운동선수들의 혈액분석을 통한 Etrythropoietin 간접도핑검사 (Blood Analysis for Indirect Doping Control of Erythropoietin in Sports)

  • The use of recombinant human erythropoietin (rhEPO), a stimulator of erythropoiesis, banned in sports because of the medical risk associated with thrombosis. Due to analytical difficulties to differentiate between natural human EPO (hEPO) and rhEPO, blood parameters of erythropoiesis such as contents of hemoglobin (cut-off value <17.5 g/d l for man, and < 16.0 g/dl for women), hematocrit and reticulocytes (cut-off value <2.0%) were measured to focus the misuse of rhEPO. We conducted anti-doping test for 122 blood samples of the World Cup athletes. The mean values of key parameters are as follows; 14.5$\pm$1.0 g/dl for hemoglobin, 41.7$\pm$2.8% for hematocrit, and 1.3$\pm$0.4% for reticulocyte. Blood sample was found to be stable up to 8 hours for the reticulocyte measurement. In addition, the soluble transferrin receptor and ferritin levels were measured by immunoassay methods using plasma samples (n=28) in which the mean value was 0.8$\pm$0.5 $\mu\textrm{g}$/$m\ell$ and 54.6$\pm$33.7 ng/$m\ell$, respectively. The results indicate that all samples tested were negative for the blood parameters of indirect anti-doping test for hEPO misuse. The statistical evaluation suggest that several other parameters such as red blood cell, mean corpuscular hemoglobin concentration, mean corpuscular volume, mean corpuscular hemoglobin and white blood cell could be considered as factors influencing hEPO function in addition to five parameters mentioned.

Erythrocyte Sedimentation Rate: Its Determinants and Relationship with Risk Factors Involved in Ischemic Stroke

  • Erythrocyte sedimentation rate (ESR) evaluation is a useful tool for monitoring disease activity in various inflammatory and non-inflammatory conditions. ESR is known to be influenced by a multitude of confounding factors. The present study aimed to assess the possible determinants of the ESR and its relationship with various risk factors involved in ischemic stroke. ESR and other hematological and biochemical parameters were investigated in 163 ischemic stroke patients (107 males and 56 females) selected based on imaging techniques including magnetic resonance imaging (MRI) or computed tomography (CT) scans. Statistical analysis was performed using the SPSS 16.0 software. Linear regression analysis showed a significant inverse relationship of hemoglobin (Hb) and hematocrit or packed cell volume (PCV) (P<0.001 for females; P<0.01 for males) with the ESR. It was observed that the red blood cell (RBC) count was not strongly correlated with the ESR (P<0.05 for both males and females). It was also observed that sex significantly affected the variables determining the ESR levels, whereas age had no effect. Gender differences were also observed with respect to Hb, RBC, PCV, mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), and ESR. The possible determinants of higher ESR levels in ischemic stroke may be sex, Hb, hematocrit, and RBC count, but the role of other clinical and laboratory parameters cannot be underestimated.

Advances in the use of dried blood spots on filter paper to monitor kidney disease

  • Patients with kidney disease require frequent blood tests to monitor their kidney function, which is particularly difficult for young children and the elderly. For these people, the standard method is to evaluate serum creatinine or cystatin C or drug levels through venous sampling, but more recently, evaluation using dried blood spots has been used. This narrative review reports information from the literature on the use of dried blood spots to quantify the main markers used to detect kidney diseases. The ScienceDirect and PubMed databases were searched using the keywords: "dried blood on filter paper," "markers of renal function," "renal function," "creatinine," "cystatin C," "urea," "iohexol," and "iotalamate." Studies using animal samples were excluded, and only relevant articles in English or Spanish were considered. Creatinine was the most assessed biomarker in studies using dried blood spots to monitor kidney function, showing good performance in samples whose hematocrit levels were within normal reference values. According to the included studies, dried blood spots are a practical monitoring alternative for kidney disease. Validation parameters, such as sample and card type, volume, storage, internal patterns, and the effects of hematocrit are crucial to improving the reliability of these results.

