The Study on the Books of Oriental Medicine Which Deal with Variation in Diagnosis on the Neck and Nuchal Pain (경항통(頸項痛)의 변증(辨證)에 관한 문헌고찰(文獻考察))

  • Objectives : The studies on variation in diagnosis on the neck and nuchal pain has not been done thoroughly as we can use it in clinical practice of these days. For this reason, I examined the variation in diagnosis mentioned in the classics of Oriental Medicine as the preceding study on standardization of variation in diagnosis on neck and nuchal pain. Methods : I gathered the twenty kinds of classics of Oriental Medicine that were computerized, the textbooks on Oriental Medicine which are being used these days, and the theses on current clinical research. After gathering these data, I analyzed these according to the variation in diagnosis. Results : The classics of Oriental Medicine on the neck and nuchal pain mentioned very much about the neck and nuchal pain occurred by the pathogenic factor of Wind, Cold, and Dampness, disharmony created by deficiency of Liver and Kidney, and pathogenic state of Meridians of Taiyang. According to the texts of these days, the differentiation of syndromes can be divided into four kinds of items such as Wind-Cold pathogen, Wind-Dampness pathogen, Phelgm-Heat, and disharmony between Qi and Blood. The theses of these days rarely mentioned about variation in diagnosis on the neck and nuchal pain. Conclusions : The differentiation of syndromes on the neck and nuchal pain can be divided into four kinds items as affection by exopathogen like Wind, Cold, Dampness, Heat, and so on, stagnation of Qi and the coagulation blood, deficient syndrome of Liver and Kidney, and deficient syndrome of Qi and Blood.

Two Cases of Chest Fever with Dysphoria in Stroke Patients Treated with Yangkyuksanhoa-tang (중풍환자의 흉중번열증(胸中煩熱症)에 대한 양격산화탕(凉膈散火湯)투여 2례(例))

  • Fire is known as one of the most common causes of stroke. Chest fever with dysphoria caused by heat in the stomach is one of the fire symptoms, meaning feeling oppressed in the chest. We observed two stroke patients who had chest fever with dysphoria. These patients' clinical symptoms were headache, thirst, heat in the upper part of the body and constipation. They were diagnosed as Chest fever with dysphoria. It is assumed that Exuberance of Yang causes heat. We treated them with Yangkyuksanhoa-tang(凉膈散火湯) based on Dong Ui Su Se Bo Won for about 2 weeks. During this period, we observed that their symptoms of heat in the upper part of the body and constipation had been improved. We suggest that Yangkyuksanhoa-tang makes the balance between water and fire, and improves chest fever with dysphoria of stroke patients.

  • The results of analyzing "Shanghanlun-Shanghan examples" of Song Dynasty Text and Liuwansu's writings in the pursuit of comprehending his views written in "Fire-Heat" & "Warm disease" -are as follow. 1.He introduced Exterior-Interior Shanghan and Latent in the Winter & recurrent in the Spring Shanghan concept in the Febrile disease pathology as the link and the bifurcation point. His perception on Febrile disease encompassed Shanghan and Febrile disease on the same plane but actually discriminated in the treatment and medication between the two actually. 2.He suggested Febrile disease pathology in the concept of Shanghan and Febrile disease, which is coherent in the formation of interior heat as the result of the invasion of the lung by warm pathogen described in Warm disease Wei, Qi, Ying and Xue Fen Syndrome differentiation. He expanded the concept of esoteric portal 玄府 of "Suwon Tiaojinglun", to that of the omni-present portal & way of Qi's all directions movement, which is in accordance of invasion of upper energizer 上焦 by warm or hot pathogen via mouth and nose as used in Warm disease three-energizer differentiation. 3.He recognized both exterior-interior febrile disease in the context of both exterior-interior contact cause disease. He prescribed pungent-cool exterior releasing medicinal similar to that of exterior-releasing treatment adopted in warm disease. As lily-talc powder百合散 introduced in "Golden chamber synopsis" dispells heat by promoting excretion of urine and defecation, it is regarded as the original formula of later ikwonsan yiyuansan 益元散.

