최근 디지털 인상채득 시스템이 개발되면서 이를 기존의 CAD/CAM 시스템과 접목한 인레이, 온레이, 크라운이나 간단한 계속가공의치 등의 고정성 보철물의 제작이 활발히 소개되고 있다. 그러나 임플란트 보철에서는 주로 기존의 아날로그식 인상채득 후 이를 스캔하여 제작하는 방법이 사용되어 왔으나, 구강 내 디지털 스캔한 데이터를 이용하여 나사형 보철물을 제작하는 술식은 아직 활발히 이루어지지 않았다. 이에 본 증례보고에서는 6명의 환자를 대상으로 구강 내 스캐너인 iTero를 사용하여 디지털 스캔을시행한후그중다섯명은맞춤형지대주와 시멘트 합착 후 나사형 임플란트 보철물(screw-retained implant prosthesis after cementation, or SCRP)을 제작하였고, 임상적으로 기능과 심미적인 측면으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.
상악 전치부에 임플란트를 이용해 심미적으로 수복 하는 것은 많은 어려움이 따른다. 이는 치조골 흡수와 치간 유두 수축 등과 같은 임플란트 주변 경, 연조직 문제와 금속지대주 및 금속도재관의 변연노출과 낮은 광투과도 등의 보철적 한계 때문이다. 이에 본 증례에서는 경, 연조직의 증대술 및 성형술, 발치 후 즉시 식립 등과 함께 임시수복물의 장기간 사용 및 수정으로 임플란트 주위 연조직의 형태와 건강을 형성한 후, 맞춤형 인상 코핑을 사용하여 이를 최종 모형에 재현하였다. 또한 맞춤형 지르코니아 지대주와 완전 도재관을 이용해 최종 수복함으로써 기능적이고 심미적인 결과를 얻었기에 이를 보고하고자 한다.
In the case of failed fixed implant prosthesis accompanied by abutment screw fracture, fractured screw fragment must be removed to use the existing implant fixtures. A 61-year-old male patient, who had a failed maxillary fixed implant prosthesis accompanied by three abutment screw fracture, hoped to reconstruct the maxillary implant prosthesis, while maintaining the existing implant fixtures. To use the existing implant fixtures, fractured screw fragments were removed. A maxillary implant overdenture using available existing implants was planned. Bar-attachment with Locator was used for implant splinting, denture stability, and retention. Final impression was taken after treatment of peri-implantitis. Jaw relation registration was taken to evaluate available interarch space for bar-attachment. After fabricating bar-attachment, centric relation was taken. Implant overdenture using bar-attachment with Locator was delivered after wax-denture evaluation. This case report showed that a satisfactory clinical result was achieved by implant overdenture using existing implant fixtures in a maxillary edentulous patient.
Surgeon dentists usually rely on their experiential judgments from patients' oral plaster casts and medical images to determine the positional and directional information of implant fixtures and to perform drilling tasks during dental implant surgical operations. This approach, however, may cause some errors and deteriorate the quality of dental implants. Computer-aided methods have been introduced as supportive tools to alleviate the shortcomings of the conventional approach. In this paper, we present an approach of 3D dental implant simulation which can provide the realistic and immersive experience of dental implant information. The dental implant information is primarily composed of several kinds of 3D mesh models obtained as follows. Firstly, we construct 3D mesh models of jawbones, teeth and nerve curves from the patient's dental images using software $Mimics^{TM}$. Secondly, we construct 3D mesh models of gingival regions from the patient's oral impression using a reverse engineering technique. Thirdly, we select suitable types of implant fixtures from fixture database and determine the positions and directions of the fixtures by using the 3D mesh models and the dental images with software $Simplant^{TM}$. Fourthly, from the geometric and/or directional information of the jawbones, the gingival regions, the teeth and the fixtures, we construct the 3D models of surgical guide stents which are crucial to perform the drilling operations with ease and accuracy. In the application phase, the dental implant information is combined with the tangible interface device to accomplish 3D dental implant simulation. The user can see and touch the 3D models related with dental implant surgery. Furthermore, the user can experience drilling paths to make holes where fixtures are implanted. A preliminary user study shows that the presented approach can be used to provide dental students with good educational contents. With future work, we expect that it can be utilized for clinical studies of dental implant surgery.

