사상체질처방 복합제제의 보험급여화 추진 과제 (Tasks for Insuring the Composite Herbal Preparations of Sasang Constitutional Medicine in the National Health Insurance)

  • Reforming the insurance scheme for herbal drugs in the National Health Insurance is a long-cherished desire for Korean Medicine doctors. Because most Korean Medicine doctors distrust the quality of existing insured herbal drugs, which are powdered mixes of each herbal extract, the use and the expenditure of insured herbal drugs have been decreased in the last ten years. To address this, it has been demanded to insure the composite type of herbal preparation, which is the extracted powder of the whole prescription, to the benefit coverage for herbal drugs. Many stake holders, however, have so far been unable to reach an agreement on this. In this situation, Sasang Constitutional prescriptions are expected to make a breach of insuring the composite herbal preparations, because some of them were approved as prescription drugs in 1999. In this review, I discussed the problems of insured herbal drugs, the necessity of insuring the composite herbal preparations and Sasang Constitutional prescriptions, and the tasks of Sasang Constitutional Society to insure them.

약사법규 중의 기존한약서 활용에 대한 고찰 (Study on the Application of Established Korean Medical Classics in the Korean Pharmaceutical Affairs Act)

  • Purpose: There is an issue in applying various principles introduced in established Korean medical classics to "Pharmaceuticals Approval, Notification and Review" of "herbal medicinal preparations" and "new drugs from natural products" that are used for western forms of medical treatment. Thus, an analysis of the origin, purpose, and application of established Korean medical classics in the Korean Pharmaceutical Affairs Act is essential. Methods: We collected data regarding the origin, purpose, and application of established Korean medical classics in the Korean Pharmaceutical Affairs Act, and classified them by periodical change and subjects. Results: Established Korean medical classics are applied as follows: 1) as criteria for Korean medicine distributors' sales of mixed herbal drugs (Pharmaceutical Affairs Act; since 1953), 2) as official compendiums for pharmacists' preparation of Korean medicine (Ministry of Health and Welfare's authoritative interpretation; from early 1970s to 1993), 3) as standards for oriental pharmacists' quality measurement of preparations (notification of the Ministry of Health and Welfare; since 1995), 4) as criteria for "Pharmaceuticals Approval, Notification and Review" of herbal medicinal preparations and crude drug preparations (notifications regarding drug approval process by the Ministry of Health and Welfare and the Ministry of Food and Drug Safety; since 1978), and 5) as standards for the quality of materials of health functional food (from 2004 to 2011). Conclusion: The application of Korean medical classics has been closely related with the change of the laws, regulations, and systems that are relevant to Korean medicine, and it seems to be more favorable for pharmacists than oriental pharmacists. Meanwhile, regulations that apply prescriptions that are recorded in Korean medical classics - dosage, indications, and preparation methods - as criteria for the approval of crude drug preparations for western medical treatment should be abolished.

최근 5개년 (2013~2017)간 기관지확장증(J47) 환자에게 처방한 급여한약제제 현황 분석 - 건강보험청구자료 중심으로 (Herbal medicine prescription analyses of bronchiectasis patients with claim data during 5 years (2013~2017))

  • Objectives : Korean national health insurance data is a useful real-world data representing whole medical bills submitted to Health Insurance Review Agency. This study aims to understand recent benefit trend of insurance herbal preparations for treating bronchiectasis(disease code J47) utilizing insurance data. Methods : We reviewed national health insurance claims data from 2013 to 2017 which have main diagnosis or sub diagnosis code of J47 and with the record of prescribing insurance herbal medication. Frequency analysis was performed to analyze the most frequently prescribed prescription. Results & Conclusions : Both the number of claims statement(770 to 1,746cases) and patients(266 to 484) of insurance herbal preparations increased considerably from 2013 to 2017. Top 10 preparations based on the number of claims statement were 'Samso-eum', 'Yeonkyopaedok-san', 'Socheongryong-tang', 'Bojungikgi-tang', 'Hyangsapyungwi-san', 'Yijin-tang', 'Saengmaek-san', 'Jaeumganghwa-tang', 'Ojeok-san' and 'Gungha-tang'. Top 10 preparations based on the number of patients were 'Samso-eum', 'Socheongryong-Tang', 'Saengmaek-san', 'Yeonkyopaedok-san', 'Haengso-tang', 'Hyangsapyungwi-san', Yijin-tang', 'Jaeumganghwa-tang', 'Bojungikgi-tang' and 'Hyeonggaeyeongyo-tang' in respectiv order. Claims of top 10 frequent preparations occupied more than 60% of total claims. We hope this finding to be utilized as basic data for future research of evidence-based bronchiectasis treatment utilizing Korean traditional medicine.

