Lack of Correlations among Histopathological Parameters, Ki-67 Proliferation Index and Prognosis in Pheochromocytoma Patients

  • Background: In this study prognostic correlations of histopathologic parameters and the Ki-67 proliferation index and as well as the diagnostic value of immunohistochemical markers in pheochromocytomas were evaluated. Materials and Methods: A total of 22 patients diagnosed with a pheochromocytoma between 2000-2010 in Izmir Katip Celebi University Ataturk Training and Research Hospital were included. Diagnostic value of the PASS scoring system, and prognostic correlations of histopathologic parameters and Ki-67 proliferation index were investigated. SPSS for Windows 17.0 software was used for statistical analysis. Results: There was no statistically significant correlation between recurrence and clinicopathologic parameters or the PASS score (PASS>4). In addition, there were no statistically significant correlations between PASS score and clinicopathologic parameters, such as diameter (5 cm), weight (>100g), gender (female/male ratio) and age (25-45/45-55/>55). Besides, there were no significant correlation between diameter and clinicopathological parameters and also recurrence. However, there was a statistically significant correlation between Ki-67 proliferation index and capsule invasion (p=0.047). Conclusions: Some but not most of the findings in our study were concordant with the literature. To clarify relationships, investigations with standard scoring systems which are not affected by subjective factors and feature appropriate histopathological criteria should be made on larger study groups.

Primary Splenic Paraganglioma in a Dog

  • A 6-year-old intact female Shih-tzu dog was referred due to anorexia. Physical examination, complete blood count, serum chemical analysis, radiography, and ultrasonography were evaluated. Physical examination and hematological analysis showed normal findings. Abdominal radiographs and ultrasound revealed well-defined masses in the spleen. Other abdominal organs showed no significant abnormalities. Tissue samples taken via sono-guided fine needle aspiration of the splenic mass showed many bare nuclei, which were variable in size. Results of histopathological and immunohistochemical (IHC) analyses performed after splenectomy were consistent with paraganglioma. Based on these findings, we diagnosed this patient with a paraganglioma of splenic origin. Two months after splenectomy, abdominal ultrasonography revealed a new neoplastic lesion in the liver. The clients refused further management and the patient expired three months after initial diagnosis. Necropsy as well as histopathological and IHC examinations of other systemic organs including the liver, adrenal gland, kidney, brain, urinary bladder, lung, aortic body, carotid body, and pancreas were performed. The neoplastic tissue in the liver also demonstrated features of a paraganglioma, and there were no remarkable findings in all other organs.

  • Non-Hodgkin's lymphoma (NHL) constitutes a group of malignancies those arises from cellular components of lymphoid or extranodal tissues. The head and neck is the most common area for the presentation of these lymphoproliferative disorders. Primary involvement of salivary glands is uncommon. This report described a case of a 73-year-old female patient who presented with involvement of both nodal and extranodal sites, with predominant involvement of salivary glands. The tumor staging worked up along with imaging, histopathological, and immunohistochemical findings were discussed. Computed tomographic images showed the involvement of Waldeyer's ring, larynx, orbit, and spleen. This report described imaging and prognostic tumor markers in diagnosing, treatment planning, and prognosis.

하악에 발생한 횡문근육종 (Alveolar rhabdomyosarcoma involving the mandibular ramus and its surrounding tissues)

  • Rhabdomyosarcoma, when it occurs in the head and neck, is primarily found in children. Alveolar rhabdomyosarcoma is rarely seen in the oral lesion, comparing to the embryonal and the pleomorphic variants. This is a report of a case of alveolar rhabdomyosarcoma in the mandible in a ten-year old girl who complained of a non-painful swelling on the right cheek. The right lower 1st molar was mobile. Her radiographs revealed an extensive radiolucency with somewhat irregular border on the right mandibular ramus. The right mandibular 1st and 2nd molars lost their lamina dura and were floating. CT images revealed smooth-outlined soft tissue mass occupying the pterygomandibular space, the infratemporal space, and the masseteric muscle with thinning and perforation of the right mandibular angle and ramus. Histopathological and immunohistochemical findings established the final diagnosis of alveolar rhabdomyosarcoma.

경부 피부에서 발생한 저등급 근섬유모세포 육종 증례 보고 (A Case Report of Cutaneous Low-grade Myofibroblastic Sarcoma in the Neck)

  • Myofibroblastic sarcoma can be classified as low-, intermediate-, and high-grade. Low-grade myofibroblastic sarcoma (LGMS) is uncommon and rarely appears on the skin. LGMS is diagnosed based on histopathological and immunohistochemical findings. Additionally, LGMS metastases are rare as well. Herein, the authors describe a case of cutaneous LGMS in the neck. To the best of our knowledge, there are no cases regarding cutaneous LGMS in the neck present in the existing literature. Surgical resection is considered as the most important treatment for LGMS. Therefore, a complete tumor resection was performed in this case. However, another mass in the uterus was identified on abdominal computed tomography performed post-surgery.

Rapid-Growing Juvenile Xanthogranuloma on the Scalp in 18-Month-Old Girl

  • Juvenile xanthogranuloma (JXG) is an uncommon histiocytic cutaneous lesion. An 18-month-old girl visited our clinic due to rapid growing orange-yellowish lesion on scalp. Enlarging time from 1 mm to 12 mm was just 8 weeks. We excised the tumor and adjacent normal tissue. Histopathological study showed numerous eosinophils and Touton giant cells within the lesion. Immunohistochemical study revealed positive immunoreactivity for CD68 in most areas. No recurrence was seen during 12 months after resection. We report a case with rapidly growing JXG on scalp with peculiar histopathologic findings.

