Background: Data on perioperative outcomes of emergent versus elective resection in esophageal cancer patients requiring esophagectomy are lacking. We investigated whether emergent resection was associated with increased risks of morbidity and mortality. Methods: Data on patients with esophageal malignancy who underwent esophagectomy from 2005 to 2020 were retrospectively analyzed from the American College of Surgeons National Surgical Quality Improvement Program database. Thirty-day complication and mortality rates were compared between emergent esophagectomy (EE) and non-emergent esophagectomy. Logistic regression assessed factors associated with complications and mortality. Results: Of 10,067 patients with malignancy who underwent esophagectomy, 181 (1.8%) had EE, 64% had preoperative systemic inflammatory response syndrome, sepsis, or septic shock, and 44% had bleeding requiring transfusion. The EE group had higher American Society of Anesthesiologists (ASA) class and functional dependency. More transhiatal esophagectomies and diversions were performed in the EE group. After EE, the rates of 30-day mortality (6.1% vs. 2.8%), overall complications (65.2% vs. 44.2%), bleeding, pneumonia, prolonged intubation, and positive margin (17.7% vs. 7.4%) were higher, while that of anastomotic leak was similar. On adjusted logistic regression, older age, lower albumin, higher ASA class, and fragility were associated with increased complications and mortality. McKeown esophagectomy and esophageal diversion were associated with a higher risk of postoperative complications. EE was associated with 30-day postoperative complications (odds ratio, 2.39; 95% confidence interval, 1.66-3.43; p<0.0001). Conclusion: EE was associated with a more than 2-fold increase in complications compared to elective procedures, but no independent increase in short-term mortality. These findings may help guide data-driven critical decision-making for surgery in select cases of complicated esophageal malignancy.

In this study, we designed and verified a research model in order to determine whether ESG management for public libraries affects the perception of libraries, user satisfaction, and the intention to revisit. A survey was conducted among the users of six public libraries in the Gyeonggi region and 25 libraries in Seoul, and 247 valid responses from the survey were analyzed. The analysis revealed that the environment and society factors of ESG management had a positive effect on the perception of libraries and user satisfaction. It also showed that the libraries' perception had a positive impact on user satisfaction and the intention to revisit, and user satisfaction showed a positive correlation for the intention to revisit. From the analysis, we can infer that the effects of ESG management were reflected in everyday life via public libraries as ESG management of the public libraries influenced the perception of libraries, user satisfaction, and the intention to revisit. We confirmed that users showed the intention to revisit if the library provides positive and sincere satisfaction through ESG management, due to public libraries having ethical and moral significance to users. We have also put forward practical marketing strategies and identified areas for enhancement that can prove beneficial to public libraries.

