A Case of Hepatotoxicity by Salvia Plebeia R. Br.

  • Objective: To investigate the possibility of hepatotoxicity by supplemental foods or folk herbs such as Salvia Plebeia R. Br. Methods: A hospitalized male patient with alcoholic liver disease and electrolyte imbalance had recovered, and then followed by rapid hepatic serum enzymes after taking S. Plebeia. This study monitored the clinical outcome and biochemical parameters. Result: A 58-year male had drunk frequently, which led to alcoholic steatohepatitis and hospitalization. Two weeks after his discharge from hospital, he felt nausea, dizziness, and mild difficulties in speech and walking, resulting in re-hospitalization at the Korean Medical Hospital. The symptoms disappeared on correction of the electrolyte imbalance suspected to have been caused by severe sweating while working in the outdoors, and the patient was discharged. During treatment and monitoring of his health as an outpatient, the serum hepatic enzyme rapidly elevated approximately 10-fold in hepatic enzymes; the enzyme levels fluctuated according to whether or not he was taking the boiled water of S. Plebeia. The RUCAM score was 12, which met the criteria for toxic hepatitis by S. Plebeia. His general condition and abnormal hepatic enzymes recovered with cessation of S. Plebeia and administration of Chungganplus syrup (CGX). Conclusion: This study reports the hepatotoxic risk of Salvia Plebeia, which is commonly used as a folk remedy in Korea.

The High Expressed Serum Soluble Neural Cell Adhesion Molecule, a High Risk Factor Indicating Hepatic Encephalopathy in Hepatocelular Carcinoma Patients

  • Objective: To investigate whether the expression of serum soluble neural cell adhesion molecule (sNCAM) is associated with hepatic encephalopathy (HE) in hepatocelular carcinoma (HCC) patients. Materials and Methods: The Oncomine Cancer Microarray database was used to determine the clinical relevance of NCAM expression in different kinds of human cancers. Sera from 75 HCC cases enrolled in this study were assessed for expression of sNCAM by enzyme linked immunosorbent assay (ELISA). Results: Dependent on the Oncomine Cancer Microarray database analysis, NCAM was down regulated in 10 different kinds of cancer, like bladder cancer, brain and central nervous system cancer, while up-regulated in lung cancer, uterine corpus leiomyoma and sarcoma, compared to normal groups. Puzzlingly, NCAM expression demonstrated no significant difference between normal and HCC groups. However, we found by quantitative ELISA that the level of sNCAM in sera from HCC patients with HE ($347.4{\pm}151.9ng/ml$) was significantly more up-regulated than that in HCC patients without HE ($260.3{\pm}104.2ng/ml$), the p-value being 0.008. sNCAM may be an important risk factor of HE in HCC patients, the correlation coefficients was 0.278 (P<0.05) on rank correlation analysis. Conclusions: This study highlights that up-regulated level of serum sNCAM is associated with HE in HCC patients and suggests that the high expression can be used as an indicator.

Effects of 12 Weeks Regular Aerobic Training on Hepatic Enzyme in Type 2 Diabetes Mellitus (T2DM) patients. (12주 규칙적인 유산소 트레이닝이 제 2형 당뇨(T2DM) 환자의 간 효소(Hepatic enzyme)에 미치는 영향)

  • The purpose of the this study was to examine the effects of 12 weeks of regular aerobic exercise training on hepatic enzymes in type 2 diabetes mellitus (T2DM) patients. The subjects consisted of 13 middle-aged male type 2 diabetes mellitus (T2DM) patients, all of whom had no other complications. Subjects participated in regular aerobic exercise training for 12 weeks, in which they started to exercise for $20{\sim}60$ min, at $60{\sim}80$% $HR_{max}$ (exercise intensity was increased gradually), per day, $3{\sim}5$ times a weeks. The results after 12 weeks were compared to baseline values. Weight and BMI, %body fat, and fasting glucose significantly decreased, and $_{peak}VO_{2}$, exercise time (ET) significantly increased after 12 weeks of aerobic exercise training. On the other hand, there were no significant differences in hepatic enzymes of Albumin, Total bilirubin, Alkaline phosphatate, AST, and ALT after training compared to baseline values. Conclusively, 12 weeks of aerobic exercise training may result in a decrease of insulin resistance factors (Weight, BMI, % body fat, fasting glucose) and an increase of aerobic capacity, but hepatic enzymes did not significantly decrease in middle age T2DM patients.