무심폐기하 관상동맥우회수술에서 아스피린과 Clopidogrel의 수술 전 병합투여가 수술 후 출혈에 미치는 영향 (Effects of Preoperative Combined Antiplatelet Agents on the Postoperative Bleeding that Occurs during Off-pump Coronary Artery Bypass Surgery)

  • 배경: 심폐기하 관상동맥우회술시 수술직전 항혈소판제의 병파투여는 수술 후 출혈과 출혈로 인한 수술 등의 합병증에 영향을 주는 것으로 알려져 있다. 하지만 무심폐기 하에서의 관상동맥우회술시 항혈소판제의 병합투여가 수술결과에 미치는 영향에 대해서는 잘 알려져 있지 않아 조사하고자 한다. 대상 및 방법: 2003년부터 2005년까지 무심폐기 하에서 시행한 관상동맥우회수술 중 항혈소판제(아스피린, clopidogrel)를 수술 12시간 전까지 지속적으로 사용한 19예를 대상으로 후향적 조사를 하였다. 수술 전 사용한 항혈소판제의 투여방법에 따라 아스피린 단독투여군 10예와 아스피린+clopidogrel 병합투여군 9예로 나누었는데 수술 전후의 혈소판, hemoglobin, hematocrit, prothrombin 시간, aPTT 그리고 수술 후 출혈과 출혈로 인한 수혈요구와 재수술여부에 대해 두 군을 비교하였다. 결과: 두 군간에 수술시간, 수술 중 2회 이상 측정한 ACT는 차이가 없었다. 수술 후 24시간 흉관을 통한 출혈량은 단독투여군 $697{\pm}271mL$, 병합투여군 $944{\pm}432mL$로 두 군간에 차이가 없었다. 수혈을 받은 환자의 수도 두 군 모두 3명으로 차이가 없었으며 출혈로 인해 재수술을 받은 경우는 두 군 모두 없었다. 수술 전후 측정한 헤모글로빈과 Hematocrit의 수치는 두 군간에는 차이가 없었으나 두 군 모두 수술 후의 수치가 수술 전보다 유의하게 감소하였다. 그러나 수술 전후의 감소량에 있어서는 두 군간에 차이가 없었다. 결론: 무심폐기하 관상동맥우회술에 있어 수술 전 aspirin과 clopidogrel의 병합투여는 aspirin 단독투여에 비해 수술 후 출혈로 인한 수혈이나 재수술 등의 수술조기결과에 있어 차이가 없었다. 하지만 저자의 경우는 대상 군의 수가 적어 향후 이에 대한 추가적인 연구가 필요할 것으로 사료된다.

An Intravenous Replenishment of Salivary Components and Dry Forage Intake in Freely Drinking Large-type Goats

  • Large-type goats eating dry forage secreted large volumes of saliva which resulted in the loss of $NaHCO_3$ from the blood and decreased plasma volume (hypovolemia). This research investigated whether or not the loss of $NaHCO_3$ from the blood and hypovolemia brought about by dry forage feeding actually depresses feed intake in large-type goats under free drinking conditions. The present experiment consisted of three treatments (NI, ASI, MI). All treatments in this experiment were carried out under free drinking conditions. In the NI control (NI), a solution was not infused. In the ASI treatment, i.v. infusion of artificial saliva was initiated 2 h before feeding and was continued for a total of 3 h concluding 1 h after the commencement of the feeding perod. In the MI treatment, mannitol solution was infused to replenish only water lost from the blood in the form of saliva. The hematocrit and plasma total protein concentrations during feeding in the NI control were observed to be higher than pre-feeding levels. This indicated that dry forage feeding-induced hypovolemia was caused by the accelerated secretion of saliva during the initial stages of feeding in freely drinking large-type goats. Increases in hematocrit and plasma total protein concentrations due to dry forage feeding were significantly suppressed by the ASI treatment. While hematocrit during feeding in the MI treatment was significantly lower than the NI control, plasma total protein concentrations were not different. From these results, it is clear that the MI treatment was less effective than the ASI treatment in mitigating the decreases in plasma volume brought about by dry forage feeding. This indicates that plasma volume increased during dry forage feeding in the ASI treatment which inhibited production of angiotensin II in the blood. The ASI treatment lessened the levels of suppression on dry forage feeding, but the MI treatment had no effect on it under free drinking conditions. The results indicate that despite the free drinking conditions, increases in saliva secretion during the initial stages of dry forage feeding in large-type goats caused $NaHCO_3$ to be lost from the blood into the rumen which in turn caused a decrease in circulating plasma volume and resulted in activation of the renin-angiotensin system and thus feeding was suppressed.

