Food Habits, Nutritional Knowledge, Nutritional Risk Factors and Health-Related Lifestyle of Korean Outpatients Aged Over 50 Years Taking Supplements

  • The purpose of this study was to investigate food habits, nutritional knowledge, nutritional risk factors, health-related lifestyle, health status and dietetic therapy in Korean middle-aged and elderly outpatients taking supplements. A cross-sectional survey was conducted from July to December in 2001. Subjects were 1702 (male 731, female 971) age-related chronic disease outpatients aged over 50 years. Data was collected using a standardized Questionnaire by in-person interview and analyzed by SPSS system. The subjects with supplement had significantly lower nutritional knowledge, higher nutritional risk factors, undesirable lifestyle, and lower self-reported health status compared to those without supplement. However, the subjects with supplement had desirable (cod habits and more concern about health compared to those without supplement. Age-related chronic disease group with supplement had significantly undesirable food habits, lower nutritional knowledge and higher nutritional risk factors compared to control group with supplement. Age-related chronic disease group without supplement had significantly desirable food habits, higher nutritional knowledge, lower self-reported health status and higher stress compared to control group without supplement. Therefore, these results may provide basic information for proper supplement of Korean middle-aged and elderly outpatients.

Enhanced Acid Tolerance in Bifidobacterium longum by Adaptive Evolution: Comparison of the Genes between the Acid-Resistant Variant and Wild-Type Strain

  • Acid stress can affect the viability of probiotics, especially Bifidobacterium. This study aimed to improve the acid tolerance of Bifidobacterium longum BBMN68 using adaptive evolution. The stress response, and genomic differences of the parental strain and the variant strain were compared by acid stress. The highest acid-resistant mutant strain (BBMN68m) was isolated from more than 100 asexual lines, which were adaptive to the acid stress for 10th, 20th, 30th, 40th, and 50th repeats, respectively. The variant strain showed a significant increase in acid tolerance under conditions of pH 2.5 for 2 h (from 7.92 to 4.44 log CFU/ml) compared with the wild-type strain (WT, from 7.87 to 0 log CFU/ml). The surface of the variant strain was also smoother. Comparative whole-genome analysis showed that the galactosyl transferase D gene (cpsD, bbmn68_1012), a key gene involved in exopolysaccharide (EPS) synthesis, was altered by two nucleotides in the mutant, causing alteration in amino acids, pI (from 8.94 to 9.19), and predicted protein structure. Meanwhile, cpsD expression and EPS production were also reduced in the variant strain (p < 0.05) compared with WT, and the exogenous WT-EPS in the variant strain reduced its acid-resistant ability. These results suggested EPS was related to acid responses of BBMN68.

North American ginseng influences adipocyte-macrophage crosstalk regulation of inflammatory gene expression

  • Background: Adipocyte-macrophage communication plays a critical role regulating white adipose tissue (WAT) inflammatory gene expression. Because WAT inflammation contributes to the development of metabolic diseases, there is significant interest in understanding how exogenous compounds regulate the adipocyte-macrophage crosstalk. An aqueous (AQ) extract of North American (NA) ginseng (Panax quinquefolius) was previously shown to have strong inflammo-regulatory properties in adipocytes. This study examined whether different ginseng extracts influence adipocyte-macrophage crosstalk, as well as WAT inflammatory gene expression. Methods: The effects of AQ and ethanol (EtOH) ginseng extracts ($5{\mu}g/mL$) on adipocyte and macrophage inflammatory gene expression were studied in 3T3-L1 and RAW264.7 cells, respectively, using real-time reverse transcription polymerase chain reaction. Adipose tissue organ culture was also used to examine the effects of ginseng extracts on epididymal WAT (EWAT) and inguinal subcutaneous WAT (SWAT) inflammatory gene expression. Results: The AQ extract caused significant increases in the expression of common inflammatory genes (e.g., Mcp1, Ccl5, Tnf-${\alpha}$, Nos2) in both cell types. Culturing adipocytes in media from macrophages treated with the AQ extract, and vice versa, also induced inflammatory gene expression. Adipocyte Ppar-${\gamma}$ expression was reduced with the AQ extract. The AQ extract strongly induced inflammatory gene expression in EWAT, but not in SWAT. The EtOH extract had no effect on inflammatory gene expression in either both cell types or WAT. Conclusion: These findings provide important new insights into the inflammo-regulatory role of NA ginseng in WAT.

