It is necessary to set the correct occlusal plane and to distribute the occlusal force uniformly considering the state of the opposing dentition during the prosthetic of the single edentulous patient with severe alveolar bone resorption. Implant supported overdenture is superior to complete denture in terms of maintenance and stability, and limited implants are used in fully edentulous patients with high alveolar bone resorption. Telescopic attachments using a newly introduced material based on poly-aryl-ether-ketone (PAEK) have the advantages of typical telescopic copping, excellent abrasion resistance, and are lighter and more economical than conventional implant overdentures. In this case, we restored maxillary arch with a implant retained overdenture using the telescopic attachment made of Pekktonand the mandible was restored with fixed implant prosthesis. Through these procedures esthetic aspects and functional outcomes were satisfactorily achieved.

Purpose. The purpose of this study was to compare the type and frequency of prosthetic complications associated with attachment types for implant overdenture. Material and methods. In this retrospective study, 38 patients (mean age, 63.5 years) have been treated with implant overdentures from 2007 to 2014. Ten patients received a bar-clip attachment. Eleven patients had received a milled bar with Locator attachment. Seventeen patients had received a Locator attachment. The mean follow-up period was 36.9 months (range, 15-83 months). The type and frequency of prosthetic complications was recorded. The frequency was analyzed to determine the statistical difference among the 3 different attachments by using one-way ANOVA (${\alpha}=.05$) and Bonferroni post hoc method at a 5% level of significance. Results. The total number of prosthetic complications was higher in the bar-clip attachment (55 events) than in the milled bar with Locator attachment (39 events) and the Locator attachment (34 events). There were no statistically significant differences, and the most common prosthetic complication was the loss of retention. In the bar-clip attachment group, the average frequency of prosthetic complications was 3.0 events per prosthesis during the first year. In the milled bar with Locator attachment and Locator attachment groups, the average frequencies were 1.45 events and 2.35 events, respectively. Statistically significant differences were observed in the frequency of the complication. (p = .043) Conclusions. Compared to the bar-clip attachment, implant overdentures that use milled bars with the Locator attachment have a significantly lower incidence of prosthetic complications in the first year of follow-up after placement.

Bisphosphonate is used widely for osteoporosis treatment, but a rising concern is the risk of osteonecrosis after long-term bisphosphonate use. Such cases are increasing, suggesting a need for research to prevent and treat bisphosphonate-related osteonecrosis of jaws. A 63-year-old female took bisphosphonate (Fosamax$^{(R)}$) for four years for treatment of osteoporosis and stopped medication two months ago because of unhealed wound. She was treated with marginal mandibulectomy maintaining the inferior border, and a metal plate was placed to prevent mandible fracture. Four months after the mandibulectomy, mandible reconstruction surgery using iliac bone and allograft was done. Six months after reconstruction, implant placement and treatment with an overdenture was done without complications. This study presents a case with a successful result.

A 23-year-old female with aggressive periodontitis was treated using dental implants and LAVA system. The severely compromised teeth were extracted irrespective of initial conservative periodontal treatment. An implant-supported overdenture with 4 implants was fabricated for the maxilla and all-ceramic restorations for the mandible. Esthetic and functional goals were achieved with team approach involving periodontists and prosthodontists. This case report describes a treatment procedure for a generalized aggressive periodontitis patient with severe bone resorption.

To determine the vertical dimension and centric relation during the construction of implant overdentures, the record base and wax rim may need to be adjusted. The conventional method has several drawbacks, as it requires repeated tightening and loosening of the impression coping. Here, we report a useful and novel method for interocclusal records using the healing abutments in implant overdentures. Our case demonstrates that this method is easier and simpler and prevents gingival collapse.