PURPOSE. Early loading of implant can be determined by excellent primary stability and characteristic of implant surface. The implant system with recently improved surface can have load application 4-6 weeks after installing in maxilla and mandible. This study evaluated the effect of healing period to the stability of hydrophilic tapered-type implant at maxillary posterior area. MATERIALS AND METHODS. This study included 30 patients treated by hydrophilic tapered-type implants (total 41 implants at maxilla) and classified by two groups depending on healing period. Group 1 (11 patients, 15 implants) was a control group and the healing period was 12 weeks, and Group 2 (19 patients, 26 implants) was test group and the healing period was 6 weeks. Immediately after implant placement, at the first impression taking, implant stability was measured using Osstell Mentor. The patients also took periapical radiographs after restoration delivery, 12 months after restoration and final followup period. The marginal bone loss around the implants was measured using the periapical radiographs. RESULTS. All implants were survived and success rate was 97.56%. The marginal bone loss was less than 1mm after 1 year postoperatively except the one implant. The stabilities of the implants were not correlated with age, healing period until loading, insertion torque (IT), the diameter of fixture and the location of implant. Only the quality of bone in group 2 (6 week) was correlated with the stability of implant. CONCLUSION. Healing period of 6 weeks can make the similar clinical prognosis of implants to that of healing period of 12 weeks if bone quality is carefully considered in case of early loading.
최근 심미성에 대한 요구가 점점 증가함에 따라, 이제는 임플란트 수복에 있어서도 기능적인 면 뿐만 아니라 심미적인 면이 더욱 강조되고 있다. 상악 전치부 임플란트 수복은 임상가들에게 항상 도전적인 과제로 다가오는데 그 이유는 다음의 세 가지 요소를 모두 필요로 하기 때문이다. 첫째, 충분한 경조직이 필요하고 둘째, 충분한 연조직이 필요하며 그리고 셋째, 심미적인 수복물이 필요하다. 연조직의 심미성은 그 하부에 있는 경조직에 의존하게 되는데 그 이유는 하부의 경조직의 골격적인 지지가 있어야만 그것을 바탕으로 그 위에 건강하고 심미적인 연조직이 안정적으로 유지될 수 있기 때문이다. 그러므로, 경조직 재건은 심미적인 임플란트 수복에 있어서 첫번째 단계이며, 특히 3차원적으로 적절한 임플란트의 식립 위치 설정은 심미성 있는 최종 수복물을 얻기 위해서 가장 중요한 단계라고 할 수 있다. 그 다음으로 두번째 단계가 순측으로 충분한 두께의 연조직을 얻기 위한 수술 기법이며, 마지막 세번쨰 단계가 적절한 출현외곽을 갖는 임시 수복물을 통해서 얻어진 심미적인 최종 수복물이다. 본 임상 증례 보고는 순측의 골 열개 결손에서의 골 증대술 과정과 전치부 영역에 주로 사용되는 VIP-CT 라고 일컫는 유경 판막술을 이용한 연조직 증대술, 그리고 임시 수복물을 통한 연조직 형태 만들기와 맞춤형 인상 코핑을 이용한 인상채득법 등을 소개함으로써 경조직, 연조직, 수복물의 세 가지 요소가 심미적인 최종 수복물을 위해 서로 어떻게 조화를 이루어내는지 알아보고자 한다.
Breast implant insertion is one of the most commonly used methods for breast reconstruction after total mastectomy. However, infection is a common postoperative complication of implant insertion. In most cases, these infections can be managed with antibiotics and supportive therapy. However, severe septic conditions, such as toxic shock syndrome (TSS), can sometimes occur. TSS is an extremely rare but life-threatening complication, for which early diagnosis and proper management play a crucial role in determining patients' outcome. Although only 16 cases of TSS after breast implant insertion have been reported in the literature, most of those cases involved a serious clinical course. The reason for the seriousness of the clinical course of TSS in these cases is that the initial impression and presentation of these patients are nonspecific, and patients can easily be misdiagnosed as having a simple upper respiratory infection, causing the underlying condition to be neglected. Herein, we present two patients who were diagnosed with TSS after receiving breast reconstruction surgery via a silicone implant after total mastectomy. Both patients were misdiagnosed at the initial examination since they showed no local infectious signs on the postoperative wound.
