  • This paper has been studied a EMR Sharing System using HL7 Message and CDA Document. HL7 Message is a Transaction for clinical data sharing between hospital-based. The CDA for document exchange supports shared care between hospital-based and community-based physicians, knowledge integration by permitting external links to other documents, and outcomes research through the capture of discrete and coded clinical data. And then this paper has designed XML based CDA for document exchange and interactive Hospital-based Transaction based on HL7 Message.

  • In recent years there are so many medical informations that surgeons should know to handle or analyze their large amount of surgical cases. Proper use of computer system offers new opportunities for the storage and manipulation of their hospital informations. But little is reported about which system, is appropriate, how much can we do with such a system, or what kind of work can be done with that, especially in the area of Thoracic and Cardiovascular Surgery section. Authors designed a computer-based patient file management system using 16 Bit AT IBM personal computer and dBASE IV program, and developed a coding system for the diagnosis and operation name, which offers the basis for the classification of the surgical patient data. And the result of some experiences which was got from the total surgical cases of Thoracic and Cardiovascular Section, Seoul District Armed Forces General Hospital during past 5years, was described.

  • The concepts of modern type computer are so called "General purpose, stored program and digital computer" that is proposed by Charles Babbage. ENIAC, the initial operational electronic digital computer model, was produced in 1946. During the last 50 years, an epoch-making development of the personal computer was marked. The computerization of all levels of society is going on and also computerization of the general hospital and medical college is developing. But patient data management system for clinician is not used generally. We suggest the use of computer aided data management application programs for the clinical informations of the patients of the Department of Thoracic and Cardiovascular Surgery for better management and to make best of medical informations, to co-operate with the current of this times, and to prepare against the Hospital Information Systems[HIS], actively. Also, we suggest to standardize the format and structure of database files to store the clinical data of the patients By standardization of the database files, we can integrate and relate the data of the individual department or hospital, build up the regional or national statistics of the patients easily, and promote the generation of application programs. The medical network by the communication and computer would be utilized to collect the database files. And finally, we suggest the use of code system to input and search the informations about the diagnosis and operation such as the code system of International Classfication of Disease[WHO] and the table of the classfication of operation of the Ministry of Health and Social Affairs, Korea. In this article, we tried to show the new standards, the essential items for computerization of clinical informations of the patients of the Department of Thoracic and Cardiovascular Surgery.r Surgery.

  • Currently, many computer systems are used in many areas of medicine including radiation oncology. For the most part, the computer system has proved to be useful in radiotherapeutic planning and dose calculation. There has been attempts to develop computer system including information management of patients, patient tracing, and office automation in radiation oncology department. But some of these available commercial systems have shortcomings. We developed a management system of patients in our radiation oncology department that integrated most of items for the evaluation of patents. In particular, the data were stored in a natural language (noncoded) and made themselves easily understandable by all clinical groups. In addition , the data could be isolated in files from which the computer could generate graphs and static data by the use of some simple commands. The system provided us with not only the functions of case review but functions of preparation of conferences, lectures and resident teaching.

  • Laboratory information system (LIS) is a key tool to manage laboratory data in clinical pathology. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology.

  • This clinical note introduces a method to assist surgeons in performing single-tooth dento-osseous osteotomy. For use in this method, a surgical guide was manufactured using computer-aided design/computer-aided manufacturing technology and was based on preoperative surgical simulation data. This method was highly conducive to successful single-tooth dento-osseous segmental osteotomy.

  • In the medical fields, many efforts have been made to develop and improve Hospital Information System (HIS) including Electronic Medical Record (EMR), Order Communication System (OCS), and Picture Archiving and Communication System (PACS). However, materials generated and used in medical fields have various types and forms. The current HISs separately store and manage them by different systems, even though they relate to each other and contain redundant data. These systems are not helpful particularly in emergency where medical experts cannot check all of clinical materials in the golden time. Therefore, in this paper, we propose a process to build an integrated data model for medical information currently stored in various HISs. The proposed data model integrates vast information by focusing on medical images since they are most important materials for the diagnosis and treatment. Moreover, the model is disease-specific to consider that medical information and clinical materials including images are different by diseases. Two case studies show the feasibility and the usefulness of our proposed data model by building models about two diseases, acute myocardial infarction (AMI) and ischemic stroke.

Development of a Computer-assited Patients Menu Planning System for Hospital Dietetics (병원 영양과의 환자식 식단 관리 전산 프로그램 개발에 관한 연구)

  • The purpose of this study was to develop a computer-assisted patients menu planning program for hospital dietetics in order to improve the quality of menu for patients and to release managers from repetitive and routine tasks. Using this program, dietitians can spend more time on professional tasks. Moreover, few studies have been done on computer assisted patients menu planning for patients who need special diets for treatment in domestic hospital dietetics, therefore this program could be a great contribution. A 16-bit personal computer compatible with IBM-PC/AT was used. The data base files and processing program were created by Clipper package ver 5.0. This system can collect a number of meals, plan patients menu and computerize nutrient analysis. The future study will develop program(s) for purchasing, inventory control and data correction. The contents of computerized system are summarized as follows. 1. The number of daily meals of special and general diets given to the patients are collected and saved in database. These data were for the monthly list of meal census which could be printed out on the screen and/or the printer. 2. The menu planning was largely consisted of 2 sections. One was for the patients who require special diets and the other was general diet. And the special diets was divided into 6 sub-sections: diabetic, low-salt, low-fat, low-salt/low-protein, low-fat/low-cholesterol and low residue diets. 3. The nutrient analysis was composed of 11 diet. Sections and diebetic diet was divided into 9 sub-sections according to the calorie requirement. The calculated results were compared with the standards which were established by the hospital dietetic department.

  • Medical data is one of the data that must be kept in safe containers, far from intrusion, viewing and modification. With the technological developments in hospital systems and the use of cloud computing, it has become necessary to save, encrypt and even hide data from the eyes of attackers. Medical data includes medical images, whether they are x-ray images of patients or others, or even documents that have been saved in the image format. In this review, we review the latest research and the latest tools and algorithms that are used to protect, encrypt and hide these images, and discuss the most important challenges facing these areas.