비둘기의 체내 조직 중 중금속 농도의 상관관계 (Correlation between Heavy Metal Levels in Tissues of Feral Pigeons(Columba livia))

  • 전국 6개 지역에서 서식하는 비둘기(Columba livia) 60마리를 채집하여 간, 신장, 뼈, 허파 조직의 Fe. Zn, Mn, Cd, Pb 농도를 측정하고 조직간, 원소간 상관관계를 분석하였다. 조사 결과 조직간, 원소간에 다양한 상관관계가 있는 것으로 밝혀졌다. 조직별 원소간 상관관계에서는 독성 원소인 카드뮴의 경우 체내 축적은 장점막내의 흡수 수용체로부터의 철 흡수를 저하시키며, hemoglobin과 hematocrit의 낮은 수치를 유도하므로 두 원소간 음의 상관관계가 있는 것으로 나타났다. 간 및 신장 조직 중 독성 원소인 카드뮴과 필수 원소인 아연과는 정의 상관관계가 나타났으며, 이는 체내 항상성 유지를 위한 독성방어 메커니즘과 밀접한 관련이 있음을 시사한다. 간 조직 중 납과 철과의 정의 상관관계는 6개 지역 중 상대적으로 농도가 높은 부산지역에서만 나타났으며, 이는 납이 hemoglobin 합성을 방해하여 간 조직 내 철의 보유량을 증가시킨다는 사실을 암시한다. 원소간 조직별 상관관계에서는 오염원소인 납과 카드뮴에서 상대적으로 다른 원소에 비해 다양한 상관관계가 나타난 것으로 조사되었으며, 이러한 조직간, 원소간 상관관계는 조류를 이용한 특정지역의 오염 및 위해 정도를 모니터링하는 데 필히 고려되어야 한다는 사실을 시사한다.

hEPO 형질전환 돼지 후대의 임신기간내 혈액조성 변화

  • 본 연구는 사람 조혈촉진유전자(hEPO)가 도입된 형질전환돼지 Fl 수컷과 암컷간 교배로 F2 생산시, Fl 모돈에 대한 임신기간(약 114일)내 혈액조성(적혈구, 백혈구, Hemoglobin, Hematocrit value, MCV, MCH, MCHC, 혈소판) 변화를 구명하기 위하여 실시하였다. 시험에 이용된 암퇘지는 형질전환 F1 암컷 2마리와 일반돼지 1마리였고 교배와 동시에 임신기간동안(114일) 5일 간격으로 5ml의 혈액을 경정맥(jugular vein)에서 채취하였으며 혈액조성분석은 Celltac MEK 5108K(Nihon Kohden, Japan)를 이용하였는데 얻어진 결과는 다음과 같다. 임신기간중 혈액내 백혈구의 변화는 일반돼지의 경우 13.8($\pm$2.4)$\times$$10^3$/ul 로임신기간내에 일정한 변화양상을 보였으나 형질전환돼지의 경우 29.4($\pm$19.4)$\times$$10^3$와 22.5($\pm$14.1)$\times$$10^3$/ul 으로 일반돼지보다는 높게 나타났으며 임신기간내에 매우 높은 변화 양상을 나타내었다. 또한 적혈구는 일반돼지(7.2$\pm$0.7 $\times$$10^{6}$/ul)에 비하여 형질전환돼지(11.5$\pm$0.5 와 11.9$\pm$0.5 $\times$$10^{6}$ul)에서 매우 높게 나타났으며 변화양상은 일반돼지와 마찬가지로 작게 나타났다. 적혈구와 함께 Hemoglobin(Hb)과 평균혈구용적율(Hematocrit value ;, HCT) 모두 형질전환돼지가 일반돼지보다 높게 나타났으며 변화양상 또한 작게 나타남으로서 사람 조혈촉진유전자(hEPO)가 형질전환된 돼지에서는 지속적으로 발현되어 조혈촉진이 이루어지고 있으며, 조혈촉진에 의해 헤모글로빈 및 적혈구가 생성되어 혈액내 높은 혈구용적율(HCT)을 이루고 있음을 알 수 있었다. 적혈구 평균용적(Mean Corpuscular Volume ; MCV), 평균적혈구혈색소량(Mean Corpuscular Hemoglobin ; MCH), 평균적혈구혈색소농도(Mean Corpuscular Hemoglobin Concentration ; MCHC) 그리고 혈소판(Platelets) 분석결과 형질전환돼지가 일반돼지보다 약간 높은 수치를 나타냈으며 변화양상 또한 유사한 결과를 나타내었다. 이와 같은 시험분석결과를 토대로 사람 조혈촉진유전자(hEPO)가 형질전환된 돼지는 유즙으로 발현할 수 있도록 형질전환 되었음에도 불구하고 헤모글로빈 및 적혈구가 증가함으로서 형질전환돼지 개체의 혈장으로도 사람 조혈촉진인자가 분비하고 있음을 간접적으로 알 수 있었다. 정상돼지 보다 형질전환돼지가 약 30% 높은HCT 수준을 보였으며 이러한 현상은 사람에서는 적혈구증다증(erythrocytosis)으로 분류되고 있다. 이에 대한 고찰은 형질전환돼지 자체의 생리적 문제점(side effects)에 대한 해결과 더불어 기존의 인간질병에 대한 모델동물로서의 이용 가능성을 제시할 수 있는 것으로 사료된다.

