PURPOSE. This research evaluated clinical outcomes of two types of hydroxyapatite (HA)-coated implants: OT (Osstem TS III-HA, Osstem implant Co., Busan, Korea) and ZM (Zimmer TSV-HA, Zimmer dental, Carlsbad, USA). MATERIALS AND METHODS. The research was conducted on 303 implants (89 of OT, 214 of ZM), which were placed from January 16, 2010 to December 20, 2012. The prognosis was evaluated in terms of success rates, survival rates, annual marginal bone loss, and implant stability quotients (ISQ). The samples were classified into immediate, early, conventional, and delayed groups according to the loading time. RESULTS. Overall, there were no significant differences between OT and ZM in success rates, survival rates, and annual marginal bone loss, except for the result of secondary stability. OT showed $77.83{\pm}8.23ISQ$, which was marginally higher than $76.09{\pm}6.90ISQ$ of ZM (P<.05). In terms of healing periods, only immediate loading showed statistically significant differences (P<.05). Differences between OT and ZM were observed in terms of two indices, the annual marginal bone loss ($0.17{\pm}0.58mm/year$ < $0.45{\pm}0.80mm/year$) and secondary stability ($84.36{\pm}3.80ISQ$ > $82.48{\pm}3.69ISQ$) (P<.05). OT and ZM did not have any statistically significant differences in early, conventional, and delayed loading (P>.05). CONCLUSION. OT (97.75%) and ZM (98.50%) showed relatively good outcomes in terms of survival rates. In general, OT and ZM did not show statistically significant differences in most indices (P>.05), although OT performed marginally better than ZM in the immediate loading and 1-stage surgery (P<.05).
In the anterior maxillary area, dental implants for tooth replacement are challenging due to the need to satisfy high esthetic level as well as functionality. Immediate implant placement and provisionalization can dramatically reduce the edentulous period, and then fulfill patient's demand for esthetics. The aim of present case report is to demonstrate two cases that successfully restored single tooth with immediate implant placement and provisionalization in the anterior maxillary area. A 47 years old female was scheduled to replace her maxillary right central incisor due to crown-root fracture by trauma. Another 54-year-old female was planned to place dental implant following tooth extraction of maxillary right lateral incisor owing to continuous pus discharge despite repetitive treatments including apicoectomy. In these two cases, surgical and prosthetic procedures progressed in a similar way. After minimal flap elevation, atraumatic tooth extraction was performed. Implant was placed in proper 3-dimensional position and angulation with primary stability. Bone graft or guided bone regeneration for peri-implant bone defect was conducted simultaneously. Provisionalization without occlusal loading was carried out at the same day. Each definitive crown was delivered at 7 and 5 months after the surgery. Two cases have been followed uneventfully for 2 to 5 years of loading time. In conclusion, Immediate implant placement and provisionalization could lead to esthetic outcome for single tooth replacement with dental implant under proper case selection.
Purpose: Patients who treated implant immediate loading within a week after implant placement at Wonkwang University Dental Hospital Implant Center were evaluated marginal bone resorption. These retrospective analyses are intended to reinforce the clinical evidence for the implant immediate loading. Materials and methods: Medical history and radiographic data were investigated, which were the patients' who treated implant immediate loading and restoration with provisional prostheses between January 2005 and June 2016, at Wonkwang University Dental Hospital Implant Center. Total number of implants was 70, marginal bone resorption was measured according to implant length, diameter and connection type. To measure marginal bone resorption, periapical radiographs were taken when the implants were placed and after 6 month. Statistical analysis was done in Mann-whitney U test and Kruskal-wallis test with SPSS 22.0 software (P<.05). Results: Mean marginal bone resorption around immediately loaded implants according to implant connection type was shown $1.24{\pm}0.72mm$ in internal hexagon connection type and $1.73{\pm}1.27mm$ in external hexagon connection type. There was no statically significant difference in marginal bone resorption with implant length and diameter. Conclusion: Implants with immediated loading in internal hexagon connection type showed less marginal bone resorption significantly than in external hexagon connection type.

