Study on Factors Affecting Nurses' Experience of Non-Reporting Incidents (병원 간호사의 사건보고 불이행 경험여부에 영향을 미치는 요인)

  • Purpose: This study was designed to identify the factors affecting the nurses' experience of non-reporting adverse incidents in hospital. Methods: This study is a cross-sectional, descriptive survey design and nonrandom, convenience sampling. Study subjects were 392 clinical nurses, who have agreed to be the subject of this research. The measuring instrument of attitudes toward incident reporting was developed by the authors. The questionnaire which consisted of 17 items about worry about appraisal, the belief in improvement, the intention of reporting, and knowledge was measured by 5-point Likert-type scale. The estimate of internal consistency was alpha =.84. Analysis of data was done with use of mean, t-test, ANOVA, logistic regression with SPSS program. Results: Clinical nurses had experience of reporting incident (51.3%), non-reporting incident (76.5%). Statistically, significant differences were found between experiences group and non experience group in intention on reporting, belief on improvement, and worry about appraisal. Logistic regression analysis showed that the significant predictors were caused by report no-fault cases, belief on improvement, worry about appraisal. Conclusion: The result also indicated that, to improve the incident reporting and risk management, it might be necessary to give a belief that it results on improvement and remove concern about punishment through construction of no-blame system.

A Study on the Revision of the Notification Form and Procedures of Marine Incident (준해양사고 통보서식 및 절차 개정에 관한 연구)

  • Accident prevention is more important than follow-up, which is based on Heinrich's law. The marine incident system is a very meaningful system that can prevent similar accidents, and was introduced in 2010 in Korea in accordance with the enforcement of the Code for the Investigation of Marine Casualties and Incidents (CI Code). Based on the CI Code, ship owners or ship operators are required to notify the Central Chief Inspector using the designated notification form in the event of a marine incident, but the number of voluntary notifications is still small. In this regard, this study intends to provide a direction for improvement by conducting an in-depth analysis focusing on the lack of notification procedures and forms of the marine incident system. To this end, we analyzed related regulations, cases of excellent overseas shipping countries such as the United Kingdom and Singapore, cases of similar domestic transportation systems such as aviation and railways, and marine incident notification procedures and forms of leading shipping companies. Major improvements in the notification process include the transition of the marine incidents to voluntary reporting, the expansion of the reporting subjects, and the identification of the security of the informer's identity. The main contents of the notification form revision include the use of the term "reporting" instead of "notification," the content of the identity guarantee in the notification form, and the increase in statistical value through the expansion of optional entries.

Case Analysis of the Aviation Accident·Incident During the Last 10 Years Using the ECCAIRS 5 Data (ECCAIRS 5 Data를 활용한 최근 10년간 항공 사고·준사고 사례분석)

  • Recently, in an effort to decrease aviation accident worldwide, there has not been any significant decline of the aviation accident rate. Therefore, in the international society, it is more focused on removing risk factors based on data collected and analyzed, in order to improve the aviation safety. This paper introduces on the characteristic of ECCAIRS 5 which become the standard for the worldwide aviation safety reporting and data exchange system. Also, using domestic aviation accident data collected during last 10 years(from 2003 to 2012), we analyze the distribution of the aviation accidents/incidents, annual and monthly aviation accident rate, flight phase, and occurrence category according aircraft type(Fixed/Rotary wing). The analyzation regarding the tendency of aviation accident/serious incident will give the direction to approach the quantitative safety management.

  • As various services are linked to IoT(Internet of Things) and portable communication terminals, cyber attacks that exploit security vulnerabilities of the devices are rapidly increasing. In particular, cyber attacks targeting heterogeneous devices in large-scale network environments through advanced persistent threat (APT) attacks are on the rise. Therefore, in order to improve the effectiveness of the response system in the event of a breach, it is necessary to apply a data enrichment mechanism for the collected artifact data to improve threat analysis and detection performance. Therefore, in this study, by analyzing the data supplementation common elements performed in the existing incident management framework for the artifacts collected for the analysis of intrusion accidents, characteristic elements applicable to the actual system were derived, and based on this, an improved accident analysis framework The prototype structure was presented and the suitability of the derived data supplementary extension elements was verified. Through this, it is expected to improve the detection performance when analyzing cyber incidents targeting artifacts collected from heterogeneous devices.