해상가두리 양식장 어류의 혈액 검사에 사용된 동물용 생화학 분석장치의 활용 가능성 (Application of Veterinary Chemistry Analyzer used to Hematological Analysis of Marine fish Cultured in Floating Netcage)

  • 본 연구는 동물용 생화학 분석장치를 이용하여 분석한 어류 혈액검사 항목 가운데 적용할 수 있는 분석항목의 활용도를 평가함과 동시에 어류 혈액학적 건강진단에 필요한 분석항목의 참고자료를 얻고자 하였다. 경남 지역의 해상가두리 어류 양식장에서 사육중인 육안적으로 보아 건강한 조피볼락 (Sebastes schlegeli), 참돔 (Pagrus major), 돌돔 (Oplegnathus fasciatu) 및 감성돔 (Acanthopagrus schlegeli) 총 522마리를 대상으로 hematocrit 와 간편하게 혈액성분을 측정할 수 있는 FUJI DRI-CHEM 3000 기종을 이용하여 혈장화학적 성분 분석을 실시하였다. Hematocrit (Ht)는 microhematocrit법에 의해 그리고 hemoglobin (Hb), 총단백 (TP), 알부민 (ALB), 알칼리성포스파타제 (ALP), 요소질소 (BUN), 젖산탈수소효소 (LDH), 중성지방 (TG), 총콜레스테롤 (TCHO), 크레아티닌 (CRE), aspartate aminotransferase (AST), alanine aminotransferase (ALT), 포도당(GLU)은 정상범위의 표준자료가 이미 프로그램화되어 장치된 동물용 생화학 분석장치인 FUJI DRI-CHEM 3000 기종을 이용하여 분석하였다. 그 결과, 측정된 Hb와 혈장화학성분의 실제 값들은 본 기종의 하한 및 상한 한계치를 상당히 벗어났다. 본 장비로부터 분석값을 검출하지 못하였던 비율을 살펴보면, ALB 및 ALT는 68~66%, LDH, TP, ALP 및 GLU는 42~21%, TCHO, AST, TG, Hb 및 CRE는 18~3%의 범위를 나타내었으나, BUN의 분석값은 모두 검출한계를 벗어나지 않았다.

