계통발생에 따른 척추동물의 뇌후구에 대한 전자현미경적 연구 (Electron Microscopic Studies on Olfactory Bulbs in the Vertebrates by Phylogenetics)

  • Authors are trying to unveil the ultrastructural organization of olfactory bulb, which has been summerized under light microscopic level or communicated only in some detail in different view point until now. For the critical point of view, since the phylogenetical approach will give the ultimate value in the correlative study between structural and functional bases (Brodal, 1969), the present study was carried out light and electron microscopic analyses of the structures of the neurons and synaptic organizations in olfactory bulbs from different animals in phylogenetical scale. We selected each one species from five animal classes: the house rabbit(Oryctolagus cuniculus var. domesticus [Gmelin]) from Mammalia, the domestic fowl (Gallus gallus domesticus Brisson) from Aves, the viper (Agkistrodon hylys [G.P. Pallas]) from Reptilia, a frog (Bombiana orientalis Boulenger) from Amphibia and the crussian carp (Carassius carassius [Linne]) from Pisces. For light microscopic study, samples were fixed in 10% formalin and paraffin sections were stained with hematoxylin-eosin. For the electron microscopic study, the tissues were fixed by perfusion through the heart or immersion with 1% paraform-aldehyde-glutaraldehyde mixture (phosphate buffer, pH 7.4), and final tissue block trimmed under dissecting microscope were osmicated (1% OsO4), they were embedded in Araldite or Epon 812, and ultrathin sections were made by LKB-V ultratome following the inspection of semi-thin sections stained with toluidine blue-borax solution. Ultra-thin sections contrasted with uranyl acetate and lead citrate were observed with JEM 100CX electron microscope. We have summerized our morphological analyses as follows: 1. The olfactory bulb of rabbit, viper and frog shows the eight layers of fila olfactoria, glomerular, external granular, external plexiform, mitral cell, internal plexiform, internal granular, medullary but domestic fowl shows the five layers of glomerular, fibrillar, mitral, granular and medullary and the three layers of fibrilla, glomerular and medullary in crussian carp. The sharpness of demarcation between the layers shows deferential tendency according to phylogenetical order. 2. Mitral cells of vertebrate have large triangular or oval shape with spherical nuclei which contain not so much chromatin. The cytoplasm contains numerous cell organelles, of which Nissl's bodies or granular endoplasmic reticula arranged as parallel strands. Development of granular endoplasmic reticula were declined as the phylogentical grade is going lower. 3. Tufted cells of all animal are mostly spindle or polygonal contour and contain oval nuclei which located in periphery of cytoplasm. The nuclei of rabbit, fowl, viper and frog has relatively space chromatin, but a nucleus of crussian carp contain irregularly aggregated chromatin in karyoplasm. Their cytoplasmic volume and cell organelle contents are in between those of mitral cell and granular cell. They contain moderate amount of mitochondria, granular endoplasmic reticula, a few Golgi complex, polysomes, lysosome, etc. 4. Granule of cells of all the vertebrate amimals studied exhibit similar features; cells and their dense nuclei show spherical or oval contour, and they have the thin rim of cytoplasm which contain only a few cell organelles. 5. In rabbit, the soma of mitral cells were in contact with boutons with two types of synaptic vesicles, that is, round and flat vesicles, especially flat vesicles in boutons were showing reciprocal synapses. However, in domestic fowls, vipers, frogs and crussian carps, there were found boutons showing only spherical synaptic vesicles. 6. The boutons containing round synaptic vesicles were made contact with the some of tufted cell of olfactory bulb in the rabbits, fowls, vipers and frogs, but no synaptic boutons were observed in soma of tufted cells in crussian carps. In the frogs, there were observed dendrites were contact with the soma of tufted cells. 7. In the neuropils of plexiform, granular and glomerular layers olfactory bulbs in the vertebrate, the synapses were axo-large dendrites, axo-median and small dendrites, dendrodendritic, and axo-axonal contacts. However, in the neuropil of crussian carps, synapses were observed only in glomerular layer.

