Idiopathic Parkinson's Disease patients' speech is hypokinetic dysarthria and their speech is possibly the consequence of impaired respiratory support. The purpose of this study was focused on the respiratory characteristics of speech breathing in de novo IPD who were not given prior vocal or anti-Parkinson treatment. A total of 40 subjects participated in the study: 20 de novo IPD patients between the ages of 50 and 80, and 20 normal subjects with similar age, height, and weight matches. Forced Expiratory Vital Capacity (FVC), Forced Expiratory Volume in 1 sec (FEV1) and $FEV_1$ as a percentage of FVC (FEV1/FVC) was measured with a PC-based spirometer (Cosmed). In addition, Maximum Phonation Time (MPT), Mean Airflow Rate (MFR), Subglottal Pressure (Psub) and the number of syllables produced per breath were measured with a Phonatory Aerodynamic System (Kay PENTAX). All subjects were asked to read a standardized Korean paragraph and the following measurements were obtained from the task. Results indicated no statistically significant differences in respiratory function (FEV1/FVC%) and aerodynamic function between the two groups, but the number of syllables per breath was significantly lower in the IPD patient group than in the normal group and it could be predicted by FVC and MFR. Therefore, the study shows that the MFR from the lungs during speech in de novo IPD patients is used inefficiently.
Purpose: The purpose of this study is to explore the roles and function of family in mediating the relationship between depression and quality of life of patients with Parkinson's disease (PD). Most studies have found that depression is particularly common in patients with PD and further associated with poor quality of life. Family function, as a mediator, is based on a strength orientation perspective that emphasizes not only their responsibilities and risks but also recuperative powers and growth potential. Methods: Overall 157 adults with idiopathic Parkinson's disease were enrolled in this study via outpatient clinic and completed a set of assessment to measure depression using BDI, family APGAR questionnaire, and patients' quality of life using PDQ-8. Hierarchical multiple regression analysis was conducted to examine the mediating role of family APGAR score in the relationship between BDI and PDQ-8. Results: Patients' depression, gait disturbance, duration of illness, and family function were statistically significant on quality of life. These factors accounted for 60% of the variance in quality of life. Family function has a partial mediating effect on the relationship between depression and quality of life. Conclusion: Findings from the study suggest that although PD patients' depression impacts their quality of life, by having strong family function, the extent to which depression impacts the quality of life can be favorably mitigated. Additionally, these outcomes have important implications for future model development regarding PD patients.

The acoustic vowel space has been used as an acoustic parameter in dysarthric speech. The aim of this work was to examine mathematical formulae for acoustic vowel space and to apply these to Korean speakers with idiopathic Parkinson's disease(IPD). Five acoustic parameters were chosen from earlier works and one new parameter was proposed, the pentagonal vowel space. The six parameters included triangular vowel space (3 area), irregular quadrilateral vowel space (4 area), irregular pentagonal vowel space (5 area), vowel articulatory index (VAI), formant centralization ratio (FCR) and F2i/F1u ratio (F2 ratio). An experimental group of 32 IPD patients(male:female=16:16) and a control group of twenty healthy people (male:female=8:12) participated in the study and repeated vowels (/a-i-u-e-o/) three times. A correlation analysis was performed among the six parameters, 2-way ANOVA was done with gender and groups as independent factors, and an independent sample t-test was conducted between the male and the female group as post hoc comparison. All parameters were highly correlated with each other and only the FCR showed a high negative correlation with the others. The results of ANOVA showed a significant difference in F2 ratio, 3 area, 4 area and 5 area between gender and in 4 area and 5 area between groups. For the male members of the two groups, significant statistical differences were found in all parameters whereas no such differences were found for the female members. These findings indicated that the vowel space of the female group was wider than the vowel space of the male group. These differences may have been caused by gender-specific speech styles rather than by patho-physiological mechanisms. We also claim that the pentagonal vowel space is better than the other vowel spaces at representing the disordered speech in natural speech situations.

The purpose of this study is to investigate the angular velocity of forearm pronation/supination movement in Parkinson's disease patients, as a quantitative measure of the bradykinesia. Thirteen Parkinson's disease patients ($64{\pm}11.0$ yrs, male:6, female:7) participated in the experiments. The subjects' both right and left forearms were scored by a rater according to rapid alternating movement of hands category in the UPDRS (unified Parkinson's disease rating scale) and the angular velocity of forearm pronation/supination was measured at the same time. As analysis parameters, RMS (root mean square) angular velocity and RMS angle were used. The parameters showed negative correlation with the clinical score (RMS angular velocity: r= - 0.914, RMS angle: r= -0.749). The RMS angular velocity of all clinical scales were significantly different one another except for the non significant difference between those of scale 3 and 4. RMS angle of scale 0 was significantly different from those of scale 2, 3, 4 and that of scale 1 was significantly different from those of scale 3 and 4. This suggests that RMS angular velocity can be used for a quantitative measure of bradykinesia in motor examination.
Clinical measures that Quantify falling risk factors are needed for the accurate evaluation of patients and to plan an intervention strategy. The purpose of this study was to examine the test-retest and interrater reliability of the dynamic gait index (DGI) for persons with Parkinson's disease (PD). A total of 22 idiopathic PD patients were recruited from rehabilitation hospital, Korea in this study. The DGI was assessed in two sessions that were, three days apart. We also measured Berg balance test (BBT) and geriatric depression scale (GDS) for concurrent validity with DGI. Intrarater and interrater reliability (.96 and .98 respectively) for DGI were high. indicating good agreement. The DGI was showed a good positive correlation with the BBS (r=.852). but not GDS (r=-.462). Intrarater and interrater reliability of DGI were high in people with PD. The DGI could be a reliable measure to evaluate functional postural control during gait activities in the PD population, and the ability of DGI to detect real change is acceptable in research and clinical settings.