가물치의 혈청단백질에 미치는 자외선전신조사의 영향 (Effects of Whole Body Ultraviolet -Light Irradiation on Serum Protein in Snake Head, Ophicephalus argus(CANTOR))

  • The present paper deals with the changes in serum protein fraction, total serum protein, hematocrit, red blood cell, haemoglobin, and weight of liver or kidney : body weight ratio of irradiated and non-irradiated snake head, Ophicephalus argus(CANTOR) . Irrardiation doses are 2537$\AA$-7 minutes (7M) and 2537$\AA$-15 minutes (15M). Serum electrophoretic patterns showed a marked decrease in albumin fraction at 1, 3, 9, 12 and 15 days on 7M group and 15 M group. On both experimental groups percentage increases in $\alpha$1 and $\beta$ fractions occurred at different time periods in general but are interpreted as only apparent changes accompanying the greater albumin fall. ${\gamma}$-Globulin decreased at 1, 6 and 15 days on 7M group. and at 1, 3, 6 and 9 days on 15M group. Also, A/G ratio was significantly low in groups subjected to above conditions as compared to the controls. A/G ratio decreased at 3 and 15 days remarkably on both groups. On the average, the reductions in the A/G ratio were not proportional to themagnitude of ultraviolet-light. Total protein of serum changed according to suggestive changes in electrophoretic patterns of serum . Total protein of serum declined at 1, 3, 6, 9 and 12 day periods on 7M groups and at 1, 3, 9, 12 and 15 day-periods on 15 M group, and increased approaching control values at 15 and 18 day-periods on both groups. Hematocrit increased remarkably at 1, 3, 6 and 12 days and decreased at 9 and 15 days on 7M group, and increased throughout the sampling period on 15M . Red blood cell decreased throughout the sampling period and increased slightly at 6 days on 7 M group and decreased at 1, 6, 9, 15 and 18 days and increased remarkably at 3 and 12 days on 15M group. Hemoglobin decreased remarkably at 1 day-period and increased at other days on both groups. The liver weight was not remarkably changed after whole body irradiation on both groups. and kidney was increased from 1st day on 7M group and 3 rd day on 15M group respectively. It appears that changes in electropphoretic patterns of serum, A/G ratio, total protein of serum, hematocrit, red blood cell, hemoglobin, and liver or kidney weight act to the detriment of the animal following non-ionizing irradiation.

토복령약침자극(土茯笭藥鍼刺戟)이 $HgCl_2$중독(中毒) 흰쥐의 손상신(損傷腎)에 미치는 영향(影響) (The Effect of Smilacis glabrae Rhizoma aqua-acupuncture on injured kidneys Induced by $HgCl_2$ Intoxication in Rats)

  • Purpose: In order to study the therapeutic effect of TBR(Smilacis glabrae Rhizoma) aqua-acupuncture on the $HgCI_2$(mercury bichloride) intoxicated kidneys of rats. Methods: 10% TBR extract solution was aqua-punctured into the bilateral loci of Shin-su(BL23), every other day for 18 days, and the amount of leucocytes, erythrocytes, thrombocytes, hematocrit, hemoglobin, serum BUN and creatinine levels were measured and renal mercury level and histopathological findings were studied as well. The experimental groups were into nonmaI group, control group, sample I group, sample II group and sample III group. Results: 1. There were not any significant changes of leucocytes, erythrocytes and serum creatinine level in the TBR aqua-acupuncture group, saline aqua-acupuncture group and TBR aqua-acupuncture group respectively as compared with control group. 2. It showed a significant increase of hematocrit and hemoglobin level in the TBR aqua-acupuncture group on the 8th experimental day as compared with the control group. 3. It showed a significant increase of platelet in the TBR aqua-acupuncture group on the 8th experimental day, whereas significant decreases on the 18th experimental day as compared with the control group. 4. It showed a significant decrease of serum BUN level in the TBR aqua-acupuncture group on the 8th and 18th experimental day as compared with the control group. 5. It showed a significant decrease of renal mercury level in the TBR aqua-acupuncture group on compared with the control group. 6. In the histopathological findings, TBR aqua-acupuncture group showed slight atrophy of the renal tubules, mitosis and regeneration of tubular lining cells with visible nuclei, whereas the control group showed acute tubular necrosis, especially severe necrosis of tubular epithelial cells. Conclusions: Smilacis glabrae Rhizoma aqua-acupuncture on injured kidneys Induced by HgCl2 Intoxication in Rats increases hematocrit, hemoglobin and platelet level, and decreases BUN, renal mercury level. This suggests that Smilacis glabrae Rhizoma aqua-acupuncture helps to recover kidney from HgCl2 intoxication.
