Hemodynamic Instability due to Adrenal Insufficiency after Open Heart Surgery (개심술 후 부신피질 결핍증에 의한 혈역학적 불안정)

  • Unexplained hypotension during a stay in the ICU is not uncommon in patients who underwent major surgery such as open heart surgery. When the cardiac output of patients is low, the heart may be the origin of this problem. If the heart function is normal, then we have to consider adrenal insufficiency as a possible cause of this hypotension. Adrenal insufficiency is a rare condition in the general population, yet patients who are under a stressful condition might experience adrenal insufficiency more frequently. We report here on a case of a patient who was in an unstable postoperative state with adrenal insufficiency after surgery and the patient dramatically recovered after the replacement of hydrocortisone.

  • The use of glucocorticoids (GCs) in the perinatal period is suspected of being associated with adverse effects on long-term neurodevelopmental outcomes for preterm infants. Repeated administration of antenatal GCs to mothers at risk of preterm birth may adversely affect fetal growth and head circumference. Fetal exposure to excess GCs during critical periods of brain development may profoundly modify the limbic system (primarily the hippocampus), resulting in long-term effects on cognition, behavior, memory, co-ordination of the autonomic nervous system, and regulation of the endocrine system later in adult life. Postnatal GC treatment for chronic lung disease in premature infants, particularly involving the use of dexamethasone, has been shown to induce neurodevelopmental impairment and increases the risk of cerebral palsy. In contrast to studies involving postnatal dexamethasone, long-term follow-up studies for hydrocortisone therapy have not revealed adverse effects on neurodevelopmental outcomes. In experimental studies on animals, GCs has been shown to impair neurogenesis, and induce neuronal apoptosis in the immature brains of newborn animals. A recent study has demonstrated that dexamethasone-induced hypomyelination may result from the apoptotic degeneration of oligodendrocyte progenitors in the immature brain. Thus, based on clinical and experimental studies, there is enough evidence to advice caution regarding the use of GCs in the perinatal period; and moreover, the potential long-term effects of GCs on brain development need to be determined.

  • Background: Celecoxib, a COX-2 specific inhibitor, has recently been used for the treatment of rheumatoid arthritis. However, the molecular and cellular mechanisms of celecoxib against RA inflammation remain to be defined. To elucidate the action mechanism of celecoxib on inflammatory cells, we investigated the effect of celecoxib on the production of two important mediators of inflammation, nitric oxide and PGE2 Methods: RAW 264.7 cells stimulated with LPS were preincubated with various concentrations of celecoxib (from $10^{-8}$ to $10^{-5}$ M) and $10{\mu}M$ hydrocortisone, respectively. The production of NO and PGE2, the end products of iNOS and COX-2 genes, were estimated in culture supernatants by Greiss method and EIA, respectively. The expression of iNOS gene, COX-2 gene, $NF-{\kappa}B$, and $I-{\kappa}B$ were determined by RT-PCR and western blot analysis. Results: Celecoxib and hydrocortisone inhibited the production of NO and PGE2 in dose dependent manner, when RAW 264.7 cells were stimulated with LPS. The expression of iNOS was also down-regulated by celecoxib and hydrocortisone. Interestingly, COX-2 gene differentially expressed according to the dose of celecoxib, a decrease with lower dose ($10^{-8}$ M) but an increase with higher dose ($10^{-5}$ M). $NF-{\kappa}B$ binding activity was decreased by lower dose of celecoxib, whereas was not affected by higher dose of it. The expression of $I-{\kappa}B$ was suppressed by higher dose of celecoxib. Conclusion: The celecoxib strongly suppressed the production of NO and PGE2 in LPS-stimulated RAW264.7 cells. The decrease of NO seems to be linked to the inhibition of iNOS by celecoxib. The lower and higher dose of celecoxib differentially regulated the COX-2 expression and $NF-{\kappa}B$ activity.

