Noise pollution, both in the environment and in the workplace, has been recognized as a major health hazard -one that can impair not only a person's hearing but also his physical and mental well-being. As industrialization progresses, the prevalence rate of occupational diseases is increasing, especially hearing loss, which has the highest prevalence rate among the occupational diseases. The major cause of noise is the construction of various large industries without any regulation of noise sources. Therefor, we must establish an enactment to control mechanical noise sources. as soon as possible. For the purpose of controlling the noise source, we must have exact data about such things as the sound level, the frequency of the peak sound and the revolutions per minute (r.p.m.) of the machine (a measure of the power of its motor). This study was undertaken in order to define the noise characteristics, the power of the machine's motor, the change of the sound level and the peak sound as the r.p.m. increases, and the permissible exposure time. The sample size of this study was 74 machines at 11 plants in 6 industries. The results are as follows; 1. The breakdown of the types of mechanical noise noted was : 63.6% continuous normal sound, 26.9% intermittent sound, 4.7% continuous repeating sound and 4.6% impulsive sound. 2. With respect to the type of industry, the overall sound level was the highest in the mechanical industry, with $103.8{\pm}2.8dB(A)$, and lowest in the textile industry, with $89.2{\pm}1.43dB(A)$. 3. With respect to the type of machine, the highest sound level was 124 dB(A) caused by Gauzing(II), in the mechanical industry, and the lowest was 76 dB(A) caused by Attachment (Jup Chack) (I) in the timber industry. 4. The shortest permissible exposure time to Gauzing(II) in the mechanical industry was less than 15 minutes. 5. Among 74 machines, 68.2% of the peak sound was situated in the high frequency range (52.7% at 2 KHz, 4.1% at 4 KHz and 1.4% at 8 KHz). 41.8% of the peak sound was in the middle frequency range (4.1% at 250Hz, 14.8% at 500Hz and 22.9% at 1KHz). 6. If one machine had two motors or more, the peak sound was shifted to the low frequency range. 7. As the r.p.m. increased, the overall and peak sound levels were increased without any change of the frequency of the peak sound. 8. Whenever the machines had the same kind and the same r.p.m., the overall and peak sounds were changed by the physicochemical characteristics of the raw materials and the management.

Background: The use of extracorporeal life support (ECLS) in the setting of cardiopulmonary resuscitation (CPR) has shown improved outcomes compared with conventional CPR. The aim of this study was to determine factors predictive of survival in extracorporeal CPR (E-CPR). Methods: Consecutive 85 adult patients (median age, 59 years; range, 18 to 85 years; 56 males) who underwent E-CPR from May 2005 to December 2012 were evaluated. Results: Causes of arrest were cardiogenic in 62 patients (72.9%), septic in 18 patients (21.2%), and hypovolemic in 3 patients (3.5%), while the etiology was not specified in 2 patients (2.4%). The survival rate in patients with septic etiology was significantly poorer compared with those with another etiology (0% vs. 24.6%, p=0.008). Septic etiology (hazard ratio [HR], 2.84; 95% confidence interval [CI], 1.49 to 5.44; p=0.002) and the interval between arrest and ECLS initiation (HR, 1.05 by 10 minutes increment; 95% CI, 1.02 to 1.09; p=0.005) were independent risk factors for mortality. When the predictive value of the E-CPR timing for in-hospital mortality was assessed using the receiver operating characteristic curve method, the greatest accuracy was obtained at a cutoff of 60.5 minutes (area under the curve, 0.67; 95% CI, 0.54 to 0.80; p=0.032) with 47.8% sensitivity and 88.9% specificity. The survival rate was significantly different according to the cutoff of 60.5 minutes (p=0.001). Conclusion: These results indicate that efforts should be made to minimize the time between arrest and ECLS application, optimally within 60 minutes. In addition, E-CPR in patients with septic etiology showed grave outcomes, suggesting it to be of questionable benefit in these patients.

Background: Several trials have reported on whether adjuvant chemotherapy for resected stage IB non-small cell lung cancer is needed. The aim of our study was to investigate prognostic factors for recurrence to help identify patients who should receive adjuvant chemotherapy. Material and Method: We reviewed the cases of 48 stage IB non-small cell lung cancer patients between 1997 and 2006. Disease-free survival and overall survival rates were calculated by the Kaplan-Meier method. Univariate analysis was performed with the log rank test and multivariate analysis was done using Cox's proportional hazard model. Result: The median follow-up time was 48 months. The overall survival rate was 55.9%, and the disease-free survival rate was 48.6%. Of 8 variables, two factors, visceral pleural invasion and Iymphovascular invasion, were prognostic factors of disease-free survival (univariate analysis). Visceral pleural invasion was a significant prognostic factor in multivariate analysis, and overall survival in com-pared one or more variable such as visceral pleural invasion or, and lymphovascular invasion with the other variables. Conclusion: Visceral pleural invasion was identified as a poor prognostic factor and it may help select which patients will benefit from adjuvant chemotherapy in addition to more comprehensive follow-up.
