Developement of the reinforced acrylic-based hybrid denture composite resin with vinyloligosilsesquioxane (POSS)

  • The mainly used polymeric material for the denture is PMMA because of its cost and easiness to handle. So it was widely used material among dentists for past decades. But the acrylic-based denture materials have several common weak points such as shrinkage after curing and lack of strength. In order to solve these problems, we adapted one of hybrid system using acrylic polymer and vinyloligosilsesquioxane(POSS). POSS, which is a well known expandable monomer during polymerization process, may eventually suppress volumetric shrinkage. And the hybrid system makes it possible for the polymer to be stable in various severe conditions. Eight different kinds of samples were designed and synthesized. Each samples were characterized with dynamic mechanical analyser(DMA) to confirm their thermodynamic properties, fractured to analyze the cross-sectional morphology of the samples. And elongation, flexural and impact tests were also executed to evaluate the mechanical properties of the samples. From the results, hybrid composites had well defined crosslinked network structure compared to the widely used denture materials, and the mechanical strength improved without changing any surface condition as increment with POSS ratio in hybrid system. Fractured morphology showed homogeneous surfaces in spite of mutli component system, therefore we can conclude that the adoption of the POSS brought the reinforcement of the denture resin.

DOUBLE CROWN RETAINED REMOVABLE PARTIAL DENTURE - 4 YEARS IN RETROSPECT (Double crown retained RPD의 4년 임상 관찰)

  • Statment of problem : Little in known about the importance of selection of various double crowns as retainer in determining the outcome of treatment with double crown retained removable partial denture. Purpose : To obtain information about the effects and the results of this treatment modality. Material and methods : This study describes 61 double crown retained removable partial dentures worn by 51 patients from Samsung Medical Center, Seoul a time ranging between 6 and 48 months and evaluate occlusal contacts on the denture teeth, denture movement, incidence of denture relining, denture retention, fracture of dentures and abutments, hygiene, residual ridge inflammatory changes, number of lost abutment,. interruption of denture use. Results : One tenth of all the restorations were relined. Restorations in 21 arches fractured repeatedly for various reasons . There was no apparent interrelationship between fractures and the five groups. Conclusion : Good prognoses of removable partial dentures were shown in all groups(Kennedy Classes I, II, III, combination and few remaining abutment).

A clinical case of hybrid telescopic double crown using friction pin with an isolated few remaining teeth (고립된 소수 잔존치 환자에서 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관 국소의치의 임상증례)

  • A few authors have reported good clinical results using double crown removable partial denture (RPD) with a few remaining teeth. Hybrid telescopic double crown is a good indication for a patient with poor periodontal condition and/or few remaining teeth after extraction, especially located in cross-arch position. In this case, there was a poor periodontal condition with teeth mobility. Several teeth with poor prognosis were extracted. Remaining anterior teeth was restored with fixed prostheses and edentulism was restored with Kennedy class I removable partial denture in maxilla. In mandible, it was hard to restore with clasp removable partial prostheses because of bilaterally isolated remaining teeth so that hybrid double crown removable partial denture with friction pin was suitable for this case. The objective of this report is to discuss the characteristics and the utility of hybrid double crown prostheses using a few remaining teeth in mandible.

  • Hybrid telescopic double crown have ever been good treatment option for patient with periodontally unfavorable few remaining teeth for successful prognosis. Tooth and implant combined telescopic double crown can be used for improving retention and support for denture with strategically placed implants on edentulous ridge. In this case, an 55-year-old female patient had chronic periodontitis with few remaining teeth on maxilla and fixed partial prosthodontics on mandible. Treatment of hybrid telescopic double crown with friction pin lasted 9.5 years only with natural teeth. After 9.5 years, additional implants was installed due to fracture of cast-posted abutment teeth. After implantation, tooth and implant combined double crown had fabricated. Through re-treatment, no complications of new denture has been found during 2 years follow up. This case presents fair prognosis of tooth and implant combined double crown denture in periodontally unfavorable condition.

  • This report presents two cases concerning strategies for improving denture retention in pediatric patients with oligodontia caused by hypohidrotic ectodermal dysplasia (HED). Both patients presented with multiple missing teeth, conical canines, alveolar bone atrophy, and a skeletal Class III tendency. In the first case, a modified form of conical-crown-retained denture was used to cover the canines. This approach was carried out without tooth extraction or coping. In the second case of severe alveolar bone resorption and a distally tilted lower left canine, the tooth was restored with a hybrid ceramic crown. It was subsequently converted into a clasp-retained removable partial denture, utilizing a suction mechanism. Both patients are currently receiving regular check-ups for the maintenance of their prosthetic appliances and the evaluation of their growth patterns. This study presents innovative prosthetic treatment methods for pediatric patients with HED who have inadequate denture retention.