신채호의 「독사신론」의 구성과 '민족사'의 재구 (The organization of Shin ChaeHo's Doksasillon and reorganization of the Nation history)

  • 신채호의 한국 역사에서의 주된 관심 분야는 고대사였다. 그리고 그의 역사 서술의 특징은 민족을 역사의 중심에 두는 민족주의적 성향이 목적의식적으로 강하였다는 점이다. 일반적으로 당시의 민족주의론은 신채호의 경우에서처럼 '강한' 민족을 강조하고, 동시에 그것을 역사의 전면에 부각시키려는 사회진화론의 입장에 서 있는 것이었다. 사회진화론과 결합된 민족주의는 제국주의를 한편으로는 비판하면서도 다른 한편으로는 선망하는 구조를 갖게 되었다. 그리고 이것이 신채호의 고대사 연구라 할 수 있는 "독사신론"에서 그대로 드러난다고 할 수 있다. 부여족을 한국 고대사의 주류로 '재발견'하고 고대사를 재구성, 구체화 시킨 신채호가 '부여족'을 한국 고대의 주 종족으로 선택하고 부여족을 한국 민족의 대명사처럼 사용한 이유는 부여 고구려가 가장 강성했기 때문이다. 한국 역사 속에서의 강성한 민족의 강조는 국가적 위기상황을 극복하고자 했던 구한말 시기 자강운동가의 면모를 잘 반영하는 것이기도 하며 한편으로는 한국에서 민족주의가 어떻게 형성되었는가를 보여주는 것이기도 하다. 신채호는 국권이 침탈당하는 상황에서 근대화에 뒤쳐진 민족으로서 유일하게 동원할 수 있는 무기가 민족으로서의 동질성을 강조하는 민족주의밖에 없었던 시기에 살았던 인물로 간주된다. 이와같이 독사신론에서 보여지고 있는 단군, 부여족의 역사의 재구성과 민족의 구원자로 중시했던 영웅을 발견해 내는 과정 등이 신채호에게는 역사를 쓰는 이유였던 것이다.

만성 기침 환자에서 기관지와 흉곽외 기도의 Methacholine 유발검사의 의의 (Methacholine Responsiveness of Bronchial and Extrathoracic Airway in Patients with Chronic Cough)

  • 연구배경 : 만성 기침은 임상에서 흔한 증상으로 대부분은 후비루증(posterior nasal dripping : PND), 천식, 위식도 역류증, 기관지염 등으로 인한 것으로 알려져 있다. 만성 기침 환자 중 일부분은 기침과 동반하여 천명이나, 호흡곤란 등과 같은 기관지 천식과 유사한 증상을 경우가 많아 기관지 과민반응 검사를 시행하게 된다. 기관지 과민반응 검사는 대부분 $FEV_1$을 측정하지만 흉곽외 기도의 과민반응은 측정하지 못하는 단점이 있다. 본 연구는 만성 기침 환자를 대상으로 기관지와 흉부외 기도의 methacholine 유발 검사를 시행하여 기도 과민 반응을 평가하여 상관 관계를 알아보고자 본 연구를 시행하였다. 