Effects of aquatic drugs and immune response in color carp, Cyprinus carpio, to Aeromonas hydrophila (Aeromonas hydrophila에 대한 비단잉어의 면역반응과 수산약제의 영향)

  • This study was carried out in order to investigate the immunosuppressive factor and immune response of color carp. The protection and serum antibody production of juvenile color carp aganist Aeromonas hydrophila were investigated on the effect of temperature differences and injected several aquatic drugs, i.e. Hydrocortisone, Oxytetracycline, Chloramphenicol and Ascorbic acid. The fish were injected intraperitoneally with 1mg/fish of HKC and FKC at three different temperature conditions as $16^{\circ}C$, $24^{\circ}C$, and $32^{\circ}C$ respectively. There were induced better protection and serum antibody production in the fish which had been kept at $24^{\circ}C$ than in the fish which had been kept at $16^{\circ}C$ and $32^{\circ}C$. The FKC immunized fish were followed 24 hrs later with intraperitoneal injection of 40mg/kg body weight of Hydrocortisone, 60mg/kg body weight of Oxytetracycline. 60mg/kg body weight of Chloramphenicol and 30mg/kg body weight of Ascorbic acid, respectively. The control fish were injected PBS only. The fish given the above aquatic drugs reduced serum antibody production level and protection rate when compared to control fish. As the results, immune response of juvenile color carp immunized FKC at $24^{\circ}C$ was more effective than $16^{\circ}C$ or $32^{\circ}C$ and immune response of juvenile color carp injected several aquatic drugs which was seemed to be immunosuppressive factor.

Effect of Mori Folium. on the anti-allergic inflammatory response (상엽(桑葉)이 항알러지 염증반응에 미치는 영향)

  • Objective : This experimental study was performed to examine the anti-allergic infiammatory effects of Mori Folium. Method : Macrophage 264.7 cells were pretreated for 1hour with Sangyup. After pretreatment, macrophage were incubated with lipopolysaccharide(LPS) $100ng/m{\ell}$ 12h and media collected and $TNF-{\alpha}$, $IL-1{\beta}$, IL-6, IL-10 concentrations in supernatants were measured each by Enzyme linked immuno-sorbent assay. Sangyup were used $1mg/m{\ell}$, $500{\mu}g/m{\ell}$, $250{\mu}g/m{\ell}$, $100{\mu}g/m{\ell}$, $50{\mu}g/m{\ell}$. Hydrocortisones were used $10^{-4}M,\;10^{-5}M,\;10^{-6}M,\;10^{-6}M,\;10^{-6}M$. Results : Macrophage 264.7 cells were pretreated with Hydrocortisones and Sangyup. After pretreatment, macrophage were incubated with lipopolysaccharide(LPS). To investigate the anti-inflammatory effect of Sangyup, we measured the amount of cytokines, and the results are as follows; 1. Mori Folium. showed statistically significant inhibitory effect on antiinflammation in $TNF-{\alpha}(p<0.02)$ in all five concentrations compared with the (-)controls treated with LPS. Mori Folium showed inhibitory effect on antiinflammation in $TNF-{\alpha}$ in similarpattern in all five concentrations compared with the (+)control pretreated with hydrocortisone. 2. Mori Folium. showed statistically significant inhibitory effect on antiinflammation in IL-6(p<0.01) in all five concentrations compared the (-)controls treated with LPS. Mori Folium. showed inhibitory effect on antiinflammation IL-6 similar pattern in all five concentrations compared with the (+)control pretreated with hydrocortisone. 3. Experimental Group pretreated with Mori Folium. showed statistically significant difference of antiinflammation in $IL-1{\beta}$ in concentrations of $50{\mu}g/m{\ell}(p<0.01)$, $250{\mu}g/m{\ell}(p<0.05)$ compared with the (-)control treated with LPS. Experimental Group pretreated with Mori Folium. showed increased level of $IL-1{\beta}$ in all concentrations compared the (+)control treated with hydrocortisone. 4. Experimental Group pretreated with Mori Folium. did not show statistically significant effect on IL-10 compared with the (-)control. Conclusion : By the findings of this experiment, Mori Folium is observed to have antiallergic and antiinflammation effect.