To obtain better esthetic results when immediately placing a dental implant, the soft tissue surrounding the implant must be conditioned during healing of the extraction socket. To this end, the emergence profile can be customized through immediate restoration of the provisional prosthesis, and good clinical results can be obtained at the time of definitive restoration in the future, resulting in high patient satisfaction. In this case, horizontal root fracture occurred after trauma to both maxillary central incisors. Immediate implant placement and loading was planned considering aesthetics and alveolar bone condition. By taking an impression using a digital intraoral scanner, a digital diagnostic wax-up was performed to make a more aesthetic prosthesis without applying external force to the traumatized teeth. Based on this, the ideal placement location was determined and immediate implant placement was performed using a 3D printed surgical guide. The provisional prosthesis was restored 5 days after placement, and the definitive zirconia crown was restored through soft tissue conditioning and customization using the shape of the provisional prosthesis for 3 months.
Statement of Problem. Commonly used classification systems do not inform for dentists the dimension of the available bone at a potential implant site although regarding a variety of morphologic and pathophysiologic aspects using schematic graphs. However, for the implantologist the availability of bone substance is most important independent whether it concerns the jaw basis or the alveolus. Purpose of Study. The present article refers to a new evaluation form, to analyze the available bone with regard to optional immediate loading site by site. According to a new systematic implant planning concept will be presented in two case reports. Results. The feasibility of the classification for planning and documentation of immediately loaded implants is presented in two case reports. Conclusion. The factor of bone support for immediate functional stability is important in dental implantology. The new systematic implant planning helps to systematically estimate the dimension (ASCIi classification) of the alveolus site by site to evaluate the possibility of immediate loading. The Gottingen classification thus aids to determine the degree of stability that can be expected for the planned solution.
Nowadays, it seems like that the concept of the early loading protocol has been intruded between the concept of the immediate loading supported by advanced clinicians and the concept of the conventional loading supported by the prudent clinicians. In particular, the stability dip, which is found to occur usually at 4~6 weeks after dental implant placement because the decrease of the primary mechanical stability is not compensated by the increase of the secondary biologic stability, is frequently pointed out as one of the major causes of early loading failure. In this article, based on our recent clinical investigation about the crestal bone preservation effect of the early loaded NanoTite$^{TM}$ Tapered Certain Prevail$^{(R)}$ (Biomet 3i, USA), the concept of the early loading is revisited. In addition, various methods to overcome the stability dip are reviewed as the oral and maxillofacial surgeon's view point.
PURPOSE. A large number of studies have suggested the practicability and predictability of immediate implant function, but few studies have reported marginal bone level changes during sequential loading periods. The purpose of this study was to evaluate the marginal bone remodeling of immediately loaded self-tapping implants both at each time point and during each loading period between two time points. MATERIALS AND METHODS. The patients included in this retrospective study were treated with immediately loaded NobelSpeedy Replace implants between August 2008 and July 2009. Differences in the marginal bone level (MBL) at each time point and the marginal bone level change (ΔMBL) between two time points were analyzed with Bonferroni correction (P < .05). RESULTS. Overall, 24 patients (mean age, 47.3 ± 12.8 years) with 42 immediately loaded implants and a median follow-up of 6.5 years (IQR, 67.8 months) were included. The cumulative survival rate after 10 - 12 years was 95.2%. Continuous but slow marginal bone loss was observed during long-term follow-up. MBL at both 7.5 years and 11 years was significantly lower than that at loading, 6 months, 2 years and 4 years (P < .05). No bone loss difference was found in any period before 4 years of follow up (P > .05). The loading period of 4 years to 7.5 years showed the largest ΔMBL compared to those of other time periods (P < .05). CONCLUSION. Slight bone loss occurred continuously, and more radical changes of marginal bone can be observed during the period of 4-7.5 years. Thus, long-term effective follow-up of immediately loaded implants is needed.