The Antihyperlipidemic Effect of Lion's Mane Mushroom (Hericium erinaceus) in Hyperlipidemic Rats Induced by High Fat and Cholesterol Diet (고지방과 콜레스테롤 식이로 유도된 고지혈증 흰쥐에서 노루궁뎅이버섯의 항고지혈증 효과)

  • This study was conducted to investigate the dietary supplementation of fruiting body of Hericium erinaceus (HE) mushroom on lipid profiles of serum and histological changes of the liver in rats with high fat and cholesterol diet. Five-week old female Sprague-Dawley albino rats were divided into three groups of 8 rats each: The normal control diet (NC group), high fat and cholesterol diet (HFC group), and HFC diet supplemented with 5% fruiting powder of Hericium erinaceus (HFC+HE group). In the HFC+HE group, serum total cholesterol, low density lipoprotein, and triglyceride concentrations were significantly reduced compared with the NC group. Body weight gain of those in the HFC+HE group were lower than those in the HFC group; whereas HFC+HE had no effect on the levels of plasma albumin, creatinine, blood urea nitrogen, uric acid, glucose, and total protein. The enzyme activities related to the liver function, such as aspartate aminotransferase (AST), alanine aminotransferase (ALT), and, alkaline phosphatase (ALP), were lower in the NC group than in the HFC group, but without significance. Feeding the mushroom increased the excretion of total lipid and cholesterol. A histopathological analysis showed that the those in the HFC group developed hepatic steatosis, whereas those in the HFC+HE group developed small fat droplet. In conclusion, these results suggest that 5% HE supplementation to HFC diet provided health benefits by acting on lowering atherogenic lipid profile in rats with high fat and cholesterol diet.

A case of subcutaneous paragonimiasis presented with pleural effusion (흉수를 동반한 피하조직 이소기생 폐흡충증)

  • Paragonimiasis is a parasitic infection that occurs following the ingestion of infectious Paragonimus metacercariae from crabs or crayfish. Pulmonary paragonimiasis is the most common clinical manifestation of this infection, but several ectopic paragonimiasis cases have also been reported. Among them, cases of subcutaneous paragonimiasis are rare, especially in children. We report a case of subcutaneous paragonimiasis of the right abdominal wall with pleural effusion with hepatic involvement and without abnormal pulmonary infiltration in a boy aged 2 years and 5 months. He had eaten soybean sauce-soaked freshwater crabs (kejang) 6 months prior to complaining of right abdominal wall distension. On evaluation, right pleural effusion without abnormal pulmonary infiltration was detected, as well as blood eosinophilia, an elevated serum IgE level, pleural fluid eosinophilia and a positive enzyme-linked immunosorbent assay that detected P. westermani antibody in the serum. Thoracentesis, praziquantel administration, and excision of subcutaneous lesions were performed. After treatment, the eosinophil count and serum IgE level were decreased, and the subcutaneous lesions did not recur. The frequency of paragonimiasis has decreased recently, but it is still prevalent in Korea. Paragonimiasis should be suspected if pleural fluid eosinophilia is associated with blood hypereosinophilia and a high level of serum IgE; however clinicians should obtain a thorough history of travel and food habits.

  • Garlic occupies a special position among the many foods of vegetable origin because it is the sole food for Koreans during the their lives. And vitamin A has been ingested by forms of food or additives. Cadmium has been described as one of the most dangerous trace elements in the food and environment of man and livestocks. Since the de novo synthesis of stress proteins can be detected early after exposure to some agents, analysis of cadmium-induced changes in gene expression , ie. alterations in patterns of protein synthesis, may be useful to develop as biomarkers of exposure and damage for food hygiene. He acute and chronic combine effects of cadmium (Cd, CdCl2 20mg/kg), garlic oil(Dds: diallyl disulfide 50mg/kg, 3 times a week) and vitamin A(Ra: retinol acetate 50,000 IU/kg, 3 times a week) on Wistar male rats were evaluated concerning cadmium contents, tissues enzyme activity, HSP expression histopathological and electron microscopical examinations. The results of the study are as follows ; 1. Less cadmium was absorbed through the digestive tracts, but the ratio of contents in tissue were not changed by the simultaneous adminstration of diallyl disufide or retinol acetate. 2. ALT(alanine aminotransferase) , AST(aspartate aminotransferase), glucose, BUN (blood urea nitrogen), creatinine, the key indices of the clinical changes in hepatic and renal function were significantly hanged by the cadmium treatment after 1 week in liver, after 4 weeks in kidney. 3. Histopathological changes in cadmium treated rats were appeared at 8 weeks age treatment in kidneys. Homogenous eosinophilic material was accumulated in cortical and collecting tubular lumens at 16 weeks. Degenerated or necrotized tubular cells were observed in cortex and medulla. Degenerated seminiferous tubules and homogeneous eosinophilic material was seen in interstitial tissue of rat treated with cadmium for 16 weeks. Calcium deposits were seen in degenerated seminiferous tubules and the tubules showed severe calcification of rat treated with cadmium for 16 weeks. Electron microscope changes in kidney were observed in rats treated with CdCl2 20 mg/kg. Proximal convoluted tubule cells showed selling of cytoplasm and narrow lumen. Capillary endothelial cells showed cytoplasmic vacuoles and swelling. Degenerated epithelial cells were accumulated in tubular lumen of kidney. 4. Enhanced synthesis of 70 KDa relateve molecular mass proteins were detected in 2 hours after cadmium, exposure, with maximum activity occurring at 8~48 hours. Induction of HSP 70 was evident at proximal tubules and glomeruli in kidney. Testicular cells produced enough HSP to be detected normally. From the above results, it could be concluded that HSP70 induction by the cadmium treatment was a rapid reaction to indicated the exposure of xenobiotics, and retinol acetate reduced the cadmium induced nephrotoxicity.