The effective control of cost overrun is a crucial issue for construction companies to secure profitability. Especially in situations where cost pressures are significant due to factors such as rising raw material prices and increased financial costs due to high-interest rate policies, cost overruns resulting from project failures have a highly detrimental impact on the profitability of construction firms. The objective of this study is to analyze the current status of cost overrun control measures adopted by construction companies using the IPA technique and provide key characteristics and implications. The IPA analysis results showed that practitioners in general contractors exhibit a high level of interest and effort regarding cost overrun while the performance level is relatively low. Nevertheless, the measures considered important to control cost overruns generally show a high tendency for execution as well. Cost overrun control measures that show high importance but low execution are primarily related to collaboration and communication sectors. To effectively control cost overruns, enhancing collaboration and communication with construction supervisors/CM, headquarters, and regulatory authorities emerged as the most urgent need. Through this study, the current status and areas for improvement regarding cost overrun control measures in general contractors can be identified. This can be valuable for deriving directions and enhancements for future cost overrun control strategy development.
The purpose of this study is to investigate various factors affecting the relationship between digital leadership and digital transformation in companies. In particular, focusing on the mediating effect of dynamic digital capabilities that quickly sensing and seizing advanced and convergent digital technologies and apply them to the organization, this study also intends to examine the moderating effect of the organization's acceptance intention. The hypotheses were verified with a sample of 258 copies of data collected by conducting a survey on companies participating in mini-clusters in industrial complexes nationwide. As a result of the study, dynamic digital capabilities showed a significant mediating effect in the relationship between digital leadership and digital transformation, and the effect of dynamic digital capabilities on the level of digital transformation differed according to the organization's digital acceptance intention. This study presented a new concept and measurement items of digital dynamic capabilities, and how to consider digital leadership, dynamic digital capabilities and acceptance level of organization when promoting digital transformation in companies.
This study explores variations in preferences for human and AI agents within the medical and financial services. Study 1 investigates whether there are preferential disparities between human and AI agents across these service domains. It finds that human agents are favored over AI agents in medical services, while AI agents receive greater preference in the financial services. Study 2 delves into the underlying reasons for the preference differentials between human and AI agents by assessing the significance of certain capabilities as perceived by users in each domain. The findings reveal a mediating role of perceived empathy importance in the effect of service domains on human-AI preference. Furthermore, perceived empathy is deemed a more critical capability by users for preferring human over AI agents across both service domains compared to other capabilities such as experience and agency. This research is noteworthy for elucidating the variances in preferences for human and AI agents across medical and financial services and the rationale behind these differences. It enhances our theoretical comprehension of the pivotal factors influencing preferences for human and AI agents, underscoring the significance of human experiential capabilities like empathy.
Background and Objectives: There is limited data on the impact of type 2 myocardial infarction (T2MI) during the coronavirus disease 2019 (COVID-19) pandemic. Methods: The National Inpatient Sample (NIS) database from January 2019 to December 2020 was queried to identify T2MI hospitalizations based on the appropriate International Classification of Disease, Tenth Revision-Clinical Modification codes. Monthly trends of COVID-19 and T2MI hospitalizations were evaluated using Joinpoint regression analysis. In addition, the multivariate logistic and linear regression analysis was used to compare inhospital mortality, coronary angiography use, and resource utilization between 2019 and 2020. Results: A total of 743,535 patients hospitalized with a diagnosis of T2MI were identified in the years 2019 (n=331,180) and 2020 (n=412,355). There was an increasing trend in T2MI hospitalizations throughout the study period corresponding to the increase in COVID-19 hospitalizations in 2020. The adjusted odds of in-hospital mortality associated with T2MI hospitalizations were significantly higher in 2020 compared with 2019 (11.1% vs. 8.1%: adjusted odds ratio, 1.19 [1.13-1.26]; p<0.01). In addition, T2MI hospitalizations were associated with lower odds of coronary angiography and higher total hospitalization charges, with no difference in the length of stay in 2020 compared with 2019. Conclusions: We found a significant increase in T2MI hospitalizations with higher in-hospital mortality, total hospitalization costs, and lower coronary angiography use during the early COVID-19 pandemic corresponding to the trends in the rise of COVID-19 hospitalizations. Further research into the factors associated with increased mortality can increase our preparedness for future pandemics.
The Construction Technology Information System collects, processes, and provides the public with free of charge 16 types of construction technology-related texts or index information, such as construction standards, construction practices, and construction reports, required at construction sites to enhance domestic construction technology. This study judged that user satisfaction could be increased if the budget was allocated first to the most effective construction technology information DB establishment. For each type of construction technology information, the level of satisfaction felt by the population for the quantity provided and the value of use according to the acquisition and utilization of information were investigated. Reliability between independent variables and dependent variables through a multiple regression analysis process to identify how the demographic characteristics of the population affect user satisfaction and the factors that affect the use value of information for each type of construction technology information. and the correlation was analyzed. The correlation between the demographic characteristics of respondents and users' satisfaction with the provided quantity was generally low, but construction experience, age, etc. were found to have an effect on satisfaction with the provided quantity. In particular, among the demographic measurement items of respondents, the more construction experience they had, the higher their satisfaction level with the quantity provided of low-level technical standards, and it was analyzed that older age had an impact on satisfaction with the quantity provided for small and medium-sized business support information.

