Market in Medical Devices of Blockchain-Based IoT and Recent Cyberattacks

  • The creativity of thesis is that the significance of cyber security challenges in blockchain. The variety of enterprises, including those in the medical market, are the targets of cyberattacks. Hospitals and clinics are only two examples of medical facilities that are easy targets for cybercriminals, along with IoT-based medical devices like pacemakers. Cyberattacks in the medical field not only put patients' lives in danger but also have the potential to expose private and sensitive information. Reviewing and looking at the present and historical flaws and vulnerabilities in the blockchain-based IoT and medical institutions' equipment is crucial as they are sensitive, relevant, and of a medical character. This study aims to investigate recent and current weaknesses in medical equipment, of blockchain-based IoT, and institutions. Medical security systems are becoming increasingly crucial in blockchain-based IoT medical devices and digital adoption more broadly. It is gaining importance as a standalone medical device. Currently the use of software in medical market is growing exponentially and many countries have already set guidelines for quality control. The achievements of the thesis are medical equipment of blockchain-based IoT no longer exist in a vacuum, thanks to technical improvements and the emergence of electronic health records (EHRs). Increased EHR use among providers, as well as the demand for integration and connection technologies to improve clinical workflow, patient care solutions, and overall hospital operations, will fuel significant growth in the blockchain-based IoT market for linked medical devices. The need for blockchain technology and IoT-based medical device to enhance their health IT infrastructure and design and development techniques will only get louder in the future. Blockchain technology will be essential in the future of cybersecurity, because blockchain technology can be significantly improved with the cybersecurity adoption of IoT devices, i.e., via remote monitoring, reducing waiting time for emergency rooms, track assets, etc. This paper sheds the light on the benefits of the blockchain-based IoT market.

Setting Priority Criteria for Classification of Self-Testing In Vitro Diagnostic Medical Devices Using Analytic Hierarchy Process Technique (Analytic Hierarchy Process 기법을 활용한 개인용 체외진단의료기기 분류기준에 대한 우선순위 연구)

  • Background: The coronavirus disease 2019 pandemic has been challenging the healthcare service, i.e., the vitalization of the point of care accompanying self-testing in vitro diagnostic medical devices (IVDs). This study aims to suggest priority criteria to classify self-testing IVDs using the analytic hierarchy process technique. Methods: Two dimensions of the characteristics embedded in the IVDs and the diseases to be diagnosed with self-testing IVDs were parallelly considered and independently investigated. In addition, three expert panels consisting of laboratory medical doctors (n=11), clinicians (n=10), and citizens (n=11) who have an interest in the selection of self-testing IVDs were asked to answer to questionnaires. Priorities were derived and compared among each expert panel. Results: First of all, ease of specimen collection (0.241), urgency of the situation (0.224), and simplicity of device operation (0.214) were found to be the most important criteria in light of the functional characteristics of self-testing IVDs. Medical doctors valued the ease of specimen collection, but the citizen's panel valued self-management of the disease more. Second, considering the characteristics of the diseases, the priority criteria were shown in the order of prevalence of diseases (0.421), fatality of disease (0.378), and disease with stigma (0.201). Third, medical doctors responded that self-testing IVDs were more than twice as suitable for non-communicable diseases as compared to communicable diseases (0.688 vs. 0.312), but the citizen's group responded that self-testing IVDs were slightly more suitable for infectious diseases (0.511 vs. 0.489). Conclusion: Our findings suggested that self-testing IVDs could be primarily classified as the items for diagnosis of non-communicable diseases for the purpose of self-management with easy specimen collection and simple operation of devices, taking into account the urgency of the situation as well as prevalence and fatality of the disease.

