Background: Early identification of patients who are highly likely to develop severe illness among confirmed cases of coronavirus disease 19 (COVID-19) can be expected to lead to effective treatment. This study therefore aimed to determine whether the presence of nonalcoholic fatty liver disease (NAFLD) has an impact on the exacerbation of COVID-19 symptoms. Methods: The study used the Korean National Health Insurance claim data for treatment of COVID-19 patients in 2020. NAFLD includes nonalcoholic fatty liver (NAFL) and nonalcoholic steatohepatitis (NASH). The outcome variables used were hospitalization and the use of medical devices. Hospitalization was defined by a length of stay exceeding one day and the use of medical devices was defined as one or more uses of a ventilator or extracorporeal membrane oxygenation. Multivariable logistic regression analysis was performed to determine if there was a difference in the hospitalization and use of medical devices of COVID-19 patients depending on the presence of NAFLD. Results: The odds ratio of hospitalization was 1.059, indicating slightly higher odds of hospitalization for patients with NAFL or NASH compared to those without the conditions, but it was not statistically significant (0.969-1.156). On the other hand, the odds ratio of use of medical devices was high at 1.667 and was statistically significant (1.111-2.501). Conclusion: The study results found NAFLD to be a risk factor that can exacerbate symptoms in COVID-19 patients. Accordingly, it is necessary to identify NAFLD patients through preemptive screening and provide them with appropriate treatments.
Purpose: This study was done to identify the patterns and significant predictors influencing hospitalization of Korean men for alcohol use disorder. Methods: A descriptive study design was utilized. Data were collected using self-report questionnaires from 143 inpatients who met the DSM-5 alcohol use disorder criteria and were receiving treatment and 157 social drinkers living in the community. The questionnaires included Alcohol Use Disorders Identification Test (AUDIT), Alcohol Problems, Alcohol Expectancy Questionnaire (AEQ), Life Position, and The Korean version of the Children of Alcoholics Screening Test (CAST-K). Data were analyzed using descriptive statistics, t-test, ${\chi}^2$-test, F-test, Pearson correlation coefficients, and logistic regression with forward stepwise. Results: AUDIT had significant correlations with alcohol problems, alcohol expectancy, and parents' alcoholism. In logistic regression, factors significantly affecting hospitalization were divorced (OR=4.18, 95% CI: 1.28-13.71), graduation from elementary school (OR=28.50, 95% CI: 8.07-100.69), middle school (OR=6.66, 95% CI: 2.21-20.09), high school (OR=6.31, 95% CI: 2.59-15.36), drinking alone (OR=9.07, 95% CI: 1.78-46.17), family history of alcoholism (OR=2.41, 95% CI: 1.11-5.25), interpersonal relationship problems (OR=1.28, 95% CI:1.17-1.41), and sexual enhancement of alcohol expectancy (OR=0.83, 95% CI: 0.72-0.94), which accounted for 53% of the variance. Conclusion: Results suggest that interpersonal relationship programs and customized cognitive programs for social drinkers in the community are needed to decreased alcohol related hospitalization in Korean men.
This study investigates effcts of HMO internal structural arrangements on performance, specially cost reduction measured by hospitalization rate. This study formulates formalization, centralization measured by decision-making participation, differentiation, and coordination as structural factors, considering coordination as an intermediate factor between the rest of structural factors and hospitalization rate. The commonly used HMO types is assumed not effective in explaining performance differences. For the empirical test, I use bootstrap regression analyses with 48 HMOs. The results of the analyses show that HMO types fail to explain differences in hospitalization rate. However, dicision-making participation and differention effectively reduce hospiatalization rate, while frmalization increases hospitalization rate and coordination has nonessential effect on hospitalization rate. And, formalization and decision-making participation positively contribute to achieve coordination in HMO. These findings suggest that the theoretical framework derived from rational-citingency theory of formal organization better explains performance differences of HMOs than HMO types.
The purpose of this study was to investigate conception of children's hospitalization stress and to make clear concepts, and to use Walker & Avant's concept analysis method. Children's hospitalization stress attributes were derived from physiological changes, individual-environmental interactions, cognitive and psychosocial developmental stage responses, and intrinsic and outward changes due to coping styles. The prerequisites for hospitalization stress of children were disease severity, biological factors, and support system, and the results were hospitalization adaptation, disease and health recovery, improvement of child development, and acquisition of coping skills. In order to solve the prerequisites for Hospitalization stress of children, it is necessary to be aware of the disease and to establish a support system of parent-family. Afterwards, we suggest more qualitative research on the hospitalization stress of children, the development of tools that reflect the characteristics of the hospitalization stress of children, and the development of programs to solve the Hospitalization stress.
This study concerns whether the public spirit of medical services, which is the foundational goal of national hospitals, is being realized. To derive results regarding this question, it is necessary to identify the determinants that influence the choice of national hospitals. The data are based on the number of cases of medical service use among the data for 6 years from 2008 to 2013 in the Korea Medical Panel, and the subjects were limited to those who were 18 years old or older. In the final analysis, 14,365 cases of hospitalization service uses were employed. For the research method, Andersen's behavioral model theory was applied, and predisposing, enabling, and individual factors were determined to be variables. Logistic regression analysis was conducted to analyze the determinants of national hospital choice factors. It was found that the rate of use of national hospitals was very low, with 5.23% for hospitalization services. Furthermore, socioeconomically vulnerable people, such as the elderly, low-income people, the national meritorious and medical care beneficiaries, chronic patients, and disabled patients are more likely to choose national hospitals than private hospitals. Therefore, for the appropriate management of medical service use for low-income vulnerable groups, it is necessary to discuss the enhancement of primary medical care in national healthcare.
