The Rare Causes of Rhabdomyolysis; Parainfluenza Virus type I Infection and Hypernatremia (드문 원인에 의한 횡문근융해증; 제 1형 파라인플루엔자 감염과 고나트륨혈증 각 1례)

  • Rhabdomyolysis, the clinical syndrome caused by the injury to skeletal muscle resulting in the release of muscle cell contents into the systemic circulation, has been described in association with various factors. The causes include crush injury, skeletal muscle overuse, heat, drug, abuse of alcohols and metabolic disorders as well as several types of viral and bacterial infections. We report two cases of rhabdomyolysis, which were complicated by uncommon causes, parainfluenza virus type I infection and hypernatremia.

  • This article is a study on to which categories of modern diseases blood deficiency pattern types are assigned by reference to modern clinical papers. Clinical papers were searched in China National Knowledge Infrastructure(CNKI) from 1994 to 2013. Results are as follows. First, diverse diseases classified in qi-blood depletion pattern and pattern of blood deficiency and wind-dryness are reported and pattern types designated by the name of viscera are the minority. Second, among pattern types in Korean Standard Classification of Diseases(KCD), diseases classified in heart blood deficiency pattern, liver blood deficiency pattern and heart-liver blood deficiency pattern are a few. Third, the level of designation by the combined patterns such as qi deficiency, fluid deficiency, yin deficiency, kidney deficiency, essence deficiency, wind-cold, cold-dampness, dampness-heat, liver hyperactivity, liver depression and static blood is more specific than KCD, which makes pattern types more useful to clinical application. The detailed relation between modern diseases and pattern types can be an another topic.

A review on Clinical Trials by Using a Computerized Tongue Diagnosis System (국내외 설진기를 활용한 인간 대상 연구현황)

  • Objectives The purpose of this study was to survey the status of clinical use of a computerized tongue diagnosis system (CTDS) Methods We searched domestic/international articles using the CTDS from online medical databases including OASIS, NDSL and pubmed. We selected articles on clinical application or reliability of CTDS but excluded articles on mechanical design or software programming for developing a new CTDS. Finally we found 15 articles and classified the articles according to the study purpose. Results Out of the 15 articles, 8 were focused on the clinical application including halitosis, cold/heat syndrome, lung cancer, xerostomia etc. Other 5 articles were aimed at evaluating and improving reliability of CTDS. The other 2 articles were studied for development of differential diagnostic criteria on tongue coating thickness. Conclusion We found out that until now the researches on clinical application of CTDS mainly had been performed for producing a variety of CTDSs. Considering the importance of the tongue color in the traditional Korean medicine, we suggest that at first standard operating procedure for CTDS be developed and researches to develop differential diagnostic criteria on tongue body/coating color be performed and then explore its applications.

A Study of Indoor Thermal and Air Quality in Middle and High School Classrooms during Winter (겨울철 학교교실의 실내열.공기환경 실태와 학생들의 주관적 반응)

  • This is a basic study to improve air quality of school classrooms in winter time. The purposes are to check indoor thermal and air environment in school classrooms during winter and to analyze influencing factors on indoor environment. The measurements of students' physical elements with questionnaire surveys were carried out in a total of 6 classrooms. As a result, this research shows that the temperature of one classroom was below indoor thermal standard level, three classrooms had lack of heat, and two classrooms are heated much, which induce relatively low humidity. All of 6 classrooms had lack of ventilation, being high level of $CO_2$ concentration and 2 classrooms are in condition of high PM10 concentration. The majority of students(76%) answered that the cause of their 'heated space syndrome' is because of the lack of ventilation. Students' opening windows for ventilations is hardly carried out at normal times, except that indoor temperature is over standard. That is, we can suggest one of solutions, which is to enable students to operate heating and ventilating system by themselves according to students' physical condition.

  • The four taboos in using Baekhotang as explained by Odang(吳瑭), are identical to the standard symptoms of Severe Exterior Heat Syndrome[表熱重證]. There are similarities between Sanghan and Onbyeong in using Baekhotang(白虎湯). But there are significant differences between Sanghan and Onbyeong in explaining the pathogenesis of human body. In pattern identification by the Wi-Gi-Yoeng-Hyeol(衛氣營血) system, body fluid[津液] is the key feature, whereas in that of the Yuk-Gyeong(六經) system, Yanggi(陽氣) is the point. Therefore, we can understand that the standard symptoms of pattern identification are slightly different. However, that Sanghan and Onbyeong present different explanations does not mean that the disease itself strictly 'belong' to one category. They are different approaches, not explanations for two different subjects. Therefore, Sanghan and Onbyeong should be studied in line with this concept.

