Environmental Factors Influencing the Prevalence of Falls in South Korea

  • Objective: This study is a systematic review conducted to analyze the environmental factors that cause falls in the older adult. Design: Systematic review Methods: The study was conducted by searching the Research Information Sharing Service (RISS), Korean studies Information Service System (KISS), and Data Base Periodical Information Academic (DBpia) databases for literature published in South Korea up to July 2020. A total of 12 studies were selected for analysis based on the inclusion and exclusion criteria. Results: The results of the analysis revealed that all the selected literature were surveys and the study subjects were 65 years of age and above. The sample size ranged from a minimum of 95 subjects to a maximum of 3,278. A total of eight tools were used to measure the environmental factors associated with falls. The prevalence and recurrence of falls increase with age and deterioration of health. Older adult individuals who experience falls encounter difficulties in recovering from impaired physical function and disability; moreover, in severe cases, falls may lead to death. Falls are largely associated with a combination of intrinsic and extrinsic (i.e., environmental) factors. The purpose of this study was to assess potential extrinsic risk factors for falls. Falls occur in indoor environments, such as washrooms, bathrooms and living rooms, and outdoor environments, including roads and stairways, depending on the season, time of day, and use of ambulatory aids. In such environments, falls are mainly caused by slipping and stuttering. Conclusions: Therefore, as the rate of fall is influenced by several factors, extrinsic factors should be improved by developing comprehensive accident prevention programs that address the improvement of environmental risk factors around places of residence to reduce risk factors among the older adult, who, especially, are at a high risk for falls.

  • Globally researchers at medical institutions are actively sharing COHORT data of patients to develop vaccines and treatments to overcome the COVID-19 crisis. OMOP-CDM, a common data model that efficiently shares medical data research independently operated by individual medical institutions has patient personal information (e.g. PII, PHI). Although PII and PHI are managed and shared indistinguishably through de-identification or anonymization in medical institutions they could not be guaranteed at 100% by complete de-identification and anonymization. For this reason the security of the OMOP-CDM database is important but there is no detailed and specific OMOP-CDM security inspection tool so risk mitigation measures are being taken with a general security inspection tool. This study intends to study and present a model for implementing a tool to check the security vulnerability of OMOP-CDM by analyzing the security guidelines for the US database and security controls of the personal information protection of the NIST. Additionally it intends to verify the implementation feasibility by real field demonstration in an actual 3 hospitals environment. As a result of checking the security status of the test server and the CDM database of the three hospitals in operation, most of the database audit and encryption functions were found to be insufficient. Based on these inspection results it was applied to the optimization study of the complex and time-consuming CDM CSF developed in the "Development of Security Framework Required for CDM-based Distributed Research" task of the Korea Health Industry Promotion Agency. According to several recent newspaper articles, Ramsomware attacks on financially large hospitals are intensifying. Organizations that are currently operating or will operate CDM databases need to install database audits(proofing) and encryption (data protection) that are not provided by the OMOP-CDM database template to prevent attackers from compromising.

Social Media and Communication in Times of Public Health Crisis: Analysis of COVID-19 YouTube Vlog activities in the sharing of patient experience and information

  • This study analyzes the content of YouTube Vlog videos created by patients of Coronavirus disease 2019 ("COVID-19") in South Korea and viewer comments on those videos. As this new infectious disease started to sweep the world in late 2019 and early 2020, the public started facing fear and uncertainty stemming from the lack of sufficient and accurate information about the virus. At the same time, as COVID-19 patients in South Korea were treated in isolation to prevent the spread of the virus, the patients themselves were experiencing anxiety and exclusion from the society. During this period, there was an increase in YouTube Vlog videos created by the patients in which they shared their experiences going through the treatment and recovery processes. To understand how these YouTube Vlog videos were being used by the patients to connect with the society and seek support in a state of isolation and anxiety, this study conducted a qualitative multi-case analysis of three sample YouTube Vlog video channels to analyze their content, as well as a lexicon-based sentiment analysis of viewer comments to understand the experiences and reactions of viewers. The patients' YouTube Vlog videos showed that they shared similar stages of progress, despite each emphasizing a different main theme. Overall, the tone of the viewer comments became increasingly positive over time, although with some variance among different patient cases and stages. The results confirmed that Vlogs of patients played a significant role in reducing the uncertainty around COVID-19 and strengthening social support for the patients. The findings of this study can improve an understanding of the psychological and behavioral aspects of patient experience in isolated treatment and the impact of shared communication among members of society in times of crisis.