  • Impulse of ki refers to palpitation around the navel, it is mostly caused by impairment of yang due to erroneous sweating, deficiency of the spleen yang. Behcet's syndrome caused by prolonged damp-heat and pathogenic germs. Its symptoms are characterized by conjunctival congestion, blue canthus and ulceration of the oral cavity, restlessness all the time, etc. It is advisable to follow the therapeutic principles of clearing away heat and drying damp, detoxicating and sterilizing. Lily disease is one of the emotional disease. It caused by yin deficiency of the heart and lung, found in mental depression or convalescence after a critical disease. Its symptoms are characterized by restlessness, reticence, insomnia, failure to walk, poor appetite, a subjective hot or cold, bitter taste and dark urine. It is advisable to follow the therapeutic principle of nourishing yin to clear away heat. Cholera refers to the disease marked by sudden severe vomiting and diarrhea, and colic of the heart and the abdomen. It is mostly caused by deficiency of the ki in the middle-energizer, emotional stress, etc. It is characterized by such symptoms as sudden severe vomiting and diarrhea, restlessness, etc. It is advisable to follow the therapeutic principles of warming yang and removing toxic substances, and strengthening the spleen and stomach. Heat invasion of blood chamber due to affection of the exterior pathogens, pathogenic heat invading the blood in deficiency and fighting with the blood. Its symptoms are marked by fever, feeling of the fullness, etc. There are deficiency of blood, hot blood and stasis of blood. The different therapeutic treatment should be taken according to different types thereof.

  • Allergy has a born predominant cause of atopy in body and react oversenstively several meterial. The cause of allergy are tick, house dust, egg, milk, bean, cosmetics, virus, bacteria, flesh and meat in Western medicine. While, Oriental medicine did not have an application on the allergy, but recognized that allergy is connected with fetal toxicosis, heat-syndrome in new born, syndrome characterized by dyspnea, sneezing, stuffy nose, nasal discharge and phlegm. Therefor, We knowed method of medical treatment following cause of disease and pathogenesis against allergy. Allergic disease(atopic dermatitis, bronchial asthma, rhinitis) were related with lung, spleen, kidney. To prevent allergic disease in born, Oriental medicine taked a serious view of fetal education. In Western medicine, The cause of allergy applied to exopathogen of Oriental medicine. In Oriental medicine, Treatment of allergy used generally strengthening the body resistance to eliminate pathogenic factors.

A Study about trends of using Shihogayonggolmoryo-tang granule (시호가용골모려탕 과립제의 임상경향 분석 및 활용)

  • Objectives : This study was carried out to investigate trends of using Shihogayonggolmoryo-tang granule on neuropsychiatry fields. And suggest that using herbal extracts change non-prescription medicine to prescription medicine. Methods : I researched 86 outpatients who treated with Shihogayonggolmoryo-tang granule at oriental medicine hospital. And I analyzed their medical records of chart review study. Results : 1. Average medication time in this research was 14.9 days and the main usage was supplementary to herbal medicine, such cases amounted to 62%. 2. There are 52 patients out of 86 who are much better, such cases amounted to 55%. 3. The improved diagnosis is Syndrome of heart-fire flaring up, Syndrome of upper energizer dryness-heat, headache, and tinnitus. The improved symptom is anxiety, headache, dizziness, tic disorders, and chest discomfort. Conclusions : I expect to using herbal extracts change non-prescription medicine to prescription medicine based on this study.

Study on the Dossisosihotang (도씨소시호탕(陶氏小柴胡湯)에 대한 연구(硏究))

