폐쇄성(閉鎖性) 수면무호흡증(睡眠無呼吸症)에서 지속적(持續的) 상기도(上氣道) 양압술(陽壓術)이 혈력학적(血力學的) 변화(變化)에 끼치는 영향(影響) (Blood Pressure Reactivity during Nasal Continuous Positive Airway Pressure in Obstructive Sleep Apnea Syndrome)

  • 목 적 : 폐쇄성 수면무호흡증(이하 수면무호흡증) 환자에서 고혈압이 흔히 병발되나 지속적 상기도 양압술(이하 양압술)에 따른 고혈압 해소 정도는 연구방법에 따라 상이한 결과를 보이고 있다. 이에 저자들은 지속적-비관혈적 혈압 측정법을 이용하여 기초 혈압치가 다양한 수면무호흡증 환자군에서 양압술을 적용하면서 야간 수면중 혈압 및 심박 동수 반응에 양압술이 미치는 영향을 연구하고자 하였다. 또한 양압술에 의한 혈역학적 변화에 영향을 줄 수 있는 요인들을 찾고자 하였다. 방 법 : 연구대상으로는 수면무호흡증의 임상 증상을 지니고 있으며 기초 측정일의 야간 수면다원검사 결과 호흡장애지수가 5이상인 환자 10명(평균연령 52.2$\pm$12.4세, 남자 9명, 여자 1명)을 선택하였다. 양압술을 시행하지 않은 상태인 기초 측정일과 양압술을 시행한 양압술 시행일 모두에서 야간 수면다원기록과 동시에 지속적-비관혈적 혈압측정기($Finapres^{(R)}$)를 사용하여 매 박동(beat-to-beat)시의 수축기 혈압, 이완기 혈압, 그리고 심박동수를 측정하였다. 또한 매 2초마다 혈중산소포화도($SaO_2$)를 측정하였다. 기초 측정일과 양압술 시행일 두 군간에 혈역학적 및 호흡기적 변인들의 평균치를 비교하였다. 기초 측정일 값에서 양압술 시행일 값을 뺀 수축기 혈압 델타값 및 이완기 혈압 델타값 각각과 기초 측정일의 측정치들(수축기 혈압, 이완기 혈압, 그 각각의 혈압 변이도, 심박동수 및 심박동수 변이도, 혈중산소포화도 평균치, 그리고 호흡장애지수), 양압술 시행일의 측정치들(양압술 처방압력 및 총 각성시간 분율), 연령, 그리고 비만도 간의 상관관계를 분석하였다. 결 과 : 1) 혈중 산소포화도의 평균치와 호흡장애지수가 기초 측정일에 비해 양압술 시행일에서 각각 증가(p<.01) 및 감소(p<.01)하였으나, 혈역학적 변인을 포함한 다른 비교 변인들에서는 차이가 없었다. 2) 기초 측정일의 혈압에서 양압술 시행일의 혈압을 뺀 델타값들을 분석한 결과 기초 측정일의 혈압치와 대상군의 연령에 따라 혈압 반응이 차별적으로 나타났다. 수축기 혈압 델타값과 기초 측정일의 수축기 혈압간에 유의한 정적 상관관계를 보였으나(p<.01), 이완기 혈압 델타값과 기초 측정일의 이완기 혈압간에는 각성기(p<.01)를 제외한 다른 수면단계에서 유의한 상관관계가 없었다. 렘수면단계를 제외한 모든 수면단계에서 이완기 혈압의 델타값과 연령 사이에 유의한 역 상관관계가 있었지만(p<.05), 수축기 혈압의 델타값과 연령 간에는 유의한 상관관계가 없었다. 3) 수축기 혈압 델타값 및 이완기 혈압델타값 각각과 기초 측정일의 측정치들, 양압술 시행일의 측정치들, 그리고 비만도 간의 상관관계를 분석한 결과, 이완기 혈압 델타값과 양압술 시행일의 각성시간 분율간의 유의한 정적 상관관계(p<.01) 외에는 유의한 상관관계가 없었다. 결 론 : 양압술 적용에 따라 수축기 혈압 델타값과 이완기 혈압 델타값이 기초 측정일의 수축기 혈압치와 대상군의 연령에 따라 각각 증가 또는 감소하였다. 따라서, 본 연구는 양압술에 따른 혈압 반응은 단순 감소로 나타나는 것이 아니라 여러 변인의 조건에 따라 차별적 혈압 반응 양상으로 나타나는 복합적인 현상이며 앞으로 좀더 세밀하게 연구되어야 할 대상임을 시사한다.