Effect of Fiber Type and Combination on the Reinforcement of Heat Polymerized Denture Base Resin (섬유의 종류와 조합이 열중합 의치상 레진의 강화에 미치는 영향)

  • The aim of this study was to evaluate the effect according to the fiber type and combination on the reinforcement of heat-polymerized denture base resin. The heat-polymerized resin(Vertex RS, Dentimax, Netherlands) was used in this study. Glass fiber(GL; ER 270FW, Hankuk Fiber Glass, Korea), polyaromatic polyamide fiber(PA; aramid; Kevlar-49, Dupont, U.S.A.) and ultra high molecular weight polyethylene fiber(PE, polyethylene; P.E, Dong Yang Rope, Korea) were used to reinforce the denture base resin specimens. The final size of test specimen was $64mm{\times}10mm{\times}3.3mm$. The specimens of each group were stored in distilled water at $37^{\circ}C$ for 50 hours before measurement. The flexural strength and flexural modulus were measured by an universal testing machine(Z020, Zwick, Germany) at a crosshead speed of 5 mm/min in a three-point bending mode. In this study, all fibers showed reinforcing effects on denture base resin(p<0.05). In terms of flexural strength and flexural modulus, glass fiber 5.3 vol.% showed most effective reinforcing effect on heat polymerized denture base resin. For flexural modulus, PA/GL was the highest in denture base resin specimen for hybrid FRC using two combination (p<0.05). Glass fiber 5.3 vol.% and PA/GL are considered to be applied effectively in reinforcing the heat polymerized denture base resin.

Prosthetic rehabilitation for patient with hemi-maxillectomy: Obturator combined with a hybrid telescopic double crown using friction pin (편측 절제된 상악골 환자에서 하이브리드 텔레스코픽 이중관 의치를 이용한 구강 폐색기 수복 증례)

  • When oral cancer develops in the maxilla, oro-nasal communication occurs after surgical treatment including removal of the primary site. Restoration through an obturator is necessary to prevent food from storing due to non-oral opening, and to ensure proper pronunciation and aesthetic restoration. In this case, the patient was treated with right hemi-maxillectomy due to oral cancer and has residual abutment and poor periodontal support due to the effect of head and neck radiotherapy. The obturator was treated with a hybrid telescopic double crown denture. Reporting a successful prognosis in 18 months of follow-up.

  • The successful results of the treatment using a double crown denture have been reported in several papers for some of the few remaining teeth in patient. In particular, double crown dentures may be an alternative to treatment in cases where the periodontal status is poor and clasp type removable partial dentures are inappropriate (crown/root ratio > 1). In this case, the patient visited clinic with a chief complaint of difficulty in mastication due to loss of posterior teeth and overall teeth mobility. After teeth with severe periodontitis were extracted, treatment plan of mandible is Kennedy class I removable partial denture (RPD) and treatment plan of maxilla is hybrid telescopic double crown RPD with a friction pin. Patient is well adapted after the prosthetic treatment and we report due to achieving satisfactory results in pronunciation, mastication, retention and aesthetics.

  • The biomechanical prognosis of conventional removable partial denture is questionable in case of patient with few remaining teeth or periodontally compromised teeth. In this case, hybrid telescopic double crown RPD may be a successful treatment alternative. Hybrid telescopic double crown RPD has following advantages over conventional RPD: secondary splinting effect between abutments, more vertical stress direction and more convenient in repairing the denture after extraction of abutment tooth. In this clinical case, patient had deep overbite in anteriors and partially edentulous. The maxilla was restored with hybrid telescopic double crown RPD and the mandible was restored with implants and fixed prostheses. Long-term follow-up and supportive periodontal treatment were performed, and satisfactory results were achieved in terms of function and aesthetics.

A COMPARISON OF THE MASTICATORY FUNCTION BETWEEN TWO DIFFERENT TYPES OF IMPLANT SUPPORTED PROSTHESES AND COMPLETE DENTURE FOR FULLY EDENTULOUS PATIENTS

  • STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.
