한우 체외수정란이 이식된 수란우의 임신과 유산에 영향을 미치는 시술자 측 요인들 (Effect of Technical Aspect in Practitioner on the Pregnancy and Abortion after Transfer of In Vitro Produced Korean Native Cow Embryos)

  • 본 연구는 임상 조건하에서 수정란이식의 효율성 증대를 위하여 이식 시술자 측면의 숙련도, 난이도, 소요시간, 주입 위치, 자궁의 출혈 및 위치 조절이 체외수정란이 이식된 수란우의 임신과 유산에 미치는 효과를 검토하였다. 임신율은 숙련된 시술자$(54.2\%)$, 자궁각 2/3 지점에 이식$(46.9\%)$ 및 위치 조절군$(52.9\%)$이 유의하게 높은 경향이었으나, 이식 난이도, 소요시간 및 출혈에 따른 차이는 인정되지 않았다. 한편 시술자 측의 요인에 따른 유산율은 유사한 경향이었다.

활로 4징증의 근치수술후의 원격 방실전도 차단 (Complete A-V Block 3 Months after Total Correction of Tetralogy of Fallot)

  • There appears some conduction defects frequently after total correction of Tetralogy of Fallot. Common defect is right bundle branch block due to surgical intervention. We experienced complete A-V block which occured 3 months later after total correction of Tetralogy of Fallot in a 8 year old boy. The patient was completely free of any A-V block after the operation for 3 months, and sudden onset of A-V block with coupled premature ventricular contractions resulted him in shock state during the attack of severe bronchopneumonia for 4 days prior to the second visit. Emergency implantation of Cordis demand type temporary pacemaker was necessary to control the complete heart block with bradyarrhythmia and frequent ventricular fibrillation. Permanent cardiac pacemaker was implanted two weeks later as indicated with Cordis Stanicor lambda demand pacemaker, and the patient was discharged uneventfully on the 8th post implantation day with the heart rate of 72/min. Another 3 months after the implantation, the patient was transported to this hospital as dead on arrival after an accidental fall from a 2 meter height, and all possible cardiopulmonary resuscitation was performed for 60 minutes at the emergency room in vain. Autopsy was done to find out the cause of sudden death and the etiology of complete heart block. Microscopic focal infarctions with scar formation were noted along the course of conduction system in the interventricular septum, which might be the main cause of complete heart block during the attack of severe bronchopneumonia complicated with acute bacterial endocarditis. The tip of the pacemaker wire was slipped from the granulation scar at the apex of the right ventricular cavity, and this might be the direct cause of pacing failure and death.

  • 경피적 폐생검술은 폐 및 늑막 질환의 진단에 보편적으로 흔히 시행되며 이에 의한 합병증의 발생 빈도는 매우 낮다. 이중 매우 드물지 만 치명적 일 수 있는 합병증 중의 하나는 경피적 폐생검술 후 바늘의 경로를 통한 암세포의 흉벽으로의 전이이다. 환자는 59세 된 여자로 편평상퍼 세포 폐암으로 진단되어 우측폐 상엽절제술을 시행한 후 외래 추적 관찰 6개월째 수술 전 시행한 경피적 폐생검 부위에 종괴가 촉지되어 폐암의 흉벽 전이로 판단하고종 괴를 포함하여 주변 근육들과 함께 확대 절제하였고 이후 방사선 치료 및 항암제 치료를 병 행하였다. 본 인제대학교 의과대학 흉부외과학교실 상계 백병 원에서는 1 례의 폐암 환자에서 경피적 폐생검술 후 암세포가 흉벽으로 전이되 어 외과적으로 치험하였기에 보고하는 바이다.

  • This study evaluated the influence of smooth and brushite-coated implant surfaces in dogs. The first through fourth mandibular premolars were extracted from eight young adult dogs. Twelve weeks after extraction. implantation was performed at the extraction sites. In total, 40 implant fixtures were implanted in the dog mandibles. Twenty machined implants served as controls and twenty brushite-coated surfaces served as tests. Dogs were sacrificed 2 and 4 weeks after implantation. The hemi-mandibles were obtained and processed histologically to obtain non-decalcified sections. Longitudinal sections of each implant were made and analyzed using light microscopy. The overall implant success rate was 83.3%. Histomorphometrically. the experimental group had a better percentage of bone-implant contact than the control group (p<0.05) and there was a significant difference between the 2- and 4-week groups after implantation (p<0.05) Our results suggest that the implant surface morphology influences the increase in peri-implant osteogenesis in the early period of peri-implant healing.