  • Cyclobuxine D, a steroidal alkaloid, was extracted from Buxus microphylla var. koreana Nakai. The effects of cyclobuxine D on carrageenin-induced pleurisy and croton oil-induced granuloma pouch in rats was investigated and compared with those of aspirin, hydrocortisone ana dexamethasone. Intrapleural injection of 2% carrageenin caused the accumulation of exudate. The rate of plasma exudation, measured by the exuded dye amounts for 20 min in the pleural cavity after intravenous injection of pontamine sky blue, showed a peak at 5 hr. Cyclobuxine D (5, 20 and 50 mg/kg, i.p.) suppressed dose-dependently the accumulation of the pleural exudate and the exudation of dye. Among several methods used for screening and evaluation anti-inflammatory agents, granuloma pouch technic introduced by Hans Selye (Hans seyle, 1953) is considered as a simple and reliable method. An air pocket was produced in the subcutaneous tissue of the interscapular region by injection of 1 ml of 1% croton oil as irritant. Inflammatory exudate accumulated in the pouch during the succeding 14 days. Cyclobuxine D (5 and 20 mg/kg) decreased fluid volume in pouch and weight of pouch wall in granulomatous inflammation.

A study on the development of hydrocortisone certified reference material in cosmetic cream using isotope dilution-mass spectrometry (동위원소희석-질량분석법을 이용한 화장품 크림 중 히드로코르티손 인증표준물질 개발 연구)

  • Steroids have a temporary skin improvement and whitening effect by controlling vasodilation, but they also cause side effects when used for a long time. Therefore, steroids were designated and managed as raw materials that cannot be used in cosmetics in Korea. However, steroids are continuously being detected in cosmetics, causing social issues. In this study, we developed a certified reference material (CRM) for the determination of steroids such as hydrocortisone in cosmetics. A cream-type cosmetic CRM was manufactured and subsequently certified following the guidelines outlined in ISO Guide 35. Homogeneity, short-term stability, and long-term stability were evaluated using isotope dilution mass spectrometry (ID-MS). The certified values were determined by using NIST's primary reference material to ensure traceability. From now on, we intend to supply the certified reference material as a cosmetic CRM to national and international companies, as well as research institutes after certification as certified reference material from KOLAS and registering on COMAR.

Antiinflammatory Action of Phytolacca Saponin

  • The antiinflammatory activity of Phytolacca saponin and phytolaccagenin isolated from the roots of Phytolacca americana has been investigated. When Phytolacca saponin and phytolaccagenin were given parenterally exhibited potent inhibitory effects on acute edema in rats and in mice. while oral administrtion required 6 times more dosage in comparison with that of intrapertioneal injection. The anti-exudative and anti-granulomatous activities of the saponin showed as much as the dose of 8 times higher than that of hydrocortisone. The saponin appeared to be of no effects on adrenal, however, showed a severe thymolysis at high dose.

  • Acetone-dried cells of Arthrobacter simplex were entrapped in several preparations of hydrophilic urethane prepolymers and their steroid converting ability was examined. SeVeral solvents, such as methanol and propylone glycol, wereeffective for the conversion of hydrocortisone to prednisolone. The stability of the immobilized cells during storage and on repeated reactions was also examined. Thisconvenient entrapping method was also applicable for the immobilization of cellular organelles. yeast peroxisomes. The entrapped peroxi-somessh owed the activities of alcohol oxidase and catalase.

  • Transport of drug entrapped in a liposome-hydrogel formulation was significantly retarded in an in vitro topical application. Liposomes containing hydrocortisone acetate, a hydrophobic antiinflammatory agent, were prepared by the precipitation method, and the liposomal suspension was mixed with hydrogel into a semisolid gel-type ointment.(omitted)
