scale model was used to investigate how participants regarded individual test items of two instruments in relation to item difficulty calibrations. Results: All items except the three items fit the Rasch model. One item (anxiety/depression) of the EQ-5D and two items (dependence on medical aids and negative feelings) of the WHOQOL-BREF are misfit. The WHOQOL-BREF targets the survivors well, while the EQ-5D is able to target the survivors with lower HRQOL levels with some ceiling effects. By inspecting the item difficulty calibrations of the two instruments, five items of the WHOQOL-BREF are selected as common items in relation to the EQ-5D. These five items are considered compatible with each other. Differential item functioning (DIF) analysis reveals that the healthcare item of the WHOQOL-BREF vs the self-care item of the EQ-5D exhibits significant DIF. Conclusion: Findings suggest that one paired item should be taken into consideration when equating the WHOQOL-BREF and the EQ-5D applied to cancer survivors who underwent palliative care.

  • 최근 비만이 전 세계적인 문제로 대두되면서 임상 연구의 해석에 필요한 중요 자료를 제시해 줄 수 있는 동물 모델을 이용한 생리학적 기전 연구에 대한 관심이 높아지고 있다. 인삼은 많은 동물 실험에서 항비만 또는 항당뇨 효과가 보고되었으나 인체의 임상에서 비만을 연구한 논문은 거의 없는 실정이다. 이 연구에서는 신체계측치수와 대사지표를 활용한 동물 모델에서의 인삼의 항비만 효과의 근거수준을 평가하고자 한다. 전임상 단계에서 비만에 대한 인삼의 효과를 연구한 대조군 연구, 비교 연구를 포함시키고자 한다. 실험적으로 비만을 유도하는 도중 혹은 이후에 인삼을 투여하고, 일차평가변수는 신체계측치수, 이차평가변수는 지방조직의 무게, 섭취음식의 총량, 대사지표 등을 포함한다. 언어, 출판일 등 특별한 제한 없이 MEDLINE, Embase, PubMed, Web of Science, Scopus를 통해 논문 검색을 시행한다. 본 연구에서의 자료 수집은 개인 정보를 포함하지 않으며, 사생활 침해의 우려가 없으므로 윤리적 승인 대상에서 제외된다. 연구의 전체과정을 수행한 후 연구결과는 연관 저널에 출간하거나 관련 학회에 발표할 예정이다. 본 연구 프로토콜은 the Collaborative Approach to Meta-Analysis and Review of Animal Data from Experimental Studies (CAMARADES) website (http://www.camarades.info)에 등록되었다.

Fiber post의 Relining 방법과 시멘트 유형에 따른 Push-out Bond Strength의 비교 (Comparison of Push-out Bond Strengths According to Relining Procedure and Cement Type on Fiber Post)

  • 근관치료된 치아의 수복에서 fiber post 크기와 포스트 공간의 직경이 일치하지 않은 경우, 포스트 공간은 두꺼운 레진 시멘트로 채워지게 된다. 레진 시멘트가 두꺼워지면 시멘트 내에 기포가 형성되어 포스트 분리가 발생되기 쉽다. 이런 문제점을 해결하기 위한 방법은 composite resin을 이용하여 fiber post를 relining하는 것이다. 이렇게 형성된 해부학적 포스트는 포스트 공간에 잘 적합하고 시멘트의 두께를 감소시킨다. 이 연구의 목적은 relining 과정과 임상 과정의 간소화를 위해 사용되고 있는 luting agents 종류에 따른 fiber post의 push-out 결합 강도를 평가하는 것이다. 42개의 발치된 치아는 6개의 그룹(n=7)으로 나누어졌다. (A1: relined fiber post를 Luxacore/all-bond 2로 합착, A2: non-relined fiber post를 Luxacore/all-bond 2로 합착. B1: relined fiber post를 Calibra/XP-bond로 합착. B2: non-relined fiber post를 Calibra/XP-bond로 합착. C1: relined fiber post를 RelyX Unicem으로 합착. C2: non-relined fiber post를 RelyX Unicem으로 합착.) Push-out 결합 강도는 relining procedure와 cement type의 상호작용에 의해 영향을 받는 것으로 나타났다. relined fiber post 그룹이 non-relined fiber post 그룹보다 더 높은 결합 강도를 가지며 fiber post relining 과정은 결합강도에 유의한 영향을 가진다.(p<0.05) Luting agent에 따라 모든 그룹에서 RelyX Unicem의 결합강도가 Luxacore/All-bond 2와 Calibra/XP bond에 의한 것보다 유의하게 더 높았다(p<0.05).