백서의 DMBA 유도 악하선종양에서의 $Na^+,K^+$-ATPase에 대한 연구 (A STUDY OF $Na^+,K^+$-ATPase IN THE RAT SUBMANDIBULAR GLAND TUMOR INDUCED BY DMBA)

  • 체중 100g 내외의 Sprague Dawley계 정상백서 암, 수 각 50마리씩을 실험동물로 하여 분말형태의 순수 DMBA를 소구(pellet)로 압착제작하여 우측 악하선에 매식후 즉시, 1,3,5,7,9,11,13,15,17주째 10마리씩 희생시켜 좌, 우측악하선을 적출, 무게를 측정하고, 육안적, 조직학적, 생화학적으로 실험기간에 따라 비교분석하여 다음과 같은 결론을 얻었다. 1. 매식후 11 주째부터 우측 경부 악하선부위의 종창을 관찰할 수 있었으며, 결절덩어리를 촉진할 수 있었다. 2. 우측악하선의 무게의 크기는 실험기간의 경과에 따라 괄목할만한 증가를 보였다(매식후 17주째는 좌측에 비해 약 5.5배의 무게를 나타냈다). 3. 매식후 7주와 11주째부터 조직학적으로 상피이형성과 침윤성 유표피암의 소견을 각각 관찰할 수 있었다. 4. 우측 악하선에서의 유표피암의 발생율은 매식후 17주째 76%이었다. 5. DMBA 는 현저히 억제된 $Na^+,K^+$-ATPase 활성도를 야기하며 실험기간의 경과의 따라 활성도 비율도 현저히 저하시킨다.

Plasma source ion implantations for shallow $p^+$/n junction

  • Plasma source ion implantation is a new doping technique for the formation of shallow junction with the merits of high dose rate, low-cost and minimal wafer charging damage. In plasma source ion implantation process, the wafer is placed directly in the plasma of the appropriate dopant ions. Negative pulse bias is applied to the wafer, causing the dopant ions to be accelerated toward the wafer and implanted below the surface. In this work, inductively couples plasma was generated by anodized Al antenna that was located inside the vacuum chamber. The outside wall of Al chamber was surrounded by Nd-Fe-B permanent magnets to confine the plasma and to enhance the uniformity. Before implantation, the wafer was pre-sputtered using DC bias of 300B in Ar plasma in order to eliminate the native oxide. After cleaning, B2H6 (5%)/H2 plasma and negative pulse bias of -1kV to 5 kV were used to form shallow p+/n junction at the boron dose of 1$\times$1015 to 5$\times$1016 #/cm2. The as-implanted samples were annealed at 90$0^{\circ}C$, 95$0^{\circ}C$ and 100$0^{\circ}C$during various annealing time with rapid thermal process. After annealing, the sheet resistance and the junction depth were measured with four point probe and secondary ion mass spectroscopy, respectively. The doping uniformity was also investigated. In addition, the electrical characteristics were measured for Schottky diode with a current-voltage meter.

반도체 제조 이온주입 공정의 이온 임플란타 장치에서 엑스레이 발생 특성 (Characterization of X-ray Emitted in the Ion Implantation Process of Semiconductor Operations)

  • Objectives: The aims of this study are to investigate how X-rays are emitted to surrounding parts during the ion implantation process, to analyze these emissions in relation to the properties of the ion implanter equipment, and to estimate the resulting exposure dose. Eight ion implanters equipped with high-voltage electrical systems were selected for this study. Methods: We monitored X-ray emissions at three locations outside of the ion implanters: the accelerator equipped with a high-voltage energy generator, the impurity ion source, and the beam line. We used a Personal Portable Dose Rate and Survey Meter to monitor real-time X-ray levels. The SX-2R probe, an X-ray Features probe designed for use with the RadiagemTM meter, was also utilized to monitor lower ranges of X-ray emissions. The counts per second (CPS) measured by the meter were estimated and then converted to a radiation dose (𝜇Sv/hr) based on a validated calibration graph between CPS and μGy/hr. Results: X-rays from seven ion implanters were consistently detected in high-voltage accelerator gaps, regardless of their proximity. X-rays specifically emanated from three ion implanters situated in the ion box gap and were also found in the beam lines of two ion implanters. The intensity of these X-rays did not show a clear pattern relative to the devices' age and electric properties, and notably, it decreased as the distance from the device increased. Conclusions: In conclusion, every gap, in which three components of the ion implanter devices were divided, was found to be insufficiently shielded against X-ray emissions, even though the exposure levels were not estimated to be higher than the threshold.

