MRI Findings to Predict Neurodevelopmental Outcomes in Preterm Infants Near Term-Equivalent Age

  • Purpose: Preterm infants are at high risk for adverse neurodevelopmental outcomes. Magnetic resonance imaging (MRI) has been proposed as a means of predicting neurodevelopmental outcomes in this population. It is controversial whether diffuse excessive high signal intensity (DEHSI) represents damage to the white matter or delayed myelination in preterm infants. This study investigated MRI findings for predicting the severity of neurodevelopmental outcomes and assessing whether preterm infants with DEHSI near term-equivalent age have abnormal neurodevelopmental outcomes. Materials and Methods: Preterm infants (n = 64, gestational age at birth < 35 weeks) undergoing brain MRI near term-equivalent age and subsequent neurodevelopmental outcomes were evaluated between 18 and 24 months of age. The associations of MRI findings and the risk of severe cognitive delay, severe psychomotor delay, cerebral palsy (CP), and neurosensory impairment were analyzed. The associations of DEHSI with risks of severe cognitive delay, severe psychomotor delay, CP, and neurosensory impairment (hearing or visual impairment) were analyzed. Outcome data were evaluated by logistic regression and the Fisher's exact test. Results: There were significant associations between abnormal white matter findings and delayed mental development, delayed psychomotor development, neurosensory impairment, and presence of CP. The presence of DEHSI was not correlated with delayed neurodevelopmental outcomes or presence of CP. In multivariate logistic regression analyses, cystic encephalomalacia, punctate lesion, loss of white matter volume and ventricular dilation were significantly associated with CP. Conclusion: Abnormal MRI findings near term-equivalent age in preterm infants predict adverse neurodevelopmental outcomes. No significant association between DEHSI and adverse neurodevelopmental outcomes was demonstrated.

  • Injuries and infectious diseases have been the most important public health problems since the beginning of human life. Injuries result in death of about 30,000 people each year in South Korea. In terms of years of life lost, injuries are considerably more costly than either heart disease or cancer. In terms of cost - both the direct costs of care and the indirect costs to individuals, families, and societies of a diminished life-injuries are among the most expensive of all social problems. The main purposes of this study are (1) to describe the outcomes as well as treatment process of brain injured patients and (2) to identify the factors impacting on length of stay during hospitalization and hospital fees. The research method used in this study was to review the medical records of five hundreds brain injured cases using systemic random sampling. The multiple logistic regression was administered to identify the factors impacting on the outcomes. The results are as follow : (1) the consultation .ate was found to be 72.9% while referral rate was 11.2%; (2) nearly 30% of the respondents were hospitalized over 30 days; (3) multiple logistic regression analyses revealed that the determinants influencing LOS were number of consultations, number of lab tests, and surgery; (4) the determinants of hospital fee were severity of brain injury, gender of patients, number of consultations, number of lab tests, and surgery.

Factors associate with Musculoskeletal Symptoms in Small and Medium Sized Hospital Nurses (중소병원간호사의 근골격계증상 영향 요인)

  • Purpose: The purposes of this study were to identify musculoskeletal symptoms and to investigate factors associated with musculoskeletal symptoms in small and medium sized hospital nurses. Methods: The data were collected from 206 nurses working in Jeonnam. We used self administration questionnaires of KOSHA GUIDE H-9-2012 for musculoskeletal symptoms, PWI-SF for psychosocial stress, KOSS for job stress, and HPLP-II for health behaviors. The collected data were analyzed using the $x^2$, t-test, and logistic regression analysis in SPSS Ver. 23.0. Results: Sixty-six percent of the subjects had musculoskeletal symptoms. The shoulder had the highest symptom at 38.3%, followed by lower back at 32.5%, and neck at 29.1%. The mean and standard deviation score of psychosocial stress were $24.06{\pm}6.01$, those of job stress were $2.38{\pm}0.23$, and those of health behaviors were $2.14{\pm}0.39$. The items that were significantly different according to musculoskeletal symptoms were career ($x^2=6.67$, p<.036), one week overtime ($x^2=7.27$, p<.026), subjective health status ($x^2=4.29$, p<.038), and psychosocial stress ($x^2=7.99$, p<.010). In logistic regression analysis, career, and psychosocial stress were found to affect musculoskeltal symptoms. Conclusion: It is necessary to conduct preventive intervention to reduce musculoskeletal symptoms according to career and psychosocial stress for small and medium sized hospital nurses.

