좌신 절제후 발생한 지연성 횡격막 탈장 2례 (Delayed Diaphragmatic Hernia after Left Nephrectomy -2 cases report-)

  • Since diaphragmatic injuries are difficult to diagnose, those that missed may present with latent symptoms.Delayed diaphragmatic hernia is very rare and occurs commonly after penetrating or blunt trauma. Recently we had experience with two cases of delayed diaphragmatic hernia after left nephrectomy.They were operated by herniolysis and closure of diaphragm using prosthetic patch[Dura, Dacron through the left thoracotomy. Postoperative courses were uneventful.

Small Bowel Hernia due to a Blunt Pelvic Injury

  • A 91-year-old female presented to Chonnam National University Hospital Regional Trauma Center with a lateral compression type III fracture of the pelvis. She was managed non-operatively for a week in the intensive care unit under close observation and had an emergency operation due to delayed onset of an acute obstructed direct inguinal hernia. Traumatic abdominal wall hernias are rare. However, trauma surgeons should always be aware of the possibility of such injuries because of their critical consequences.

선천성 식도폐쇄 수술 후 열공탈장의 호발과 그 유발인자 (High Incidence of Hiatal Hernia in Esophageal Atresia and Its Etiologic Factors)

  • Hiatal hernia is a very rare disease in the pediatric population. However information from our esophageal atresia postoperative follow-up program has hypotheses; "Hiatal hernia may more frequently occur in postoperative esophageal atresia patients (EA group) than in the general pediatric population (GP group)" and "The tension on the esophagus after esophageal anastomosis may be an important etiologic factor of hiatal hernia in EA group". To prove the first hypotheses, we compared the incidence of hiatal hernia in the GP group with the incidence in the EA group. The Incidence in the GP group was obtained from national statistic data from Statistics Korea and Health Insurance Review and Assessment Service of Korea. The incidence in the EA group was obtained from the medical record and the imaging studies of our esophageal atresia postoperative follow-up program. To prove the second hypothesis, the presumptive risk factors for the development of hiatal hernia in EA group, such as the type of esophageal atresia, degree of esophageal gap, the stage operation and the redo-operation with resection and re-anastomosis of esophagus were analyzed statistically. The total number of patients in the EA group was ninety-nine and there were 5 hiatus hernias. The incidence of EA group (5 %) is significantly higher than incidence of GP group (0.024 %). (p=0.0001) The statistical analysis of the presumptive risk factors for hiatal hernia development in EA group failed to show any evidence of correlation between postoperative esophageal tension and the hiatal hernia. This study shows that the postoperative patients with esophageal atresia have high occurrence of hiatal hernia and should be followed up carefully to detect hiatal hernia.

개에서 발생한 횡격막 탈장 4례 (Diaphragmatic Hernia in Dogs: 4 cases)

  • A diaphragm is a musculotendinous partition that separates abdominal and thoracic organs and assists in ventilation. Three dogs were presented after being hit by a car, and one dog was presented with the history of respiratory problems including intermittent dyspnea and coughing. Survey thoracic radiography, positive contrast celiography and sonography revealed traumatic diaphragmatic hernia in three dogs and congenital peritoneopericardial diaphragmatic hernia in one dog. In three dogs including a dog with congenital peritoneopericardial diaphragmatic hernia, herniorrhaphy was performed. Among the three dogs underwent surgery, two dogs with traumatic diaphragmatic hernia recovered uneventfully. However the dog with congenital peritoneopericardial diaphragmatic hernia died from pulmonary edema at 2 days after surgery. Positive-contrast ceilography and ultrasonography provides to investigate the integrity of the diaphragm. Chronic diaphragmatic hernia and concurrent injuries are considered as prognostic factors.

소아 서혜부 탈장에 대한 임상적 고찰; 단일병원 10년간 1244예 후향적 고찰 (Clinical Study of Inguinal Hernia in Pediatric Patients)

  • This study is a retrospective analysis of 1244 cases of the inguinal hernia in children under the age of fifteen years who were operated at the department of pediatric surgery, Inje University Busan Paik Hospital from March, 1997 to February, 2007. The ratio of male to female was 3.6:1. The type of hernia was indirect in all of the cases. The hernia was on the right side in 656 cases (53.9 %), left side in 467 cases (37.5 %), and bilateral in 121 cases (9.7 %). The hernia presented most frequently in infants under age 12 months; 364 cases (29.2 %). Fifty-nine cases (21.7 %) were in female and 305 cases (31.3 %) in male. There were 428 cases (33.6 %) in 1-3 years age group, 295 cases (23.7 %) in 4-6 years, 112 cases (9.0 %) in 7-9 years, 39 cases (3.1 %) in 10-12 years and 16 (1.2 %) in 13-15 years. The content of hernia sac was small bowel (59 %), omentum (31 %) in males and the ovary and tube (54 %) and small bowel (26 %) in female. The incidence of combined operation at the time was 3.2 %, and consisting of orchiopexy (67.5 %), frenulotomy (12.5 %), appendectomy (10 %), circumcision (5 %), and fistulotomy (5%). The incidence of combined disease was 2.8 % and consisting of undescended testis, Hirschsprung's disease, idiopathic hypertrophic pyloric stenosis, imperforate anus, and congenital heart disease. After unilateral inguinal hernia repairs, contralateral hernias developed in 34 patients. The laterality of the primary site of hernias were left in 19 cases (55.8 %), and right 15 cases (44.1 %). The 936 cases (75.2 %) were operated under general anesthesia; Mask bagging 663 cases (53.2 %), endotrachea intubation 257 cases (20.6 %), and laryngeal mask 16 cases (1.2 %). The remainder 308 cases (24.7 %) were operated under regional caudal anesthesia.

