Recently we experienced one, case of epiphrenic esophageal diverticulum of a 50 - year - old female patient who had complained heaviness in her chest after meals and occasional dysphagia for one year. Preoperative barium study showed a large epiphrenic esophageal diverticulum at about 7cm above the diaphragm which protruded to the right side of the mid thorax. On the operation field, epiphrenic esophageal diverticulum, measuring 5x 6x3cm in size, was noted. Diverticulectomy and extended myotomy was performed and the postoperative course was uneventful.
Objectives : This study was designed to find out the differences of the acupuncture sensation by body parts. Methods : Sixty-three subjects got acupuncture at five acupoints which represent five different body parts ; head($GV_{20}$), abdomen($ST_{25}$), back($BL_{24}$), upper extremity($LU_9$), lower extremity($GB_{40}$). All subjests were asked to complete questionnaire rating the intensity of 13 kinds of acupuncture sensation(acupuncture sensation scale, ASS). We compared the subjective acupuncture sensation between the body parts. Results : Intensity of acupuncture sensation of $GV_{20}$ was significantly lower than $LU_9$(p=0.001) and $GB_{40}$(p=0.000). Sum of acupuncture sensation of $GV_{20}$ was also significantly lower than $BL_{24}$(p=0.011), $LU_9$(p=0.004) and $GB_{40}$(p=0.033). Among the 13 types of acupuncture sensation scale, tingling and aching were well sensed at $GV_{20}$ and $ST_{25}$, aching, tingling and sharp pain were well sensed at $LU_9$, $GB_{40}$, dull pain, deep pressure and heaviness were well sensed at $BL_{24}$. Conclusions : Head showed significantly lower intensity of acupuncture sensation than upper extremity and lower extremity. Among the acupuncture sensation scales, tingling and aching were well sensed at head and abdomen, aching, tingling and sharp pain were well sensed at upper extremity and lower extremity, dull pain, deep pressure and heaviness were well sensed at back.
Purpose: The purpose of this study was to examine the sensation changes in upper extremity and the quality of life for post-operative mastectomy patients. Methods: This study used a descriptive study design. The participants were 132 women who had mastectomies after being diagnosed with breast cancer and were participating in breast cancer self-help groups. The sensation changes in the arm of the surgery site was measured by the arm symptoms which were subjective uncomfortable feelings of the affected arm, and the quality of life was measured by Medical Outcomes Study Short Form-36. Collected data were analyzed by ANOVA, t-test, ${\chi}^2$-test and multiple regression with SPSS WIN 12.0 program. Results: Patients with stage IV breast cancer had severe changes in sensation of the arm. Participants with lymphedema had statistically significant changes in sensation compared to participants without lymphedema, but the quality of life was not different between the two groups. Pain by the SF 36 was statistically different between the two groups, with- and without-lymphedema. The factors in upper extremity's sensation changes which influence on quality of life were pain and heaviness. Conclusion: Nursing intervention for relieving pain and heaviness of the affected arm needs to be developed in order to improve QOL of the breast cancer survivors.
PURPOSE: This study aimed to compare the degree of lumbopelvic rotation during the active straight leg raise (ASLR) test in chronic low back pain with and without lumbar segmental instability. METHODS: A total of 71 patients with chronic low back pain were recruited for this study. The subjects who tested positive for more than three of the five lumbar segmental instability tests (prone lumbar instability, lumbar passive extension test, anterior posterior mobility test, passive straight leg raise, age) were categorized into the lumbar segmental instability positive group. Patients who tested positive for less than three of the five tests were categorized into the lumbar segmental instability negative group. The lumbopelvic rotation was measured three times during ASLR and a mean was determined. Subjective heaviness during the ASLR was measured on 6 point scale. RESULTS: There was a statistically significant difference in the lumbopelvic rotation angle between the groups with and without lumbar segmental instability (p<.01). There was no significant difference in the subjective heaviness during ASLR. The mean lumbopelvic rotation angle during ASLR was $13.54{\pm}2.86^{\circ}$, and $8.81{\pm}2.47^{\circ}$ in the positive and negative groups, respectively (p<.01). The cut-off value of the lumbopelvic rotation during was $10.5^{\circ}$, the sensitivity was 82.9%, and the specificity was 80.6%. CONCLUSION: These results suggest that lumbopelvic rotation is more prevalent in patients without lumbar segmental instability. Clinically, this important when diagnosing chronic low back pain with lumbar segmental instability, as the lumbopelvic rotation angle during the ASLR test can be used to aid in diagnosis.
