  • Construction of an obturator for rehabilitation of a patient who underwent a maxillectomy is vital. Routinely a constructed obturator includes denture portion. A patient who may present anatomical limitations due to surgical or radiotherapy complications often challenges the clinician. Purpose: This clinical report describes a patient with severe trismus after surgical resection and radiotherapy treatment of a tumor in the upper left maxilla. Conclusion: This report describes the concepts of using a rotational path insertion for an obturator and a separately constructed maxillary denture. The stability and retention of the obturator were obtained from anatomical features. Where as these were achieved through magnetic attachments and the remaining edentulous ridge to ensure esthetic and function of the prosthesis.

A Safe and Reliable Method for Installing Wireless LAN into a Hospital

  • A medical environment in which patient information can be accessed anytime / anywhere is called a "ubiquitous environment". To realize such an environment, the installation of wireless LAN is quite effective. Because the maximum radio wave output (antenna power) is set low in Japan, it has been easy to safely introduce wireless LAN into hospitals, to date mainly into large hospitals. However, if the placement of access points is not done properly, problems will occur, such as signals not reaching the desired area. A solution to these types of problems is to do an electromagnetic-field propagation simulation, which should be performed before construction of the hospital. It is also necessary to protect against security problems, such as signal interception or illegal access. We herein show our procedures for the safe introduction of wireless LAN.

  • this study is aiming to obtain an index and a standard assessment which will help in forecasting circulation (critical movement-line) & behavior of pediatric out-patients for each sickness for future children's hospitals construction planning. The study above was based on the analytical results of ordinary disease types of child patients and the circulation of pediatric out-patients by each sickness, along with the comparison study on relationship between these analytical results and hospital facilities.

  • Contemporary hospital has more and more extension because the growth and change of hospital has to respond the various needs. For expanding, the sky bridge(sky walk) becomes one of the method to interconnect two or more buildings on the air. Sometimes the designers don't know exactly how the sky bridge works after construction. Beyond the role of interconnecting two buildings, the role of sky bridge is transformed the main axis of pedestrian in the whole hospital organization. This scope of analysis is about the sky bridge of Hanyang University hospital. The first method is the investigation of the utilization in this time. The second is the analysis of design drawings got form the designing of the past(1994-1998). From this analysis, the conclusions are followings.;The designer who wants to apply the sky bridge should more consider about, 1) how the sky bridge impacts the whole spatial system such as the pedestrian, patients, physicians, service's facilities etc. 2) what the inside of sky bridge has something such as natural light, ventilation, and the height, depth, width of aisle, the sign for seeking orientation. etc. In future the research relating sky bridge should be studied some hospital as well as one hospital for comparing the characteristics of the concepts and organizations

  • Information concerning a patient visit is to be sent from the OCS(Order Communication System) to the PACS(Picture archiving and Communication System) and when the image acquisition is completed, information regarding the procedures also actually preformed should be sent back to the OCS. It should be possible to present related medical information from the OCS at the same image workstation in a coherent way with the images in the PACS, Examples of the related medical information are: the report of a diagnostic image procedure, data on medication, laboratory results, admission and discharge letters, and surgery reports, because we could maximize the efficiency of Radiological Work, like decrease examination time and human mistakes, though the integration of PACS and OCS, Therefore, We research some hospitals to find integral lists of PACS and OCS but there are no sufficient ingredients. Further, the percentage of Integral lists of PACS and OCS is different in all hospitals because there is no such standard yet like HL7(Healthcare Level 7) and DICOM(Digital Imaging and Communications in Medicine). Accordingly, if all hospital would follow national standard like HL7 and DICOM, the integration of OCS and PACS would be efficient but in this situation radiological technician should take part in construction integration system of PACS and OCS positively, so we could improve efficiency of radiological work and our status.

  • Purpose: Here is little information about China's operation department of general hospital in Korean medical architecture papers, which is inconvenient for scholars engaged in medical building research and comparison. Furthermore, Surgery is to provide patients with surgery and rescue sites, is an important technical department of the hospital, and then with the development of medical technology and people's trust in medical science, the role of the operating room will become increasingly important. Therefore, the purpose of this research is to analyze history, changes and definitions of the operating room and China's operation department of general hospital related laws and regulations for future research to provide effective analysis of data, and find shortcomings. Methods: This research was conducted according to the relevant laws, books, and papers of the operation department of Chinese general hospital. Results: At present hospital construction in China is in the period of rapid development and coupled with the growing medical technology, operating room layout and configuration is still in the continuous trial of continuous improvement stage. Overall, it requires more detailed guidelines to improve the quality of the operating room. Implications: Implications: It is expected that the research of this paper will provide an effective reference for the future research of the operation department of China General Hospital, which can promote and improve the work system of the operation department of China General Hospital.