Material Hardship of the Poor Families in Korea: The Distributions and Determinants of Material Hardship (빈곤층의 물질적 궁핍, 실태와 영향요인 분석)

  • Material hardship measures have been used to supplement the traditional income-based poverty measures. Recently poverty researchers have increasingly used measure of material hardship to examine the well-being of low income families. Measuring the material hardship might be useful to a better understanding of the multi-dimensions of the poverty in Korea. Using the data of the Korea Welfare Panel Study(the fourth wave), I examine incidences of material hardship across quintiles of the income distributions and the factors that might affect the experience of material hardship among the poor families. Major findings are as follows. Descriptive results show that nearly one-fifth of all families had experienced at least one of the five material hardships in the year. Those in the bottom quintiles and the poor families are more likely to experience material hardship than the other quintiles and non-poor families. But, incidences of the material hardship in the middle income quintiles and low income families are not a few(18 percent and 37 percent). Logistic regression results show that family-consumption related variables, income other economic resources(asset and public assistance), and household's employment status affect the experience of material hardship among the poor families. But, material hardship among the poor families is not significantly related with family income. These results indicate that material hardship measures are the useful indicators to understanding the multi-dimensions of the poverty in Korea. And they suggest that an extensive reform of the public assistance policy is necessary to relieve the material hardships of the poor in Korea.

The Effects of Music Interventions on High-risk Neonates in NICUs: Systematic Review and Meta-analysis (신생아집중치료실 고위험 신생아 대상 음악중재연구에 대한 체계적 문헌고찰 및 메타분석)

  • The purpose of this study was to analyze and evaluate music intervention research conducted with high-risk neonates in NICUs in both domestic and international settings. Seventeen music intervention studies were identified, and their characteristics, including type of music, music provider, and treatment frequency, and outcomes (i.e., neonatal vital signs) were reviewed and analyzed along with meta-analysis. For music interventions targeting high-risk neonates in NICUs, the effect sizes of the neonates' vital signs were classified as either medium or large. In addition, larger effect sizes were associated with a combination of live and recorded music, nonmusical therapists as the music providers, and treatment frequency of one to five sessions per week. These research findings verify the clinical value of music for high-risk neonates and provide insights into the selection of music elements, music delivery methods, and music providers in NICU music interventions.

Expectations for Social Security and Perception of Life in Old Age in a Superaged Society : An Analysis of the Differences Between Age Groups in J apan (초고령사회 일본의 사회보장에 대한 기대인식과 노후 생활 인식 - 연령계층별 차이에 주목하여 -)

  • In this study, based on survey data from Japan, I analyzed the differences between the expectations for social security and the perception of life in old age by age group. The analysis data used in this study are from the "Survey on Life Security, 2019" conducted by the Japan Life Insurance Cultural Center, which surveyed men and women aged 18 to 69. The results of the analysis are as follows. First, expectations about health insurance are higher than expectations about other forms of social security in all age groups. Second, when it comes to expectations for public pensions, both men and women have the highest average scores in their 60s. Third, the age group with the lowest average score for public health insurance, public pension, public care insurance, and survivors' pension was found to be those in their 40s. In addition, men in their 20s had a higher average score on their perception of life in old age. Fourth, the effect of social security expectations on perception of life in old age was found to be somewhat different for gender and age groups, but overall, it was found that public health insurance expectations were an important factor that had a positive impact on the perception of life in old age.

Current Status and Health Empowerment of North Korean Women Defectors (북한이탈여성의 현황 및 건강 임파워먼트)

  • The purpose of this study is a descriptive research study to confirm the difference in health empowerment according to the current status and general characteristics of North Korean women defectors residing in South Korea. The subjects of the study were 201 North Korean women defectors living in the local community, and data were collected from September 16 to September 21, 2020 using a structured questionnaire. The collected data were analyzed by descriptive statistics, t-test, ANOVA, pearson's correlation coefficients using the SPSS/WIN 23.0 program. As a result of the study, the domain with the lowest score in the health empowerment of North Korean women defectors was "I know a positive method to cope with stress related to my health care." in the stress management question. Statistically significant differences were found in North Korean repatriation experience(t=3.77, p<.001), education(F=4.56, p=.012), economic status(t=-2.95, p=.004), subjective health status(t=-4.75, p<.001) and subjective stress(t=-2.47, p=.015). Based on the results of this study, alternatives should be prepared to strengthen the health empowerment of North Korean women defectors according to their individual characteristics.