  • To prevent the overflow of various harmful chemicals, it is necessary to modify the chemical management system with an expansion to institutionally regulated substances. This modification should be preceded by selection of the priority chemicals, with a diverse chemical ranking system (CRS) applied to select the chemicals in developed countries. In Korea, a systematic CRS was used in a project related to soil and groundwater, however, it is inadequate to compare soil and groundwater CRS to that of surfacewater. In this study, a priority chemical ranking system for surfacewater was proposed through the analysis of international and domestic CRS cases. This was then applied to 161 chemicals to derive the priority list of harmful chemicals. As a result, Chemical RAnking of surFacewater polluTants (CRAFT) is presented for the protection of human health and the aquatic ecosystem from surfacewater pollutants. The components of CRAFT are the human health toxicity, aquatic ecosystem toxicity and reliability assessment factors. Three lists were derived from the 161 priority harmful chemicals for the protection of human health, aquatic ecosystem or both. It is expected that this result can be useful to prioritize harmful chemicals for the protection of human health and the aquatic ecosystem from Korean surfacewater.

국내담수지역 인체위해성기반 준거치 산정에 활용되는 어류섭취량인자 타당성 평가 (Adequacy Evaluation of Fish Intake Parameter used for Human Health Risk Assessment to Derive Freshwater Quality Criteria in Korea)

  • Water quality criteria for human health protection are derived based on the human health risk assessment. Water quality criteria in Korean freshwater bodies have been derived according to the equations developed by the US Environmental Protection Agency. The equations include the fish intake parameter, which is very important factor that significantly influences on the criteria derivation. So far, several fish intake values were used in human health risk assessment for water quality standards and effluent standards. However, these values are not consistent and they refer to various sources. Therefore, there is a need to suggest the most appropriate value of fish intake parameter to derive freshwater quality criteria in Korea. In this study, national and international fish intake values were widely collected and evaluated to select the adequate value of fish intake parameter that can be applied in Korea. The USEPA presented fish intake parameter as the 17.5 g/day for general adults and sport fishers and 142.4 g/day for subsistence fishers. In Korean reports, wide range values of 2 to 67.7 g/day were suggested as fish intake value. These values included finfish and shellfish intakes in common but had various habits. This study found that the 52.4 g/day suggested in Korean Exposure Factors Handbook published by the Ministry of Environment in 2007 seemed to be the suitable fish intake parameter to derive the freshwater quality criteria in Korea. The value is based on water corrected intakes of finfish and shellfish present in freshwater and coastal areas. We expect that this report can be useful to select suitable fish intake value in human health risk assessment for establishing freshwater quality standard in Korea.

건설안전교육이 불안전한 행동에 미치는 영향에 관한 연구 (A Study on the Influence of Construction Safety Education on Unstable Behavior)

  • 연구목적: 건설업 기초안전보건교육이 실시된 지 10년이 지난 지금 더 이상의 보수교육이 실시되지 않고 있으며, 기초안전보건교육을 이수한 사람은 신규채용자교육도 면제되고 있어서 안전교육이 부실화되고 있다. 조속히 보수교육과 신규채용자교육의 법제화로 불안전한 행동의 유발을 방지하고 안전사고를 방지하기 위한 전략적 방안을 모색하고자 한다. 연구방법: 본 연구에서는 건설안전교육인 기초안전보건교육과 신규채용자교육이 불안전한 행동인 Human Error와 위반 행동과의 구조적 관계를 설문을 통한 회기분석을 실시하였다. 연구결과: 설문조사는 건설업 기초안전보건교육과 신규채용자 교육이 재해의 인적 요소인 Human Error와 위반 행동 감소에 많은 영향이 있는 것으로 분석되었고 지속적인 안전교육이 안전의식을 높일 수 있음을 확인하였다. 결론: 기초안전보건교육의 보수교육과 신규채용자 교육의 법 개정으로 안전교육의 부실화를 방지하여야 한다.

Human Error Probability Determination in Blasting Process of Ore Mine Using a Hybrid of HEART and Best-Worst Methods

  • Background: One of the important actions for enhancing human reliability in any industry is assessing human error probability (HEP). The HEART technique is a robust tool for calculating HEP in various industries. The traditional HEART has some weaknesses due to expert judgment. For these reasons, a hybrid model is presented in this study to integrate HEART with Best-Worst Method. Materials Method: In this study, the blasting process in an iron ore mine was investigated as a case study. The proposed HEART-BWM was used to increase the sensitivity of APOA calculation. Then the HEP was calculated using conventional HEART formula. A consistency ratio was calculated using BWM. Finally, for verification of the HEART-BWM, HEP calculation was done by traditional HEART and HEART-BWM. Results: In the view of determined HEPs, the results showed that the mean of HEP in the blasting of the iron ore process was 2.57E-01. Checking the full blast of all the holes after the blasting sub-task was the most dangerous task due to the highest HEP value, and it was found 9.646E-01. On the other side, obtaining a permit to receive and transport materials was the most reliable task, and the HEP was 8.54E-04. Conclusion: The results showed a good consistency for the proposed technique. Comparing the two techniques confirmed that the BWM makes the traditional HEART faster and more reliable by performing the basic comparisons.