자연치 교합조정에 의한 전치, 구치 개교합의 보철적 수복 - 수직고경의 의도적 감소증례 (Occlusal Adjustment and Prosthodontic Reconstruction on the Open-bite Patient. - Intentional Decrease of Occlusal Vertical Dimension -)

  • A well-planned, precise occlusal adjustment of natural teeth has some distinct advantages over other forms of occlusal therapy. It should be emphasized, however, that an occlusal adjustment is an irreversible procedure and has definite contraindications in some mouths. Generally, the treatment methods for the patients that has open-bite will be following as below. : (1) Use of removable orthopedic repositioning appliance, (2) Orthodontics, (3) Full or partial reconstruction of the dentition, (4) Orthognathic surgical procedure, (5) Occlusal adjustment of the existing natural teeth, (6) Any combination of the above. Above all, the advantages of occlusal adjustment of natural teeth are : (1) the patient is more able to adapt to the changes in jaw position and posture; (2) the phonetic or speaking ability of the patient is not significantly changed and usually is improved; (3) the esthetics of the natural teeth is not altered and often is better; (4) the hygiene of the individual teeth is easily maintained; and (5) the functional usage of the teeth as cutting and chewing devices is markedly improved. The objective of an occlusal adjustment, as with any form of occlusal therapy, is to correct or remove the occlusal interferences, or premature contacts, on the occluding parts of the teeth which prevent a centric relation closure of the mandible. A systematic, disciplined approach can be followed in treatment, the objectives should be listed. They are : (1) Centric relation occlusion of the posterior teeth. (2) Proper "coupling" of the anterior teeth. (3) An acceptable disclusive angle of the anterior teeth in harmony with the condylar movement patterns. (4) Stability of the corrected occlusion. (5) Resolution of the related symptoms. For the patient with open-bite on anterior and posterior teeth, this case report shows the treatment methods in combination the fixed prosthesis with the selective cutting of the natural teeth. Occlusal adjustment is no longer an elective procedure but a mandatory one for patients requiring restorations and those in treatment for TMD dysfunctions or those whose dentitions show signs of occlusal trauma. Occlusal adjustment is essential for all who do not display the above lists.

교근에서 보툴리눔 독소 주사점의 표지화 (Surface Mapping of Masseter for Botulinum Toxin Injection)

  • Generally, many Asian women tend to dislike the square jaw, as they believe it makes the face look wider, giving a stubborn and strong impression. Contouring of the mandible is therefore a relatively common aesthetic procedure among Asians. These days, the use of botulinum toxin for contouring of the lower face offer simple alternative to surgery. Motor point, which is the site over a muscle where its contraction may be elicited by a minimal intensity short duration electrical stimulus, is the optimal injection point of botulinum toxin. Study was undertaken to identify the location of motor point of the masseter muscle and the skin surface landmark. First, the thickest point of the masseter muscle was inspected through palpation and inspection by 3 different individual plastic surgeons and then compound muscle action potentials(CMAPs) of masseter muscle in 15 health volunteers were recorded using EMG. For the localization of the measured points, line between lateral canthus to the mandibular angle was used. Location of motor points were mapped to skin surface from lateral canthus in a percentage of the distance along the landmark line and in distance in millimeters. The clinical injection point was located at 71.69 percentile and 7.3 mm of the landmark line. The motor point test was located at 72.54 percentile and 7.1 mm of the landmark line. The depth of motor point was 16mm. There was no statistically significant difference between the clinical injection point and the motor point. We conclude that surface mapping of motor point of the masseter muscle would increase accessibility and accuracy in botulinum toxin injection for contouring of the lower face.

