입원아동어머니의 질병에 대한 불확실성의 인지와 간호요구 (A Study on Mother's Perception of Uncertainty and Nursing Need Concerning Child's Hospitalization)

  • Purpose: The purpose of this study was to provide fundamental data in ensuring a high quality of nursing needed by hospitalized child's mother through research of the mother's perception of uncertainty and nursing need concerning child's hospitalization. Method: 173 hospitalized child's mothers at G hospital in Busan are questioned. The tools used in this study are PPUS(Parent's Perception of Uncertainty Scale) and the nursing need of hospitalized children's mother. The data was analyzed by SPSS/PC program. Results: 1) The perception of total uncertainty for the hospitalized child's mother showed average 2.37 and the nursing need showed very high level, 3.37. 2) The perception of uncertainty in accordance with general character of the hospitalized child's mother shows non significant difference 3) Significant correlation indicated between the perception of uncertainty of hospitalized children's mother and nursing need. Conclusion: It is to be desired to prepare a program which can raise the level of perception developing a method that is able to practically assess and manage the level of perception of hospitalized children's mother.

  • Purpose: This study was conducted to design services for improving the quality of care of hospitalized children with acute diseases. Methods: The service design process had four phases: discovery, definition, development, and delivery. The participants were 23 mothers of hospitalized children with an acute disease, and seven nurses and three doctors working at a pediatric hospital. Data were collected through self-report questionnaires, in-depth interviews, and observations. The data were analyzed using content analysis and descriptive statistics. Results: The participants reported needs for explanations about the treatment, skillful nursing, and environmental improvements. The concept of the services was familiarity and enjoyment, aimed at solving the problems of unfamiliarity and boredom. A six-guideline was presented for improving the quality of care of hospitalized children with acute diseases: improvement of awareness, development of educational materials, improvement of skills, environmental improvements, play activities, and evaluations of user satisfaction. Conclusion: These findings indicate that nursing services should deliver familiarity and enjoyment to hospitalized children and their families. The findings of this study emphasize that the service design methodology can be used to improve the quality of care of hospitalized children with acute diseases.

Development of Educational Content for Nurses to Prevent Safety Accidents of Hospitalized Children: Delphi Approach

  • Purpose: The purpose of this study was to develop content for safety education to prevent hospital safety accidents among hospitalized children and to investigate the status of safety education performed by nurses. Methods: First, the Delphi method was used, with 18 experts, to develop educational contents for preventing safety accidents. Second, an exploratory survey was performed of the actual status of safety education for preventing safety accidents among hospitalized children using a questionnaire developed based on the Delphi method. The participants of this study were 159 nurses with at least 6 months of work experience. Results: The educational content developed through the Delphi method for preventing safety accidents among hospitalized children contained seven domains (falls, injury, electric shocks/burns, suffocation/aspiration, poisoning/abuse, kidnapping, medical devices) with 44 topics. The item mean of nurses'perceptions of the importance of child safety education was 4.18, and the actual performance score was 3.72, which was a statistically significant difference (t=11.58, p<.001). Conclusion: These seven comprehensive domains of accident prevention education for hospitalized children are expected to be useful for interventions to support the safety of hospitalized children.

병원감염에 관한 판례의 동향 (Trends of the Precedent Case concerning Hospitalized Acquired Infection)

  • The Hospitalized Acquired Infection is defined as the case where the hidden infection or not found at the time of hospitalization occurs during the hospitalized period or, within 30 days to those who performed the surgery operation and then left the hospital. About 2/3 of the Hospitalized Acquired Infection are found as having the internal infection cases that are occurred by the patients' own virus due to the lowered immune system, while about 1/3 are found as having the external infection. The latter 1/3 of the external infection cases can be prevented through the infection management. And in case the new Hospitalized Acquired Infection case occur to the patient who was treated in the hospital, its responsibility issue will matter. As well in the disputes over the Hospitalized Acquired Infection cases, the cause-result relation between the damages and the medical staff's fault and as to whether there is failure of the medical staff or not. personnel should be proved in the medical-malpractice cases. In addition, the difficulties in proving such as expertise, secrecy propensity, discrete propensity and incompleteness will be considered to ease the burden of patient side's proving. Probability theory, Fact based assumption theory, Most adequate plaintiff preassumption or Expressed evidence theories are being discussed as the theories of eased burden of proof. In the result of gathering and reviewing Korea's precedent cases concerning the Hospitalized Acquired Infection, there are only a few accumulated prece dent cases and the attitude of the court also are also not consistent. Therefore, there are the precedents where the cause-result relation and the failure are immediately assumed when (1) timely proximity between the medical behavior and malpractice results, (2) proximity between the medical behavior-applied parts and the malpractice results-found parts, and (3) lack of other causes are separately evidenced; while the are the precedents only when 'the existence of the medical faults based on the common sense' is separately evidenced. It was found that the former and latter cases coexisted. The former is considered as based on the theory that separates the fault and cause-result relation not to consider them together, or regarded as based on the doubts that assumes the medical staff's neglect even though the Hospitalized Acquired Infection might be completely prevented by their efforts. However, the modern medical technology has the limitation as far as the prevention of the Hospitalized Acquired Infection. In conclusion, the assumption of the cause-result relation and that of the fault should be separately reviewed. Therefore, the latter precedents are considered as more reasonable, in the point the faulty behavior may be proved based on the common sense.