시판 황련해독탕 Ex제제의 성분함량 모니터링 연구 (A monitoring study of marker contents in the Hwangnyeonhaedok-tang Ex preparations on the market)

  • Objectives : Hwangnyeonhaedok-tang(HNHDT) is the one of well-known traditional herbal formula and has been widely used for the treatment of various diseases in Korea. For the quality improvement of commercial traditional herbal formula, we investigated the quality of prepared HNHDT. Methods: We measured the contents of baicalin, berberine and geniposide in the HNHDT Ex preparations on the market by HPLC and compared with the official notice of ingredients. Results: In 3 preparations of HNHDT covered with the national health insurance(NHI), the content of baicalin was various from 32.83mg to 185.05mg, the total berberine was $12.0{\sim}50.75mg$ and geniposide was $12.57{\sim}44.84mg$. Contents of marker ingredients in NHI Preparations were below the standard and showed a large variation. In 7 non NHI Preparations of HNHDT and the standard(STD) HNHDT manufactured in laboratory, the content of baicalin was $37.16{\sim}115.69mg$, the total berberine was $5.62{\sim}28.43mg$ and geniposide was $10.98{\sim}113.47mg$. But, because composition of non-NHI preparations is different from NHI preparations, every maker's contents were up to the notice for baicalin and berberine. There was newly enforced standard regulations of geniposide in non-NHI preparations, so contents of geniposide was up to that standard only in JI and STD. Conclusions: There was a large variation of contents in the HNHDT Ex preparations on the market and the quality differences were existed between NHI and non-NHI preparations of HNHDT. Further studies for evaluation of efficacy differences and standardization of manufacturing processes are necessary.

Is ginseng free from adverse effects?

  • Ginseng is a perennial herb widely used in China, Japan, and Korea. It is also one of the most commonly used herbal medicines in the U.S. Although it is generally considered safe to use, adverse effects associated with ginseng use have been reported. Inappropriate ginseng use, such as high dose administration, may cause insomnia, headaches, diarrhea, as well as cardiovascular and endocrine disorders. Other factors that may contribute to adverse effects of ginseng include the variety of ginseng species, variability in commercial ginseng preparations, and potential ginseng-drug interactions. To minimize adverse effects of ginseng, consumers should be advised to use it appropriately, and the herbal industry should try to provide standardized ginseng preparations.

한국의 한약제제 생산현황과 발전방안 (The Present State of Korean Herbal Preparation Production and Possible Improvement Plan)

  • Recently. the herbal industry has expanded into the herbal dietary supplements industry, the herbal cosmetics industry, etc. But still the main form of herbal drug usage is water extraction- packaging after herb preparation in private clinics. There was no report or data of investigations about industrial herbal preparations. To search for a better plan of herbal drug improvement and popularization. we surveyed the present state of Korean herbal preparation production. We made an industrial herbal preparation list, investigated the total amount of production, the chief items of manufacture. names of prescriptions each item is based on. scales of each production company, etc., and report on them on this article.

