  • A 3-year-old boy was transferred to emergency room (ER) with lethargy and abdominal pain. Physical examination revealed drowsy mental status but neurologically intact. Basic evaluation in ER shows hypoglycemia (43 mg/dL), hyperglycerolemia, ketonemia and ketonuria. Initial urine organic acid was performed and the result showed severe hyperglyceroluria. Under suspicion of isolated GKD, GKD gene was obtained from his DNA from white blood cell in peripheral blood and sequencing was performed. Isolated glycerol kinase deficiency (GKD) is an X-linked inborn error of metabolism that is either symptomatic or asymptomatic. GKD is due to deletions of, or mutations within, the GK gene, and there is no genotype-phenotype correlation. Gene study that we performed showed normal at a well-known mutation site, but found 4-base insertion at 79 base pair away from the beginning of exon 11.

Effects of Extracts from Mixed Culture with Tricholoma Matsutake Mycelium and Cordyceps Militaris Mycelium on Blood Glucose in Streptozotocin-Induced Diabetic Rats (송이버섯과 동충하초 균사체 혼합배양 추출물의 투여가 Streptozotocin으로 유발한 당뇨 쥐의 혈당에 미치는 영향)

  • This study was performed to investigate the influence of extracts from mixed culture with Tricholoma matsutake mycelium and Cordyceps militaris mycelium on hypoglycemia in streptozotocin-induced diabetic rats. Experimental animals were divided into 7 groups : normal control group(NC), diabetes control group (DC), positive control group(PC), non-fermented OCM(oriental medicine & cereal medium) extracts group (UM), crude polysaccharide of non-fermented OCM extracts group (UME), fermented OCM extracts group (UF), crude polysaccharide of fermented OCM extracts group (UFE). NC, DC groups were orally administered saline, PC group was orally administered acarbose. UM, UME, UF, UFE groups were orally administered each extract once a day for 14days. Blood glucose level was lower in the all administering OCM extract groups (UM, UME, UF, UFE) than in the diabetes group(p<0.05), and specially UF, UFE groups were similar to tendency of PC group. ALT, ALP activity in OCM groups were not significantly lowered than PC group(p<0.05). AST activity was not different with PC group. The results of this study show that extracts from mixed culture with Tricholoma matsutake mycelium and Cordyceps militaris mycelium may have a beneficial effect on the hypoglycemia.

Electrolyte and acid-base imbalance in native calves with enteropathogenic diarrhea

  • Diarrhea is the most common cause of death in calves, and remains a major health challenge. Although there are many studies on the related pathogens, the understanding of the clinicopathological changes is limited. This study aimed to identify the pathogens and observe the clinicopathological changes in electrolytes and acute phase proteins (APPs) associated with diarrhea. Blood samples and fecal samples were collected from 141 calves for the determination of APPs, electrolyte and acid-base status and identification of enteropathogens, respectively. Single or co-infections with enteropathogens, including virus (bovine viral diarrhea virus, coronavirus, and rotavirus), Eimeria, Cryptosporidium, and Escherichia coli K99 were detected in both non-diarrheic and diarrheic calves. Levels of APPs such as serum amyloid A, haptoglobin and fibrinogen were comparable between diarrheic and non-diarrheic calves. Hypoglycemia, high blood urea, electrolytes and acid-base imbalance (hyponatremia, hypochloremia, and decreased bicarbonate), and strong ion difference (SID) acidosis showed a significant association in diarrheic calves (p < 0.01). Particularly, significant hyponatremia, bicarbonate loss, SID acidosis, hypoglycemia, and elevated blood urea nitrogen were found in rotavirus-infected calves. Monitoring the clinicopathological parameters of APPs and electrolyte levels could be vital in the clinical management of diarrheic calves.