Sleeping is important for human being in that it has a great effect on their daily activities. Considering that an average adult sleeps seven to eight hours a third of our life is spent in bed. The present study intends to make consumers aware of the important of the bedding science and help the manufactures develop efficient, up-to-date beds. For these two purposes, consumers' recognitions and opinions on the features of the bedding were surveyed. In the study, a questionnaire consisting of various items on the features of the bedding was designed and distributed to consumers in 8 regions & Taegu in Korea to survey the patterns in which they buy, use, and maintain their bedding. The result of the study are as follows; Air conditioner is widely used popular because of hot weather in Taegu. People in Taegu mostly purchase their bedding from market, in every 5~6 years. The important point to be considered for bedding purchases were humidity absorption, air permeability, light weight in summer comforters, thermal insulation, flexibility, color figure design in winter comforters, humidity absorption, flexibility, color figure design in mats. As summer bedding, a sheet of single-layer quilt and rush mat were most popular which indicates climate of hot and humid weather in Taegu. Bedding uses in winter were in order a sheet of cotton, silk, wool quilt, and silk quilt showed high level in contrast to other areas. Dissatisfactions with summer quilts were humidity absorption, air permeability, heaviness and in winter quilts they were dissatisfied with thermal insulation, heaviness, flattening. In details about bedding managements, people in Taegu most frequently disinfected their bedding by sunlight in one month, and refinished every 5 years.
The problem of selection of a subset containing the largest of several slope parameters of regression equations is considered. The proposed selection procedure is based on the weighted median estimators for regression parameters and the median of rescaled absolute residuals for scale parameters. Those estimators are compared with the classical least squares estimators by a simulation study. A Monte Carlo comparison is also made between the new procedure based on the weighted median estiamtors and the procedure based on the least squares estimators. The results show that the proposed procedure is quite robust with respect to the heaviness of distribution tails.
This study was intended to identify the desired image and fabric preference of woman college student for apparel and the relationship between these variables. Data were collected by questionnaire distributed to 260 woman college students through August to September of 2000. After eliminating incomplete questionnaires, 236 were analyzed. The most desired image was neat, and the most preferred fabric was natural fiber textile in fiber contents and light and soft in fabric sensation. The avoided image was masculine, and the negative sensation was heaviness. As a result of factor analysis, 7 factors -cute, intellectual, animate, neat, mature, sporty, sexy- were determined in image. And 8 factors -soft, durable, flat, rigid, light, bulky, heavy, pliable- were determined in fabric sensation. These factors explained large percentage of variance respectively. Multidimensional scaling was employed to analyze the relationship between desired image and fabric preference. Two dimensions were accepted to interpret the relationship. One of the results showed the closeness among the natural fiber textile, lightness, durability and natural image. And the short distance among the blended textile, elasticity, warmness and sporty image was presented.
Objective : To study needling sensation and the flow of the needling sensation the ancient and the present literatures were reviewed. Results : 1. The needling sensation was getting the Qi and a normal reaction of acupuncture. 2. The flow of the needling sensation was promoted the Qi and normalized the flow of the Qi 3. The needling sensation of a patient expressed soreness, distension, heaviness, numbness, tingling, flushing, fervor, chill, itching, and so forth. 4. The needling sensation of a patient expressed simple or complex syndrome. 5. The needling sensation of a doctor expressed objectively more than the needling sensation of a patient in the hand feeling of a doctor. 6. The doctor was compared good needling sensation and bad needling sensation. 7. The needling sensation and the flow of the needling sensation were processed continuously. 8. The needling sensation and the flow of the needling sensation were connected with a nervous system and expressed variously in the anatomical structure and hand acupuncture tenchnique.
From July 1988 to January 1991 six patients, aged 29 to 70 years underwent transfemoral thrombectomy for the treatment of deep vein thrombosis. Preoperative venograms showed thrombi in the following locations: calf veins[five], superficial femoral or popliteal veins [four], common femoral veins[three], and iliac veins[three], Durations of symptoms before admission were from 1 day to 20days. Operations were performed under local anesthesia and all the patients were requested for doing Valsalva maneuver during thrombectomies. All patients were received heparin pre-and postoperatively, which was switched to Coumadin for preventing of rethrombosis. One patient was transferred to other hospital 4 months after operation due to regional reason, and the remained five patients were evaluated with a mean follow-up time of 20 months. There was no evidence of postoperative pulmonary embolism. Three of five patients were clinically asymptomatic. One complained of the heaviness of involved leg in the evening, and the other had discomfort on walking Even though our cases were a few in number, we concluded that thrombectomy is a valuable treatment modality of deep vein thrombosis.
Fabric touch was evaluated psychophysically in order to determine the relationship between mechanical properties and subjective sensation. For subjective touch sensation, eight aspects such as hardness, smoothness, coarseness, coolness, pliability, crispness, heaviness and thickness were evaluated using free modulus magnitude estimation (FEME) technique. KES-FB was used to measure the mechanical properties of fabrics. Woolen fabric with the highest values of WC and weight was evaluated as the coarsest, heaviest and thickest. While silk crepe do chine with the lowest LT, G, 2HG, thickness and weight was rated as smoother and more pliable than any other fabrics. And flax with the highest values of LT and SMD was evaluated as hard, cool and crisp. Fabric touch and satisfaction were predicted well from the mechanical properties, especially from SMD, by regression analysis. Satisfaction for touch increased as smoothness increased.
