Purpose: This retrospective chart review study was carried out in order to examine the frequency of delirium, and to identify the risk factors associated with the development of delirium in elderly patients with arthroplasty. Methods: Data were collected from medical records of patients who received arthroplasty during one and half year in a hospital. Three hundred sixty five patients were selected for the study. The onset of delirium was reviewed based on the result of psychological doctor's consultation or nurse's assessment with Confusion Assessment Method and delirium onset risk factors were examined. Data were analyzed with descriptive statistics, t-test, $x_2$-test, Fisher's exact test and logistic regression analysis. Results: Delirium occurred with 31 patients (8.5%) out of 365 patients who underwent arthroplasty. There were statistically significant differences between incidence of delirium and gender ($x_2$=6.11, p=.025), age ($x_2$=32.81, p<.001), hearing difficulty ($x_2$=8.08, p=.012), albumin level of preoperational day (t=-3.43, p<.001), albumin (t=-2.20, p=.028) and hemoglobin level (t=-2.83, p=.005) of operational day. Age and gender were the most significant predictive factors with regard to the incidence of delirium. Conclusion: Through understanding of these results, nurses will be able to identify those patients who may be at risk for developing delirium in early stages.
Since the middle of 1990s, in Korea a few researches on the optimal management technologies combining numerical model and GIS for the management of water environment in drinking watershed area and reservoir such as Paldang Lake have been carried out. In this study, the integrated water environment management system was been suggested to efficiently reflect the public awareness of the environment by integrating the web based distributed data collection system, GIS, public hearing system and water quality model. As all the components of the system have been developed using the World Wide Web and all data have been collected from the relevant agencies through the Internet, the water quality model could be implemented on the web directly. In consequence, the environmental geographic information in Paldang Lake could be acquired and analyzed through the Internet. The system can rapidly respond to the public right to know on environment, so the public will willingly participate in the governmental projects on environment. To verify the usability of the developed system, it has been applied to Paldang Lake. Especially when the web based model has been used, users can easily and confidentially get the prediction results by applying the minimum number of parameters for the water quality model. This model will provide clearness and scientific bases in the process of water quality prediction for the sensitive sites where there are critical conflicts between the residents and the developers. In this study, rapid water environment management technique without spatial and time limit has been suggested, which can contribute to the efforts on the government and the public participation.
Purpose: The purpose of this study was to investigate the actual condition of the disabled due to Occupational accidents based on the functional evaluation tool. Method: The data were collected from 2 Workers Accident Medical Corporation, 311 subjects between February and August, 2007. The tool comprised of six components including physical function, cognitive function, communication, emotional and disturbing behavior, nursing skills and rehabilitation needs. Results: Among the disabled, the average point of physical function of 5 was found in 61.92% and to express and understand were evaluated as normal in 36.69% and 33.44%, respectively. Also, hearing and vision were evaluated as normal in 53.50% and 37.98%, respectively. 19.01% of the disabled had emotional problems and 13.78% had disturbing behavior. Most disabled were in need of nursing skills with 97.73% ROM and activities. The 78.18% and 76.90% of disabled were in need of rehabilitation because of right and left leg in full movement disorders, respectively. Conclusion: The evaluation score for the disabled caused by Occupational accidents was diverse showing 10 to 19 points and 140 to 150 points. The result means the care cost should be determined depending on the care needs of the disabled because there is such a diverse range of care needs for them.
The B$\'{e}$k$\'{e}$sy audiometer is used to determine not only hearing threshold but also assumption of recruit phenomenon and cause of disease. However, current B$\'{e}$k$\'{e}$sy audiometer takes an operational model, so called face-to-face model, in which model one audiometrist can assess only one subject at a time. As a result, the face-to-face model leads to high audiometrist's labor cost and long waiting time when there exist many subjects. To improve such weakness, this paper extends PC based B$\'{e}$k$\'{e}$sy audiometer to support client/server model in which model one audiometrist enables to concurrently access several subjects. As providing capabilities such as real-time transfer of assessment result, video monitoring of subject and video chat, the same effect as face-to-face model but overcomes weakness of face-to-face model.