대학병원 근로자의 지식경영 준비도에 관한 연구 (The Readiness of the University Hospital Employees for the Knowledge-based Management)

  • This study purports to investigate the readiness of the university hospital employees in the knowledge-based management. Data were collected from 550 employees including administrative, nursing, and technical staff of 9 university hospitals located in Seoul and Kyunggi Province through the self-administered questionnaires. The response rate was 79% and 425 questionnaires were used as final data and analyzed using 2 test, t-test, and ANOVA. The main findings of the study are as follows. 1) It seems that most employees of the study hospitals have basic knowledge on the concept of knowledge-based management. This finding implies that the implementation of the knowledge-based management in Korean university hospitals will not likely to face strong resistance from their employees. 2) The results show that Korean hospital employees are still not so accustomed to using e-mail as the main communication tool. This finding suggests that it is necessary to use various communication tools which include electronic data interchange, teleconference, and cyber chatting for facilitating the knowledge-based management in Korean university hospitals. 3) It is desirable to appoint a chief knowledge officer(CKO) for operating knowledge-based management system effectively. 4) A reward system for employees who show a distinguished performance in the creation and sharing of new knowledge should be established. Knowledge mileage system, selection of the best knowledge employee and team will be a good example of the effective reward system. 5) The participation and support from the chief executive officers (CEO) of the hospitals is an important factor for successful knowledge-based management. Furthermore, to make physicians actively participate in the knowledge-based management is another important factor for obtaining valuable outputs from the system. 6) It is found that the knowledge and skills of the hospitals employees on the information technology (IT) are not sufficient for making knowledge-based management more popular. This implies that it is very important to select IT-oriented employees and educate them continuously on the knowledge-based management.

Horticultural Activity Interventions and Outcomes: A Review

  • The objective of the present study was to review the current research about the outcomes of horticultural activity interventions in order to determine research trends and analyze the characteristics of horticultural activity interventions. We reviewed the literature using search engines such as the Web of Science, Proquest Dissertation and Theses, Academic Search Premier, Research Information sharing Service, and Korean National Assembly Digital Library to locate journal articles that include relevant search terms (e.g., gardening activity, gardening program, allotment garden, urban agriculture, horticultural therapy, and horticultural activity). We initially identified 14,414 journal articles that were published before April 2014 and selected 509 of the papers for review. We found that studies investigating treating children and adults 8 to 64 years old were the most common, and horticultural activities such as gardening outdoors, planting indoors, making crafts with live plants, arranging flowers, making crafts with artificial or pressed flowers, and other activities were usually combined. Short/medium term (11 to 20 sessions) horticultural activity programs were the most frequent, and most interventions were of medium duration (> 60 min to 120 min). Most of the studies focused on the psychological or emotional effects of horticultural intervention, such as its effects on emotional intelligence, self-esteem, stress, and depression. Further studies are needed to analyze the research methodology, specific outcomes, and strengths or weaknesses of studies investigating horticultural activity interventions.

Binding Pattern Elucidation of NNK and NNAL Cigarette Smoke Carcinogens with NER Pathway Enzymes: an Onco-Informatics Study

  • Cigarette smoke derivatives like NNK (4-(Methylnitrosamino)-1-(3-pyridyl)-1-butanone) and NNAL (4-(methylnitrosamino)-1-(3-pyridyl)-1-butan-1-ol) are well-known carcinogens. We analyzed the interaction of enzymes involved in the NER (nucleotide excision repair) pathway with ligands (NNK and NNAL). Binding was characterized for the enzymes sharing equivalent or better interaction as compared to +Ve control. The highest obtained docking energy between NNK and enzymes RAD23A, CCNH, CDK7, and CETN2 were -7.13 kcal/mol, -7.27 kcal/mol, -8.05 kcal/mol and -7.58 kcal/mol respectively. Similarly the highest obtained docking energy between NNAL and enzymes RAD23A, CCNH, CDK7, and CETN2 were -7.46 kcal/mol, -7.94 kcal/mol, -7.83 kcal/mol and -7.67 kcal/mol respectively. In order to find out the effect of NNK and NNAL on enzymes involved in the NER pathway applying protein-protein interaction and protein-complex (i.e. enzymes docked with NNK/NNAL) interaction analysis. It was found that carcinogens are well capable to reduce the normal functioning of genes like RAD23A (HR23A), CCNH, CDK7 and CETN2. In silico analysis indicated loss of functions of these genes and their corresponding enzymes, which possibly might be a cause for alteration of DNA repair pathways leading to damage buildup and finally contributing to cancer formation.

Assessment of Arsenic Levels in Body Samples and Chronic Exposure in People Using Water with a High Concentration of Arsenic: a Field Study in Kutahya

  • Objective: This study aimed to evaluate the prevalence of skin lesions, which is a health effect of chronic arsenic (As) exposure, and determine the hair/blood arsenic concentrations of people living in Kutahya villages who are using and drinking tap water with a high concentration of arsenic. Materials and Methods: A total of 303 people were included in the present cross-sectional study. A prepared questionnaire form was used to collect the participants' information and environmental history. Skin examination was performed on all participants. Hair, blood and water samples were analyzed using atomic absorption spectroscopy. The cumulative arsenic index (CAI) was calculated for all participants. Results: Villages were divided into two groups according to the arsenic level (<$20{\mu}g/L$, Group I; >$20{\mu}g/L$, Group II) in their water. The prevalence of skin lesions, hair and blood arsenic level, and CAI were found to be higher in the Group II participants. There was a positive association between body arsenic levels and CAI in the participants of each group. Conclusions: The number of skin lesions and arsenic concentrations in body samples were found to increase with the water arsenic level and exposure time. We hope that sharing this study's results with local administrators will help accelerate the rehabilitation of water sources in Kutahya.