간호교육 철학정립 및 교육과정 개발을 위한 기초조사 (A Preliminary Study on Setting Philosophy and Curriculum Development in Nursing Education)

  • The purpose of this study is to guide the direction of the Korean nursing education to analysize ⑴ the philosophy and objectives ⑵ curriculum, and ⑶ educational environment. This analysis is based on the data from 50 nursing schools (14 4-year colleges and 35 3-year colleges) The survey was conducted from Dec. 1986 through Jan. 1987 by mail. 1) Educational philosophy and objectives 10 4-year colleges and 8 3-year college program have curricular philosoph. Most popular curricular philosophies are human beings, health, nursing, nursology, nursing education, nurses role in the present and in the future. 10 nursing schools mentioned that human being is the subject to interact with : environment physically, mentally and socially. 2 schools mentioned that health is the state of functioning well physically, mentally and socially. 13 schools mentioned that the nursing is the dynamic act to maintain and to promote the highest possible level of health. 4 schools mentioned that the nursology is an applied science. 4 schools mentioned that nursing education is the process to induce the behavioural changes based on the individual ability. There is different opinion about the nurses' role between 4-year college and 3-year college. In the responses from 4-year colleges they focus on the leadership in effective changes, self-regulating and self-determining responsibilities, applying the new technology, continuing education, and participation in research to further nursing knowledge. In the responses from 3-year colleges, they focus on the education in college, primary health care nursing, direct care provider and public health education. Among 50 respondents 40 schools have educational goals which can be divided into two categories. One is to establish the moral and the other is to develop the professionalism. 2) Curriculm The analsis of curriculum is only based on the data from the 4-year colleges because the most of 3-year colleges follow the curriculum guideline set by the Ministry of Education. a) Comparison of the credits in cultural subject and in nursing major. The average required credit for graduation is 154.6 and the median credit is the range of 140-149. The average credit of cultural subjects is 43.4. In detail, the average number of credit of required course and elective courses are 24.1 and 19.3 respectively. The average credit for major subject is 111.2. In detail, the average credit for required courses and electives course are 100.9 and 10.4 respectively. In 5 colleges, students are offered even on elective course b) Comparison of the credit by class. The average earned credits are as follows : 41.1 in freshman, 400 in sophormore 38.3 in junior and 32.4 in senior. Cultural subjects are studied in early phases. c) Comparison of the compulsory and elective cultural subject by institute. The range of credit is 7-43 in compulsory cultural subjects and there are lot of differences among institutions. While all respondents require liberal arts as compulsary subjects, few respondents lists social science, natural science and behavioral science as required subjects. Social science-related subjects are frequently chosen as cultural subjects d) Distribution of creditsin cultural subjects by institute. The liberal art subjects are taught in 20 institute. English and physical education courses are taught in all instituions. The social science subjects are taught in 15 colleges and the basic Psycology and the Basic sociology are the most popular subjects. The natural science subjects are taught in 7 colleges and Biology and Chemistry are the most popular subjects among them. e) Distribution of credits in major basic courses by institute. Most of the institutes select Anatomy, Microbiology, Physiology, biochemistry and Pathology as basic major courses. f) Comparison of the required and elective courses for nursing major by institutions. Subjects and credit ranges in major are varing by institute. More than half of the respondents select the following subjects as required major subjects. (1) Adults Health Nursing and Practice (19.5 credits) (2) Mother and Child Care and Practice (8.9 credits) (3) Community Health Care and Practice (8.5 credits) (4) Psychiatric Nursing Care and Practice (8.1 credits) (5) Nursing Management and Practice (3.9 credits) (6) Fundamental of Nursing, Nursing Research and Health Assessment and Practice. Three institutions select Introduction to nursing, Rehabilitation Nursing, School Nursing, Public Health Nursing, Nursing English, Communication, Human Development as electives in nursing major. 3) Educational environment a) Nursing institution There are forty-three 3-year colleges and seventeen 4-year colleges and 81.4% of which are private b) Number of students and faculty 19.2% of the students are in 4-year colleges and 80.8% of the students are in 3-year colleges. In 4-year colleges, the number of nursing faculty members is in the other of assistant professor, instructor and professor. In 3-year colleges, the orderiis lecturer, associate professor, full time instructor and assistant professor. In 4-year colleges, 18.8 students are allocated per nursing faculty and in 3-year colleges, 33.1 students are allocated per nursing faculty. c) Clinical practices 66.7% of the 4-year colleges practice over 1201 hours in clinic and 28.5% of 3-year colleges practice over 1201 hours in clinic. In 4-year colleges, 11.5 students are allocated per nursing faculty and in 3-year colleges,17 students are allocated per nursing faculty The survey shows no difference in the procedure between 4-year colleges and 3-year colleges but 3-year colleges choose the more variety practicing site such as special hospital and community health clinic. d) Audiovisual facilities The survey shows a lot of difference in audiovisual facilities among institution and 3-year colleges are less equipped than 4-year colleges.
