제2형 당뇨병 환자에서 혈당조절에 따른 구강건강인식도 (Self-Perceived Oral Health Status according to Regulation of Blood Glucose in the Type 2 Diabetic Patients)

  • 본 연구는 제2형 당뇨병 환자가 인식하고 있는 구강건강 문제를 파악하고, 당화혈색소와 주관적 구강건강상태와의 연관성을 제시함으로써 당뇨병 환자의 전신 및 구강건강증진을 위한 기초자료를 제시하고자 시행하였다. 2012년 1월 9일부터 2012년 3월 9일까지 만 40세 이상 제2형 당뇨병을 가진 환자 174명에게 설문지 조사와 $In2it^{TM}$을 이용하여 당화혈색소를 측정한 결과 다음과 같은 결론을 얻었다. 1. 연구대상자의 구강건강문제를 파악한 결과 구강점막질환 증상 중 '입 안 점막에 염증이 있다'와 '입 안 점막이 아프다', 치주질환 증상 중 '칫솔질 시 잇몸에서 피가 난다', 구강건조증 증상 중 '평상시 내 입 안은 마른다고 느낀다'를 가장 많이 인식하는 것으로 나타났다. 2. 일반적 특성에 따라 주관적 구강건강상태를 분석한 결과, 특히 당뇨병 유병기간이 길수록 입안점막질환과 치주질환의 증상 및 구강건조증 증상을 더 많이 인식하는 것으로 나타났다(p<0.05). 3. 혈당 조절을 위한 건강행동에 따라 당화혈색소 수준을 분석한 결과, 규칙적인 식사를 잘 실천하는 집단에서 당화혈색소 수준이 더 낮은 것으로 나타났다(p<0.01). 4. 주관적 구강건강상태에 따라 당화혈색소 수준을 분석한 결과, '혀에 타는 듯한 느낌이 있다', '칫솔질 시 잇몸에서 피가 난다', '잇몸이 치아 뿌리 쪽으로 내려갔다' 및 '입에서 나쁜 냄새가 난다'를 인식한 집단에서 당화혈색소 수준이 더 높았다(p<0.05). 또한 구강건조증 증상 중 '나는 목이 말라 자다가 일어나 물을 마신다'에 응답한 집단에서 당화혈색소 수준이 더 높았다(p<0.05). 특히 다중선형회귀분석 결과 치주질환 증상은 당화혈색소와 3번째로 강한 연관성을 보인 요인으로 나타났다. 이상의 결과로 볼 때 혈당조절 정도는 다양한 구강건강문제와 관련성이 있으므로 당뇨병 환자를 대상으로 구강관리 프로그램의 적용을 더욱 강화하고, 의료인력의 협진을 통해 구강 내 당뇨합병증을 최대한 감소시켜야 할 것이다.

Hemolysis of irradiated leukoreduced red blood cells during rapid warming: An in vitro experimental study

  • Background: Although water chambers are often used as surrogate blood-warming devices to facilitate rapid warming of red blood cells (RBCs), these cells may be damaged if overheated. Moreover, filtered and irradiated RBCs may be damaged during the warming process, resulting in excessive hemolysis and extracellular potassium release. Methods: Using hand-held syringes, each unit of irradiated and leukocyte-filtered RBCs was rapidly passed through a water chamber set to different temperatures (baseline before blood warming, $50^{\circ}C$, $60^{\circ}C$, and $70^{\circ}C$). The resulting plasma potassium and free hemoglobin levels were then measured. Results: Warming RBCs to $60^{\circ}C$ and $70^{\circ}C$ induced significant increases in free hemoglobin (median [interquartile ranges] = 60.5 mg/dl [34.9-101.4] and 570.2 mg/dl [115.6-2289.7], respectively). Potassium levels after warming to $70^{\circ}C$ ($31.4{\pm}7.6mEq/L$) were significantly higher compared with baseline ($29.7{\pm}7.1mEq/L$; P = 0.029). Potassium levels were significantly correlated with storage duration after warming to $50^{\circ}C$ and $60^{\circ}C$ (r = 0.450 and P = 0.001; r = 0.351 and P = 0.015, respectively). Conclusions: Rapid warming of irradiated leukoreduced RBCs to $50^{\circ}C$ may not further increase the extracellular release of hemoglobin or potassium. However, irradiated leukoreduced RBCs that have been in storage for long periods of time and contain higher levels of potassium should be infused with caution.