개에서의 선천성 복막-심낭 횡경막허니아 1예 (Congenital Peritoneopericardial Diaphragmatic Hernia in a Dog)

  • A 5 kg, seven-month-old, female Pekingese dog was presented to Seoul National University Veterinary Medical Teaching Hospital with the history of anorexia and exercise intolerance. Muffled cardiac sound and mild abdominal pain were detected in the physical examination. In positive contrast peritoneography, contrast medium was observed in enlarged pericardial sac through the diaphragm. According to the history taking, physical examination and contrast radiographic study, the dog was diagnaosed congenital peritoneopericardial diaphragmatic hernia. Following midline celiotomy, herniated falciform ligament and greater omentum were repositioned to abdrminal cavity. The diaphragmatic defect was closed with absorbable suture. Clinical signs related to peritoneopericardial hernia disappeared immediately after surgical treatment. There had been no evidence of recurrence of the peritoneopericardial hernia for 1 year.

성인에서 발생한 Bochdalek Hernia 수술치험 1예 (A case of bochadalek hernia in adult)

  • Bochdalek hernia in adult is relatively uncommon. We experienced a surgical case of Bochdalek hernia in a 29 year old housewife. She was admitted via E-R due to left chest pain and nausea for 1 months. 5 days before admission, she had Cesarean section on private hospital. After delivery, nausea and exertional dyspnea were aggravated. On admission, chest PA showed herniated stomach, colon, small intestine on left thorax. She was diagnosed of diaphragmatic hernia and operated. Operative findings were as follows: 1. hypoplastic: lower lobe of left lung, 2. stomach, colon, small intestine, spleen were herniated, 3. the defect was located on posterolateral portion of diaphragm, measuring 10 x 8 cm in size. Herniated intestines were reduced and diaphragmatic defect was repaired as interrupted sutures with 2-0 Mersilene. Postoperative courses were uneventful and the patient was discharged on POD 10th.

식도주위 열공 탈장 -1례 보고- (Paraesophageal Hiatal Hernia)

  • We experienced a case of congenital paraesophageal hiatal hernia[Type;IV in seventeen day-old female and treated through the right thoracotomy, reduction of the herniated viscera ,stomach and some part of transverse colon and omentum by gentle finger push, and narrowing the esopahgeal hiatus. Paraesophageal hiatal hernia accounts for only 5% per cent of all diaphragmatic defects but is a potentially dangerous lesion due to compressed lung by the herniated viscera. Symptoms are related to this, including exertional dyspnea, vomiting, cough, Tachypnea but noncyanotic, etc. Barium study shows that the stomach has herniated into the right pleural cavity. The speckled appearance in the herniated stomach in the herniated stomach was due to food material. It strongly suggests paraesophageal hiatal hernia. The operation was done. We report the case with the brief review of literatures.

임신으로 인한 횡격막 탈장 1례 (A case of diaphragmatic hernia associated with pregnancy)

  • A twenty three year old, Primigravida and 32 week pregnant woman who has been complained dyspnea, chest pain, nausea and vomiting was admitted to this chest surgical department on Feb. 19, 1979. Physical findings were those of acutely ill appearance, decreased thoracic excursion and absence of breath sounds in the left hemithorax. Roentgen examination of the chest revealed reticular cystic densities in the left, particularly in lower lung field with collapse of the left lung. Correction of the diaphragmatic hernia was carried out with reduction and repair of the hernia through transperitoneal approach. On exploration, the defect of the diaphragm was 12 x 12 cm in size and was located posterolateral area of left diaphragm. Hernia contents were stomach, spleen, omentum and splenic flexure of large bowel. The baby was normal full term spontaneous delivered at 36th POD. Diaphragmatic hernia complicated by pregnancy is a rarity and mortality is extremely high. Therefore, the literatures have reviewed and the case is reported.

Morgagni 탈장증 수술 1예 (Surgical Treatment of the Morgagni Hernia: report of one case)

  • Morgagni hernia is a rare condition of the congenital diaphragmatic hernia which Is located at the anteromedial portion of the diaphragm, and is located immediately posterior to the sternum. Its cause is considered by embryologic defect and the abdominal organs are passed through a defect. The incidence is predirected women over 50 years old. Its synonym is hernia of subcostosternal, retrosternal, parasternal, rectocostoxiphoid, anterior diaphragmatic or Larrey`s. This report presents a symptomatic Morgagni hernia of ten months old male child on whom the diagnosis was established and was operated at the Busan Gospel Hospital. This patient was admitted with the chief complaints of mild cyanosis, frequent upper respiratory infections and protrusion of the right lower anterior chest. Herniorrhaphy was performed through the upper abdominal midline incision, hernial contents of the omentum and the colon, and sac as noticed from the Larrey`s space measuring 4 x 2 cm. in diameter and oval in shape. Interrupted sutures without difficulty repaired the defect. The cyanosis was disappeared and the patient had uneventful course of post-operative period. The patient was discharged at 7th. postoperative day. This case presentation with a brief review of literatures is given.