A Study on the Risk Rate of Work Type According to the Fatal Accident Cases and the Work Strength in Construction Work (중대재해사례와 직업강도를 고려한 건축공사 위험성 평가)

  • Construction accidents have not decreased in spite of much effort such as new work methods, education and research related to safety works. Specially, many fatal accidents happened in construction works which involve the apartment, building, school, church, hotel, hospital, bank work and the other works. These accidents are mainly caused by unanticipated risk factors. From these reasons, this study researched fatal accidents which happened in construction works during last 13 years($1992{\sim}2004$) and analyzed the input workers and a work period of construction work. According to the input workers and a work period, the results are as following. During 13years($1992{\sim}2004$), the fatal accidents, related to the construction work, were happened to the 1,977 cases. These results were occupied the 21.32% of the total industry accidents. According to the result of the construction, the fatal accident rate of a concrete form work was the highest rate of 16.24% (321 cases) and a temporary work, a steel frame work was the each rate of 12.39%(245 cases), 10.07%(199 cases). Comparing to other work types, the fatal accident rate of those three work types(concrete form work, temporary work, steel work) was represented highly. We surveyed input workers and work period of construction work site. From the result of survey, input workers of a concrete form work were occupied with 13,720. The risk rate of the work type, which was considered input workers and work period, was represented 0.3622(a steel frame work), 0.1142(a temporary work), 0.0782(a tower crane) and 0.0772(a concrete work).

Validity and Reliability of the Nurses' Self-Concept Instrument (NSCI) for Registered Nurses in Long-term Care Hospitals (요양병원 간호사를 위한 전문직 자아개념 (Nurses' Self-Concept Instrument, NSCI) 도구의 타당도와 신뢰도)

  • Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean Version of the Nurses' Self-Concept Instrument (NSCI) geriatric hospital nurses in Korea. Methods: Bilingual nursing professionals performed translation and back-translation. Reliability and validity of the content and construction of the instrument were confirmed. Internal consistency reliability was determined. Construction and concurrent validity were verified using factor analysis and correlation coefficients. Results: The total 14 items for the Korean version of the Nurses' Self-Concept Instrument (NSCI) were retained through item analysis. In explanatory factor analysis, four subcategories were proposed with their names of each factor: 'Leadership', 'Staff relation', 'Knowledge', and 'Care'. The four factors accounted for 78.81% of the variances. The Cronbach's ${\alpha}$ regarding internal consistency were .77~.91 for the NSCI subscales. Correlation among four subcategories ranged .62~.84. Conclusion: The findings show that the Korean version of the Nurses' Self-Concept Instrument is reliable and valid for measuring professional Self-Concept of geriatric hospital nurses in Korea.

A study on the space composition and area planning of neonatal intensive care unit through the case of regional neonatal intensive care centers (신생아 집중치료 지역센터 사례를 통해 본 신생아 집중치료실(NICU) 공간 구성 및 면적 계획에 관한 연구)

  • Purpose: This study is to present basic data and standards for calculating the space composition and area required when planning a neonatal intensive care unit. Methods: Review domestic and foreign facility standards and regulations related to the current neonatal intensive care unit, select a regional neonatal intensive care center designated by the Ministry of Health and Welfare as a case hospital, and organize a space program for the construction plan of the neonatal intensive care unit through analysis of characteristics such as facility size and function composition. Results and Implications: The size, facility area, and detailed room composition characteristics of the neonatal intensive care unit in Korea were confirmed, and essential rooms and appropriate areas were derived when planning the construction of the neonatal intensive care unit. Korea's legal facility standards related to neonatal intensive care units are lower than the actual hospital status and overseas standards, and the facility standards of the medical law need to be improved.

The Effect of Working Time Reduction in Hospital Management (근로시간 단축에 따른 병원경영 영향 분석)

  • The study was designed to investigate what effect of working time reduction which will be gradually expanded to corporate size from June, 2004 could give to medical industries, and to provide basic information for hospitals and government to prepare the policy. 276 hospitals were surveyed about medical service income and manpower realities during the first half of 2003, using a structured survey tool. Response rate was 8% and 20 hospitals were finally analyzed. The effect of working time reduction in hospital management was different to the size of hospitals and the alternative. Income to existing service income was decreased by $2.2{\sim}4.6%$ in tertiary hospitals, by $3.2{\sim}5.7%$ in general hospitals with more than 300 beds, and by $3.7{\sim}6.0%$ in general hospitals with less than 300 beds. In preparation against such decrease in income, government is required to raise insurance payment, to calculate added service charge for day-off on Saturday forenoon, to retain emergency care payment, to expand emergency care facilities, to secure duty doctors, and to support middle and small sized hospitals. Hospitals are required to give self improving efforts such as fortifying of weekday care, development of weekend care program, strengthening of care capacity and function of emergency care center, and making manpower operation efficient.
