Patients who have impacted tooth are found commonly in orthodontic treatment. Although it is difficult to find the cause of impacted teeth, the most common, causes are prolonged retention of the deciduous teeth, trauma, aberrant sequence of eruption, lack of space and deficiency of Vitamin D. Impacted teeth may lead to esthetic and functional problems and root resorption of adjacent teeth, so we should treat it as soon as possible. Commonly used treatment method is the following: After surgically uncovering of the impacted teeth, a bond of orthodontic appliance is established, and orthodontic traction is started with a removable or fixed appliance. We used the modified lingual arch with a soldered auxiliary appliance in lingual arch for traction of lower impacted teeth. The modified lingual arch could control the magnitude and direction of the applied force with one-arch treatment, and also could give continuous force to impacted tooth without patient patient cooperation. We achieved good results with the modified lingual arch.
The author has observed 906 full mouth roentgenograms in Korean young man who had the impacted and supernumerary teeth. Impacted and supernumerary teeth were analysed upon several items, such as incidences and positions in the dental arch. The result were as follows; 1. The incidences of impacted teeth were 59.05% and the ratio of upper jaw to lower one was 1:3. 2. The frequency of the supernumerary teeth were 5.52% and the ratio of maxilla to mandible was 49:1. 3. No significant differences between right and left side in both jaws, but incidence rate of the supernumerary teeth was 74% in the maxillary central incisor area. 4. The positions of impacted and supernumerary teeth were vertical and inverted shapes in maxilla, whereas horizontal and mesio-angular forms in mandible.

During orthodontic treatment of impacted teeth, use of appropriate anchorage against the traction force is important. Tooth anchorage with multi-bracket appliances is commonly used but sometimes it causes unwanted movements of adjacent teeth. Skeletal anchorage devices are therefore considered to minimize such side effects. Still their survival rate and positioning are highly limited according to the bone density and the interradicular space. This case report presents a case of two impacted teeth, one of which is dilacerated and horizontally angulated. Using the microplate with short screws and a bendable neck, negative effects on adjacent teeth were minimized and impacted teeth were repositioned with good stability.
A study on the impacted teeth of the anterior segment was carried out. The data for this study were compiled form 1739 outpatients of the Department of Orthodontics, College of Dentistry, Seoul National University. The following conclusions were obtained; 1. The frequency of the impacted teeth of the anterior segment was 2.8% and the frequency of male was 4.0%, that of female was 2.1%., respectively. 2. The order of occurrence of the impacted teeth in the anterior segment was maxillary canine mesiodens, maxillary central incisor, maxillary lateral incisor, mandibular canine. 3. The etiologic factors of the impaction were the space deficiency for eruption, cleft palate, the prolonged retention of deciduous teeth, dentigerous cyst. 4. The favorable results of treatment and prognosis were obtained from the impacted teeth of the normal shape after surgical exposure and adhered the plastic attachment and inducted them into the dental arch.
The first patient of this case was 13 year and 8 month old girl who had the Angle's class I malocclusion with uneruption of upper left central incisor and left canine. The second patient was 12 year old boy who had the Angle's class I malocclusion with uneruption of right central incisor. After surgical exposure of impacted teeth of those patients and bonding of attachment on exposed crown, those teeth were orthodontically pulled with removable appliances. The results were obtained as follow. 1. The functionally proper occlusion was established by guidance of the impacted teeth into the normal position in the arch. 2. Establishment of esthetic and balanced arch form was achieved. 3. The possibility of restorative prosthetic treatment induced by extraction of impacted teeth was eliminated. 4. The psychologic relief of pubertal patient who was very sensitive to facial esthetics was gained.
Transplantation of one's own teeth from one site to another is called dental autotransplantation. The role of autotransplantation of teeth in several clinical situations, the clinical techniques involved and factors influencing success of the transplant are discussed. Autotransplantation of impacted or retained teeth is an appropriate treatment, if orthodontic traction has failed, especially in growing patients.
Odontogenic keratocysts (OKCs) are one of the most aggressive cysts in the oral and maxillofacial area because of their high recurrence rate and infiltrative behavior. In growing patients with OKCs, a radical treatment approach might cause numerous complications, including the disturbance of jaw growth and loss of the involved tooth. This case report describes successful comprehensive orthodontic treatment combined with marsupialization of the cyst in a young girl who exhibited an OKC with impacted teeth. The 10-year-old girl presented with an OKC extending from the mandibular symphysis through the left mandibular body, with ectopic impaction of the mandibular left canine and first premolar, as well as congenitally missing bilateral mandibular second premolars. Interestingly, spontaneous improvement of the positions of the ectopic impacted teeth, along with a reduction in the size of the cyst, occurred during marsupialization. The sequential use of removable and fixed appliances enabled orthodontic traction of the impacted teeth. The treatment outcome was stable at 2.5 years after the end of the treatment. We speculate that comprehensive orthodontic treatment combined with marsupialization can be an effective treatment strategy for patients with OKCs, especially when they are encountered in young, growing patients with impacted teeth.
The author has observed on the impacted teeth of 11 cases from 484 full-mouth roentgenograms of dental students S. N. U. (except 3rd molar) These studies are very significant in oral surgery and orthodontic problems. Most of the impacted teeth are located in maxilla and among them 7 cases are impacted central incisors the others are lateral incisors, and cuspids. The form of impactions are vertical, horizontal and inverted positions.
Impacted or unerupted permanent teeth have many problems in making a diagnosis and treatment plan in dental clinic. There are several methods to treat impacted teeth. The combination of surgical exposure and orthodontic traction is usually the treatment of choice in cases with impacted teeth. Two cases are reported, which were treated with surgical intervention and orthodontic movement. and one case is treated with orthodontic movement alone. To improve esthetic problem and maintain periodontal health, We should avoid loss of attached gingiva in surgical exposure, and excessive orthodontic force during the traction of the impacted tooth.
Tooth eruption disturbance is seen commonly in mixed dentition or early permanent dentition. During this period of time, children experience the growth and development of craniofacial skeleton and dentition, so the impaction and eruption disturbance of permanent teeth make many problems in oral and maxillofacial areas. Eruption guidance of impacted tooth is not simple because many factors related to impaction are considered. Several factors such as etiology, tooth development stage, location of impacted teeth or eruption pathway, patient cooperation, etc. need to be taken into consideration in deciding the management of the impacted tooth. In the present study, it was assessed the etiology and impaction status of impacted tooth through various cases, and try to explain the treatment method to guide eruption by surgical exposure.
