The Effect of Helicobacter pylori eradication in Patients with H. pylori(+) Idiopathic thrombocytopenic Purpura : Case study (위나선균양성 특발성혈소판감소성자반증에서 위나선균박멸요법의 효과: 증례 연구)

  • Idiopathic thrombocytopenic purpura is a potentially life-threatening condition characterised by thrombocytopenia and mediated by an autoimmune mechanism. Several recent studies have suggested that an association exists between H. pylori eradication and improvement in platelet count in a significant proportion of patients with idiopathic thrombocytopenic purpura. This study confirmed the efficacy of H. pylori eradication in increasing the platelet count in steroid refractory idiopathic thrombocytopenic purpura patients. Eradication therapy is simple and inexpensive, with limited toxicity and the advantage of avoiding long-term immunosuppressive treatment.

  • There have been many reports about surgical treatments of flexible flatfoot in children and acquired adult flatfoot deformity due to posterior tibial tendon dysfunction common in the 5th and 6th decades. However there has been a controversy for surgical treatment guideline for painful idiopathic flexible flatfoot deformities in young adults. Therefore, we present a 27-year-old female with severe painful idiopathic flexible flatfoot who was treated with medial sliding calcaneal osteotomy and subtalar arthroereisis using $Kalix^{(R)}$ (Newdeal SA, Vienne, France) endoprosthesis and had good clinical outcome with high patient satisfaction at 10 months follow-up postoperatively.

  • Spinal cord herniation is a rare condition that has become increasingly recognised in the last few years. The authors report a case of idiopathic spinal cord herniation in a 33 year old woman performed with progressive Brown-Sequard syndrome. The diagnosis was made on MR imaging. After repairing the herniation, the patient made a gradual improvement. Potential causes are discussed, including a possible role of dural defect. In conclusion, idiopathic spinal cord herniation is a potentially treatable condition that should be more readily diagnosed that increasing awareness and improved imaging techniques.

  • Objective : Epilepsy is a brain condition which causes a person to suddenly lose consciousness and sometimes to have fits. The etiologic factors of epilepsy are various, but most of cases are idiopathic. The purpose of this study is to report a pediatric patient with idiopathic epilepsy. Method : We treated him with herb medicine, acupucture and anticonvulsant. Then, we stopped administering anticonvulsant, and observed the progress of his condition. Result : We had good effect with oriental medical treatment on an epileptic who quitted taking anticonvulsant. Conclusion : We report the good results of oriental medicine on a pediatric patient with idiopathic epilepsy. And the further study is needed with more cases, longer duration and other tools.

A Case of Idiopathic Isolated Hypoglossal Nerve Palsy (특발성 단독설하신경마비 1례)

  • Isolated hypoglossal nerve palsy is a rare clinical condition and it causes deviation of the tongue and dysarthria. A 50-year-old female presented with tongue deviation to the left and mild dysarthria. She had no remarkable past medical history except several recent upper respiratory infections. On examination, the other cranial nerves were intact and she had no focal neurological signs. The findings of MRI and MR angiography were normal. Cerebrospinal fluid analysis revealed only mild elevation of protein. We diagnosed her as suffering with idiopathic isolated hypoglossal nerver palsy and we administered steroid therapy. The dysarthria was improved, but the tongue deviation still remained at 50 days after onset. We report here on a rare case of idiopathic isolated hypoglossal nerve palsy.

  • Current treatment for adolescent idiopathic scoliosis(AIS) consists of three phases: observation, bracing, and surgery. Although there are many nonsurgical treatment(bracing, electrical stimulation, exercise, manipulation, acupuncture, etc), their effect is still controversial. In many paper, Schroth method was reported good immediate response to conservative care, which could be considered a sign of good prognosis. Schroth method became effective thai specialists in physiotherapy for spinal deformities teach the patient how to perform a routine of 'curve pattern' specific exercises with the purpose to facilitate the correction of the asymmetric posture and to teach the patient to maintain the corrected posture in dally activities. This Principles of correction exercise treatment are based on those developed by the German physiotherepist K. Schroth.

A STUDY OF IDIOPATHIC OSTEOSCLEROSIS IN THE PANORAMIC RADIOGRAPHS (파노라마 X선사진을 이용한 특발성 골경화증에 관한 연구)

  • The purpose of this study was to evaluate the prevalence, the distribution, and the relation with adjacent roots of idiopathic osteosclerosis in the jaws by means of analysis of 7,837 persons' panoramic radiographs. The following results were obtained ; 1. The prevalence of idiopathic osteosclerosis in the jaws in panoramic radiographs was revealed to be 9.76% in total examined persons. There was a higher prevalence in males(10.10%) than in females(9.76%). 2. There was also a higher prevalence in the mandible(97.92%) than in the maxilla(2.08%). The most frequently involved area was the mandibular molar area(34.72%), followed by mandibular premolar area(33.37%). 3. According to the types of idiopathic osteosclerosis with relation to adjacent roots, separated type(51.59%) was the most frequent, followed by apical type(32.52%), interradicular type(7.95%), apical and interradicular type(7.95%) in descending order of frequency.