입원아동 어머니의 치료적 놀이 요구 및 어머니가 지각한 간호사의 치료적 놀이 수행정도 (Therapeutic Play Need and the Nurses′s Therapeutic Play Performances Perceived by Hospitalized Children′s Mothers)

  • The purpose of this study was to identify therapeutic play need and the degree of the nurses's therapeutic play performances perceived by hospitalized children's mothers, and then to compare the difference between the degree of the need and perceived performances. The subjects of this study consist of 121 hospitalized children's mothers. A questionaire for this study was 29 item likert type 4 point scale developed on the basis of literature review, hospitalized children's mother interview and researcher's clinical experiences. Data was analyzed by Paired t-test, t-test, ANOVA & Duncan's comparison. Results of this study are summarized as follows : 1. The degree of the therapeutic play need was measured to be high (3.20). The degree of perceived therapeutic play need by five components were followed as : physiologic suffering relief & enhancing play (3.39), growth & development facilitating play (3.26), instructional play (3.24), emotional well-being promoting play (3.18) and providing play tool & environment(2.97). 2.The degree of the nurse's therapeutic play performances perceived by mothers was measured to be low (1.60). The degree of perceived performances of the therapeutic play by the five components were followed as : physiologic suffering relief & enhancing play (1.87), instructional play (1.74), emotional well-being promoting play (1.64), providing play tool & environment (1.44) and growth&development facilitating play (1.42). 3. The difference between the degree of the therapeutic play need and nurese's perfor- mances perceived by hospitalized children's mothers were significant statistically (t= 38.54, p=.0001). Also, five components of therapeutic play were significant statistically (p=.0001). Therapeutic play has unique benefits among health care intervention for children. These findings will help in building of the theoretical framework of therapeutic play and enhancing the quality of nursing care for hospitalized children. Therapeutic play program for hospitalized children according to child growth & development and physiologic status are recommended to be developed.

입원 환아 어머니의 스트레스와 대처노력에 관한 연구 (A Study on the Stress and Coping Effort of Hospitalized Children's Mother)

  • Purpose: Assessment of stress and coping effort of hospitalized children's mother are very important factor to support and recover the children. The purpose of this study was to identify the stress and coping effort in mothers whose children are hospitalized. And analyzes the correlation between mother's stress and coping effort on the hospitalized children. Method: This study surveyed 70 mothers whose children are hospitalized. The data was collected for this study at one university hospital in Daejeon from June 1,2007 to July 10, 2007. The instruments used in this study were the mother's stress scale which was developed by Ok-Nam, You(1998), modified by Ho-Jin, Mun(2001), and the mother's coping effort were measured with the Coping Health Inventory for Parents(CHIP) developed by McCubbin and Patterson(1981). The data was analyzed by using SPSS program and include ANOVA, and Pearson's Correlation. Result: The mother's stress was average 2.42 out of a possible 4 points. Type of coping effort was in order of type III(cope with it by understanding medical circumstances)(M=2.73), type I(Cope with it through positive definition)(M=2.43), type II (cope with it by maintaining psychological stability)(M=2.28). Total coping effort according to general character of mothers whose children are hospitalized showed a significant difference in occupation(P<.05). Correlations between mother's stress and total coping effort was r=-.361(P<.05). Conclusion: This study based on nursing of stress and coping effort of hospitalized children's mothers. Pediatric nurses need to establish a stronger communication board and a relationship between medical staff and children's parents so that have supportive information. And to establish a support program that strengthens the coping effort of hospitalized children's mothers.

  • The purpose of this study was to investigate the level of uncertainty and anxiety in families of hospitalized children. Data were collected through self-report questionnaires which were constructed to include parent's perception of uncertainty and state anxiety. The subjects consisted of 126 families of hospitalized children in one university-affiliated hospital in Daegu. The data were analyzed by the SPSS program. The results were as follows; 1. The mean score of uncertainty was 64.70 (Range=31-95). The mean scores of subsets of the uncertainty were followed as: lack of clarity (2.59), unpredictability (2.46), lack of information (2.22) and ambiguity (2.14). 2. The mean score of state anxiety was 47.93 (Range=20-67). 3.The level of uncertainty was positively correlated to the level of state anxiety. 4. The level of anxiety was different depending on their religion and monthly income. The above findings indicated that the level of uncertainty and the state anxiety in families of hospitalized children were positively correlated. Therefore, nursing intervention for reducing uncertainty and anxiety and improving coping method should be provided for families of hospitalized children.

입원 아동의 가족중심돌봄 개념분석 (Family-Centered Care for Hospitalized Children: Concept Analysis)

  • Purpose: This paper is a report on the concept analysis of family-centered care for hospitalized children. Methods: The concept analysis approach of Walker and Avant was used. A search of multidisciplinary literature published between 1960 and 2016 was undertaken using the keyword 'family centered care' or 'family centered nursing' combined with hospitalized children. Attributes, antecedents, and consequences were inductively derived from the citations analyzed (n=19). Results: The attributes of family-centered care included (1) family respect, (2) collaboration, (3) family support, and (4) information sharing. These attributes are influenced by the 'willingness of family to participate', 'competency and willingness of staff,' and 'institution policy and system.' Additionally, family-centered care does significantly impact 'the health of the children', 'family empowerment' and 'work satisfaction and self-confidence of staff'. Conclusion: Family-centered care of hospitalized children as defined by the result of this study will contribute to the theoretical foundation for application in pediatric nursing practice.

  • Purpose: The purpose of this study was to develop a program promoting self-esteem and to determine the effectiveness of the program for hospitalized school-aged children. Method: The sample group consisted of 68 hospitalized children in a University Hospital located in Busan. The experimental group was given the program and feedback was obtained 3 times up until the day of discharge. Data were analyzed using t-test and 2-way ANOVA with the SPSS program package. Result: There was a significant difference between experimental and control groups in self-esteem. Conclusion: It was found that the program promoting self-esteem in school aged children was effective. The program, which was based on learning theory, was shown to be an effective and strong method to promote adjustment in hospitalized school-aged children.