Purpose: This study investigated the accuracy of free-hand implant surgery performed by an experienced operator compared to static guided implant surgery performed by an inexperienced operator on an anterior maxillary dental model arch. Methods: A maxillary dental model with missing teeth (No. 11, 22, and 23) was used for this in vitro study. An intraoral scan was performed on the model, with the resulting digital impression exported as a stereolithography file. Next, a cone-beam computed tomography (CBCT) scan was performed, with the resulting image exported as a Digital Imaging and Communications in Medicine file. Both files were imported into the RealGUIDE 5.0 dental implant planning software. Active Bio implants were selected to place into the model. A single stereolithographic 3-dimensional surgical guide was printed for all cases. Ten clinicians, divided into 2 groups, placed a total of 60 implants in 20 acrylic resin maxillary models. Due to the small sample size, the Mann-Whitney test was used to analyze mean values in the 2 groups. Statistical analyses were performed using SAS version 9.4. Results: The accuracy of implant placement using a surgical guide was significantly higher than that of free-hand implantation. The mean difference between the planned and actual implant positions at the apex was 0.68 mm for the experienced group using the free-hand technique and 0.14 mm for the non-experienced group using the surgical guide technique (P=0.019). At the top of the implant, the mean difference was 1.04 mm for the experienced group using the free-hand technique and 0.52 mm for the non-experienced group using the surgical guide technique (P=0.044). Conclusions: The data from this study will provide valuable insights for future studies, since in vitro studies should be conducted extensively in advance of retrospective or prospective studies to avoid burdening patients unnecessarily.
목적: 새롭게 개발된 인상채득이 가능한 치유지대주를 이용한 인상채득과 기존의 임프레션 코핑을 이용한 인상채득을 3차원적으로 비교함으로써 개발된 시스템의 정확성을 평가하였다. 재료 및 방법: 10명의 환자에서 구치부에 임플란트(SuperLine; Dentium, Seoul, Korea)를 식립 후 치유지대주(MyHealing; Raphabio Co., Seoul, Korea)를 연결하였다. 식립 3개월 후 보철물 제작을 위한 인상채득시 인상채득이 가능한 치유지대주를 이용한 transfer 방식과 기존의 인상용 코핑을 이용한 pick-up 방식으로 두 번 인상채득 후 각각 주모형을 제작하였다. 맞춤형 보철 지대주(Myplant; Raphabio Co., Seoul, Korea)를 제작하고 각각의 주모형에 체결 후 치과용 스캐너(Scanner S600; Zirkonzahn, South Tyrol, Italy)로 가상의 모형을 형성하였다. 3차원 분석 프로그램(Geomagic Qualify 12; Geomagic, Morrisville, NC, USA)을 이용하여 두 개의 가상의 모형을 중첩하여 지대주의 위치와 기울기의 차이를 분석하였다. 측정값은 Kruskal Wallis과 Mann-Whitney U test 방법을 통해 분석하였고, 유의수준은 0.05 로 설정하였다. 결과: 두 가지 인상법에 따른 지대주의 위치는 근원심 방향에서 0.032 mm, 협설측 방향에서 0.029 mm, 상하 방향에서 0.023 mm의 차이를 보였다. 기울기는 근원심 방향에서 $0.755^{\circ}$, 협설측 방향에서 $1.275^{\circ}$, 상하 방향에서 $0.420^{\circ}$의 차이를 보였다. 결론: 인상채득이 가능한 치유지대주를 이용한 인상채득 방식은 기존의 전통적인 인상용 코핑을 이용한 방식과 비교 시 0.032 mm 이내의 차이를 보였고 기존 논문과의 비교 시 유사한 수치이다.
An absolutely passive fit at the interface with the superstructure and the abutment cylinders is need for implant longevity. In this study, a method of cast framework correction using spark erosion technology was evaluated. Electric discharge machining(EDM) is a process that uses electrical discharges, or sparks, to machine metal, The surface being machined is bombared with high-intensity electrical energy pulses that gradually melt away the stock until the desired configuration is obtained. Master model was fabricated by using metal block impression analogue, which fired 5 Branemark squared impression coping in an arc shaped metal block. Then framework using 4mm standard gold cylinder and type IV gold alloy was fabricated. In order to measure the fit of the framework we used both contact and noncontact coordinate measurement machine and data was processed by computer program. After superimposition of gold framework and master model numeric image data the distances between gold cylinder bearing surfaces and abutment replica bearing surfaces, and the angle deviations between gold cylinder and abutment replica centroid points were calculated. The results were as follows 1 The total mean distance (p<0.01) and standard deviation (p<0.001) between the gold cylinders and abutment bearing surfaces were significantly decreased after EDM 2. The total mean of maximum distances was significantly decreased after EDM (p<0.05). 3. After EDM, the mean angle deviation between centroid points was decreased.