This study attempted to empirically investigate the determinants of poverty transition of the working poor with disabilities from a dynamic perspective. Analyses were conducted on the data from Panel Survey of Employment for the Disabled(PSED, Year 1-6), included the households with a disabled head of household. The working poor were defined as the household of which income fell below 120% of the absolute poverty line among the households just described. As results, The 6-year mean poverty rate for the working poor with disabilities included in the analysis was 31.4%, approximately three times of the poverty rate of the total population and the working poor with disabilities were found to have greater difficulty with poverty exit once having fell into poverty than all households living in poverty. And it was found that the economic activity factor was the key determinant of in-work poverty. In addition, employment of the working poor with disabilities did not lead straight to poverty exit, and the quality, rather than the status of, employment was the key determinant of poverty exit. The implications of the findings of this study are that it is essential to increase decent jobs, expand the social safety net of the working poor with disabilities and establish poverty reduction measures for each class of the working poor with disabilities to exit from poverty.
The aim of this study was to understand the management level of pathogenic bacteria in HACCP system implemented animal farms. Microbial samples were collected from manure, floor, compost depot, manure on belt, low milk tank, dust in laying house and egg collector in HACCP system implemented Korean beef cattle, dairy cattle, swine, and laying Hens farms. O157, O111 and O26 strains of E. coli were not detected in HACCP system implemented Korean beef cattle farm. The detection rate of E. coli from manure and floor in HACCP system implemented cattle farms (Korean beef cattle and dairy farm) was lower than those of non-HACCP system implemented cattle farm. Salmonella spp. was detected in HACCP system implemented cattle farms (Korean beef cattle and dairy farm). Compared with pervious studies, lower detection rate of Salmonella spp. at floor and compost depot in HACCP system implemented swine and commercial layer farms were indicated. In conclusion, implementation of HACCP system in animal farms would enhance the management level of biological hazard compare to normal animal farms.
This study examined the generation of combustion toxic gases of pinus rigida specimens processed with bis-(dimethylaminomethyl) phosphinic acid (DMDAP), bis-(diethylaminomethyl) phosphinic acid (DEDAP), and bis-(dibutylaminomethyl) phosphinic acid (DBDAP). Each pinus rigida plate was coated three times with 15 wt.% flame retardants in an aqueous solution. The specimens were then dried at room temperature. The production of combustion toxic gases was investigated using a cone calorimeter (ISO 5660-1). The first time to peak mass loss rate ($1^{st}-TMLR_{peak}$) processed with the chemical additives decreased to 5.9 from 41.2% compared with the unprocessed specimen. The second time to the peak mass loss rate ($2^{nd}-TMLR_{peak}$) for the processed specimens was decreased 1.8% for DMDAP and 5.3% for DBDAP and increased 1.8% for DEDAP. The peak carbon monoxide ($CO_{peak}$) production was 1.5 to 2.0 times higher than that of the unprocessed plate. The peak carbon dioxide ($CO_{2peak}$) production was reduced 0.01 times for DMDAP and increased 1.15 to 1.19 times for DEDAP and DBDAP compared with the unprocessed specimens. In particular, the oxygen concentration was much higher than 15%, which can be fatal to humans and the resulting hazard can be eliminated. Overall, the combustion toxicity of flammable gas were increased partially by the chemical additives compared with those of the unprocessed plate.
Abandoned mine deposits are exposed to various physico-chemical geo-environmental hazards and disasters, such as acid mine drainage, water contamination, erosion, and landslides. This paper presents the ring shear characteristics of waste materials. The ring shear box with a rotatable O-ring was used in this study. Three tests were performed: (i) Shear stress-time relationship for given normal stress and shear speed, (ii) shear stress as a function of shear speed, and (iii) shear stress as a function of normal stress. For a given normal stress (50 kPa) and speed (0.1 mm/sec), the materials tested exhibit a strain softening behavior, regardless of drainage condition. The peak and residual shear stresses were determined for each normal stress and shear speed. The shear stress was measured when shear speed is equal to 0.01, 0.1, 1, 10, 50, 100 mm/sec or when normal stress is equal to 20, 40, 60, 80, 100, 150 kPa. From the test results, we found that the shear stress increases with increasing shear speed. The shear stress also increases with increasing normal stress. However, different types of shearing mode were observed in drained and undrained conditions. Under drained condition, particle crushing was observed from the shearing zone to the bottom of lower ring. Under undrained condition, particle crushing was observed only at the shearing zone, which has approximately 1 cm thick. It means that a significant high shear speed under undrained condition can result in increased landslide hazard.