급식교와 비급식교 아동의 성장발달 및 철분영양상태 비교 (A Comparison of Anthropometry and Iron Status in Children Provided with and without Natinoal School Lunch Program)

  • This study was designed to assess the iron nutritional status and growth development of children provided with and without the national school -lunch program(NLSP). The subjects consisted of 590 elementary school children (313 boys, 277 girls) in the 2nd, 4th and 6th grades provided with (n=390) and without (n=200) NSLP. anthropometric measurements were taken for body weight, height, triceps skinfold thickness and body fat percentage. Nutrient intakes were assessed by a modified 24 -hour recall method. Fastinig blood samples were obtained and analyzed for hemoglobin concentration, hematocrit, serum iron and total iron binding capacity. The results obtained are summarized as follows. No significant differences between children provided with and without NSLP were found in height and body weight, but triceps skinfold thickness and body fat(%) were significantly higher in children without NSLP than in those with NSLP. There was no significant difference in hemoglobin concentration and hematocrit between children provided with and without NSLP. However, serum iron concentration and transferrin saturation were higher in the children provided with NSLP(81.9$\mu\textrm{g}$/dl and 22.8%) than in the children without NSLP(73.1$\mu\textrm{g}$/dl and 20.9%). When comparing iron and vitamin C intakes , iron intake was significantly higher in children provided with school-lunch, but vitamin C intake was significantly higher in children provided without school-lunch. Percentages of iron -deficient anemia in underweight, normal and obese children when judged by total iron binding capacity were 14.9%, 12.5% and 25.8% respectively.

수인성 아질산 급성 노출에 따른 잉어, Cyprinus carpio의 반수치사농도, 혈액학적 성상 및 혈장성분의 영향 (Effects of lethal concentration, hematological parameters and plasma components of common carp, Cyprinus carpio by waterborne acute nitrite exposure)

  • Common carp (Cyprinus carpio) (Weight 23.05 ± 6.98 g, Length 12.65 ± 1.38 cm) were exposed to waterborne nitrite at 0, 50, 100, 200, 400 and 800 mg NO2 -/L for 96 hours. The lethal concentration 50 (LC50) of C. carpio exposed to waterborne nitrite was 398.6 mg NO2 -/L. Hemoglobin, hematocrit and RBC count were significantly decreased by waterborne nitrite exposure. The MCV (mean mean corpuscular volume) (µl), MCH (mean corpuscular hemoglobin) (pg) and MCHC (mean corpuscular hemoglobin concentration) (%) were significantly increased. The inorganic component, plasma calcium, was significantly increased, and the organic components such as plasma cholesterol and total protein were significantly increased showing a similar tendency with calcium. In enzymatic components, the AST and ALT were also significantly increased by nitrite exposure. The results of this study indicate that exposure to nitrite can affect the survival and hematological physiology of C. carpio.

  • The phenotypes of hemoglobin, albumin and transferrin of 3U2 Jindo dogs in Jindo area were studied by starch gel electrophoresis for hemoglobin and albumin, and by polyacrylamide gradient gel electrophoresis for transferrin. The results obtained were as follows: 1. In the hemoglobin phenotypes, three phenotypes, HbAA, HbAB and HbBB, which were controlled by two allelic genes, $Hb^A$ and $Hb^B$, were observed and their frequencies of appearance were 1.65%, 10.60% and 87.75% respectively. The distribution of gene frequency was calculated as 0.0695 in $Hb^A$ and 0.9305 in $Hb^B$. 2. In the albumin phenotypes, three phenotypes, Alb FF, Alb FS and Alb SS, which were controlled by two allelic genes, $AIb^F$ and $AIb^S$ were observed and their frequencies of appearance were 12.59%, 25.56% and 61.85% respectively. The distribution of gene frequency was calculated as 0.2537 in $AIb^F$ and 0.7463 in $AIb^S$. 3. Analysis of transferrin phenotypes showed 6 different types which were controlled by three allelic genes, $Tf^B$, $Tf^C$ and $Tf^D$ and their frequencies of appearance were 54.04% in TfBB, 17.54% in TfBC, 9.82% in TfBD, 8.07% in TfCC, 7.37% in TfCD and 3.16% in TfDD. The distribution of gene frequency was calculated as 0.6772 in $Tf^B$, 0.2053 in $Tf^C$ and 0.1175 in $Tf^D$.