Histopathological evaluation of the lungs in experimental autoimmune encephalomyelitis

  • Importance: Experimental autoimmune encephalomyelitis (EAE) is an animal model of multiple sclerosis characterized by inflammation within the central nervous system. However, inflammation in non-neuronal tissues, including the lungs, has not been fully evaluated. Objective: This study evaluated the inflammatory response in lungs of EAE mice by immunohistochemistry and histochemistry. Methods: Eight adult C57BL/6 mice were injected with myelin oligodendrocyte glycoprotein35-55 to induce the EAE. Lungs and spinal cords were sampled from the experimental mice at the time of sacrifice and used for the western blotting, histochemistry, and immunohistochemistry. Results: Histopathological examination revealed inflammatory lesions in the lungs of EAE mice, characterized by infiltration of myeloperoxidase (MPO)- and galectin-3-positive cells, as determined by immunohistochemistry. Increased numbers of collagen fibers in the lungs of EAE mice were confirmed by histopathological analysis. Western blotting revealed significantly elevated level of osteopontin (OPN), cluster of differentiation 44 (CD44), MPO and galectin-3 in the lungs of EAE mice compared with normal controls (p < 0.05). Immunohistochemical analysis revealed both OPN and CD44 in ionized calcium-binding adapter molecule 1-positive macrophages within the lungs of EAE mice. Conclusions and Relevance: Taken together, these findings suggest that the increased OPN level in lungs of EAE mice led to inflammation; concurrent increases in proinflammatory factors (OPN and galectin-3) caused pulmonary impairment.

The Histopathological Examination for Diagnosis of MALT Lymphoma in the Stomach

  • Primary gastric lymphoma (PGL) is derived from mucosa-associated lymphoid tissue (MALT) and it differs from nodal lymphoma in histologic features and biologic behavior. Recent studies have showed that Helicobacter pylori (H.pylori ) infection is closely related to the development of low grade gastric lymphoma, and eradication of the infection induces regression of the tumor. H. pylori infection is known to be important to the development of gastric MALT lymphoma. The aim of this study was to elucidate the histopathological behavior of PGL according to the concept of MALT and to compare the predictive value of tests frequently used for diagnosis of H. pylori. The histological features of gastric lymphoma arising from MALT are the replacement of glands by uniform dense infiltration of centrocyte-like cells in the lamina propria and lymphoidepithelial lesion. H. pylori-associated histologic changes of neutrophilic infiltration, lymphoid follicle or aggregates formation and intestinal metaplasia, and H. pylori immunoreactivity were analyzed. Detection of H. pylori in chronic active gastritis and peptic ulcer suggests a possible role of H. pylori in the pathogenesis. Giemsa, Toluidine blue and Long H&E stains were used in H. pylori detection. Histopathological examination of gastric biopsy specimens revealed lymphoepithelial lesions pathognomonic of MALT lymphoma, and immunohistochemical staining for CD20 was diffusely positive. CD3 was positive in reactive T cells. PAX-5 was negative except the follicle. Bcl-2, cytokeratin, Ki-67, and c-myc were positive. The findings may indicate a predictable transition of low grade to high grade, and c-myc may be used as a valuable marker before molecular pathology diagnosis.

페커리 (Collard peccary)에서 발생한 유선 편평 세포 선암종 (Mammary Squamous Cell Carcinoma in a Collard Peccary (Tayassy Tajacu))

  • 10년령의 암컷 collared peccary (Tayassu tajacu)가 후복부의 종양을 주증으로 내원하였다. 신체 검사상에서, 우측 하복부에 위치하고 있는 단단하고 고정되어있지 않는 종양 (15 ${\times}$ 9 cm)이 확인되었다. 흉부 방사선 검사 상에서 경계가 명확한 결절이 관찰되었으며, 이는 폐 전이 소견으로 보여졌다. 본 증례는 조직검사와 면역조직검사를 통하여 폐 전이를 동반한 유선 편평 세포 선암종으로 진단되었다. 야생 동물에서, 유선 편평 세포 암종은 매우 드문 유선 종양이다. 페커리는 진단 한달 후에 폐사하였다. 본 증례 보고는 페커리에서 처음 보고되는 유선 종양 케이스이며 두번째로 보고되는 폐 전이성 선암종이다.

개 전염성 기관기관지염에 대한 modified live vaccine의 방어효과 (Effects of the modified live vaccines against Bordetella bronchiseptica and canine parainfluenza virus)

  • The purpose of this study was to investigate the protective effects of the modified live vaccines against canine Bordetella bronchiseptica (Bb) and canine parainfluenza virus (CPIV) in puppies by nasal administration. A total of 24 puppies were classified as 3 groups consisting of 8, and each one was divided into two subgroups; vaccinated (n=4) and unvaccinated (n=4). Group I, group II and group III were challenged with Bb, CPIV, and Bb+CPIV, respectively. In group I vaccinated puppies (n=4) were experimentally challenged with Bb 2 weeks after vaccination and unvaccinated puppies (n=4) were experimentally challenged with Bb alone. The same methods of the above were applied for group II and group III. We carried out several studies including serum tests, isolation, and histopathological examination. The vaccinated puppies showed higher antibody titers than unvaccinated puppies and the titer sustained during the experimental period. In the isolation test, recovery period was shorter in the vaccinated subgroup than in the other. In clinical signs, the unvaccinated puppies showed the typical signs of tracheobronchitis (coughing, nasal and occular discharge), but another subgroup showed delayed incidence and mild clinical signs. The typical gross lesions and histopathological findings were observed in the unvaccinated puppies. In immunohistochemical findings, the vaccinated puppies showed little intensive in reaction for CPIV antigen than the other. It could be concluded that intranasal vaccination of modified live Bb and CPIV vaccine to puppies is effective to prevent canine infectious tracheobronchitis.