중환자실에 입원한 환자의 장기부전 및 예후 평가를 위한 SOFA 점수체계의 의의 (The SOFA Score to Evaluate Organ Failure and Prognosis in the Intensive Care Unit Patients)

  • 연구 배경 : 중환자의 예후를 예측할 수 있는 지표로 현재까지 APACHE II, III 점수체계와 SAPS 등이 임상에 응용되고 있다. 1996년 유럽에서 SOFA 점수체계가 제안되었고 1998년 다기관 전향적 연구를 통하여 장기부전을 평가하고 경과를 감시하는 간편하고 효과적인 점수체계임을 입증하였다. 이 연구는 춘천성심병원에서 내과계 중환자만을 대상으로 SOFA 점수체계의 유용성을 조사하였다. 방 법 : 2000년 5월 1일부터 6월 30일까지 춘천성심병원 내과계 중환자실에 입원한 154명을 대상으로 하였고, 이중 타과환자, 타병원으로 전원된 경우, ECG 관찰만을 위하여 입원한 경우, 처치 후 관찰을 위해 48시간 이내만 체류한 환자를 제외한 105명을 연구에 포함시켰으며, 91명에서 평가가 가능하였다. SOFA 점수와 APACHE III 점수는 입원 시부터 매일 24시간 간격으로 중환자실에서 퇴실할 때까지 하였다. 결 과 : 1) 사망률은 20%였고 중환자실 체류기간의 중앙값은 3일이었다. 2) 사망군과 생존군 간의 24시간 내 SOFA 점수는 각각 $8.17{\pm}4.05$, $3.85{\pm}3.24$로 유의한 차이가 있었다. 3) 장기부전이 없는 경우의 사망률은 6.5%, 5개의 다발성 장기부전이 있는 경우 사망률은 100%였다. 4) 48시간 이상 체류했던 74명을 분석한 결과 사망군에서는 최대 SOFA 점수가 81%에서 증가하였고 생존군에서는 21%에서 증가하여 유의한 차이를 보였다. 5) 24시간 내 SOFA 점수와 APACHE III 점수와의 관련성을 평가한 결과 r=0.85 (p<0.01)로 유의한 상관관계가 있었다. 결 론 : SOFA 점수체계는 다발성 장기부전이 있는 환자에서 장기부전을 평가하고 예후를 예측하며 임상경과를 관찰하는데 이용할 수 있는 간편하고 임상적 효용성이 있는 지표로서 임상적으로 많은 도움이 될 것으로 생각한다.

기관지내시경 검사시 전처치로서 Midazolam의 효과 (The Effect of Midazolam As Sedative Agent in Bronchoscopy)

  • 연구배경 : 기관지내시경은 호흡기질환의 진단과 치료에 유용한 도구로 널리 이용되고 있다. 그러나 상당수의 환자가 검사시 고통을 느끼거나 불안감을 느끼고 있어 이를 경감시켜주기 위한 전처치가 필요하다. 저자들은 전처치 약제로 midazolam의 안정성과 효과를 평가하고 또한 적절한 용량을 알아보고자 본 연구를 시행하였다. 방 법 : 연속적으로 기관지내시경을 시행한 180명의 환자를 대조군, midazolam 0.03 mg/kg군, midazolam 0.06 mg/kg군으로 분류하여 검사자와 내시경보조 간호사, 환자의 만족도를 비교하였다. 또한 검사중 혈압과 심박 및 산소포화도의 변화를 측정하였고 기침의 횟수와 lidocaine의 사용량을 측정하였다. 환자의 회복상태는 내시경검사 종료 후 10분간격으로 검사하였다. 결 과 : 각군간에 연령, 성별, 신장, 체중 및 검사시간의 차이는 없었다. 검사 전 및 검사중 혈압과 심박수 및 산소포화도는 각 군간에 유의한 차이가 없었으며 기침의 횟수, lidocaine의 투여량 역시 각 군간에 유의한 차이가 없었다. 기관지내시경 종료후 10분 내에 모든 환자에서 회복이 관찰되었다. 검사자 및 간호사의 만족도는 midazolam 0.03 mg/kg군에서 가장 높았으며 환자의 만족도는 midazolam 투여군 모두에서 대조군보다 높은 결과를 나타냈다. 결 론 : 기관지내시경 검사시 전처치로서 midazolam의 정맥투여는 안전한 방법으로 환자의 만족도를 증가시키며 저용량의 midazolam 투여는 의식하 진정작용을 통해 검사자의 만족도 또한 증가시키는 것으로 생각된다.