  • DCS (Distributed Control System), the main control system of power plants, is an automated system for enhancing operational efficiency by monitoring, tuning and real-time operation. DCS is becoming more intelligent and open systems as Information technology are evolving. In addition, there are a large amount of investment to enable proactive facility management, maintenance and risk management through the predictive diagnostics. However, new upcoming weaponized malware, such as Stuxnet designed for disrupting industrial control system(ICS), become new threat to the main control system of the power plant. Even though these systems are not connected with any other outside network. The main control systems used in the power plant usually have been used for more than 10 years. Also, this system requires the extremely high availability (rapid recovery and low failure frequency). Therefore, installing updates including security patches is not easy. Even more, in some cases, installing security updates can break the warranty by the vendor's policy. If DCS is exposed a potential vulnerability, serious concerns are to be expected. In this paper, we conduct the penetration test by using NESSUS, a general-purpose vulnerability scanner under the simulated environment configured with the Ovation version 1.5. From this result, we suggest a log analysis method to detect the security infringement and react the incident effectively.

  • Risk assessment of a railway system should be periodically conducted managing a large amount of accumulating accident/incident data and scenarios, which generally requires enormous time and efforts. Therefore, special information management system is essential for railway risk assessment, where data needed for decisions on managing the railway safety could be promptly supported. In this study, we develop the framework of a railway risk assessment information management system (RAIMS). The RAIMS is composed of two main modules: 1) hazard data processing module; 2) and risk assessment module. Hazard data could be turned into risk information using these two modules. The RAIMS will be useful in finding hazard conditions, quantitatively assessing the risk, and providing pertinent risk measures, eventually serving to prevent railway accidents and reduce severities of railway accidents.

  • In this research, the intensity of solar energy, which was injected to the different angle plane every hour day by day, was technically documented and quantitatively analyzed through actual observations. In order to group every days into days with similar intensity, graph was drawn with respect to time for every dary and each area value under the curve was calculated. Then, the search for grouped days having similar intensity curve patterns was carried out. In order to maximize the efficiency of solar energy systems, the optimum incident angle of absorber plate was derived.

Knowledge in total quality management (TQM), especially statistical quality control (SQC) developed for industry, seems to be applicable to medical care.   • With the development in field of industry and medicine, new machines and techniques are being launched. Moreover, the complexity of the techniques is associated to an increasing risk of incident. Especially, a small error in radiotherapy can lead to a serious patient-related incident, risk management is necessary in radiotherapy in order to reduce the risk of incident. However, in field of radiotherapy, there are no legally binding clauses for risk management and there is an absence of risk management systems at an institutional level. Therefore, we analyzed institutional status of risk management, reporting & classification systems, and risk assessment & analysis in 31 countries. For risk management and reporting systems, 65% of countries investigated had legislation or regulations; however, only 35% of countries used classification systems. It was found that 43% more countries had legislation for risk management in healthcare than those for radiotherapy; 19% more countries had reporting systems for healthcare than those for radiotherapy. For classification systems, 60% more countries had legislation, recommendation, and guidelines in the field of radiotherapy than those for healthcare. Recently, international institutes have published several reports for risk management and patient safety in radiotherapy, owing to which, countries adopting risk management for radiotherapy will gradually increase. Before adopting risk management in Korea, we should precisely understand the procedures and functions of risk management, in order to increase efficiency of risk management because classification & reporting system and risk assessment & analysis are connected organically, and institutional management is needed for high quality of risk management in Korea.

  • Continued efforts to build up Safety Management System(SMS) and to improve its efficiency in airports, to which customers have direct access, are taken for granted due to continuous development of the aviation industry in both quantity and quality and rapid growth of air transportation market. This thesis proposed efficient operation methods of SMS for domestic airport organizations including Incheon International Airport(IIA), the largest airport in South Korea, aiming at strengthening aviation safety from the perspective of airport operators who play a pivotal role in service provider SMS. Those are consolidation of the existing safety management organizations and various improvements to promote voluntary incident reporting system. To draw a proposal for the improvements, conducted a research on domestic safety management status, carried out an analysis on operating conditions and did a research on ICAO regulations, domestic legal system as well as statistics data. Relevant studies and researches were also gathered and analyzed. A search for further improvements can also help increase operational efficiency and promoting a higher-level of safety awareness among operators can establish mature safety culture at airports.