The Cause of Jeong-Jo's Death Based on The Annals of the Chosun Dynasty (朝鮮王朝實錄), Seungjeongwon Ilgi (承政院日記) (정조의 사망원인에 대한 연구(조선왕조신록과 승전원일기를 중심으로))

  • Objectives: There are many unknowns surrounding Jeong-Jo's death. To better understand Jeong-Jo's death, we closely inspected his medical records from the 20 days before his death in The Annals of the Chosun Dynasty (朝鮮王朝實錄) and Seungjeongwon Ilgi (承政院日記), Understanding medical treatment before death, we can correctly trace the cause of Jeong-Jo's death. Methods: According to The Annals of the Chosun Dynasty (朝鮮王朝實錄) and Seungjeongwon Ilgi (承政院日記), we examined his medical records between June 14 and June 28 of 1800, as to the change in chief complaint, cold and heat, thirst, syndrome differentiation, the opinions of Jeong-Jo, and the opinions of doctors, The original work, The Annals of the Chosun Dynasty and Seungjeongwon Ilgi were provided by NIKH (The National Institute of Korean History). The Korean translation of The Annals of the Chosun Dynasty was also provided by NIKH. Seungjeongwon Ilgi (original work written in Chinese characters) was directly translated into Korean by the study author. Results: 1. Jeong-Jo's is likely to have died of septicemia caused by an abscess on his back. 2. The cause of Jeong-Jo's death could be stroke due to hypertension, mental stress, and 'Wha (火) disease' Conclusions: The dominant view that Jeong-Jo was murdered by poison is not likely.

  • Clinical symptoms of herpes zoster include red rash, burning pain, tingling or extreme sensitivity in the affected area, usually limited to one side of the body. There might be a fever or a headache. The pain of acute herpes zoster may be severe, but it is usually temporary. Some of old patients are likely to develop postherpetic neuralgia. We treated a 73 year-old male patient who had severe pain and a headache after acute herpes zoster. In the point of Differentiation of Syndromes(辯證), this subject was diasgnosed as wind-heat syndrome(風熱證) and was administered Bangpungtongsung-san. For the purpose of making the pain easier, we used the western medication as well. After six days of treatment, pain and the other symptoms improved. Besides postherpetic neuralgia dissipated. Based on this experience, both oriental medicine and western medicine have a good effect on acute herpes zoster.

  • To evaluate GC-rhEPO, human erythropoietin produced by recombinant DNA technique, its general pharmacological properties were investigated in experimental animals administering intravenously and in vitro test system. GC-rhEPO at doses of 70,700 and 7,000 IU/kg body weight had no influence on general behavior, spontaneous motor activity, thiopental-inducted sleeping time, writhing syndrome induced by acetic acid, strychnine-induced convulsions, charchoal meal propulsion in mice, and body temperature, gastric juice secretion, urine and electrolyte excretion in rats. In anesthetized rabbits, GC-rhEPO (70, 700 and 7,000 lU/kg, i.v.) did not alter respiratory rate, blood pressure, heat rate. In in vitro experiments, GC-rhEPO did not affect the contractions of the isolated ileum of guinea pigs and the muscle twitchs of isolated neuromuscular junction of the rats. In addition, GC-rhEPO did not affect the blood coagulation time and ADP-induced platelet aggregation in plasma of rabbits. Taken together, these results indicate that GC-rhEPO does not induce any adverse effects in the experimental animals.

  • Urticaria is classified as Eunjin in Oriental Medicine. The intrinsic causes include digestive system disorder, food poison, parasite, female reproductive health problem, nervous breakdown, and disorder of endocrine hormone. The extrinsic sources include all kinds of insect bites or stings, medications, pollen, dirt, and toxic smells. Some unknown sources also produce urticaria. The urticaria that lasts for more than six weeks, is categorized as chronic urticaria which is not very different from acute urticaria that occurs rapid but for a short period. The chronic urticaria may come every day, and its symptoms can even get worse at irregular or regular intervals. The cycle is hard to predict. It could be weeks or years. In clinical treatments, the syndromes of patient's chronic urticaria with cerebral hemorrhage have been analyzed and identified as follows: The diagnosis by asking the patient's case history showed that the root cause was retention of food resulting from dysfunction of the spleen and stomach. First, Pyeongwisan was dosed. From the syndrome differentiation with four diagnostic methods, we diagnosed the symptoms as Poong yoel (wind-heat). Then, Hyeongbangpaedocksangamibang was prescribed to get very effective results.

Advanced In-Vessel Retention Design for Next Generation Risk Management

  • In the TMI-2 accident, approximately twenty(20) tons of molten core material drained into the lower plenum. Early advanced light water reactor (LWR) designs assumed a lower head failure and incorporated various measures for ex-vessel accident mitigation. However, one of the major findings from the TMI-2 Vessel Investigation Project was that one part of the reactor lower head wall estimated to have attained a temperature of 1100$^{\circ}C$ for about 30 minutes has seemingly experienced a comparatively rapid cooldown with no major threat to the vessel integrity. In this regard, recent empirical and analytical studies have shifted interests to such in-vessel retention designs or strategies as reactor cavity flooding, in-vessel flooding and engineered gap cooling of the vessel Accurate thermohydrodynamic and creep deformation modeling and rupture prediction are the key to the success in developing practically useful in-vessel accident/risk management strategies. As an advanced in-vessel design concept, this work presents the COrium Attack Syndrome Immunization Structures (COASIS) that are being developed as prospective in-vessel retention devices for a next-generation LWR in concert with existing ex-vessel management measures. Both the engineered gap structures in-vessel (COASISI) and ex-vessel (COASISO) are demonstrated to maintain effective heat transfer geometry during molten core debris attack when applied to the Korean Standard Nuclear Power Plant(KSNPP) reactor. The likelihood of lower head creep rupture during a severe accident is found to be significantly suppressed by the COASIS options.