A descriptive statistical analysis of inpatients with lumbar disc herniation at a Korean medicine hospital in 2014

  • Objectives : This is a retrospective statistical analysis of the demographic and therapeutic characteristics influencing the continued improvement of symptoms in patients treated in 2014 for herniated lumbar discs at a Korean medicine hospital; thereby, providing clinical data to further improve medical services of Korean medicine. Methods : We investigated the demographic and therapeutic variables of all patients who were diagnosed with a herniated lumbar intervertebral disc and were hospitalized for more than 1 night at Dunsan Korean medicine hospital from January 1, 2014, to December 31, 2014. IBM SPSS 21.0 was used to conduct a logistic multiple regression analysis and a covariance analysis (ANCOVA) of the demographic and therapeutic variables collected from the electronic medical records and telephone surveys. Results : 1. A longer duration of hospitalization was significantly better for the maintenance of pain relief or a decrease in the pain after discharge. 2. Younger patients were significantly less likely to be treated with a Western medical treatment after discharge. 3. Most of the demographic and therapeutic variables were not statistically significant in regards to treatment for lower back pain since discharge. Conclusion : Some of the demographic and therapeutic variables had a positive effect on the prognosis at one year or greater in patients who received integrative Korean medical treatment for lumbar disc herniation. Continued and systematic research will be needed.

Study on Factors Affecting Nurses' Experience of Non-Reporting Incidents (병원 간호사의 사건보고 불이행 경험여부에 영향을 미치는 요인)

  • Purpose: This study was designed to identify the factors affecting the nurses' experience of non-reporting adverse incidents in hospital. Methods: This study is a cross-sectional, descriptive survey design and nonrandom, convenience sampling. Study subjects were 392 clinical nurses, who have agreed to be the subject of this research. The measuring instrument of attitudes toward incident reporting was developed by the authors. The questionnaire which consisted of 17 items about worry about appraisal, the belief in improvement, the intention of reporting, and knowledge was measured by 5-point Likert-type scale. The estimate of internal consistency was alpha =.84. Analysis of data was done with use of mean, t-test, ANOVA, logistic regression with SPSS program. Results: Clinical nurses had experience of reporting incident (51.3%), non-reporting incident (76.5%). Statistically, significant differences were found between experiences group and non experience group in intention on reporting, belief on improvement, and worry about appraisal. Logistic regression analysis showed that the significant predictors were caused by report no-fault cases, belief on improvement, worry about appraisal. Conclusion: The result also indicated that, to improve the incident reporting and risk management, it might be necessary to give a belief that it results on improvement and remove concern about punishment through construction of no-blame system.

Association between Physical activity and Depressive Symptoms among Korean Male Workers (일개 제조업 남성근로자에서 신체활동과 우울증상과의 관련성)

  • Objectives: This study was conducted to assess the association between level of physical activity and depressive symptoms among Korean male workers. Methods: A self-administered questionnaire was distributed and anthropometric measurements were conducted with 1,379 male workers during a general health check. From this data, we identified the general characteristics, levels of physical activity(IPAQ), and depressive symptoms(CES-D) of the study population. We conducted univariate logistic regression to verify the variables that affect depressive symptoms and multivariate logistic regression to calculate odds ratios. Results: Among the 1,379 male workers, 43.4% were in the insufficiently active group(IPAQ category 1) and 22.8% were in the depressive symptom group($CES-D{\geq}16$). The odds ratio(OR) for depressive symptoms was statistically higher in the insufficiently active group, workers aged over 30, the shift work group, and the unmarried group. Adjusted for those variables, the ORs for depressive symptoms were found to be 1.45(95% CI=1.25-2.11), 1.15(OR=1.15, 95% CI=1.08-1.99) and 1.75(95% CI=1.48-2.02), for the insufficiently active group, workers aged over 30, and the shift work group, respectively. Conclusions: We found physical inactivity, age, marital status, and shift work to be associated with depressive symptoms among male workers. This study supports the suggestion that physical activity can be helpful to reduce depressive symptoms among workers.

Obstetric Outcomes in 68 Pregnant Patients with Recurrent Pregnancy Loss on Oreintal Treatment and Analysis of Factors Affecting the Success of Birth (한방치료 후 임신에 성공한 반복 임신손실 환자 68례의 산과적 결과 및 출산 성공 영향 인자의 분석)

  • Purpose: To estimate the effect of oriental treatment on recurrent pregnancy loss, a retrospective analysis was done. Methods: Sixty-eight pregnant women at the $\bigcirc\bigcirc$ oriental clinic, Korea, from January 2005 to May 2009 diagnosed as recurrent pregnancy loss were included in this study. The sixty-eight patients received oriental treatment such as acupuncture, moxibution, herbal acupuncture and herbal prescriptions, divided into two groups: Group A- live birth(N=58) and Group B- abortion(N=10). The maternal age, parity, menstrual history, gynecological history and period of treatment were compared. To find out factors affecting the success of birth, we performed binary logistic regression analysis(SPSS ver. 14.0 for windows). Results: The live birth rate was 85.3%. The maternal age, parity, menstrual history, gynecological history and period of treatment were not different between two groups. Logistic regression analysis showed that the significant factors predicting the occurrence of miscarriage were advanced maternal age(${\geq}35$)(P=0.005, Odds Ratio[OR]=3.809, 95% Confidence Interval[95%CI]: 1.514-9.585) and suffering from gynecological problems(P=0.044, OR=4.048, 95%CI: 1.037-15.801). Conclusions: The results suggest that oriental treatment has effectiveness on recurrent pregnancy loss. Further study will be needed.