스마트 모바일 환경에서 의료정보 동적접근 시스템 (Medical Information Dynamic Access System in Smart Mobile Environments)

  • 최근, 병원정보시스템의 환경은 다양한 스마트 기술을 접목하고 있는 추세이다. 따라서, 스마트 폰, 테블렛 PC와 같은 다양한 스마트 디바이스가 의료 정보 시스템에 활용된다. 또한, 이러한 환경은 이기종 센서, 디바이스, 시스템 및 네트워크에서 실행되는 다양한 응용 프로그램으로 구성된다. 이들 병원 정보 시스템 환경에서, 기존의 접근 제어 방식에 의한 보안 서비스를 적용하는 것은 문제가 된다. 기존 보안 방식의 대부분은 접근제어 리스트 구조를 사용한다. 이는 클라이언트 이름, 서비스 객체 메소드 이름으로 접근 제어 매트릭스에 의해 정의된 접근만을 허용한다. 가장 큰 문제점으로는 정적인 접근 방법은 변화되는 상황에 신속하게 적응하지 못한다. 따라서, 우리는 보다 유연하고, 매우 상이한 보안 요구와 다양한 환경에 적용 할 수 있는 새로운 보안 메커니즘을 필요로 한다. 또한, 환자중심의 의료 서비스 형태로 변화되고 있어, 이를 해결하기 위한 연구가 요구된다. 본 논문에서는 스마트 모바일 환경에서 의료정보 동적접근 시스템을 제안하고자 한다. 우리는 기존 병원정보 시스템의 환경을 기반으로 동적접근 제어 방법으로 의료정보 시스템에 접근하는 방법에 중점을 두었다. 물리적인 환경은 모바일 x-ray 영상 디바이스와 전용 모바일 스마트 디바이스, PACS, EMR 서버와 인증 서버로 구성하였다. 소프트웨어 환경은 모바일 X-ray영상기기는 Windows7 OS를 기반으로 동기화 및 모니터링 서비스를 위해 .Net Framework를 기반으로 개발하였다. 그리고 전용 스마트 디바이스는 Android OS를 기반으로 JSP와 Java SDK를 통한 동적접근 응용 서비스를 구현하였다. 병원의 의료영상정보 서버와 모바일 X-ray영상기기, 전용 스마트 디바이스간의 의료정보는 의료영상정보 표준인 DICOM을 기준으로 한다. 또한 EMR 정보는 H7을 기반으로 한다. 동적접근 제어 서비스를 제공하기 위해, 우리는 산소포화도, 심박수, 혈압과 체온과 같은 생체 정보의 값에 대한 조건에 의해 환자의 상황을 분류하고, 의료진의 의료정보 접속 인증 방법으로 동적인 접근 방법을 설계했다. 이는 일반 상태와 응급상태로 2부분으로 구분하여 이벤트 추적 다이어그램으로 보였다. 그리고, 인증 정보는 ID/PWD와 위치, 역할, 작업시간 그리고 응급 환자를 위한 응급 코드를 포함하였다. 동적접근 제어 방법의 일반적인 상황은 인증 정보의 값에 의해 의료정보에 접근 할 수 있다. 그러나 응급상황의 경우는 인증 정보 없이 응급 코드에 의해 의료정보에 접근하도록 하였다. 또한, 우리는 의료정보 표준에 따라 환자, 의료진 및 의료 영상 정보로 구성되는 의료정보 통합 데이터베이스 스키마를 구축했다. 끝으로, 우리는 제안 시스템의 수행 결과를 일반과 응급상황과 같은 환자의 상태에 따라 스마트 디바이스 기반으로 동적접근 응용 서비스의 유용성을 보였다. 특히, 제안 된 시스템은 동적 액세스 제어 방법에 의해 응급상황에서 스마트 디바이스기반의 효과적인 의료 정보 서비스를 제공한다. 이 결과, 제안한 시스템이 u-병원 정보 시스템과 서비스에 유용할 것으로 기대한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. A photosensitizer behaved as an added absorber. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
