목적: 지르코니아 프레임워크를 이용한 시멘트 유지형 임플란트-지지 단일 크라운과 연결 크라운의 성공률과 보철적 합병증을 임상적으로 평가하는 것이다. 대상 및 방법: 67명의 환자에서 75개 수복물(단일 51개, 연결 24개)을 지르코니아 프레임워크를 이용한 시멘트 유지형 임플란트-지지 단일 크라운 혹은 연결 크라운으로 수복했다. 모든 수복물은 임시 시멘트로 합착했다. 보철적 합병증과 성공률을 조사하고, 나이, 성별, 보철물의 위치, 대합치, 보철물의 유형이 보철적 합병증에 미치는 영향을 평가했다. 결과:평균 22.2개월의 관찰 결과, 최종 누적 성공률은 66.9 (73.2 - 60.6)%였다. 유지 상실이 16개 보철물(단일 14개, 연결 2개)에서 나타났고, 지대주 나사 풀림과 비니어 도재 파절은 단일 크라운에서만 각각 2개의 보철물에서 나타났다. 단일 크라운과 연결 크라운에 대한 Kaplan-Meier 생존분석 결과, 최종 누적 성공률은 각각 58.9 (66.6 - 51.2)%, 87.5 (96.1 - 78.9)%를 보여 통계적으로 유의한 차이가 있었지만, 다른 고려 요인들은 보철적 합병증에 통계적으로 유의성이 없었다. 결론: 보철적 합병증은 유지 상실이 가장 많았고, 지대주 나사 풀림 및 비니어 도재 파절은 단일 크라운에서만 비교적 적게 관찰되었다. 나이, 성별, 보철물의 위치, 대합치에 따른 보철적 합병증에 미치는 영향은 유의한 차이가 없었지만, 상부 보철물을 연결 크라운으로 제작했을 때, 단일 크라운으로 제작한 경우보다 높은 성공률을 보였다.
진단, 수술, 인상, 보철물 제작 등 임플란트 치료 전 과정에서 디지털 기술이 활용되고 있다. 본 증례에서는 디지털 수술 가이드를 이용하여 합병증 없이 상악동 거상술을 시행하고, 계획된 위치에 임플란트를 식립하였다. 골유착을 위한 치유 기간 이후, CAD-CAM(Computer-aided design/Computer-aided Manufacturing)으로 맞춤형 지대주 및 임시 보철물을 제작하여 장착하고, 환자의 적응도와 교합을 평가하였다. 임시 보철물상에서 교합 변화가 관찰되어, 광중합형 컴포지트 레진으로 수리하였다. 최종 보철물 제작 시, 지대주의 수직 침하, 임시 보철물의 형태와 적응된 교합 관계를 반영하기 위해, 이중 스캔과 지대주 수준의 디지털 인상을 채득하였다. 치은 압배 없이 치은 연하 변연을 인기하기 위해, CAD-CAM 소프트웨어상에서 라이브러리화된 지대주 데이터를 중첩하고, 지르코니아 최종보철물 제작하여 장착하였다. 구치부 임플란트 수복 시, 디지털 시스템을 이용하여 전통적인 방법에서 겪는 어려움을 줄이고, 수술부터 보철물 제작까지 효율적인 치료 과정과 안정적이고 예지성 있는 결과를 얻어 보고하는 바이다.
표준형 이오네스큐우심낭판막의 일차성 조직실패상의 특징의 일부를 알아보고져 승모판위치에서 각 각 행콕판막을 적출하였던 56례와 표준형 이오네스큐판막을 적출하였던 연속적 전례인 일차성 조직실 패환자 113례를 대상으로 임상 및 병리학적으로 분석 검토하였다. 양 환자군의 수술당시의 연령은 각각 31.9$\pm$9.2세와 30.4$\pm$ 12.5세였다. 행콕판막은 조직손상으로 인 한 판막폐쇄부전이 빈발한 반면 이오네스큐판막은 석회화변성의 빈도가 높고 협착병변인 경향이 우세 하였다. 판막적출기간은 행콕판막에서보다 이오네스큐판막에서 단축되 었다. 이러한 판막실패의 특징은 판막구조설계의 개선으로 기계적 요소로 인한 판막실패를 감소할 수 있을 것으로 보이나 항광물화상의 개선 없이는 조직판막의 내구성의 개선은 곤난할 것임을 시사하였다.