  • Strengthening of the health system is a safety imperative, especially in a crisis as caused by the ongoing COVID-19 pandemic. While there is a need for enhancing the number and skill sets of the public health professionals, especially the frontline workers, it will be prudent to use the digital health technologies, including artificial intelligence, in enhancing the capacity of the healthcare professional education and delivery. However, it has to be ensured that an ethical and safe approach is adopted to develop and use digital health technology and, ethically appropriate training is imparted, to enhance the capacity of the human resources for health, leading to an overall health system strengthening.

Anterior Talofibular Ligament and Superior Extensor Ankle Retinaculum Thicknesses: Relationship with Balance

  • Purpose: This study determined if anterior talofibular ligament (ATFL)/superior extensor ankle retinaculum (SEAR) thicknesses are related to dynamic balance in individuals with chronic ankle instability (CAI). Materials and Methods: The subjects were 14 males and 15 females (age=24.52±3.46 years). Ankle instability was assessed using the Cumberland Ankle Instability Tool (CAIT) with a cut off score of 25 to define two groups. SonoSite MTurbo (Fugifilm Sonosite, Inc.) musculoskeletal ultrasound (MSKUS) unit was used to assess ATFL and SEAR thicknesses. Dynamic balance was measured with the Y Balance Test (YBT) and two NeuroCom balance tests. Results: There were no significant differences in the average ATFL thickness between stable and unstable ankles in those subjects with CAI (0.25±0.03 cm and 0.21±0.05 cm, respectively) or in the SEAR thickness (0.09±0.04 cm and 0.10±0.03 cm, respectively). There were also no significant differences in the right and left ATFL thicknesses (0.23±0.07 cm and 0.21±0.04 cm, respectively) or the SEAR thicknesses (0.09±0.01 cm and 0.09±0.01 cm, respectively) in those without CAI. There were no differences between limbs in composite scores on YBT in those with CAI (p=0.35) and those without CAI (p=0.33). There was a moderate correlation between the left SEAR thickness and the large forward/backward perturbations on the NeuroCom (Natus) motor control test (r=0.51, p=0.006 and r=0.54, p=0.003, respectively). Conclusion: There were no differences in the ATFL/SEAR thicknesses or balance measures between or within the groups, likely because CAI is multi-factorial and related to mechanisms other than tissue changes alone. More sensitive technology and a better definition of the measurement process may provide more definitive results.

소규모 다중이용시설의 실내공기질 실태조사 및 건강위해성평가: 민감군 이용시설을 중심으로 (Indoor Air Quality and Human Health Risk Assessment for Un-regulated Small-sized Sensitive Population Facilities)

  • Objectives: The purpose of this study is to investigate human health risk assessment of indoor air pollutants at small-sized public-use facilities (e.g., daycare centers, hospital and elderly care facilities) that the susceptible population is mainly used. Methods: To assess indoor air quality (IAQ), the concentrations of indoor air contaminants such as HCHO, benzene, toluene, ethylbenzene, xylene, styrene, PM-10, CO, $NO_2$ and $O_3$ in air samples were measured according to the Indoor Air Quality Standard Method. By conducting the questionnaire survey, the major factors influencing IAQ were identified. Human health risk assessment was carried out in the consideration of type of use (user and worker) at 75 daycare centers, 34 hospitals and 40 elderly care facilities. Results: As a result of measurement of indoor air contaminants, the average concentration of HCHO and TVOCs in hospitals was higher than daycare centers and elderly care facilities, about 8.8 and 23.5% of hospitals were exceeded by IAQ standard. In human health risk assessment, for the user of daycare centers and elderly care facilities, the mean carcinogenic risk of HCHO inhalation was higher than acceptable value. Except for HCHO, other values were determined under acceptable risk. Similarly, for the worker of hospitals, the mean carcinogenic risk of HCHO inhalation was higher than acceptable value and other values were evaluated under acceptable risk. In contrast, the risk levels of other contaminants measured in elderly care facilities were acceptable. In the determination of factors influencing IAQ, the construction year, building type, ventilation time, and the use of air cleaner were identified. Conclusions: This study provides the information for establishing the plans of public health management of IAQ at small-sized public-use facilities that have not yet been placed under the regulation. The findings suggest the consideration of human health risk assessment results for the IAQ standards.