대상 및 방법 : 대상 환자는 2개월 이상 기침이 있는 111명의 환자를 대상으로 하였으며, 남자 55명, 여자 56명으로 평균 연령은 $37.4{\pm}13.0$세였다. 대상 환자는 심혈관 질환, 전신 질환 등이 없었으며, 과거에 기관지 진단 받은 병력이 없고 기관지 확장제나 사용한 병력이 없었던 환자였다. 기침이나 천명 또는 간헐적인 호흡곤란 등의 증상은 문진으로 확인하였고, 기침만 존재한 환자 46명, 기침과 천명이 있는 환자 24명, 기침과 호흡곤란이 있는 환자 22명, 기침, 천명 및 호흡곤란이 있는 환자 19명이었다. 대상환자의 폐기능검사는 $FEV_1$(%) $106.5{\pm}15.3$, FVC(%) $100.1{\pm}12.8$, $FEV_1$/FVC(%) $106.3{\pm}13.1$로 정상이었다. Methacholine 유발검사는 0.0625 에서 25 mg/ml의 2배수 농도를 흡입한 5분후 기관지 과민 반응은 $PC_{20}FEV_1$으로 측정하였고, 흉곽외 기도 과민 반응은 최대 중간흡입 기류속도인 $MIF_{50}$으로 측정하여 연속 3회 감소하고 25%이상 감소한 $PC_{25}MIF_{50}$으로 측정하여 25mg/ml 이하의 농도에서 양성인 경우를 기관지(BHR) 및 흉부외 기도(EAHR) 에 각각 반응이 있다고 판독하여 대상 환자의 상관 관계를 확인하여 다음과 같은 결과를 얻었다. 결 과 : 대상 환자 총 111명중에 기관지 과민반응(BHR)이 27명, 흉곽외 기도 과민반응(EAHR)이 16명, BHR과 EAHR이 함께 있는 환자 8명으로 과민 반응이 있는 환자는 총 50명이었으며, 그외의 61명은 기관지와 흉부외 기도의 과민반응이 없었다. BHR 또는 EAHR이 양성인 환자 50명중에 기침만 있는 환자는 BHR이 3명, EAHR이 9명, BER과 EAHR이 같이 있는 환자는 2명으로 EAHR이 가장 많았으며, 기침과 천명이 있는 환자는 BHR이 10명, EAHR이 2명 및 BHR과 EAHR이 같이 있는 환자는 2명으로 BHR이 가장 많았고, 기침과 호흡곤란이 있는 환자는 BHR이 7명, EAHR이 3명, BHR과 EAHR이 같이 있는 환자는 1명으로 BHR이 가장 많았다. 기침, 천명 및 호흡곤란이 모두 있는 환자는 BHR이 7명, EAHR이 2명, BHR과 EAHR이 같이 있는 환자는 3명으로 BHR이 가장 많았다. BHR와 EAHR이 모두 음성인 환자는 총 61명이었으며, 기침만 있는 환장 32명, 기침과 천명이 있는 환자 10명, 기침과 호흡곤란이 있는 환자 11명, 기침, 천명 및 호흡곤란이 모 두 있는 환자는 7명이었다. 결 론 : Methacholine에 양성 반응인 환자중 호흡곤란이나 천명이 동반된 만성 기침환자에서는 주로 기관지 과민 반응이 나타났으며, 기침 단독 증상인 환자에서는 반응이 존재함을 알 수 있었다.