Background: The currently available reverse shoulder arthroplasty (RSA) designs can be classified into medial glenoid/medial humerus (MGMH), lateral glenoid/medial humerus (LGMH), and medial glenoid/lateral humerus (MGLH) prosthesis designs. The purpose of this study was to radiologically analyze the effect of different RSA designs on humeral position following RSA. Methods: A total of 50 patients who underwent primary RSA were retrospectively analyzed. Among 50 patients, 33 patients (group A: MGMH) underwent RSA with Aequalis system (Wright, Inc, Bloomington, MN, USA), 6 (group B: LGMH) with Aequalis system using bony increased offset, and 11 (group C: MGLH) with Aequalis Ascend Flex system. The acromiohumeral distance, acromioepiphyseal distance (AED), lateral humeral offset (LHO), LHO from the center of rotation ($LHO^{COR}$), and deltoid length were radiologically measured to quantify the distalization and lateralization of the humerus. Results: The increment in postoperative AED was $19.92{\pm}3.93mm$ in group A, $24.52{\pm}5.25mm$ in group B, and $25.97{\pm}5.29mm$ in group C, respectively (p=0.001). The increment in postoperative LHO was $0.13{\pm}6.30mm$, $8.00{\pm}12.14mm$, and $7.42{\pm}6.88mm$, respectively (p=0.005). The increment in postoperative $LHO^{COR}$ was $20.76{\pm}6.06mm$, $22.04{\pm}5.15mm$, and $28.11{\pm}4.14mm$, respectively (p=0.002). Conclusions: The radiologic analysis of the effect of different RSA designs on humeral position following RSA showed significant differences in the increment in postoperative AED, LHO, and $LHO^{COR}$ between the 3 groups. Therefore, MGLH design seems to be more effective for humeral distalization and lateralization compared to original Grammont design.
The mandibular canal must be considered carefully during surgical treatment, especially surgical extraction of the impacted tooth and intraosseous implant because it contains the important inferior alveolar nerve and vessels. The author investigated the curvatUre of the mandibular canal, the positional frequency of mandibular foramen to the occlusal plane and gonial angle and the positional frequency of the mental foramen to the tooth site using orthopantomograms. The materials consisted of 295 orthopantomograms divided into seven groups ranging from the first decade to 6th. decade. The results were as follows: 1. The position of mandibular foramen was most frequently below occlusal plane in Group Ⅰ (78.6%) and Group Ⅱ (71.2%), above occlusal plane in Group Ⅲ (63.0%), Group IV (71.1%), Group V (57.6%), Group (76.7%) and Group VII (70.0%). 2. The curvature of mandibular canal was 142.8° in Group Ⅰ, 142.09° in Group Ⅱ, 139.34° in Group Ⅲ, 141.48° in Group Ⅳ, 138.45° in Group Ⅴ, 140.77° in Group Ⅵ and 143.89° in Group Ⅶ. 3. The gonial angie was 125.82° in Group Ⅰ, 123.18° in Group Ⅱ, 124.06° in Group Ⅲ, 120.45° in Group Ⅳ, 121.12° in Group Ⅴ, 121.63° in Group Ⅵ and 121.24° in Group Ⅶ. 4. The position of the menta] foramen was most frequently below the apex of mandibular first premolar in Group Ⅰ (57.2%), between the apex of mandibular first and second premolar in Group Ⅱ (59.6%) and Group Ⅲ (48.9%), and below the apex of mandibular second premolar in Group Ⅳ (39.2%), Group Ⅴ (48.5%) Group Ⅵ(46.6%) and Group Ⅶ(56.4%)
Purpose: Numerous studies have shown that crestal bone resorption around the implant was related to the location of the implant abutment junction(IAJ). Recently it was hypothesized that platform switching termed the inward horizontal repositioning of the IAJ might limit bone resorption around the implants. The purpose of this clinical study was to evaluate the effect of platform switching on crestal bone resorption. Materials and Methods: The crestal bone loss of 65 external hex implants in 26 patients were radiographically measured at crown placement and follow-up examinations. 23 standard implants(non-platform switching group, NP) were connected with the matching abutments and 42 wide implants(platform switching group, PS) were connected with the 1 mm smaller diameter abutments. Results: There was significant difference of crestal bone loss between NP group and PS group. For implants in the NP group, mean crestal bone loss was $1.18{\pm}0.68\;mm$ at crown placement and $1.42{\pm}0.41\;mm$ at follow-up. The meal bone loss in PS group was $0.47{\pm}0.52\;mm$ at crown placement and $0.60{\pm}0.65\;mm$ at follow-up. When the crestal bone changes according to placement depths of implants were compared, subcrestal position of IAJ had a significantly less bone loss in PS group, but it was not in NP group. Conclusion: Within the limits of the present study, it was concluded that platform switching technique might decrease crestal bone loss around the implants. Additionally, when the IAJ of implant was placed 1 mm deeper in the alveolar bone, the effect of platform switching on bone loss was enhanced.