Purposes: This study has the purpose to the improvement of health promotion for local residents through delivery of high-quality medical service by improving imbalance of medical use and seeking an improvement plan for accessibility of effective medical service by understanding the pattern of medical use by region. Methodology: As for the method, this study derived results at the significance level of p<0.05 through chi square test(χ2 test) and Generalized Estimating Equation(GEE) SAS 9.4 version by using the data of the 7th Korean Longitudinal Study of Ageing 2018. Findings: Study results show that local residents use medical service such as hospitalization care and outpatient visit more as compared to the residents in Gyeonggi-do/large or medium-sized cities. The more the number of chronic disease, the more they select hospitalization care rather than outpatient visit. Results also show that patients engaged in labour tend to select outpatient treatment rather than hospitalization treatment. Meanwhile, severity of disease turned out to be higher amongst medical care beneficiaries than that of national health insurance patients when comparing the types of medical security. Practical Implications: In stead of solving the problem of the difference in medical use by region from the aspect of income and economic level, an integrative solution shall be provided putting viewpoint on the social phenomenon suited to the changes of the times. This study suggests a plan for using a health and medical community care center that acts as a gate keeper of regional medical service.
Personalized Patient centered nursing care is increasingly difficult to achieve despite the fact that it remains one of our consistent goals. So, we must find away to individualized nursing care. One means, to achieve this is by use of the nursing history form, which has been developed to help the nurse make maximum use of her limited time with the patient, by obtaining systematically the information needed to plan his nursing care. The nursing history form can be used to collect data about individual nursing needs but also it lends itself to the collection of epidemiological data relevant to the needs of patient population. So this study was undertaken in an attempt to describe the general characteristics of the population studied, to find out their perceptions and expectations related to their illness and hospitalization, to find out specific basic needs and to examine the relationship between the patients nursing needs and demographic characteristics through the responses to the nursing history questions. The study population defined and selected was all the patients (70) who were admitted to Yonsei University Hospital from October 1 - 15, 1975. The direct interview method was used and the data were categorized by the investigator, according to the nature of responses to each question and were subjected to the percentile and the chi- square tests. The findings can be summarized as follows 11. General characteristics of the study population ; The population was made up mainly of urban patients who were highly educated. The age was equally distributed. 2. Patients, perceptions and expectations related to illness and hospitalization ; 88.6% of the patients knew about the reason for hospitalization but 64. 5% could state symptoms only. 37.5% recognized the cause of illness. Approximately three fourth of the patients expressed on expectation for early recovery. 60.0% of the patient responses indicated, that they expected less than 10 days of hospitalization. Of the total responses regarding hospitalization, 45.7% were negative about the post -hospitalization expectation, 45.7% expected to return to work. As a result of these findings, we can see that there is a great educational need, a psychological need and environmental need for adaptation to the hospital and a socio- economic need for the post - hospitalization period. 3. Specific basic needs : The physical needs include the problem of getting sufficient sleep (50.0%), difficulty in food intake(47.1%), problems with hospital diet (47.0%), abnormal condition of the skin(44.3%), problems with bowel elimination(35.7%), assistance with bathing(35.7%), pain(30.0%), difficulty in walking(30.0%) , difficulty in seeing(30.0%) problems in urinary elimination(20.0%) , and difficulty in hearing(10.0%), 4. Nursing needs and epidemiological characteristics ; Age distribution was related to the rendition of the mouth but no significant differences were observed statistically with the patients responses to the other nursing history questions according to the epidemiological variables of age, sex, education and residence.
Objectives : This study was performed to examine medical care utilization of psychiatric patients and to explore patients' characteristics associated with extended hospitalization. Methods : Data were extracted from information of Korean Health Insurance Review and Assessment Service. All data associated with admission and outpatient clinic visit were analysed by patient characteristics. We selected first psychiatric admission patients who diagnosed mental and behavioral disorders due to use of alcohol (main disease code: F10), schizophrenia and related disorders (F20-29) and mood disorders (F30~33) from January to June 2005. We analysed status of admission, mean length of stay, regular access to outpatient clinic and rates of extended hospitalization during 3 years. Bivariate and multivariate analyses were conducted to identify factors associated with extended hospitalization. Results : The number of psychiatric patients during the first six month of 2005 was 30,678. The mean length of stay was longest for schizophrenia and related disorders but shortest for mood disorders. Patients who experienced an extended hospitalization were 18.8% of total subjects. An extended hospitalization was more common in schizophrenia and related disorders than other diagnostic groups. The factors associated with the extended hospitalization were age, sex, diagnostic group, type of insurance and medical care utilization groups. Conclusions : The study indicates the problem of an extended hospitalization for psychiatric patients in Korea. It is suggested that variations in rates of extended hospitalization among medical care utilization group may need an active early intervention system in psychiatric treatment service. Particular attention needs to be devoted to planning and funding for reducing extended hospitalization.

This study analyzed all medical institutions located in the Yeongbuk area of Gangwon Province by dividing the use of medical institutions by "including nursing hospitals" and "excluding nursing hospitals" from 2003 to 2017. As a result, high blood pressure, dementia, musculoskeletal diseases, small-scale mechanical diseases, and other diseases were accounted for in the use of hospitalization for the elderly (including nursing hospitals) in the Yeongbuk region.
