일부 보건의료계열 학생들의 B형 간염환자 감염관리에 대한 이론적지식과 임상감염관리 실천도의 관련성 (Relationship between knowledge and infection control behavior about Hepatitis B patient in health care-related majors)

  • Purpose: The purpose of this study was to identify relationship between knowledge and infection control behavior about Hepatitis B patient in health care-related majors. Methods: Data was collected from health care related majors-nursing, emergency rescue and dental hygiene with questionaire. Total respondents were 306 and we use all of them. We analysed students' knowledge and infection control behavior about Hepatitis B patient and their relationship. Uni-variate analysis, t-test, ANOVA and correlation analysis test were conducted using SAS version 9.2. Results: Students who go the Hepatitis B lecture were significantly high Knowledge level about Hepatitis B(p<.05). Students who go to clinical education about Hepatitis B were significantly better behavior about infection control in clinical practice(p<.05). The relationship between knowledge about Hepatitis B and infection control in clinical practice was significantly negative(p<.05). Conclusions: It needs to be educated both infection control lectures and clinical practice to health care-related majors to prevent cross-infection between patients and health care providers.

Chronic Hepatitis B Virus Infection and Risk of Pancreatic Cancer: A Meta-analysis

  • Objectives: A number of studies have shown that chronic hepatitis B virus infection is implicated in susceptibility to pancreatic cancer. However, the results are still controversial. This meta-analysis aimed to quantitatively assess the relationship between chronic hepatitis B virus infection and incidence of pancreatic cancer of cohort and case-control studies. Methods: A literature search was performed for entries from 1990 to 2012 using PUBMED and EMBASE. Studies were included if they reported odds ratios (ORs) and corresponding 95% CIs of pancreatic cancer with respect to the infection of hepatitis B virus. Results: Eight studies met the inclusion criteria, which included five case-control studies and three cohort studies. Compared with individuals who have not infection of hepatitis B virus, the pooled OR of pancreatic cancer was 1.403 (95%CI: 1.139-1.729, P=0.001) for patients with hepatitis B virus infection. Sub-group analysis by study design showed that the summary OR was 1.43 (95%CI: 1.06-1.94, P=0.021) when pooling case-control studies and 1.31 (95%CI: 1.00-1.72, P=0.05) when pooling cohort studies. Conclusion: Findings from this meta-analysis suggest that chronic hepatitis B virus infection may increase the risk of pancreatic cancer. This relationship needs to be confirmed by further follow-up studies.

일부 농촌지역 초.중학생의 B형 간염 바이러스 감염 발생률에 대한 조사 연구 (Incidence of Hepatitis B Virus Infection of the School Children in a Rural Area of Korea)

  • Infection by hepatitis B virus is one of the major health problems of this nation. HBsAg positive rates of general population and school children were known to be as about 8 percent and 3.9 to 5.9 percent respectively. To study the incidence rate of hepatitis B infection in school children of rural area, author had examined 475 school children of relatively isolated agricultural area for baseline prevalence of hepatitis B virus serologic markers and followed up 415 school children during 10 months to determined the frequency of serologic conversion. The major results are summarized as followings: 1) Among the 278 susceptible children who were followed up, 26 had seroconversion for HBsAg or Anti-HBs. Therefore, the cumulative incidence rate during 10 months is estimated 9.4%. 2) The incidence rate of hepatitis B infection tends to increase with age (6-9yrs: 3.2%, 10-14yrs: 9.5%, 15-17yrs: 18.9%), and the incidence rate in male (13.0%) was higher than in female (5.7%). 3) The incidence rates of hepatitis B virus infection were not different statistically among three economic classes (The rates of lower, middle and higher class were 11.8%, 7.1% and 10.5%.). 4) The incidence rates of hepatitis B virus infection were not different statistically between visitors and non-visitors of clinic or hospital, dental clinic, persons received IV and not received IV, and persons with familial history and without familial history of liver diseases. Therefore all of these factors were not identified as risk factor of hepatitis B virus infection. And the transmissibility within the class of school was not recognized, too. 5) Among the 25 children who were HBsAg positive when enrolled, 15 (60%) were still HBsAg positive, who were identified as chronic carrier 15 of 415 school children were chronic carriers, then chronic carrier rate was estimated 3.6%, and there was no difference between sexes. 6) Of 38 children who had been Anti-HBs positive when enrolled, 5 (13.2%) lost Anti-HBs. Therefore, the loss rate of Anti-HBs per year is estimated to be 15.8%.