The benefits of implant supported overdenture are readily apparent for the fully edentulous patients and have been well documented, however, there is deficiency of the studies regarding the combination of implants with removable partial dentures for partially edentulous patients. The purpose of this article is to review the literature concerning implants with removable partial dentures and evaluate the evidence for this clinical approach. Through many clinical case reports and studies we have searched from a broad variety of journals, we present the six considerations needed to contemplate respecting implants with removable partial denture in partially edentulous patients. First, the connection between abutment tooth and removable partial denture has to be rigid and the link between implant and removable partial denture should be hinged. Second, a mesial rest acts better in the point of force distribution for distal extension removable partial denture and splinting between implants is also a favorable choice. Third, T bar has an advantage for implants which are used as abutments in distal extension removable partial denture. Forth, as we all known functional impression is better way to reproduce movement for distal extension removable partial denture. Fifth, indirect retainer and guiding plane on the proximal surfaces of terminal abutment teeth are important in preventing denture base lifting. Sixth, implants in conjunction with removable partial denture is superior in the esthetic and phonetic as well as cost-effective point of view. We also suggest that which place we should install implants for force distribution and which diameter and length of implants should be used. in this review article, we recommend to locate the implant near of the abutment tooth for esthetics or near of first molar position for good stress distribution. The diameter and length of implant also influence to stress distribution. When we compare to conservative partial denture, patients go for removable partial denture using implants due to convenience, better support and retention according to several studies. But it is true that we need to study more on this subject and collect long term follow up cases before we discuss on it. So it is enough to bring this subject into the surface of prosthetic treatment by this article.
치과와 기공실을 오가며 복잡하게 진행되는 기존 CAD-CAM 컴퓨터 가이드 제작 방식을 단 순화하기 위하여, 진료실에 설치한 바로가이드 시스템(VARO Guide, CAD, Pre-Guide, VARO-mill, NeoBiotech, Seoul, South Korea)을 통하여 Top-Down 개념과 수복 주도개념을 적용하여 1-day 수술과 임시보철을 완료하는 증례를 보고하고자 한다. Pre-Guide를 통해 환자의 중심위에서 교합을 채득하고 CT를 촬영하며, 진료실 내 컴퓨터에서 CAD를 통해 바로가이드를 1시간 내에 제작하였다. 미리 제작된 광중합 레진이 담긴 Pre-Guide 트레이를 술부에 위치시킨 후 중심위에서 CT를 촬영하였다. CT 데이터가 진료실 내 컴퓨터로 전송되면 수복 위치를 정한 후 이어서 3차원적으로 턱뼈에서 임플란트를 식립할 위치를 디자인하여 STL 파일로 추출하였다. 그리고 진료실에 설비된 밀링 기계(VARO-mill)에 Pre-Guide를 고정하여 수술용 가이드를 15분 내에 제작한다. 이 가이드로 환자는 한번 내원하여 임플란트를 계획한 위치에 안전하고 정확하게 수술해 줌과 더불어, 초기고정력이 양호한 임플란트에 대해서는 즉시임시치아까지 장착해 주어 환자에게 즉시 기능이 가능하도록 할 수 있었다.