Dumping Syndrome in a Child with Gastrojejunal Tube Feeding (위공장 경관식이(Gastrojejunal Tube Feeding) 환아에서 동반된 덤핑 증후군(Dumping Syndrome) 1례)

  • Dumping syndrome is a known complication of gastric surgery in adults, but a very rare disease in the pediatric population. We report on a case of dumping syndrome in a 19-month-old child, who underwent gastrojejunal feeding tube insertion for the treatment and prevention of gastroesophageal reflux and frequent aspiration pneumonia. At 17 months of age, 2 months after the beginning of gastrojejunal tube feeding, postprandial diaphoresis, palpitation, lethargy, bloating, and diarrhea occurred, and a single episode of convulsion with hypoglycemia were noted. Early and late dumping syndrome was confirmed by an abnormal oral glucose tolerance test with early onset hyperglycemia followed by delayed onset hypoglycemia. Diet therapy including uncooked corn starch then improved the postprandial diaphoresis, abnormal glucose levels, and her nutritional status. We conclude that dumping syndrome may be considered as a complication of gastrojejunal tube feeding in a child.

Screening of Herbal Medicines for Synergistic Effects of Metformin and Herbal Extracts Combination in RAW 264.7 Cells (RAW 264.7 세포에서 Metformin과 병행투여 시 상승효과를 나타내는 한약재의 선별 연구)

  • Objectives: This study was performed to evaluate the effects of Metformin and Lonicerae Flos, Agrobacterium Rhizogenes, Coptidis Rhizoma, Atractylodis Rhizoma Alba, Houttuyniae Herba extracs combinations on hypoglycemia in RAW 264.7 cells. Methods: Expressions of Sirt1, p-adenosine monophosphate-activated kinase (p-AMPK), AMPK-alpha, peroxisome proliferator activated receptor (PPAR)-alpha, PPAR-gamma, X-box binding protein 1 (XBP-1), tumor necrosis factor (TNF)-alpha and interleukin (IL)-6 were analyzed by real time polymerase chain reaction and Western blotting analysis. Results: The level of gene expression of Sirt1, p-AMPK, AMPK-alpha, PPAR-alpha and XBP-1 in relation to that of beta-actin were increased or decreased significantly with the Metformin and Lonicerae Flos, Agrobacterium Rhizogenes extracts combination groups. The level of gene expression of TNF-alpha and IL-6 were increased significantly with the Metformin and Houttuyniae Herba, Coptidis Rhizoma extracts combination groups. Conclusions: Metformin and Lonicerae Flos, Agrobacterium Rhizogenes extracts combination groups showed synergistic hypoglycemic effects by increasing AMPK and PPAR gene expression in RAW 264.7 cells.

Colonic Leiomyoma in Two Dogs (두 마리의 개에서 결장 평활근종의 증례)

  • Two dogs were referred with the clinical signs of depression and constipation, respectively. One dog with depression had hypoglycemia on biochemistry. On survey abdominal radiographs, a soft tissue density round mass with calcified spots was identified cranial to the urinary bladder. On ultrasonographs, a solitary oval heterogeneous complex-echoic mass with well-defined margin was found. On surgery, the mass was pedunculated and originated from the colonic wall. The resected mass was confirmed as colonic leiomyoma. The other dog with constipation had normal CBC and biochemistry values. On survey abdominal radiographs, there was a soft tissue density round mass dorsal to the descending colon and rectum. The descending colon was displaced ventrally and narrowed by the mass. On ultrasonographs, a well-defined round heteroechoic mass compressed the colon. The mass was also pedunculated and originated from the colonic wall on surgery and confirmed as colonic leiomyoma. Two dogs recovered without any clinical signs after surgery and showed permanent resolution of the clinical signs, respectively.

Preparation and Characterization of Self-assembled Glycol Chitosan Hydrogels Containing Palmityl-acylated Exendin-4 for Extended Hypoglycemic Action