Use of ALLGIO Probe Assays for Detection of HBV Resistance to Adefovir in Patients with Chronic Hepatitis B, Kerman, Iran

  • Hepatitis B virus (HBV) infection is contagious with transmissiobn vertically or horizontally by blood products and body secretions. Over 50% of Iranian carriers contracted the infection prenatally, making this the most likely route of transmission of HBV in Iran. To evaluate the resistance to adefovir (ADV) therapy in patients with chronic hepatitis B infection, a study was conducted on 70 patients (63 males and 7 females), who had received in first line lamivudine and second line adefovir. All were tested for the presence of hepatitis B surface antigen (HBsAg), hepatitis B envelope antigen (HBeAg), serum alanine amino transferase (ALT) level and HBV DNA load before and after treatment with ADV. In all samples, resistance to lamivudine and ADV was tested with real time PCR. Among seventy patients with chronic hepatitis B infection, 18 (25.7%) were resistant to LAM and 8 (11.4%) were resistant to ADV. Only one patient was negative for the presence of HBS-Ag (5.6%) and two were negative for HBe-Ag (11.1%). In this study we used a new method (ALLGIO probe assay) that has high sensitivity in detection of adefovir resistance mutants, which we recommend to other researchers. Mutant strains of the YMDD motif of HBV polymerase can be found in some patients under treatment with lamivudine and ADV. ADV has been demonstrated to be efficient in patients with lamivudine resistant HBV.

새터민의 B형간염 인식과 지식에 관한 연구 (Awareness and Knowledge about Hepatitis B among North Korean Immigrants)

  • Purpose: The purpose of this study was to identify awareness and knowledge about hepatitis B virus (HBV) infection among North Korean immigrants who had resided in South Korea. Methods: This was a cross-sectional, community based study. Data were collected from a convenience sample of 198 North Korean immigrants aged 18 years old through 65 years old among those who had attended community centers, local churches, and gathering places in four cities. The questionnaire included 15 items about participants' knowledge on HBV infection and its consequences, mode of transmission, and preventive measures. Results: The mean score of HBV knowledge was $6.3{\pm}3.8$ (range 0~15). The items that over 60% of the respondents checked correct answer regarding knowledge about HBV were 'There is blood screening for hepatitis B infection', 'Hepatitis B can be transmitted through sharing needles with infected person'. Multivariate analysis showed that received education for the prevention of HBV infection and disease morbidity were significantly associated with the knowledge of HBV infection. Overall, approximately 11% of total variability in the knowledge of HBV infection could be explained by the 2 variables in this model ($R^2=.109$, p<.001). Conclusion: The results showed that North Korean immigrants need more education on HBV infection and prevention to improve their knowledge about HBV.

  • Both the enterically transmitted forms of viral hepatitis, hepatitis A and E are self-limited and do not cause chroni chepatitis. Chronic hepatitis occurs in patients with hepatitis B and C as well as in patients with chronic hepatitis D superimposed on chronic hepatitis B. Chronic hepatitis such as hepatitis B or C is important in terms of insurance underwriting and claims. General review of hepatitis B and C was performed in this article.

일부 접객업소 여성 종사자들에 있어서의 간염 B 항원 발현빈도에 관한 관찰 (The observation about occurrence rate of HBs Antigen to waitress entertaing at restaurant business)

  • Up to the present there were the blood transfusion and the use of apparatus not to sterilize at surgical operation and medical treatments in well known infectious ways of hepatitis B virus. But all its ways were still not surely known. As the other ways of hepatitis B virus infection, it was suggested that contagion such as kissing, sexual contact, homosexuality, and varieties discharged out human body urine, stool, tear, salivary, menstrual blood, Vaginal discharge etc, and low economic status, unsanitary environmental life, alcohol and smoking related to hepatitis B virus infection. From 800 waitress who frequently contact with a lot of drinker with low economic status and unclear environmental life, this study were investigated HBs Ag (Hepatitis B surface antigen) known marker for hepatitis B virus infection in their serum, and a few conditions of their private life in order to known dangerous rate b~ing exposed to the source of hepatitis B virus infection as comparison with control group. The results were summarlized as following 1. The positive rate of HBs Ag (4.3%) was significantly higher in waitress than in control group(1.1%). 2. In waitress old, 20-24 ages group was the most as 59.5%, and positive rate of HBs Ag was trend of higher in twenties than thirties. 3. Among the waitress, one that it was less than a year were the most (62.4%) in the period to work at restaurant business, and positive rate of HBs Ag was trend of higher in propotion to period to work at there. 4. Among the waitress, one who entertained to beer hall was the most (46%), and they were trend of higher in positive rate of HBs Ag (6.3%) than other waitress. 5. Among the waitress, one to drink with smoking, and only to drink, and only to smoke, and not to drink and smoke 66.1%, 21.5%, 4.6% and 7.8%. Espically in one to drink with smoking, ther was trend of indicating the high positve rate of HBs Ag, and it was made suspicion of the relationship of alcohol smoking, contact with a lot of drinker, unclear environment to hepatitis B virus infection. In the above results, it was found that dangerous rate being exposed in hepatitis B virus infection was high in the waitress. Therefore there are required for active preventions against hepatitis B virus infection them. Also as it is possible to be infectious source in public health that waitress infected to viral hepatitis, it is thought that appropriate rules about them.