  • Human Human species run into a blind alley due to abnormal weather and climates everywhere in global village. Human beings are helpless against the nature and might begin to learn humbleness just now. However, humans cannot attribute current abnormal weather and climates to only natural phenomenon because we have never been affectionate to global environments sufficiently up to now that results in running into this blind alley. At this point, the only thing that humans can do is to love and care for the earth more. ESG is an emerging topic to cope with this issue and practice of ESG will be the pending mission for the next generation. In this research, 'active participatory learning program for ESG practice' is designed by 'connecting ESG with Entrepreneurship' through over 20 years of experienced current teachers in elementary and junior·senior high school, professors in university and field experts in education. Analysis of learning effectiveness before and after the implementation of education program showed meaningful result in elementary and junior·senior high school. Thus, I would like to suggest a proposal based on concerns about 'What should we do to overcome the global crisis?" by paying attention to ESG from elementary school.

Comparison of total parenteral nutrition-associated cholestasis according to amino acid mixtures in very low birth weight infants (총정맥영양 연관 담즙정체증)

  • Purpose : The purpose of this study was to evaluate the effect of amino acid mixtures on incidence and severity of total parenteral nutrition associated-cholestasis(PNAC) in very low birth weight infants. Methods : Retrospective review of 63 very low birth weight infants(birth weight ${\leq}1,500g$) who received total parenteral nutrition(TPN) in our neonatal intensive care unit from January 2000 to December 2004 was performed. Patients were divided into 2 groups : Group I(n=32, Jan 2000-Jun 2002) and Group II(n=31, Jul 2002-Dec 2004), where infants in Group II received taurine and glutamic acid-rich amino acid mixtures. PNAC was defined as serum direct bilirubin(DB) level greater than 2.0 mg/dL. The incidence and severity of PNAC were compared between these groups. Results : The incidence of PNAC was significantly lower in Group II than in Group I(21.9% vs 6.5%, P<0.148). Maximum and mean DB levels were also significantly lower in Group II(P<0.05). Conclusion : The incidence and severity of PNAC in very low birth weight infants may be reduced with different composition of amino acid mixtures in TPN. Further prospective randomized controlled studies are needed to determine an ideal composition of acid mixtures to prevent the development of PNAC.

A Study on the Actual Conditions of Smoking in Middle and High School Students in One Region (일 지역 중·고등학생의 흡연실태)

  • To investigate the actual conditions of smoking in middle and high school students in Chinan County, I used a sturctured questionnaire for 1,579 students attending twelve middle-high schools from December 1, 1998 to December 20, 1998. I collected and data correlated the using an $SPSS-PC^+$ 1. The smoking rate of middle-high schoo1 students in Chinan County was 17.9%, relatively high. This smoking rate was different according to the gender, grade, religion, and economic situation. In mals, high school students, non-religious, students low income family students, the smoking rate was higher. The smoking rate of high school students was almost the same as the smoking rate of adults, generally higher than that of foreign teenagers. Because the smoking rat of studinets in the third grade of middle school and in the first grade of high School was six times higher, increased education should be conducted during this time in an attempt to curb the sudden increase of the smoking rate. The smoking rate of girl students was 5.0%; this has increased mor than three times from ten years ago. Consequently, counter measures should be taken against the smoking of female students as well as juvenile smoking in general. In addition, the smoking rate of middle-high school students showed interesting differences when correnated with enviornmental factors. Students with low grades, who are not satisfied with school life, who don't have both parents, who have uncaring parents who nare too strict or too arbitary, who have smoking parents, or who have experienced smoking commonly smoked. Therefore, to lower the smoking rate we should improve the school environment, improve a student's interest in school life. And parents or siblings should lead by example and quit smoking at home. Schools should educate students more effectively concerning the harmful effects of smoking and create an accurate understanding of its dancers. From the beginning, we should teach students never ever to touch cigaretts. 2. The surve discovered that most students started out of curiosity, or solicitantion from friends or elders at middle school, and had been smoking one to five cigarettes for more than a year. They obtained cigarettes at stores and most of them have friends who smoke. As a result anti-smoking education should be conducted at elementary schools prior to middle school. More than 95% of the teenagers who smoke had friends who smote and smoked out of curiosity or the recommendation of elders. Thus, we must focus on teenagers who smoke in group, rather than individually. Fuyrthermore, the strict application of the regulation of tobacco sales as well as tobacco cooperation from retailers are needed. While students did not show any mood or academic achievement difference after beginning smoking, 58.1% of the students a health situdation that was worse. Juvenile smoking is more harmful to the juvenile than adult smoking is to the adult. This should be focused on in an anti-smoking campaign. 3. Students who smoke hada more positive attitude toward smoking than students who don't smoke. Students who smoke had a tendency to have a nuetral position and are not concerned about smoking compared to non-smoking students. The survey showed that the great number of students had a nuetral position. Because this nuetrality may increase Juvenile smoking, education that provides an exact understanding of smoking should be performed to build the correct attidude toward smoking. 4. Middle school students smoke when angry, gloomy, anxious, a lone and when they have some problems to solve, on when they feel inconveniened in other wores, they smoke to reliver stress. They also smok due to addiction. Because smoking is not a praetical method to relieve stress, a program which helps to acquire positive relief stress should be provided to help reduce smoking. 5. About 65% of students who smoke want to quit smoking because of health problem, 78% of them have tried mor than once to quit but failed due to weak will power and peer pressure from friends who smoke. Juvenile smoking is group, oriented. Thus, the program that advances less smoking will be the one that focuseds on groups. 6. As for advice to students who want to quit smoking, "persuasion" was used most commonly, followed by a "presentation on how to quit smoking". Another method were severe punishment. About 70% of the students wanted the anti-smoking guide at school. 7. Most students (73.5%) had a position that more anti-smoking education at school is needed. Obriously, then, anti-smoking education at middle-high schools should be reinfoced. Although the education which explains the harmful influence of tobacco is known as an efficient way prevent smoking; it does not influence students who already smoke. Therefore, for students who smoke, multi-dimensional approaches must be attempted that include physical training, phychokogical approache, consultation and discussion, medical chek-ups, audio-visual education technigues, and professonal instructors, in addition, because smoking students have more negative on lukewarm attitude to anti-smoking education anti-smoking education should be conducted through a communicative style by dedicated teachers who care about students. Clinical Results alter Pulmonary Endarterectomy as a Curative Surgical Method in Chronic Thromboembolic Pulmonary Hypertension: an Approach to Operative Classification of Thromboembolic Disease (만성 폐동맥 색전증의 치료로서 내막제거술의 임상적 결과: 색전증 분류에 따른 접근)