한약제제(액제)의 안전성 평가 (Safety Assessment of Liquid-Type Herbal Preparations)

  • The safety of liquid-type herbal preparations (47 samples) and beverages containing herbal medicines (18 samples), which are distributed in Gyeonggi province was studied by analyzing heavy metals, benzo(a)pyrene, preservatives and sugar content and to explain the differences between medicine and food based on the labeling standards. The herbal preparation is regarded as medicine and can be differentiated from the beverages containing herbal medicine, which is food, based on the fact that the herbal preparation should be labeled with usage, dosage, efficiency and effect. The mean concentrations of Pb, As, Cd, Hg were found to be 0.014 mg/kg, 0.113 mg/kg, 0.004 mg/kg and 0.003 mg/kg in herbal preparation and 0.009 mg/kg, 0.122 mg/kg, 0.003 mg/kg and 0.002 mg/kg in beverages containing herbal medicines, respectively. In 10 cases of herbal preparations and 6 cases of beverages containing Rehmanniae Radix Preparata (Sukjihwang), benzo(a)pyrene was not found. According to preservative test on herbal medicine, dehydroacetic acid was found less than the indicated content in 6 cases out of 37 cases, which labeled with preservatives, and benzoic acid, dehydroacetic acid and methylparaben were contained with 164.0~198.0 mg/kg, 149.8~272.5 mg/kg and 88.4 mg/kg, respectively, in 3 cases out of 10 cases, which was not labeled with preservatives. Among 16 cases of beverages containing herbal medicines, 3 cases of preservative-labeled beverages had the preservatives, and the rest of all, which was not labeled with preservatives, did not show that they contain any preservative, therefore, it was suitable for the labeling standards. As a result of sugar content test, preservative-labeled products were not appreciably different from the others.

색조화장품에 적용될 수 있는 17종의 천연한약재가 피부 염증억제에 미치는 임상적 연구 (Effects of Herbal Extracts on the Inflammatory Reactions Which Use the Makeup Preparations)

  • This study was carried out to investigate the effects of herbal extracts on the skin inflammatory reactions which use the makeup preparations. In experiment 1, among the herbal ingredients of herbal extracts, ethanol extracts and 1,3-BG(Butylene Glycol) extracts of Galla Rhois showed potent radical scavenging activity, more than 91% at all concentrations, tested by DPPH (1,1-diphenyl-2-picryl-hyrazyl) method. In experiment 2, ethanol extracts of Chrysanthemi Flos, Gardenias Flos, Galla Rhois showed potent inhibitory activity of the lipopolysaccharide-induced nitric oxide(NO) production, more than 87% at $10{\mu}g/m{\ell}$, by the macrophage RAW 246.7 cells. And 1,3-BG extracts of Taraxaci Herbs, Corm Fructus, Galla Rhois showed potent inhibitory activity of nitric oxide production, more than 89% at $10{\mu}g/m{\ell}$. In experiment 3, ethanol extracts of Chrysanthemi Flos, Gardeniae Flos, Galla Rhois showed potent inhibitory effects of cyclooxygenase-II activity, more than 78% at $10{\mu}g/m{\ell}$, by using ELISA kit. And 1,3-BG extracts of Galla Rhois, Carthami Flos, Chrysanthemi Flos, Taraxaci Herba, Corm Fructus showed potent inhibitory effects of cyclooxygenase-II(COX-II) activity, more than 80% at $10{\mu}g/m{\ell}$. Therefore, 1 expect that herbal extracts, especially Galla Rhois may be used as a drug for treatment on skin inflammation and a material of the makeup preparations.

"동의보감(東醫寶鑑)" 고제(膏劑)에 대한 연구 (A Study of Paste Preparations in "Dongeuibogam")

  • Objectives : The study contains formulas, medicinal herbs, how to make of paste preparations in "Dongeuibogam". Methods : I will try to find paste preparations in "Dongeuibogam" using a word, paste(膏). Results : Paste preparations are categorized into two types: the external and the internal medicine. Paste preparations were used not only in the dermatology disease but in the internal disease. The same formula name to have different information was written by other people. Various kinds of medicinal herbs, minerals, animals were used and type of oil or juice was used to dissolve these. Methods to process paste preparations were to grind medicines or extract from medicinal materials. Paste preparations for the external was used to put in eyes or to push into wound and apply them to the skin. Paste preparations for the internal was used to take with wine, water and herbal tea. Some paste preparations were used both external and internal. Conclusion : These results explain that Paste preparations were variously used in "Dongeuibogam".
