The Effect of Characteristics of Hospital Choice, Security and Hospital Service Quality Characteristics on Revisiting Intent (병원 선택 특성과 보안성 및 병원 서비스 품질 특성이 재이용의도에 미치는 영향)

  • The purpose of this study was to investigate the effect of characteristics of hospital choice and hospital service quality on service value, customer satisfaction and hospital revisiting intent and a survey involving patients and their caregivers who used the hospital before was conducted for empirical analysis. Thus, the theory and factors concerning the characteristics of hospital choice, hospital service quality, service value, customer satisfaction and hospital revisiting intent and relevant preceding research were explored after examining the preceding research, the research model was developed accordingly. A total of 459 collected questionnaires were analyze after carrying out the survey on a national scale. The findings of this study have significance since it identified the motivation that affect the choice of hospital and provide information. Also, this study could be utilized as reference data for hospitals to survive in the fierce competition, as it carried out factual survey on considerations when making choice of hospital. Furthermore, due to the low expectation of patients and caregivers for hospital administration, the study indicated that it's linked to a significantly low chance of customer satisfaction and provide the cause. This study was differentiated with other research on hospital choice since it selected security as a factor of hospital choice, though it's not discussed in the preceding research and moreover, conducted the empirical analysis. And the results of empirical analysis revealed that customers felt that service value was improved when the information security was strengthened. In other words, the strengthening of hospital information security could be hospital choice motivation, which means this study provided practical implication.

An application of datamining approach to CQI using the discharge summary (퇴원요약 데이터베이스를 이용한 데이터마이닝 기법의 CQI 활동에의 황용 방안)

  • This study provides an application of datamining approach to CQI(Continuous Quality Improvement) using the discharge summary. First, we found a process variation in hospital infection rate by SPC (Statistical Process Control) technique. Second, importance of factors influencing hospital infection was inferred through the decision tree analysis which is a classification method in data-mining approach. The most important factor was surgery followed by comorbidity and length of operation. Comorbidity was further divided into age and principal diagnosis and the length of operation was further divided into age and chief complaint. 24 rules of hospital infection were generated by the decision tree analysis. Of these, 9 rules with predictive prover greater than 50% were suggested as guidelines for hospital infection control. The optimum range of target group in hospital infection control were Identified through the information gain summary. Association rule, which is another kind of datamining method, was performed to analyze the relationship between principal diagnosis and comorbidity. The confidence score, which measures the decree of association, between urinary tract infection and causal bacillus was the highest, followed by the score between postoperative wound disruption find postoperative wound infection. This study demonstrated how datamining approach could be used to provide information to support prospective surveillance of hospital infection. The datamining technique can also be applied to various areas fur CQI using other hospital databases.

A Case Study of Health Information Service for Patient Users in Overseas Hospital Libraries (국외 병원도서관에서의 환자이용자를 위한 건강정보서비스 사례연구)

  • The purpose of this study is to suggest implications from the case of health information service for patients in overseas hospital libraries. A total of 89 overseas hospital libraries were selected, including general hospitals, specialized hospitals, women's hospitals, children's hospitals, and veterans' hospitals. The health information service that is provided in general and common in 89 hospital libraries was surveyed and the differentiated health information service was investigated next. As a result, first, it can be seen that the establishment of hospital libraries and the provision of health information services are common outside of Korea. Second, various human resources such as librarians, health information specialists, medical specialists, social workers, clinical librarians, health education specialists, and volunteers are utilized. Third, it provides not only print materials but also various information sources such as electronic materials, websites, pamphlets, brochures, and provides health information in various languages. Fourth, in providing health (information literacy) education and programs, services are provided through linkage with hospitals, local public libraries, and local communities. The implications for domestic hospital libraries are as follows: First, the change of awareness of the establishment of hospital libraries and the provision of health information services; second, the support of the curriculum and associations and the need for continuing education; third, it is necessary to link with related organizations for mandatory and diversification of health information services in hospital libraries.