  • Background: Pulmonary endarterectomy is widely accepted as a treatment for chronic thromboembolic pulmonary hypertension. Based on our experiences, we sought to find ways to reduce perioperative complications and to improve surgical outcomes in patients undergoing pulmonary endarterectomy. Material and Method: This study was designed as a retrospective analysis of 20 patients with pulmonary hypertension who underwent pulmonary endarterectomy between January 1998 and March 2008. All patients presented with chronic dyspnea. Deep vein thrombosis (DVT) was the major cause of chronic pulmonary thromboembolism (55%). Seventeen patients (85%) underwent inferior vena cava (IVC) filter placement. Thirteen patients underwent surgery under total circulatory arrest, while the others underwent surgery while on low flow cardiopulmonary bypass. Concomitant tricuspid annuloplasty was done in 6 patients (66%) whose tricuspid regurgitation was as severe as grade IV/IV. The mean follow-up duration was $45{\pm}32$ months. Result: Using of University of California, San Diego (UCSD), thromboembolism classification, 4 patients (20%) were type 1, 8 patients (40%) were type II, and 8 patients (40%) were type III. Right ventricular systolic pressure was reduced significantly from $77{\pm}29$ mmHg to $37{\pm}19$ mmHg after pulmonary endarterectomy (p<0.001). The degree of tricuspid regurgitation and the NYHA functional class were all improved postoperatively. Reperfusion edema occurred in 7 cases (35%). The incidence of reperfusion edema was higher in the UCSD type III group than in the other group (25% vs 50%, p=0.25) and the length of postoperative intensive care unit stay was longer in type III group ($5{\pm}2$ days vs $9{\pm}7$ days, p=0.07). The early mortality rate was 10%, and the late mortality rate was 15% (n=3); one death was due to progression of underlying non-Hodgkin's lymphoma, and the other deaths were related to recurrent thromboembolism and persistent pulmonary hypertension, respectively. Conclusion: Pulmonary endarterectomy, as a curative surgical method for treating chronic thromboernbolic pulmonary hypertension, should be performed aggressively in patients diagnosed with chronic thromboembolic pulmonary hypertension, and an effort should be made to reduce the frequency of perioperative complications and to improve surgical outcomes.