Background: Gefitinib and erlotinib are useful, molecular targeted agents in patients with non-small-cell lung cancer (NSCLC) who failed previous chemotherapy. We compared the efficacy and toxicity of two drugs in patients with squamous cell lung cancer, most of whom are male smokers. Methods: We retrospectively reviewed the clinical information on patients with NSCLC who were treated with gefitinib or erlotinib treatment at Chonnam National University Hwasun Hospital between July 2002 and November 2009. The overall response rate (ORR), overall survival (OS) and progression-free survival (PFS) were compared between the two drugs. Results: A total of 182 (100 gefitinib vs. 82 erlotinib) of 584 patients treated by targeted agents had squamous histology. Of the 182 patients, 167 (91.7%) were male and 159 (87.4%) were smokers. The ORR and disease control rate (DCR) were 4.9% and 40.6%, and there was no significant difference between gefitinib and erlotinib (ORR, 5.0% vs 4.8%; p=0.970; DCR, 40.0% vs 41.4%; p=0.439). The median OS in the gefitinib group was 12.1 months, and that in the erlotinib was 12.7 months (hazard ratio [HR], 1.282; 95% confidence interval [CI], 0.771~2.134; p=0.339). The median PFS for the gefitinib group was 1.40 months, compared with 1.37 months for the erlotinib group (HR, 1.092; 95% CI, 0.809~1.474; p=0.564). Skin rash ${\geq}$grade 3 was more common in erlotinib (12.2%) than gefitinib (1.0%, p=0.003) groups. Conclusion: This retrospective study showed that the two drugs appear to have similar antitumor efficacy and toxicity except for skin rash.
Purpose: To study the efficacy of capecitabine or S-1 plus oxaliplatin (CAPOX or SOX) for treating thymidine phosphorylase (TP)- or dihydropyrimidine dehydrogenase (DPD)-positive advanced gastric cancer. Materials and Methods: Eighty-six patients with stage IIIC to IV gastric cancer were assessed for TP and DPD expression by immunohistochemistry. The association between CAPOX or SOX efficacy and TP/DPD expression was retrospectively analyzed. Results: There were no significant differences in the objective remission rate (ORR, 52.27% vs. 47.62%; P>0.05), disease control rate (72.73% vs. 73.81%, P>0.05), progression-free survival (hazard ratio [HR], 1.119; 95% confidence interval [CI], 0.739-1.741; P=0.586), and overall survival (OS; HR, 0.855; 95% CI, 0.481-1.511; P=0.588) between CAPOX and SOX. A higher number of stage IV patients showed TP positivity, while DPD-positive patients predominantly showed intestinal type of gastric cancer. In TP-positive patients, the ORRs associated with CAPOX and SOX treatments were 57.14% and 38.10%, respectively; OS was better with CAPOX than with SOX (HR, 0.447; 95% CI, 0.179-0.978; P=0.046). Among DPD-positive patients, the SOX treatment-associated ORR (60.87%) was significantly higher than the CAPOX treatment-associated ORR (43.48%). Furthermore, SOX treatment resulted in better OS than did CAPOX treatment (HR, 2.020; 95% CI, 1.019-4.837; P=0.049). Conclusions: No significant difference in clinical efficacy was found between CAPOX and SOX. TP-positive patients might respond better to CAPOX while DPD-positive patients may respond better to SOX. Our findings might serve as a guide for personalized chemotherapy for gastric cancer.

In Jeju-Do, there is the 1100 Road, 516 Road, and Bijarimro, which are typical mountain sight-seeing roads in Jeju. These roads are local roads that have been the location for many traffic accidents. This study focused on these roads, categorized the type of traffic accident and analyzed the accident characteristics. The major accident factors were analyzed through trip AHP analysis, Comparative analysis of the velocity distribution and the factors affecting traffic accidents were analyzed. Tourists took many trips on these roads. The mixing rate of the rental cars was 36.70%~71.60% in 1100 road and Bijarimro. Currently, these local roads are regulated by a speed limit of 60km/h. However, it might be necessary to reduce the speed limit to 40km/h considering the geometric line form of the road and the climate in these areas. The speed limit of more than 40km/h is found 87.0% on 516 Roads, 88.57% on 1100 roads, and 93.1% on Bjarimro, In these roads, the speed ratio is higher as described above. Therefore, these roads have been found to have a higher risk of traffic accidents by overspeeding driving. The overspeed driving ratio of these roads was 87.0%~93.1%, The overspeed driving enforcement method at one spot has only the effect of reducing the speed at that enforcement place; the effect cannot be expected for the other places or sections. It is necessary to introduce a section overspeed driving enforcement system utilizing the average velocity in these areas to prevent traffic accidents.