  • Objective: The purpose of this study was to evaluate the relation between idiopathic scoliosis and facial deformity in the horizontal, vertical, and anteroposterior planes. Methods: A total of 123 female patients aged 14 years or older, who visited the Spine Clinic at the Department of Orthopedics, Korea University Guro Hospital for treatment of idiopathic scoliosis, were enrolled. Whole-spine anteroposterior and lateral radiographs were taken with the patient in a naturally erect position, and frontal and lateral cephalograms were taken in an erect position with the Frankfort horizontal line parallel to the floor. Scoliosis was classified according to the Cobb angle and Lenke classification of six curve types. Cephalometric tracing in all cases was carried out with V-Ceph 5.5 by the same orthodontist. The Kruskal-Wallis test was performed to determine whether any relation existed between each group of the idiopathic scoliosis classification and the cephalometric measurements of frontal and lateral cephalograms. Results: The measurements did not reveal any significant association between the Cobb angle and cephalometric measurements and between the curve type based on the Lenke classification and cephalometric measurements. Conclusions: Based on the results of this study, no apparent relation was observed between the severity of scoliosis and facial form variations in idiopathic scoliosis patients.

Development of Wearing of Musculo-Skeletal Functional Garment for Adolescents' Idiopathic Scoliosis -With the Principle of Sports Taping Applied- (청소년 척추 측만증 환자를 위한 근골격계 보정 웨어 개발 - 스포츠 테이핑 원리 응용 -)

  • We aimed to analyze the muscle activity of adolescent patients with idiopathic scoliosis during gait and develop the wearing of musculo-skeletal functional garment by applying the principle of sports taping based on the result of the analysis. We selected 20 male students between the ages of 13 and 18 and divided them into 2 groups: one group consisted of 10 patients with idiopathic scoliosis <20 degrees of Cobb's Angle: the other group had 10 normal students. Using, we measured and analyzed the muscle activity of 8 different regions: left and right latissimus dorsi, left and right thoracolumbar fascia, left and right gluteus medius, and left and right biceps femoris during gait. Our results can be summarized as follows: Firstly, in patients with idiopathic scoliosis, the gait showed a significantly low activity of the right latissimus dorsi muscle when the left foot was supported on the ground(p<.05). Secondly, in the overall gait cycle, the patients showed a higher activity of the right thoracolumbar fascia and right gluteus medius than that seen in the normal students: however, this difference was not statistically significant. Thirdly, by applying sports taping on the bisis of the results, this study developed the wearing of musculo-skeletal functional garment that could maximize the stimuli of the right latissimus dorsi and alleviate muscle contraction of the right thoracolumbar fascia and right gluteus medius, while expanding the spinal line upward and downward, by focusing on the difference between left and right muscular strength of the muscle activity of the bright latissimus dorsi. Overall, we expect that by wearing of musculo-skeletal functional garment, the muscular functions in adolescents with idiopathic scoliosis.

Suspected Idiopathic Acute Transverse Myelitis : Retrospective Analysis of 27 Cases (특발성 급성 횡단성 척수염 : 27예의 후향적 분석)

  • Background: Acute transverse myelitis(ATM) is a group of disorders characterized by focal inflammation of the spinal cord and resultant neural injury. It can be diagnosed by Transverse Myelitis Consortium Working Group(TMCWG) criteria. But there are some cases which were not satisfied with idiopathic ATM criteria, both clinically and radiologically, especially in acute stage. So we analyzed 27 cases retrospectively, which were diagnosed as idiopathic ATM. Methods: All the records of the patients at Gil Medical Center with a diagnosis of idiopathic ATM from 2001 to 2005 were reviewed. And clinical manifestations including neurological examination, radiologic features and cerebrospinal fluid (CSF) findings were analyzed. Results: Among the patients(20 men and 7 women; mean age, 45.3 years), 11 cases could not be diagnosed as idiopathic ATM according to the TMCWG criteria ; 6 cases did not have well marginated upper sensory level and 5 cases were not satisfied with spinal cord inflammation. Conclusions: Although most cases of suspected idiopathic ATM were suitable for TMCWG criteria, some cases were not satisfied with this diagnostic criteria, especially in acute stage. Subsequent study might be needed to evaluate the reliability and clinical application of the criteria.