일본(日本) 의학(醫學)의 '절충파(折衷派)'에 관(關)한 연구(硏究) (A Study on the ' Zhe Zhong Pai'(折衷派) of the Traditional Medicine of Japan)

  • The outline and characteristics of the important doctors of the 'Zhe Zhong Pai'(折衷派) are as follows. Part 1. In the late Edo(江戶) period The 'Zhe Zhong Pai', which tried to take the theory and clinical treatment of the 'Hou Shi Pai (後世派)' and the 'Gu Fang Pai(古方派)' and get their strong points to make treatments perfect, appeared. Their point was 'The main part is the art of the ancients, The latter prescriptions are to be used'(以古法爲主, 後世方爲用) and the "Shang Han Lun(傷寒論)" was revered for its treatments but in actual use it was not kept at that. As mentioned above The 'Zhe Zhong Pai' viewed treatments as the base, which was the view of most doctors in the Edo period. However, the reason the 'Zhe Zhong Pai' is not valued as much as the 'Gu Fang Pai' by medical history books in Japan is because the 'Zhe Zhong Pai' does not have the substantiation or uniqueness of the 'Gu Fang Pai', and also because the view of 'gather as well as store up'(兼收並蓄) was the same as the 'Kao Zheng Pai'. Moreover, the 'compromise'(折衷) point of view was from taking in both Chinese and western medical knowledge systems(漢蘭折衷). Generally the pioneer of the 'Zhe Zhong Pai' is seen as Mochizuki Rokumon(望月鹿門) and after that was Fukui Futei(福井楓亭), Wadato Kaku(和田東郭), Yamada Seichin(山田正珍) and Taki Motohiro(多紀元簡). Part 2. The lives of Wada Tokaku(和田東郭), Nakagame Kinkei(中神琴溪), Nei Teng Xi Zhe(內藤希哲), the important doctors of the 'Zhe Zhong Pai', are as follows. First Wada Tokaku(和田東郭, 1743-1803) was born when the 'Hou Shi Pai' was already declining and the 'Gu Fang Pai' was flourishing and learned medicine from a 'Hou Shi Pai' doctor, Hu Tian Xu Shan(戶田旭山) and a 'Gu Fang Pai' doctor, Yoshimasu Todo(吉益東洞). He was not hindered by 'the old ways(古方)' and did not lean towards 'the new ways(後世方)' and formed a way of compromise that 'looked at hardness and softness as the same'(剛柔相摩) by setting 'the cure of the disease' as the base, and said that to cure diseases 'the old way' must be used, but 'the new way' was necessary to supplement its shortcomings. His works include "Dao Shui Suo Yan(導水瑣言)", "Jiao Chiang Fang Yi Je(蕉窗方意解)" and "Yi Xue Sho(醫學說)". Second. Nakagame Kinkei(中神琴溪, 1744-1833) was famous for leaving Yoshimasu Todo(吉益東洞) and changing to the 'Zhe Zhong Pai', and in his early years used qing fen(輕粉) to cure geisha(妓女) of syphilis. His argument was "the "Shang Han Lun" must be revered but needs to be adapted", "Zhong Jing can be made into a follower but I cannot become his follower", "the later medical texts such as "Ru Men Shi Qin(儒門事親)" should only be used for its prescriptions and not its theories". His works include "Shang Han Lun Yue Yan(傷寒論約言)". Third, Nei Teng Xi Zhe(內藤希哲, 1701-1735) learned medicine from Qing Shui Xian Sheng(淸水先生) and went out to Edo. In his book "Yi Jing Jie Huo Lun(醫經解惑論)" he tells of how he went from 'learning'(學) to 'skepticism'(惑) and how skepticism made him learn in 'the six skepticisms'(六惑). In the latter years Xi Zhe(希哲) combines the "Shen Nong Ben Cao Jing(神農本草經)", the main text for herbal medicine, "Ming Tang Jing(明堂經)" of accupuncture, basic theory texts "Huang Dui Nei Jing(皇帝內經)" and "Nan Jing(難經)" with the "Shang Han Za Bing Lun", a book that the 'Gu Fang Pai' saw as opposing to the rest, and became 'an expert of five scriptures'(五經一貫). Part 3. Asada Showhaku(淺田宗伯, 1815-1894) started medicine at Zhong Cun Zhong Zong(中村中倧) and learned 'the old way'(古方) from Yoshimasu Todo and got experience through Ouan Yue(川越) and Fu Jing(福井) and received teachings in texts, history and Wang Yangmin's principles(陽明學) fmm famous teachers. Showhaku(倧伯) meets a medical official of the makufu(幕府), Ben Kang Zong Yuan(本康宗圓), and receives help from the 3 great doctors of the Edo period, Taki Motokato(多紀元堅), Xiao Dao Xue Gu(小島學古) and Xi Duo Cun Kao(喜多村栲窻) and further develops his arts. At 47 he diagnoses the general Jia Mao(家茂) with 'heart failure from beriberi'(脚氣衡心) and becomes a Zheng Shi(徵土), at 51 he cures a minister from France and received a present from Napoleon, at 65 he becomes the court physician and saves Ming Gong(明宮) Jia Ren Qn Wang(嘉仁親王, later the 大正天皇) from bodily convulsions and becomes 'the vassal of merit who saved the national polity(國體)' At the 7th year of the Meiji(明治) he becomes the 2nd owner of Wen Zhi She(溫知社) and takes part in the 'kampo continuation movement'. In his latter years he saw 14000 patients a year, so we can estimate the qualjty and quantity of his clinical skills. Showhaku(宗伯) wrote over 80 books including the "Ju Chuang Shu Ying(橘窻書影)", "Wu Wu Yao Shi Fang Han(勿誤藥室方函)", "Shang Han Biang Shu(傷寒辨術)", "Jing Qi Shen Lun(精氣神論)", "Hunag Guo Ming Yi Chuan(皇國名醫傳)" and the "Xian Jhe Yi Hua(先哲醫話)". Especially in the "Ju Chuang Shu Ying(橘窻書影) he says "the old theories are the main, and the new prescriptions are to be used"(以古法爲主, 後世方爲用), stating the 'Zhe Zhong Pai' way of thinking, In the first volume of "Shang Han Biang Shu(傷寒辨術)" and "Za Bing Lun Shi(雜病論識)", 'Zong Ping'(總評), He discerns the parts that are not Zhang Zhong Jing's writings and emphasizes his theories and practical uses.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times.