  • Injectable chitosan hydrogels have attracted great potential due to sustained-release property and safety. Here, palmityl-acylated glycol chitosan (Pal-GC) was used to generate physically cross-linked hydrogels by virtue of hydrophobic attraction of linear fatty carbons. Glycol chitosan was chemically modified with N-hydroxysuccinimide-activated palmitic acid in dimethylsulfoxide (DMSO) containing dimethylaminopyridine. Through a series of preparation steps of (i) dialysis with DMSO, (ii) addition of palmityl-acylated exendin-4 (Ex4-C16), and (iii) dialysis with water, Pal-GC was self-assembled to form physically cross-linked hydrogels entrapped with Ex4-C16. The Pal-GC derivative was analyzed by using 1H NMR, and the surface morphology of Pal-GC hydrogels formed was examined by scanning electron microscopy. Also, the hypoglycemic effect induced by Pal-GC hydrogels containing Ex4-C16 (250 nmol/kg) was evaluated in non-fasted type 2 diabetic db/db mice and compared with GC hydrogels containing native Ex4 at the same dose. Results showed that palmityl group was successfully conjugated with the amines of glycol chitosan, and that Pal-GC efficiently generated the hydrogels formation. Moreover, Pal-GC hydrogels containing Ex4-C16 was found to greatly prolong the hypoglycemia duration (~ 4 days). This was due to the dual-functions of the palmityl groups present in both GC and exendin-4 such as hydrophobic attraction and plasma albumin-binding. We consider this new type of self-assembled GC hydrogels loaded with Ex4-C16 would be a promising long-acting sustained-release system with anti-diabetic property.

  • Congenital hypopituitarism is a possible cause of neonatal cholestasis, but the mechanism is still unknown. The pathogenesis of cholestasis may be due to hormone deficiency, which has effects on the physiological maturation of bile acid synthesis and transport. We experienced a case presenting with cholestasis and recurrent hypoglycemia associated with congenital hypopituitarism. Cholestasis resolved with thyroxine and hydrocortisone replacement therapy.

The Impact of Psychological Insulin Resistance on Self-Care Activities in Patients with Type 2 Diabetes Mellitus Undergoing Insulin Therapy (인슐린요법을 받는 제2형 당뇨병 환자의 심리적 인슐린저항성이 자가관리에 미치는 효과)

  • Purpose: The purpose of this study was to identify the level of psychological insulin resistance and self-care activities and to evaluate the factors affecting self-care activities in patients with type 2 diabetes mellitus undergoing insulin therapy to provide basic data for the development of educational programs. Methods: Data were collected through the interviews using a structured questionnaire from August 29 to October 20, 2017, from the patients with type 2 diabetes mellitus visiting the Diabetes Mellitus Center at H-General Hospital in J-city. The subjects were 168 patients who had been being treated via self-injection for at least three months after the start of insulin therapy. Data analyses were conducted using t-test, ANOVA, Pearson's correlation coefficients, and stepwise multiple regression using the SAS WIN 9.2 program. Results: The mean score of psychological insulin resistance was 61.25 (range 19-95) and the mean score of self-care activities was 53.19 (range 18-90). Self-care activities were significantly different by gender (t = -2.94, p= .004), perceived health status (F= 7.00, p< .001), and hypoglycemia during the last three months (t= -2.47, p= .015). Negative correlation was observed between psychological insulin resistance and self-care activities (r= -.33, p< .001). Self-care activities were significantly predicted by psychological insulin resistance, perceived health status, gender, and hypoglycemia during the last three months, and 19.0% of the variance in self-care activities was explained (F= 9.01, p< .001). Conclusion: Psychological insulin resistance in patients undergoing insulin therapy and its effects on self-care activities identified in this study will be useful in starting and maintaining insulin therapy in the future.

Effect of Alpha-2 Adrenergic Agonist on Beta Adrenoceptor-Nediated Control of Blood Glucose in the Fasted Mouse

  • Dose-dependent increasesin blood glucose were produced by epinephrine and clonidine in fasted male mice. Isoproterenol was ineffective in increasing blood glucose at lower doses ($10^{-8}M$/kg-$10^{-7}M$/kg); with higher dose ($10^{-6}M$/kg) the glucose level was increased. The hyperglycemia induced by epinephrine was inhibited by yobimbine, prazosin and propranolol, indicating that the hyperglycemic effect of epinephrine is mediated by alpha-1, alpha-2 and beta adrenoceptor. When clonidine (10$^{-6}$ M/kg) was administered simultaneously with sioproterenol ($10^{-6}M$/kg), an enhenced hyperglycemic effect was observed. The increment produced by clonidine plus isoproterenol was higher than that by clonidine alone. These increment produced by clonidine plus isoproterenol was higher than that by clonidine alone. These results suggest that stimulation of alpha-2 adrenoceptor may be reponsible for the exertion of the hyperglycemic effect by beta agonists in fasted mice.