Objective: To determine the impact of dedicated subspecialized radiologists in multidisciplinary team (MDT) discussions on the management of lower gastrointestinal (GI) tract malignancies. Materials and Methods: We retrospectively analyzed the data of 244 patients (mean age ± standard deviation, 61.7 ± 11.9 years) referred to MDT discussions 249 times (i.e., 249 cases, as five patients were discussed twice for different issues) for lower GI tract malignancy including colorectal cancer, small bowel cancer, GI stromal tumor, and GI neuroendocrine tumor between April 2018 and June 2021 in a prospective database. Before the MDT discussions, dedicated GI radiologists reviewed all imaging studies again besides routine clinical reading. The referring clinician's initial diagnosis, initial treatment plan, change in radiologic interpretation compared with the initial radiology report, and the MDT's consensus recommendations for treatment were collected and compared. Factors associated with changes in treatment plans and the implementation of MDT decisions were analyzed. Results: Of the 249 cases, radiologic interpretation was changed in 73 cases (29.3%) after a review by dedicated GI radiologists, with 78.1% (57/73) resulting in changes in the treatment plan. The treatment plan was changed in 92 cases (36.9%), and the rate of change in the treatment plan was significantly higher in cases with changes in radiologic interpretation than in those without (78.1% [57/73] vs. 19.9% [35/176], p < 0.001). Follow-up records of patients showed that 91.2% (227/249) of MDT recommendations for treatment were implemented. Multiple logistic regression analysis revealed that the nonsurgical approach (vs. surgical approach) decided through MDT discussion was a significant factor for patients being managed differently than the MDT recommendations (odds ratio, 4.48; p = 0.017). Conclusion: MDT discussion involving additional review of radiology examinations by dedicated GI radiologists resulted in a change in the treatment plan in 36.9% of cases. Changes in treatment plans were significantly associated with changes in radiologic interpretation.

Around the world, many countries experiencing the issue of shrinking cities are continually expanding high-speed rail networks to enhance regional accessibility and address imbalances. This study analyzed the effects of high-speed train operations on the age-specific population migration in South Korean municipalities from 2012 to 2019, taking into account the risk levels of shrinking cities. For this purpose, an analysis was conducted using age-specific net in-migration population as the dependent variable, employing the spatial panel autoregressive model. The research results indicated that the influence of high-speed rail on regional population inflow varies depending on the risk level of shrinking city. In other words, high-speed railway operations had positive effects on population inflow in the capital areas and some major cities, while explained population outflow in the other regions. High-speed railways particularly exerted a significant impact on the inflow of the young and middle-aged population, representing the working age, but this effect was also limited to regions with a low risk of shrinkage. The findings of this study emphasize the importance of considering planned population and industrial attraction when installing high-speed rail with the goal of achieving regional balanced development and mitigating shrinkage. The results of this study also suggest the need for subsequent research to explore factors that positively influence population structure and inflow based on the level of shrinkage risk in each region, as well as the introduction of new policies tailored to the specific situations of each local government.

Purpose : This study investigated the effects of concentric and eccentric contractions on muscle strength using an isokinetic dynamometer (ID) in college students in their 20s. It aimed to understand the impact of differences between the dominant and non-dominant sides on strength asymmetry and to elucidate the clinical implications of these differences to establish an appropriate posture and environment for patients. Methods : The experiment was conducted with 30 healthy adult participants. Prior to the experiment, participants underwent a warm-up targeting the shoulders, and efforts were made to eliminate factors that could potentially influence the measurement results. Subsequently, the maximum safe range of motion of shoulder joint abduction, extension, and flexion was measured using an isokinetic muscle function testing device. Muscle strength was assessed using concentric and eccentric contractions alternating between the dominant and non-dominant sides, and paired sample t-tests were used for the analysis. Results : There was no significant difference between bilateral peak torques for eccentric contraction in shoulder joint abduction, extension, and flexion (p>0.05). There was also no significant difference between bilateral peak torques for concentric contraction in shoulder joint abduction, extension and flexion (p>0.05). Conclusion : This study found no statistically significant difference in muscle strength between the dominant and non-dominant sides during concentric and eccentric contractions. However, previous studies have shown significant differences between the dominant and non-dominant sides during eccentric and concentric contractions during internal shoulder rotation in the general population, as well as significant differences in the upper trapezius muscle. Therefore, further research is needed to support the application of different intensities for bilateral muscle strengthening exercises in clinical practice.