Epstein-Barr Virus Infection with Acute Pancreatitis Associated with Cholestatic Hepatitis

  • Infection-induced acute hepatitis complicated with acute pancreatitis is associated with hepatitis A virus, hepatitis B virus or hepatitis E virus. Although rare, Epstein-Barr virus (EBV) infection should be considered also in the differential diagnosis if the patient has acute hepatitis combined with pancreatitis. We report a case of EBV infection with cholestatic hepatitis and pancreatitis with review of literature. An 11-year-old female was admitted due to 1-day history of abdominal pain and vomiting without any clinical symptoms of infectious mononucleosis. Diagnosis of reactivated EBV infection was made by the positive result of viral capsid antigen (VCA) IgM, VCA IgG, Epstein-Barr nuclear antigen and heterophile antibody test. We performed serologic tests and magnetic resonance cholangiopancreatography to exclude other viral or bacterial infection, autoimmune disorder, and structural problems. The patient's symptoms recovered rapidly and blood chemistry returned to normal with conservative treatment similar to previously reported cases.

Detection of HBV Resistance to Lamivudine in Patients with Chronic Hepatitis B Using Zip Nucleic Acid Probes in Kerman, Southeast of Iran

  • HBV infection is contagious and may be transmitted vertically or horizontally by blood products and body secretions. Over 50% of Iranian carriers have contracted the infection prenatally, making this the most likely route of transmission of HBV in Iran. This study assesses the resistance to Lamivudine in patients with chronic hepatitis B infection using a new ZNA probe Real Time PCR method. To evaluate the effectiveness of Lamivudine therapy for chronic hepatitis B infection, a study was conducted on 70 patients (63 men and 7women), who had received the drug first line. All patients were tested for the presence of HBsAg and HBeAg, the serum ALT level and the HBV DNA load before and after treatment. In all samples resistance to Lamivudine was tested with the ZNA Probe. Our results showed that ZNA Probe Real Time PCR method could detect wild type,YMDD, and its mutants, tyrosine-isoleucine-aspartate-aspartate and tyrosine-valine-aspartate-Aspartate. Among an estimated seventy patients with chronic hepatitis B infection, 18 (25.7%) were resistant to lamivudine. Only one patient was negative for presence of HBS-Ag (5.6%) and two patients were negative for HBe-Ag (11.1%). Real-time PCR with Zip nucleic acid probes is a sensitive, specific and rapid detection method for mutations in the YMDD motif, which will be essential for monitoring patients undergoing Lamivudine antiviral therapy.

A Study on the Trend of Changes in the Positive Rate of Hepatitis B Surface Antigens and Antibodies over the Last 9 Years in One Health Care Center

  • The purpose of this study was to provide basic data on hepatitis B infection control in the community through the results of the hepatitis B surface antigen and antibody tests conducted at the Cheongyang-County Health Medical Center. From 2012 to 2020, we retrospectively analyzed the HBsAg, HBsAb, HBeAg, HBeAb, and HBV DNA results of 7,329 hepatitis B-related testers. Among 7,329 subjects, the HBsAg positivity rate was 1.7%, and the positivity rate according to age was the highest at 4.4% in their 30s, 4.2% in their 40s, 4.1% in their 50s, 2.0% in their 60s, 1.9% in their 70s and over, and 10 it was shown in the order of 0.3% from less than large. The HBsAb positivity rate was 43.1% for men, 38.2% for men, and 46.7% for women (P<0.001). To summarize the above results, for infection control of hepatitis B in Cheongyang-County, hepatitis surface antigen proton management is required for those in their 30s or older, and it is thought that efforts to acquire immunity are necessary for those in their 20s or younger.
