편마비 환자를 위한 보행 재활기구 개발 (Development of the Gait Rehabilitation Equipment for Hemiplegic Patients after Stroke)

  • The aim of this study is to design and develop the gait rehabilitation equipment that judge patient's movement of his/her center of gravity using pressure sensors, and to aid hemiplegic patients to balance themselves using an automatic stepper that changes the patient's center of gravity. It is hard to bear the weight on the affected side for hemiplegic patients. The gait rehabilitation equipment detects the footing phase of hemiplegic patient during training and moves the unaffected footing side of the stepper up and moves the affected footing side down simultaneously so that the patient's center of gravity can shift from unaffected side to affected side. The gait rehabilitation system was developed and applied for hemiplegic patients during exercise. Eight hemiplegic patients and one normal adult were studied. The developed gait rehabilitation system could judge not only the normal adult's intention but also the patient's intention to move his/her center of gravity. Even though the most of hemiplegic patients exercised in automatic mode and a few hemiplegic patients exercised in manual mode, the developed gait rehabilitation system can aid the hemiplegic patients to train more easily.

가속도센서를 이용한 편마비성보행 평가 (Evaluation of Hemiplegic Gait Using Accelerometer)

  • The study aims to distinguish hemiplegic gait and normal gait using simple wearable device and classification algorithm. Thus, we developed a wearable system equipped three axis accelerometer and three axis gyroscope. The developed wearable system was verified by clinical experiment. In experiment, twenty one normal subjects and twenty one patients undergoing stroke treatment were participated. Based on the measured inertial signal, a random forest algorithm was used to classify hemiplegic gait. Four-fold cross validation was applied to ensure the reliability of the results. To select optimal attributes, we applied the forward search algorithm with 10 times of repetition, then selected five most frequently attributes were chosen as a final attribute. The results of this study showed that 95.2% of accuracy in hemiplegic gait and normal gait classification and 77.4% of accuracy in hemiplegic-side and normal gait classification.

  • This paper describes the detection of spatio-temporal parameter using an accelerometer and footswitches to evaluate a symmetry and balance of hemiplegic patients. We detected gait data using a 3-axis accelerometer that mounted between L3 and IA intervertebral area and footswitches made by FSR-Sensor attached insole. To minimize the error of the gait parameters to be detected incorrectly in case of using only accelerometer, we enhancement the performance of detection by measuring an accelerometer and foots witches data at the same time. So, it was possible to detect more accurate gait parameters. As a result, we can confirm the symmetry and balance of hemiplegic patients. In the future. these results could be used to evaluate the walking ability in hemiplegic patients in clinical pratice.

편마비 환자에서 트레드밀 보행훈련이 보행에 미치는 효과 - 지면 보행훈련과의 비교 - (Effects of Treadmill Gait Training on Gait Patterns in Hemiplegic Patients comparison with conventional gait training)

  • The aim of this study was to investigate the effects of treadmill gait training on the functional characteristics and the temporal-distance parameters of gait in hemiplegic patients, as compared with conventional gait training. The subjects of this study were 32 hemiplegic patients who had been admitted or were visited out-patients at Kangdong Sacred Heart Hospital, Hallym University, from March 3 through April 25, 2003. These subjects were randomly divided into treadmill gait training group or conventional gait training group. We evaluated the gait ability, motor functions, muscle strength, spasticity, physiological cost index, and temporal-distance parameters. We analyzed the changes between pre and post training in each groups, and the difference between two groups. Temporal-distance parameters were obtained using the ink footprint method and then energy consumption using physiological cost index. The results were as follows: 1. After a six-week training, treadmill gait training group significantly improved, as. compared to pre-training, in gait ability, motor functions for the leg and trunk and gross function, muscle strength of the lower limb, gait speed, cadence, step length both on the affected and on the unaffected side, step length symmetry, and energy consumption(p<0.05). 2. After a six-week training, conventional gait training group significantly improved, as compared to pretraining, in gait ability, motor functions for the leg and trunk, muscle strength of the lower limb, spasticity the upper limb, gait speed, cadence, step length both on the affected and on the unaffected side, and energy consumption(p<0.05). 3. After a six-week training, the treadmill gait training group significantly improved, as compared to the conventional gait, training, in gait speed and step length on the unaffected side. These results show that treadmill gait training was improved gait speed and step length on the unaffected side of hemiplegic patients, as compared with conventional gait training. Further research is needed to confirm the generalization of these findings and to identify which hemiplegic patients might benefit from treadmill gait training.