어린이집에서의 IoT 헬스케어 디바이스 활용에 대한 교사와 부모의 인식 및 요구 (A Study on the Recognition and Demand of Teachers and Parents of Using IoT Health-Care Devices in Day-Care Centers)

  • 본 연구는 어린이집 IoT 헬스케어 디바이스 활용에 대한 부모와 교사의 인식과 요구를 실증적으로 알아보고자 수행되었다. 이를 위하여 부모 200명, 어린이집 교사 200명을 대상으로 설문조사를 실시하였다. 수집된 자료는 SPSS WIN 22.0을 이용하여 빈도분석, t-검정, 𝑥2 검정을 실시하였다. 연구결과 첫째, IoT 헬스케어 디바이스 활용에 대하여 교사가 부모에 비해 긍정적 인식과 지지도를 나타내어 더 높은 호감도를 보였으며 정보유출문제에 대해 부모보다 높은 인식을 나타내었다. 둘째 어린이집 IoT 헬스케어 디바이스 활용 요구로 교사, 부모 모두 응급상황 대처 영역과 웨어러블 형태에 대한 요구를 가장 높게 나타내었다. 비용은 부모, 교사 모두 기관과 부모가 공동으로 금액 부담의 요구를 가장 높게 나타내었다. 본 연구결과를 중심으로 어린이집에서 IoT 헬스케어 디바이스 활용 가능성을 논의하였다.

임업 사고 응급대응체계의 개선방안에 관한 연구 (A Study on Improvement of Emergency Medical System for forestry accident)

  • The estimated on-site accident rate in Forestry is relatively high. According to statistics of the accident, in the recent 5 years, from 2014 to 2018, forestry accidents have resulted in 98% of injuries and 87% of fatalities. Especially, there are significant geographical constraints to access to the scene in case of an accident. Even though the capacity of first aid capacity is notably emphasized its importance to minimize the scale of damages, the relevant employees have been educated only basic first aid, which is not considered circumstances or geographic limitations, by Occupation Safety and Health Acts. Therefore, the purpose of this study is to derive a direction for a forest emergency service system to increase forestry workers' survival and prevent secondary injury through securing 'Golden Time.' This study conducts analyzing relevant laws and regulations in domestic and international settings as well as looking at several concerned accident cases. The outcome of analysis presents an issue regarding the implementation of onsite first aid in forestry and existing risk factors depending on the working process. Finally, we suggest two ways to improve the forest emergency service that are 1) an appropriate curriculum and kit for forest first aid; and 2) a system for emergency transfer through sharing information between National Fire Agency (NFA) and emergency medical service center, and emergency and rescue mission using helicopter from NFA and Korea Forest Service.

가상현실을 이용한 모아간호 실습교육이 간호 대학생의 실습역량에 미치는 영향: 체계적 문헌고찰 (The effects of maternal-child nursing clinical practicum using virtual reality on nursing students' competencies: a systematic review)

  • Purpose: This study aimed to investigate the effects of virtual reality used in maternal-child nursing clinical practicums on nursing students' competencies through a systematic review. Methods: The inclusion criteria were peer-reviewed papers in English or Korean presenting analytic studies of maternal-child nursing practicums using virtual reality. An electronic literature search of the Cochrane Library, CINAHL, EMBASE, ERIC, PubMed, and Research Information Sharing System databases was performed using combinations of the keywords "nursing student," "virtual reality," "augmented reality," "mixed reality," and "virtual simulation" from February 4 to 15, 2022. Quality appraisal was performed using the RoB 2 and ROBINS-I tools for randomized controlled trials (RCTs) and non-RCTs, respectively. Results: Of the seven articles identified, the RCT study (n=1) was deemed to have a high risk of bias, with some items indeterminable due to a lack of reported details. Most of the non-RCT studies (n=6) had a moderate or serious risk of bias related to selection and measurement issues. Clinical education using virtual reality had positive effects on knowledge, skills, satisfaction, self-efficacy, and needs improvement; however, it did not affect critical thinking or self-directed learning. Conclusion: This study demonstrated that using virtual reality for maternal-child nursing clinical practicums had educational effects on a variety of students' competencies. Considering the challenges of providing direct care in clinical practicums, virtual reality can be a viable tool that supplements maternal-child nursing experience. Greater rigor and fuller reporting of study details are required for future research.