Information Searching Behavior of Health Care Consumers by Types of Medical Institutions (의료소비자들의 의료기관 종별 정보탐색 행태에 관한 연구)

  • The purpose of this study was to evaluate the information searching behavior of consumer by type of medical institution. A questionnaire survey was conducted of 1,507 persons who were selected through a multi-stage stratified area cluster sampling in nationwide level, excluding Jeju-Do. Personal survey was conducted through door-to door survey from 27 July to 10 August 1999. The main results of this research was as following; 1. The proportion of information searching of respondents ranged from 91.5-95.2%. Even though the proportion of user in university hospital was slightly high, there was not significant statistically by type of medical institution. In terms of information source, personal informer was most common information source in all type of medical institution. Public informers were more frequently used in university hospital visitors and professional informer in general and university hospital visitors. 2. Comparing to searching intensity, user informer and professional informer's influences were more powerful, but not statistically significant. In analysis of unit influence for information source, written informer or public informer was more powerful in clinic visitor, professional informer and written informer in university hospital visitor. 3. Information which consumer want to know mostly were about on special potential and career of physician. The clinic visitor wanted to know about institutional location and kindness of medical personnel. The university hospital visitor also wanted to know about facilities and convenience of process. Comparing to institution selection criteria of consumers at 1991, quality related criteria were recognized more importantly in outpatient and dental services. But in case of inpatient services, convenience factor was recognized more importantly. In conclusion, the effort for specific marketing plan by type of medical institution should be needed. And more concern on information searching behavior of consumer will be needed.

Exploring the Appropriate Operation Ratio on Hospital Revenue Cost and Profit

  • Many previous researchers tried to analysis relationship between financial index of hospitals such as revenue, expenses, and profit and hospital outcome such as number of inpatient and outpatient or, between that financial index and hospital size including number of hospital beds. However, these studies did not find exact relationship between financial index and hospital efficiency and productivity. Therefore, purpose of the study explores exact relationship between hospital financial outcome and hospital efficiency and productivity using adjusted inpatient days concept from American Hospital Association. Through the empirical analysis, the researchers find that hospital profit has the U-shape quadratic function to operation ratio. 66.9% of operation ratio is changing point and hospitals with 55.8% through 75.0% of operation ration have experience deficit situation. Considering the hospital circumstance, Korean hospitals would be to maintain general hospital type with various specialty departments.

  • RFID is estimated to have great potential for hospitals to reduce various costs, reconsider patients' safety, and provide high quality medical service. As the ubiquitous technology allows the medical market to expand, medical centers all around the country are fiercely competing against one another. In order to increase the hospital's competitive edge by reducing IT expenses and concentrating on medical practice, the need to establish a u-Hospital using advanced IT technology is rapidly rising. This paper implements an RFID based u-Hospital system by using ubiquitous computing technology to design a location tracking device for all surgical patients, which can secure the patients' safety and increase the quality of medical service through interlocking hospital information systems.

  • Under the quickly changing health care environment in our society competitions among hospitals are getting harder and accordingly the hospital authorities do their best efforts to renovate their hospital management and let directors of food and nutrition services in hospital be seeking the drastic changes in their clinic-support operations. To attain this end it is essential to build an information system in food and nutrition services in hospital for practicing those operational changes efficiently. By building an information system we can totally manage a number of information about hospital food and nutrition services. This kind of information system can not only relieve dieticians and food-service workers from their repetitively routine jobs, but also connect with hospital management information systems organically. Resultantly productivity in this service area can be improved and the efficiency of hospital management will be increased. And accordingly the competitive advantage of the hospital can be greater than ever and that brings patients' and hospital employees' satisfaction. I would like to name this kind of information system for hospital food and nutrition services "TASTY", abbreviated from "Time-based Advanced Service Technology for Yong-Dong Severance Hospital, Nutrition Department" There are one basic information management area and five business management areas in TASTY. Five specific business areas are divided by menu, procurement, clinical nutrition service, production(including distribution and meal service), and financial management.inancial management.

  • Hospital managements are faced with severe competition and need more complicated information systems for its efficiency than before. Hospital managers have to deal with these environments and provide patients with better services than ever to survive. This thesis analyzes and redesigns the hospital administration processes in terms of patient-oriented view. For that purpose we define the processes of the hospital administration using process design tool like ARIS-Toolset. We also introduce basic concepts of Hospital Information Systems(HIS). We finally reviewed the result of reengineering of hospital management process with AHP methodology.

  • Although the accounting system is a very useful information system for manager, that of the National University Hospitals is not settled until now. Specially, when it will adapt the responsibility management system itself, it will more important tools for them. The purpose of this research is to make the standard accounting system for National University Hospital. It was surveyed the 7 National University Hospital's accounting system, and compared to that of Private University Hospital. The results are fellows; First, it must be match financial report with budgeting system. By comparing to each other, it can make to evaluate the performance of hospital. Second, it must make same format and contents in financial report among 7 hospitals. Specially, the cost information is needed to be made more details. This research suggest the new direction to make the basic information form. It was confirmed with financial manager in private university hospital. We hope it will contribute to make the new accounting system for National University Hospital.