팔걸이가 뇌졸중 환자의 보행속도 및 에너지 소모량에 미치는 영향 (Effect of Arm Sling on Walk Speed and Energy Consumption in Patients with Stroke)

  • This study was undertaken to identify the influence which affect on gait speed and energy consumption regarding putting on arm sling during gait of the 40 hemiplegic patients selected from University Hospital and rehabilitation center in seoul during two months. The analysis of data was performed using the paired samples ttest to compare the differences of gait velocity, heart rate, oxygen consumption and oxygen cost in gait of preand post- arm sling. The results of this study were as follows; 1. When comparing the result before putting on arm sling in the gait of hemiplegic patient, gait velocity after putting on arm sling was statistically significantly increased(p<.05). 2. When comparing the result before putting on arm sling in the gait of hemiplegic patient, heart rate after putting on arm sling was statistically significantly decreased(p<.05). 3. When comparing the result before putting on arm sling in the gait of hemiplegic patient, oxygen consumption per weight after putting on arm sling was statistically significantly decreased(p<.05). 4. When comparing the result before putting on arm sling in the gait of hemiplegic patient, oxygen consumption rate per weight after putting on arm sling was statistically significantly decreased(p<.05). When putting together the above result, the gait with arm sling in comparison with the gait without arm sling was to increase gait velocity, decrease heart rate, decrease oxygen consumption and was finally to decrease energy consumption in the gait of hemiplegic patient.

새로 개발된 3차원 전기측각기를 이용한 편마비 환자의 보행분석의 결과 (The Result of Gait Analysis of Hemiplegic Patients with the Newly Developed Three Dimensional Electrogoniometer Domotion®)

  • Background: The purpose of this study is to evaluate the difference between the hemiplegic patients and controls with the newly developed three demensional electrogoniometer gait analysis program. Methods: The basic kinematic data of hip, knee and ankle joints on the sagittal plane and of temporospatial gait parameters were obtained from 25 hemiplegic patients and 25 healthy adults with three-dimensional electrogoniometer Domotion$^{(R)}$ Results: Significant difference were observed between patients and controls in kinematic parameters. Mean maximal hip flexion of healthy adults and hemiplegic limb of patient was $32.89{\pm}1.8^{\circ}$ and $18.24{\pm}4.8^{\circ}$, maximal knee flexion was $50.32{\pm}2.4^{\circ}$ and $34.98{\pm}10.4^{\circ}$, maximal ankle dorsiflexion was $5.34{\pm}1.2^{\circ}$ and $1.22{\pm}2.8^{\circ}$, and maximal ankle plantar flexion was $15.63{\pm}2.0^{\circ}$ and $8.46{\pm}3.2^{\circ}$(p<0.05). Mean maximal hip flexion of healthy adults and unaffected limb of hemiplegic patient was $32.89{\pm}1.8^{\circ}$ and $28.36{\pm}6.6^{\circ}$, and maximal ankle plantar flexion was $15.63{\pm}2.0^{\circ}$ and $8.62{\pm}3.7^{\circ}$, respectively(p<0.05). Conclusions: The gait parameters of hemiplegic patients showed significant differences as compared with normal gait parameters with the using of three dimensional gait analysis with electrogoniometer.

  • Objectives: This study investigated the change of foot rotation angle of gait analysis parameters according to gait improvement in post-stroke hemiplegic patients. Methods: We measured the foot rotation angle of eight post-stroke hemiplegic patients at the time of dependent and independent gait. Results: The foot rotation angle of the paralyzed side reduced closer to normal average according to gait improvement, but the non-paralyzed side not significantly. Conclusions: Improvement of foot rotation is an important thing for independent gait of post-stroke hemiplegic patients so this treatment seems worthy of being considered in clinical trials.