  • Objectives : 28 prescriptions with the name 'Dossi' can be found in Yixuerumen, and all these prescriptions are highly useful due to their clinical effectiveness. Among them, Dossisosihotang has an important role as the essential prescription utilizing harmonizing and releasing formula. Therefore, this paper aims to ponder upon this prescription. Methods : Dossisosihotang's origin and effectiveness were studied, and the findings served as the basis for analyzing the varied forms of this prescription which came about in the later generations in order to consider more detailed usage of this prescription. Results & Conclusions : Dossisosihotang directs its patient to consume sosihotang added with chenpi, shaoyao, chuanxiong, then simmered, and added with three spoons worth of raw wormwood juice. Its origin can be traced back to shachechuifa's 'Sihossanghaeeum,' and it is mainly effective for treating the blood aspect syndrome of lesser yang disease such as women's cold damage and heat entering the blood chamber. In treating the symptom of half-exterior and half-interior, taohua developed prescriptions depending on the syndrome, such as Sihijigyungtang, Sihohamhyoungtang, Samhosambacktang, Siryungtang, Sihosamultang, Sigalhaegitang, and Samhoseoggotang. These prescriptions became widely known for their clinical efficacy, leading to many later generations of clinical doctors to use them.

The understanding of Gwan-Gyeok in "Nangyeong" ("난경(難經)"의 관격(關格) 인식)

  • Gwan-Gyeok(關格) is one of the dangerous conditions that can lead to death and is considered important in clinical practice. However its true concept is unclear and arguments on the subject have been diverse over generations. This kind of confusion is largely due to an insufficient understanding of the study results on Nangyeong(難經). In Nangyeong, Gwan-Gyeok is divided into 'in bowels(在腑)' and 'in viscera(在臟) and distinction is made between 'damaged by heat(傷熱)' and 'damaged by dampness(傷濕) thus establishing a broad outline of differentiation of syndrome(辨證). Moreover, the clinical progress is systematically divided into 3 stages thus providing a very useful viewpoint on diagnostics. A sharp perspective on the pathogenesis is also shown by emphasizing the kidney and Myeongmun(命門) through the comparison between cheok pulse(尺) and chon pulse(寸). This point of view in Nangyeong is truly proposing a permanent standard on the understanding of Gwan-Gyeok. Therefore it is the author's hope that this study will work as a start to look back on the flow of the research on Gwan-Gyeok which has been rather confusing since Sanghallon(傷寒論).

  • There are many factors to evoke pain, for example cold, heat, seven emotion, digestion, fatigue, venemous worms, deficiency of blood etc., and its pathogenesis is complicated. Treatments for pain in oriental medicine are as follows. 1. Pharmacotherapy is a method to treat pain by different effects of herbal medicine, for example, regulating vital energy, activating blood circulation, eliminating phlegm, and dispelling the cold evil etc. 2. Acupuncture and moxibustion therapy is a kind of external application, it has a distinct effect in relieving pain. Recently its operation method has been more developed, so new acupuncture therapies are used in clinics. There are electroacupuncture, auricular acupuncture and injection of drugs at an acupuncture points etc. 3. Oriental physiotherapy is a treatment method to stimulate acupuncture points and meridians. (1) Hot pack, Paraffin bath, Microwave, Ultrasound, Short wave, Infra-red etc. are used to treat cold syndrome by dispersing cold by warming the channels. (2) E.S.T., I.C.T., T.E.N.S., S.S.P., cupping treatment etc. are used to treat disorders of vital energy. (3) Health ion, Carbon, T.D.P., etc are used to treat conic and deficiency syndrome.

  • According to the literatural study om ryuk-jeol disease(歷節病), we obtained the result as follows : First, ryuk-jelo disease(歷節病) was stated a kind of arthralgia syndrome(痺病), Because ryuk-jeol disease(歷節病) is only joint disease while arthrlgia syndrome(痺病) affects skin, muscle, joint, bone, organs, etc. Second, ryuk-jeol disease(歷節病) can bne thought to be a category of rheumatoid arthritis in western medical science. Third, etiology factors of ryuk-jeol disease(歷節病) was classified endogenous and exogenous pathogenic factors. The formers was the deficiency in both Qi and Blood(氣血不足), the deficiency in both the liver and kidney(肝腎虧損), dam(痰), blood stasis(瘀血). The latter was Windg風), Cool(寒), Dampness(濕), Heat(熱). Forth, the therapy of ryuk-jeol disease(歷節病) was based on 'diagnosis and trentment based on over all analysis of symptoms and sings(辨證施治)'