당뇨병 환자의 합병증검사 및 당화혈색소검사 수진 관련 요인 (The Associated Factors with Utilization of Tests for Diabetes Complication and Hemoglobin A1c among Some Diabetes Patients)

  • Background: This study was performed to identify factors associated with the utilization of tests for diabetes complication and hemoglobin A1c (HbA1c) among diabetes patients in Jeollanam-do, Korea. Methods: The study subjects were 2,310 diabetes patients participated in 2014 community health survey in Jeollanam-do, Korea. Dependent variables were the utilizations of fundus examination, microalbuminuria test, and HbA1c test. The used statistical analysis methods were chi-square test and hierarchical regression analysis with weight in consideration of complex sample design. Results: The utilization rates of fundus examination, microalbuminuria test, and HbA1c test were 25.8%, 27.4%, and 12.3%, respectively. In the results of hierarchical regression, fundus examination was significantly related to age, education level in predisposing factors, residential area in enabling factors and recognition of blood sugar, drug therapy, and subjective health status in need factors. Microalbuminuria examination was significantly related to monthly income, residential area in predisposing and health screening, recognition of blood sugar, drug therapy, diabetic education, number of chronic disease, and subjective health status in need factors. HbA1c examination was significantly related to age, education level, marital status in predisposing factors, residential area in enabling factors and drinking, recognition of blood sugar, drug therapy, and diabetic education in need factors. Conclusion: The results of this study were shown that perception of their disease seriousness, education about diabetes management, and accessibility of tests were important to utilization of test for diabetes complication and HbA1c. It might be necessary to the develop and strength strategies for enhancing the utilization of tests for diabetes complication and management in diabetes patients.

두릅나무과 열수추출물이 당뇨흰쥐의 혈당과 혈액 성분에 미치는 영향 (Effects of Araliaceae Water Extracts on Blood Glucose Level and Biochemical Parameters in Diabetic Rats)

  • This study was conducted to investigate the effects of Aralia elata, Acanthopanacis cortex and Ulmus davidiana water extracts on blood hemoglobin, $HbA_{1c}$, levels and biomarkers in the streptozotocin (STZ)-induced diabetic rats. Male Wistar rats divided into normal and diabetic groups. The diabetic groups subdivided into the control group (DM) and Araliaceae water extracts supplemented groups: Aralia elata (AE), Acanthopanacis cortex (AC) and Ulmus davidiana (UD). The extracts were supplemented in diet base on 11.42 g of raw Araliaceae/kg diet for 7 weeks. The diabetes was induced by injecting 572 (55 mg/kg B.W., i.p.) once 2 weeks before sacrifying. Relative weights of liver were significantly lowered in the DM group compared to the normal group, whereas those of kidney and heart were significantly increased in the DM group. Supplementation of the Araliaceae water extracts improved reduced liver weights in STZ-induced diabetic rats. Blood glucose level was significantly higher in the DM group than in the normal group, whereas insulin contents were significantly lowered in the DM groups. However, these parameters were normalized in the An, AC and UD supplemented groups, respectively. Blood hemoglobin and $HbA_{1c}$ levels were significantly higher in the DM group than in the normal group. When all of Araliaceae water extracts were supplemented to the diabetic rats lowered hemoglobin and $HbA_{1c}$ levels. Red blood cell, white blood cell and Lymphocyte were significantly higher in the DM group than in the normal group. The supplementation of Araliaceae family water extracts significantly lowered these parameters compared to the DM group. MCV, MCH contents were declined in the DM group, while the supplementation of Aralia elata, Acanthopanacis cortex and Ulmus davidiana water extracts elevated of these contents in STZ-induced diabetic rats. Accordingly, these results indicate that Aralia elata, Aeanthopanacis corex and Ulmus davidiana water extracts would seem to improve the blood biomarkers in STZ-induced diabetic rats.

Direct Coombs Test Positivity in B-Chronic Lymphoid Leukemia: a Marker of Advanced Clinical Disease

  • Background: Chronic lymphoid leukemia (CLL) is a malignant hematopoietic disorder, the most common of all adult leukemias with a distinctive immunophenotype. It is well established that CLL patients can have autoimmune complications, amongst them autoimmune hemolytic anemia as the most frequent. This study was carried out to determine the frequency of direct Coombs Test positivity in CLL patients and its possible correlation with Rai staging, hematological parameters and biochemical markers. Materials and Methods: This descriptive cross sectional study was carried at Liaquat National Hospital from January 2011 to June 2013. Sixty untreated patients with B- chronic lymphoid leukemia were enrolled. Complete blood count, direct Coombs test, serum urea, creatinine, uric acid and LDH levels were determined. Data were compiled and analyzed using SPSS version 21. Results: Out of 60 patients, 42(70%) were males and 18(30%) were females. Mean age was $59{\pm}9.2years$. Male to female ratio was 2.1: 1. The frequency of direct antiglobulin test (DAT) positivity was found to be 23.3%. The monospecific IgG was positive in 11 patients (18.3%); C3d positivity was evident in 1 patient (1.6%) and 2 patients (3.3%) had dual IgG and C3d positivity. The mean hemoglobin was $10.8{\pm}2.4gm/dl$. Significantly low mean hemoglobin of $8.3{\pm}3.0gm/dl$ was seen in Coombs positive patients compared with negative patients having a mean hemoglobin level of $11.7{\pm}1.6gm/dl$ (P<0.001). DAT positivity also demonstrated a positive association with advanced Rai stage III disease (P<0.01). No associations were noted with age, gender and biochemical markers. Conclusions: Direct Coombs test positivity in CLL in our patients, unlike in Western studies, appears relatively high, indicating significant autoimmune hemolytic anemia and advanced Rai stage in our setting. DAT positivity can be considered as a surrogative marker for advanced clinical disease.