Purpose: This study was conducted to identify behavioral items for Korean infants under 24months by naturalistic observation. These items will be the basis for developmental scale for Korean infants. Method: Data were collected by observing and interviewing fifty one infants and their mothers during the period from November 15 to December 14, 2003. Raw data in descriptive form were obtained by unstructured naturalistic observation, and were reformed to behavioral items through discussion with professors in pediatric nursing and experienced nurses in child care department. Also, behavioral items were classified into five developmental areas. Result: Total number of behavioral items was two hundreds and thirty eight. 52 items for motor development, 66 items for personal-social, 40 items for hearing and speech, 41 items for eye-hand coordination, and 39 items for performance. Conclusion: This is meaningful that fundamental items for Korean developmental scale for infants were identified by observing behaviors of Korean infants. On the basis of these items, average age will be calculated for passing each item by testing Korean infants. We hope to develop Korean infant developmental scale as a final outcome in the future research.
Electrodermal activity(EDA) is a bio-electric signal which occurs at the skin surface during the sweating. EDA reflects the activity of the sympathetic axis of the autonomic nervous system. EDA is associated with the eccrine sweat gland at the palmar and plamar surface. This study was aimed to characterize the relationship between EDA and auditory stimulus intensities. Acoustic stimulus used in this study were 500 Hz, 1 kHz, 2 kHz of narrow band noise, which were representative of speech frequencies in audible range. Stimulus intensity between 90 and 30 dB in 10 dB within dynamic range. After deriving the minimum stimulus intensity(threshold of skin potential) which elicited skin potential, and then the latency and amplitude were derived from waveform of skin potential, each latency and amplitude were compared to stimulus intensity. The waveform of skin potential were recorded stably, and the threshold of skin potential appeared nearly the hearing threshold level of the participant. The latency was decreased and the amplitude was increased according to the increase of the stimulus intensity. These results suggest that auditory evoked skin potential can be applicable to auditory assessment and audiological diagnosis tool.
Purpose: The purposes of this secondary analysis study was to examine prevalence, risk factors and unmet healthcare needs among adults with hypertension. Methods: A sample of 3,386 adults over the age of 40 with hypertension were drawn from the Korea Health Panel Study (2013). Using SPSS 22.0 version, descriptive statistics including frequency, percentage, chi-square and logistic regression were performed. Results: Results showed that 18.9% of the sample reported unmet healthcare needs with the most frequently cited one was financial burdens (43.2%). The reported experiences of unmet healthcare needs differed by gender, marital status, vision or hearing impairment, memory problem, impaired mobility, subjective health status, total family income, depressive episode and the difficulty in making decisions. The sample participants were more likely to report unmet healthcare if they had vision impairment, low income and perception that their health status as moderate to poor. Those without vision impairment were less likely to report unmet healthcare needs. Conclusion: The identified risk factors of unmet healthcare needs should be addressed which would enhance access both to health care and to resolution of unmet healthcare needs. Since visual ability seems to impact perception of unmet healthcare needs, it may be useful to find ways to address this factor.
This study investigates the effect of voice quality on speech intelligibility and the relationship between voice quality and intelligibility for children with spastic CP. We recruited 36 children with spastic CP (mean age 10.43 year, 17 girls, 19 boys, spastic type 34, mixed 2) from a special school and a rehabilitation hospital. Voice samples for the perceptual analysis of voice quality were extracted from a sustained vowel /a/ and were rated on the GRBAS scales by two experienced speech language pathologists. Ten adult subjects with no hearing problems evaluated speech intelligibility for the 37 words listed in the Assessment of Phonology and Articulation for Children on a 7-point interval scale. The children with spastic CP were divided into three groups according to the rated G scores on the GRBAS scales (G1(n)=10, G2(n)=13, G3(n)=13). Analyses of ANCOVA and Pearson correlation showed that there was a significant difference in speech intelligibility among three groups. There was also a significant correlation in G scale (grade), A scale (asthenia), B scale (breathy) score, and speech intelligibility. These findings suggest that poor speech intelligibility of spastic CP might be related to asthenia and breathiness. Vocal intensity should be increased and vocal functioning should be improved for speech therapy to improve speech intelligibility of the children with spastic CP.
The application of analytical techniques for the rational determination of problems arising from management science and corporate management. In a way that is the opposite of the repair plan that can predict accurate results is increasing utilization of the complex-based analysis methodology. In this study, we examined the application of physical space and the methodological utilization of the pedestrian model analysis that applied the simulation to the Health Checkup Center. The conclusions are as follows. First, the spatial analysis and measurement for empirical research has confirmed that the efficiency assessment through the pedestrian model simulation can lead to an objective evaluation. Second, it seems to be able to reduce the queue through a change in the number of services of the low-pressure and the hearing laboratory, the recovery room with a high proportion of male disturbances and relatively long use time. The third, the spatial density analysis and the time required to reduce the density change in comparative analysis, and the spatial layout changes, the increase in the capacity of 80 people, approximately 16 minutes to shorten the process duration.