최근 CAD-CAM (computer-aided design-computer-aided manufacturing) 기술 및 3D 프린팅 기술의 발전과 함께 다양한 디지털 기법들의 도입으로 top-down 방식의 최종 보철 수복의 정확성과 효율성이 증대되고 있다. 본 증례는 전후 엇갈린 교합을 가진 환자에서 총 9개의 상하악 구치부 임플란트 식립을 통해 안정적인 교합 지지를 얻으면서 잔존 치조골의 고도 흡수 경향을 보인 상악 전치부 무치악 부위는 케네디 4급 임플란트 보조 국소의치로 수복함으로써 연조직의 심미성을 회복하였다. 전산화단층촬영 가이드 수술로 계획된 위치에 임플란트를 식립하고, 이중 스캔 기법으로 임시 수복 단계에서 안정화된 교합을 최종 보철물에 반영하며, 코핑과 프레임워크를 금속 3D 프린팅으로 제작하여 효율적이며 예측 가능한 top-down 방식의 전악 구강 수복을 달성하였기에 이를 보고하고자 한다.
손상된 부분 무치악제는 임플란트 식립에 많은 어려움을 야기한다. 이런 치조제의 손상은 외상성 발치, 안면 외상, 치근단 수술, 만성 치주염으로 인한 발거, 임플란트 실패 등에 기인하며, 특히 상악 전치부의 경우는 큰 좌절을 유발할 수 있다. 치조제 손상의 분류 및 임상 처리 방법에 대한 많은 보고가 있었으나, 수술에 앞서 방사선 영상을 통해 경조직을 평가하여 심미적이고 연조직과 조화로운 임플란트 시술을 위한 임플란트의 이상적인 수복 위치에 따른 분류법은 소개된 적이 없었다. 본 논문의 목적은 컴퓨터 단층촬영 분석을 이용하여 이상적 임플란트 수복위치에 따른 상악 전치부 치조제의 손상을 분류하고자 하는 것이다. 본 논문에서는 뉴욕대학교 치주${\cdot}$임플란트과의 데이터를 이용했으며, 다음 경우에 한하여 자료를 수집한 후, Simplant 8.0과 3.25(10mm의 가상 임플란트를 이용하여 55개의 증례를 통한 144개의 임플란트 수복 부위를 평가하였다. (1) 상실된 상악 전치부 증례만 포함 (2) 최소 2개의 인접 치아가 상실된 증례 (3) 원래의 치아 각도를 알 수 있는 최소 1개의 잔존 치아 존재 (4) 치아외형 형판을 컴퓨터 단층 촬영시 착용 분석 결과, 2mm 이상의 순측골을 가지며 열개나 창이 없는 경우를 제1군 A, 2mm 이하의 순측골을 가지며 열개나 창이 없는 경우를 제1군 B, 한쪽 편의 열개를 가지고 있고 창은 없는 경우를 제2군 A, 협/설측 모두 열개를 보이며 창은 없는 경우를 제2군 B, 열개는 없고 한쪽 편의 창을 보이는 경우를 제3군 A, 열개는 없고 협/설측 모두 창을 보이는 경우를 제3군 B, 열개 및 창을 모두 보이는 경우를 제4군을 분류하였다. 144개의 임플란트 수복 부위를 분류해보면, 제4군이 30.6%로 가장 많았으며, 제2군 A가 20.8%, 제1군 A가 19.4%, 제2군 B가 12.5%, 제1군 B가 10.4%, 제3군이 6.3% 순이었다. 제4군이 가장 많은 것은 초진시 치조제 변형을 감지하여 컴퓨터 촬영을 의뢰했기 때문으로 여겨진다. 본 연구에서 사용한 임플란트보다 큰 일반적 크기의 임플란트 적용시 더 많은 숫자의 치조제 손상을 보일 것으로 예상되므로, 임상가들은 이런 손상된 치조제 증례에서도 좋은 예후를 보일 수 있도록 정확한 진단과 골 증대술과 같은 수술적 접근법에도 익숙해질 수 있도록 노력해야 할 것이다.