Breast Feeding Attitudes and Correlates of Intention of Breast Feeding of Mothers (모유수유 결정 관련요인에 관한 연구)

  • The purpose of this study was to examine the knowledge and attitude of breast feeding and to explore the predictive variables for the intention of breast feeding of mothers. One hundred and thirty-five mothers who delivered at the D University hospital during the period of May to June in 1996 comprised the sample. Data were collected by questionnaire methods before discharge at the hospital. Data were analyzed using percent, 1-test, and logistic regression. The results were as follows : 1. During their pregnancy, majority of mothers (74.8%) got the breast feeding information. Information sources were book (34.5%), family and relatives(32.4%), mass media(24.3%), and professionals such as nurses and doctors (8.8% ). The frequently reported sources of most encouragement for breast feeding were mother in law(20.7%) and baby's father (11.1% ). 2. The mean score of the items of Knowledge and Attitude toward Breast Feeding Scale were 42.56 (SD=5.47) and 39.07(SD=5.15) , representing positive attitude toward breast feeding. The correlation between knowledge and attitude score was significant(r 〓.54, p<.001). Knowledge of breast feeding were significantly different between breast feeding intention group (including partial breast feeding) and artificial feeding intention group(t=2.79, p<.01) 3. Logistic regression analysis revealed that feeding method in the hospital, delivery type, knowledge toward breast feeding, disease related to pregnancy, complication related to delivery, and educational level of mother were predictives of the intention of breast feeding. 4. The most frequently rated reasons for the plan for mixed feeding were concern about insufficient milk (37.9%) and work(27.6%), The major reasons for plan for artificial milk feeding were having premature baby(25.9%) and maternal health problems including infection(14.8% ) and drug use due to chronic illness (14.8%). From the result of the study, it is recommended to develop supportive nursing intervention strategy to promote breast-feeding intention and practice. The intervention could be more effective to begin early in pregnancy and include teaching for breast feeding skills as well as information provision for positive attitude formation.

Clinicopathologic correlation with MUC expression in advanced gastric cancer

  • Purpose: To investigate the relationship between MUC expression and clinicopathologic factors in advanced gastric cancer. Methods: A total of 237 tumor specimens were assessed for MUC expression by immunohistochemistry. The clinicopathologic factors were investigated with MUC1, MUC2, MUC5AC, and MUC6. Results: MUC1, MUC2, MUC5AC, and MUC6 expression was identified in 148 of 237 (62.4%), 141 of 237 (59.5%), 186 of 237 (78.5%), and 146 of 237 (61.6%) specimens, respectively. MUC1 expression was correlated with age, human epidermal growth factor receptor 2 (HER2) status, lymphatic invasion, Lauren classification and histology. Further multivariate logistic regression analysis revealed a significant correlation between MUC1expression and lymphatic invasion, diffuse type of Lauren classification. MUC5AC expression was correlated with HER2 status, Lauren classification and histology. Further multivariate logistic regression analysis revealed a significant correlation between MUC5AC expression and HER2 status, diffuse and mixed type of Lauren classification. MUC2 and MUC6 expression were not correlated with clinicopathologic factors. The patients of MUC1 expression had poorer survival than those without MUC1 expression, but MUC2, MUC5AC or MUC6 were not related to survival. In an additional multivariate analysis that used the Cox proportional hazards model, MUC1 expression was not significantly correlated with patient survival independent of age, N-stage, and venous invasion. Conclusion: When each of these four MUCs expression is evaluated, in light of clinicopathologic factors, MUC1 expression may be considered as a prognostic factor in patients with advanced gastric cancer. Therefore, careful follow-up may be necessary because the prognosis is poor when MUC1 expression is present.

Triggers and Outcomes of Falls in Hematology Patients: Analysis of Electronic Health Records (혈액내과 입원 환자의 낙상 위험 요인과 환자 결과: 전자의무기록 분석)

  • Purpose: The goal was to use electronic health records to identify factors and outcomes associated with falls among patients admitted to hematology units. Methods: This retrospective case-control study included data from a tertiary university hospital. Analysis was done of records from 117 patients with a history of falls and 201 patients with no history of falls who were admitted to the hematology unit from January 1, 2013 to December 31, 2014. Risk factors were analyzed using hierarchical logistic regression; patient outcomes were analyzed using multiple logistic regression, Cox proportional hazards regression, and multiple linear regression. Results: Clinical factors such as self-care nursing (OR=4.47, CI=1.64~12.11), leukopenia (OR=6.03; CI=2.51~14.50), and hypoalbuminemia (OR=2.79, CI=1.31~5.96); treatment factors such as use of narcotics (OR=2.06, CI=1.01~4.19), antipsychotics (OR=3.05, CI=1.20~7.75), and steroids (OR=4.51, CI=1.92~10.58); and patient factors such as low education (OR=3.16, CI=1.44~6.94) were significant risk factors. Falls were also associated with increased length of hospital stay to 21.58 days (p<.001), and healthcare costs of 17,052,784 Won (p<.001). Conclusion: These findings can be a resource for fall prevention education and to help develop fall risk assessment tools for adults admitted to hematology units.