시험관 아기 시술에서 여성의 연령이 수정란의 질과 다태 임신 발생에 미치는 영향 (Influence of Maternal Age on Embryo Quality and the Frequency of Multiple Pregnancy in IVF-ET Program)

  • Objective: This study was performed to evaluate the influence of maternal age on embryo quality and the frequency of multiple pregnancy in IVF-ET program. Method: 86 conventional IVF-ET cycles were divided into three groups according to the age by 5 year (group A: 26-30, group B: 31-35, group C: 36-40 yrs). The in vitro fertilization and development outcome (fertilization, cleavage and high quality embryo rate) and the pregnancy outcome (pregnancy, implantation, G-sac/high quality embryo and multiple pregnancy rate) were examined. And then, these results were compared among the groups. Results: The rates of fertilization (62.7, 68.5 and 65.4%, respectively) and cleavage (95.6, 97.6 and 98.0%, respectively) were not different among the groups. And the high quality embryo (HQE) rate also was not different among the groups (61.8, 62.9 and 62.8%, respectively). The pregnancy rate of group C (23.3%) was significantly lower than that of group A (41.2%) and B (48.7%). And the implantation rate was significantly decreased with advance in maternal age (group A; 17.3%, B; 12.6% and C; 6.0%). The G-sac/high quality embryo rate was significantly higher in group A (70.8%) when compared to group B (32.2%) and C (40.0%). On the other hand, the multiple pregnancy rate was significantly lower in group C (14.3%) when compared to group A (71.4%) and B (36.8%). Conclusion: The pregnancy rate was significantly decreased over 35 years. The G-sac/HQE and multiple pregnancy rate were significantly high below 31 years. Thus, these results suggest that the number of high quality embryo transferred should be limited by the age and another criteria for embryo quality evaluation were required for single embryo transfer.

저 반응군의 체외수정시술을 위한 과배란유도에 있어 GnRH Antagonist 요법과 GnRH Agonist Flare Up 요법의 효용성에 관한 연구 (GnRH Antagonist Versus Agonist Flare-up Protocol in Ovarian Stimulation of Poor Responder Patients)

  • 목 적: 난소 반응이 저하된 환자에서 GnRH agonist flare up protocol과 GnRH antagonist protocol의 효용성을 비교하고자 하였다. 연구방법: 2003년 1월부터 2005년 8월까지 체외수정시술을 받은 환자들 가운데 이전의 체외수정시술 주기에서 5개 이하의 난자가 채취되고 기저 FSH 농도가 12 mIU/ml 이상인 총 144명 가운데 73명은 GnRH agonist flare up요법을 사용하였고 71명은 GnRH antagonist 요법을 사용하였다. 양군간에 주기의 취소율, 채취된 난자수, 양질의 수정란의 수, 착상율, 임신율, 출산율을 비교하였다. 결 과: 각 군간에 나이는 평균 37.4세와 38.1세로 antagonist group에서 높았으나 통계학적 유의성은 없었다. 그 외에 기저 FSH 농도와 이전 주기의 취소율도 통계학적 유의성은 없었다. GnRH agonist flare up 주기와 GnRH antagonist 주기에서 취소율은 각각 30.1%, 53.5%로 GnRH antagonist protocol에서 유의 있게 높게 나타났다. 채취된 난자수도 각각 4.18개와 2.16개로 차이를 보였으며 난소 과자극 기간은 각각 10.5일과 9.2일로 antagonist protocol에서 약간 낮은 모습을 보였다. 최고 혈중 E$_2$의 농도와 good embryo 개수도 GnRH agonist flare up요법에서 유의 있게 높게 나타났다. 각 군에서의 착상율, 이식 주기당 임신율, 이식 주기당 출생율은 GnRH agonist flare up요법에서 약간 높은 경향을 보이기는 하였으나 통계학적인 유의성은 없었다. 결 론: 두 군간의 비교에서 GnRH agonist flare up 요법이 시작 주기당 임신율, 출산율에서 높은 경향을 보였으나 통계학적 의의는 없었다. 하지만 난소기능이 저하된 환자에서 난자의 채취 개수는 GnRH agonist flare up 요법이 GnRH antagonist를 사용한 주기보다 의의있게 높게 나타났다. 이러한 연구 결과로 볼 때 저 반응군에 있어서 GnRH antagonist flare up요법이 저 반응군에 있어 향상된 난소 반응을 기대할 가능성이 높을 것으로 생각된다.

Depth Profiling에서 Sputtering Rate의 영향 (The influence of sputtering rate during depth profiling)

  • 시료에 주입된 이온의 깊이방향에 따른 농도분포를 알아보기 위하여 시료표면을 sputtering 하면서 튀어나온 주입된 이온을 depth profiling한다. Depth profiling 측정 시에 깊이방향에 영향을 주는 sputtering rate가 변화하는 효과를 SRIM simulation을 이용하여 계산하였다. 시료에 이온이 주입하게 되면 시료의 원자밀도는 약간 증가하게 되는데, 그 결과로 sputtering yield가 변화하게 된다. 이러한 변화가 결과적으로 depth profile 측정시에 깊이방향에 영향을 줄 수 있는 sputtering rate를 변화시키는 원인이 된다. SRIM(Stopping and Range of Ions in Matter) Monte Carlo simulation code를 사용하여 이온주입에 의한 시료의 원자밀도의 변화에 따른 sputtering yield를 구하여 sputtering rate를 계산하고, 그 차이가 depth profiling 측정에서 깊이방향 분포에 영향을 줄 수 있다는 것을 확인하였다.