한국 당뇨병 성인의 혈당조절 장애요인 (Barrier Factors Influencing Glycemic Control in Korean Adults with Diabetes)

  • 본 연구는 한국 30세 이상 60미만 성인을 대상으로 당화혈색소 6.5% 이상 그룹과 미만 그룹간의 일반적 특성, 신체적 요인, 정신적 요인, 건강문해력 특성 등의 차이를 비교하고, 혈당조절에 영향을 미치는 요인을 성별에 따라 비교하기 위해 시행하였다. 2019년도부터 2021년도까지 국민건강영양조사 원시자료를 이용히여 층화집락추출 및 가중치를 반영하여 복합표본분석 방법을 적용하였다. 한국 30세 이상 60세미만 성인을 대상으로 당화혈색소 6.5% 이상 그룹과 미만 그룹의 인구사회학적 특성, 신체적 요인, 정신적 요인, 건강문해력 등에 따른 차이는 Rao-Scott χ2 test로 분석하였고, 성별에 따른 영향요인을 확인하기 위해 로지스틱 회귀분석 방법으로 분석하였다. 본 연구의 대상자는 438명 이었으며 당화혈색소 6.5% 이상 그룹은 348명으로 나타났다. 당화혈색소 6.5% 미만 그룹과 이상 그룹 간 유의한 차이가 있는 요인은 성별(RCχ2: 7.9, p=.012), 비만(RCχ2: 13.1, p=.006), 당뇨병 유병기간(RCχ2: 9.55, p=.029)이었다. 건강 전문가는 성별, 비만, 당뇨병 유병기간 등이 고혈당 위험성을 높이는 요인임을 파악하고, 혈당 조절 개선을 위한 중재 적용 시 대상자의 신체적 요인, 건강문해력 등을 고려한 건강 증진 전략을 제시할 필요가 있다.

경기도 일개 종합병원에서 건강검진을 받은 한국 성인의 혈청 De Ritis 비율과 당뇨 검사와의 관계 (Relationship between the Serum De Ritis Ratio and Diabetes Tests in Korean Adults Who Underwent Health Screening at a General Hospital in Gyeonggi-do)

  • 본 연구의 목적은 당뇨 검사결과와 간기능검사와의 관계연구이다. 간성당뇨는 제2형 당뇨와는 다르게 간기능 이상으로 기인하는 것이다. 본 연구에서 간기능 검사의 주요 효소검사인 아스파르트산 아미노전이효소(aspartate aminotransferase, AST), 알라닌 아미노전이효소(alanine aminotransferase, ALT) 그리고 AST/ALT ratio (De Ritis ratio)와 당뇨관련 검사와의 관계를 주로 확인하였다. 연구 결과 AST와 글루코스(glucose) (r=0.14, P<0.01); ALT 및 글루코스(r=0.21, P<0.01); AST 및 당화혈색소(HbAlc) (r=0.15, P<0.01); ALT와 HbAlc (r=0.20, P<0.01), 모든 변수는 양의 상관관계를 나타났으며, De Ritis ratio는 글루코스 (r=-0.20, P<0.01)와 당화혈색소 (r=-0.14, P<0.01)와 음의 상관관계를 보였다. AST와 ALT 그리고 De Ritis ratio 를 독립변수로 하고 글루코스 (R2=0.05) 와 HbA1c (R2=0.04) 를 종속변수로 하여 회귀분석한 결과 독립변수는 종속변수에 통계적으로 유의하게 영향을 나타내는 것으로 나타났다. AST는 ALT 보다 혈당과 당화혈색소에서 상관관계가 낮게 나타났으며, ALT가 증가하는 것은 즉, De Ritis ratio 감소의 원인이 된다. 따라서, De Ritis ratio는 당뇨관련 검사와의 관계에서 의미가 있는 것으로 볼 수 있다.