  • Purpose : The purpose of this study was to evaluate the effect of craniocervical flexion exercise on deep cervical flexor thickness and gait in children with hemiplegic cerebral palsy. Methods : Twelve children with hemiplegic cerebral palsy were recruited for this study. All subjects performed active craniocervical flexion exercise 3times a week over the course of 6weeks. using a pressure bio feedback unit. Ultrasonography was used to assess deep cervical flexor thickness, and a 10m walking test was used to assess gait function. For the statistical analysis, a paired t-test was used to compare the differences pre- and post-value. SPSS Statistics version 20.0 was used for statistical analysis, and statistical significance was defined as a p-value less than 0.05. Result : The results of this study indicate that children with hemiplegic cerebral palsy experienced statistically significant positive changes in both deep cervical flexor thickness (p<.05). and gait(p<.05) following the intervention. Conclusion : In conclusion, craniocervical flexion exercise can positively affect deep cervical flexor thickness in children with hemiplegic cerebral palsy, which in turn positively affects gait.

집중적 전기 자극치료가 만성 뇌졸중 환자의 보행에 미치는 효과 (The Effect of Intensive Functional Electrical Stimulation on the Gait in Chronic Hemiplegic Patients)

  • Background: The purpose of this study is to investigate the effect of the intensive functional electrical stimulation(FES) on the improvement of the gait pattern of the chronic hemiplegic patients. Method: Six hemiplegic patients, who could walk independently but have equinovarus deformity during the gait cycle, participated in this study. The affected peroneus longus and tibialis anterior muscles of all subjects were stimulated for 2 weeks period (20 minutes duration, 6 times/day). We measured the activities (mean voltage) of those muscles during the walking, using dynamic EMG. Results: After treatment, there were significant improvements in the strength of peroneus longus and tibialis anterior muscles and the gait speed, but there was no interval change of the spasticity of plantar flexor. The mean voltages of two muscles are significantly increased in all the patients (p<0.05). Conclusion: The results showed that the intensive FES on affected peroneus longus and tibialis anterior muscles in chronic hemiplegic patients could be useful for the improvement of functional gait.

노력성 호흡운동을 통한 편마비환자의 기능적 보행지수 개선 (The improve of hemiplegic patients functional ambulation profile by forceful respiratory exercise)

  • The purpose of this study was to evaluate the change of functional ambulation profile(FAP) and temporal-spatial gait parameters in hemiplegic patient by forceful respiratory exercise. 28 Hemiplegic patients due to stroke was randomized in 3 groups, forceful expiratory training(FET), forceful inspiratory training(FIT) and control group. In the experimental groups, ordinary physical therapy with forceful expiratory training and forceful inspiratory training for 20 minutes duration 3 times per week for 6 weeks were respectively performed. In the control group, only ordinary physical therapy was done. FAP and temporal-spatial gait parameters was measured at before and after experiments. The results of this experimental study were as follows : 1. In comparison of FAP before and after experiment, the FAP was significantly increased in the FET and FIT group (p<.01). In comparison of difference of FAP among 3 groups, there was the significant difference between the FIT group and the control group (p<.05). 2. The results of temporal-spatial gait parameters are as follows : 1) In comparison of gait velocity before and after experiment, the gait velocity was significantly increased in the FET and FIT group (p<.05). In comparison of difference of the gait velocity among 3 groups, there was the significantly difference between the FIT group and the control group (p<.05). 2) In comparison of gait cadence before and after experiment, the gait cadence was significantly increased in FIT group (p<.05). In comparison of the difference of the gait cadence among 3 groups, there was no significant difference between the FIT group and the control group (p>.05). Based on these results, it is concluded that the forced respiratory exercise program for 6 weeks can be improve the FAP and temporal-spatial gait parameters in hemiplegic patients. Therefore, the forced respiratory exercise is useful to improve the walking ability in hemiplegic patients.