More recently, clinical and laboratory investigations have led to the theory that dysregulation of iron, an essential metal to body function, may underlie IPD by initiating free radical reaction, diminishing the mitochondrial energy production, and provoking the oxidative cytotoxicity. The participation of iron in neuronal cell death is especially intriguing in that iron acquisition and regulation in brain are highly conservative and thus vulnerable to interference from other metals that bear the similar chemical reactivity. Manganese neurotoxicity, induced possibly by altering iron homeostasis, is such an example. In fact, the current interest in manganese neurotoxicology stems from two primary concerns: its clinical symptoms that resemble Parkinson's disease and its increased use as an antiknock agent to replace lead in gasoline. This article will commence with addressing the current understanding of iron-associated neurodegenerative damage. The major focus will then be devoted to the mechanism whereby manganese alters iron homeostasis in brain.

Objective : Subthalamic nucleus (STN) deep brain stimulation (DBS) is an effective treatment of choice for patients with advanced idiopathic Parkinson's disease (PD) who have motor complication with medication. The objectives of this study are to analyze long-term follow-up data of STN DBS cases and to identify the factors related to outcomes. Methods : Fifty-two PD patients who underwent STN DBS were followed-up for more than 3 years. The Unified Parkinson's Disease Rating Scale (UPDRS) and other clinical profiles were assessed preoperatively and during follow-up. A linear regression model was used to analyze whether factors predict the results of STN DBS. We divided the study individuals into subgroups according to several factors and compared subgroups. Results : Preoperative activity of daily living (ADL) and the magnitude of preoperative levodopa response were shown to predict the improvement in UPDRS part II without medication, and preoperative ADL and levodopa equivalent dose (LED) were shown to predict the improvement in UPDRS part II with medication. In UPDRS part III with medication, the magnitude of preoperative levodopa response was a predicting factor. Conclusion : The intensity of preoperative levodopa response was a strong factor for motor outcome. And preoperative ADL and LED were strong factors for ADL improvement. More vigorous studies should be conducted to elucidate how levodopa-induced motor complications are ameliorated after STN DBS.
Objectives: The purpose of this study was to investigate the effects of Korean Medicine on postural instability and gait difficulty(PIGD) in patient with parkinsonism. Methods: We reviewed the charts of patients with parkinsonism who were admitted to the department of Korean internal medicine, stroke and neurological disorders center, Kyung Hee University Hospital at Gangdong from October 2009 to May 2017. We analyzed the data of UPDRS (Unified Parkinson's disease rating scale) and calculated PIGD related UPDRS before and after the admission. Results: The average of PIGD score for the 23 patients of Parkinsonism significantly decreased from $8.35{\pm}5.13$ to $4.52{\pm}3.68$ after treatment. The average of PIGD score for the 16 patients of idiopathic parkinson's disease and 7 patients of atypical Parkinsonism decreased from $9.13{\pm}4.46$ to $4.63{\pm}4.15$ and from $6.57{\pm}6.45$ to $4.29{\pm}2.56$ after treatment, respectively. Conclusions: These results provide that Korean Medicine has an effect on the motor function of patients who suffer from Parkinsonism with Postural Instability and Gait Difficulty. Furthermore it could be effective for improving activities of daily life.
The purpose of this study was to compare spatio-temporal parameters during walking between patients with idiopathic Parkinson's disease and a control group matched for age, height, and weight. Thirty-three subjects were included in this study. Fifteen normal subjects (age, $63.3{\pm}5.8$ yrs; height, $164.1{\pm}8.7$ cm; weight, $60.7{\pm}17.5$ kg) and eighteen patients (age, $64.0{\pm}7.7$ yrs; height, $164.7{\pm}7.3$ cm; weight, $63.6{\pm}7.7$ kg) participated in the study. The Vicon 512 Motion analysis system was used for gait analysis in each group during walking, with and without an obstacle. The measured spatio-temporal parameters were cadence, walking speed, stride time, step time, single limb support time, double limb support time, stride length, and step length. Results in stride length and step length, when walking without an obstacle, showed a significantly greater decrease in the patient group compared to the control group. During walking with an obstacle, the patient group showed a significantly greater decrease in the step length as compared to the control group. For the control group, there were significant decreases in parameters of cadence and walking speed and increases in parameters of stride time, step time, and single limb support time when walking with an obstacle. The patient group had lower cadence and walking speed and higher stride time, step time, and single limb support time during walking with an obstacle than in walking without an obstacle. These results suggest that patients with Parkinson's disease who walk over an obstacle can decrease cadence, stride length, and step length. Further study is needed, performed with more obstacles and combined with other external cues, such as visual or acoustic guides.
