  • 외과수술용 핸드피스는 현재 국내 외 병원에서 사용되고 있는 수술 장비 중 필수 보유 장비이다. 선진 수술 기법인 인공관절 대체 수술 시 뼈(Bone)절단, 다듬질 구멍 등, 초 섬세 가공을 목적으로 하는 기기이다. 기존의 공압식(Air Type)은 수술 시 많은 양의 질소(N2 Gas)가 소모됨으로써 질소탱크 보관에 많은 공간이 필요하고 잦은 교체를 필요로 한다. 또한 기계소음과 공압식 호스의 이동으로 시술자와 환자사이에 불편함이 발생하고 감염의 위험성이 있으므로 최근에는 충전식을 선호하고 있다. 이러한 이유로 기존의 공압식을 대체할 배터리형 외과수술용 핸드피스를 연구하게 되었다.

  • Subcutaneous emphysema and pneumomediastinum is a relatively uncommon phenomenon. It may occur secondary to dental treatment using high-speed air turbine handpieces, especially after extraction of tooth. Subcutaneous emphysema is often limited only to the areas of head and neck, but also can involve deeper structures. Thorough examination and conservative treatment of these problems are essential in preventing life-threatening complications such as airway obstruction and mediastinitis. The subject of this report is a 57-year-old woman with subcutaneous emphysema and pneumomediastinum during the extraction of maxillary third molar using high-speed air turbine handpiece. If there isn't any appropriate measure, severe complications may occur. Therefore it is important to be well-informed of proper diagnosis and treatment. This article shall present a case report with literature review.

Percutaneous self-injury to the femoral region caused by bur breakage during surgical extraction of a patient's impacted third molar

  • Extraction of an impacted third molar is one of the most frequently performed techniques in oral and maxillofacial surgery. Surgeons can suffer numerous external injuries while extracting a tooth, with percutaneous injuries to the hand being the most commonly reported. In this article, we present a case involving a percutaneous injury of the surgeon's femoral region caused by breakage of the fissure bur connected to the handpiece during extraction of the third molar. We also propose precautions to prevent such injuries and steps to be undertaken when they occur.

  • A 3-month-old, female Persian cat was presented with sclera mass. The mass was detected after cat's squeals during the playing with a dog before 2 days. On ophthalmic examination, a $2{\times}5mm$ dark purple colored raised mass was observed 1 mm behind the limbus in the right eye. Anterior chamber was filled with blood and epithelial defect was detected at 2 o'clock in the right central cornea. After general anesthesia, resection of the protruded iris was performed with iris scissors and bipolar cautery. Ruptured sclera and conjunctiva were closed with simple interrupted and simple continuous suture, respectively. Also, irrigation and aspiration (I/A) of the anterior chamber was performed to remove blood clot with bimanual I/A handpiece in the right eye. Four months after the surgery, the right eye was recovered completely and menace response was positive in spite of the retinal lesions.

  • The objective of this study was to compare the apical shaping ability of the ProFile .04 taper, Lightspeed and HERO 642 rotary instruments and to assess the combined effect of GT Rotary files to Lightspeed and HERO 642 instruments. Forty-eight resin simulated root canal blocks divided into 4 groups with 12 canals. Forty-eight blocks were prepared by one operator with GT Rotary files using a high torque handpiece at 300 rpm. Thirty-six blocks in the three experimental groups were prepared with ProFile 04 taper file, Lightspeed file, HERO 642 file.(omitted)

Analyses of Influence of Frictional Heat on the Contact Stress of High-speed Micro-gears

  • When a small gear rotates at a very high speed over 40,000 rpm, frictional heat is generated on the gear surfaces. Thermal deformations and stresses arising from frictional heat may lower the efficiency and fatigue life of the high-speed gear. Especially, such frictional heat has much stronger effects on the performance of millimeter-sized high-speed gears used for surgical and dental hand-pieces, due to a small surface area. An analytical equation was derived to calculate frictional temperature on a mating gear surface and conduction heat transfer analysis was performed. Thermal deformation and contact stresses were then calculated using FEM for gears used for medical hand-pieces. The contact stresses of the meshed gear and pinion increase by 19.4% and 16.4%, respectively, when the frictional thermal deformations are considered.

  • The purpose of this study was to determine human pulp reactions to temporary cements such as zinc oxide-eugenol cement, modified zinc oxide-eugenol cement (Cavitec) and calcium hydroxide cement (Dycal). Deep class V cavities were prepared in the human teeth with ultrahigh-speed handpiece operating at a free running speed of 300,000 r.p.m., using # 701 bur and water spray coolant. The cavities were flushed with water, dried with cotton pellets and filled with zinc oxide-eugenol cement, modified zinc oxide-eugenol cement and calcium hydroxide cement respectively. The teeth were divided into two groups, which one group was extracted after One day and the other was extracted after seven days. The samples were examined with microscope and the findings were as follows; 1. The pulp reactions to temporary cements were generally mild. Among them the reactions were moderate in zinc oxide-eugenol cement and, slight in calcium hydroxide cement. 2. Calcium hydroxide cement may be used properly as temporary cement for the purpose of pulp protection.

  • Brushless DC(BLDC) 모터는 브러시가 없어 고속 회전에 유리하기 때문에 핸드피스 모터에 많이 적용하는 추세이다. 핸드피스 모터는 부피가 작을수록 유리하여 홀 센서를 장착하지 않은 경우가 많기 때문에 센서리스 제어를 하여야 한다. BLDC 모터의 상 역 기전력 zero crossing 감지 방법을 이용한 센서리스 제어를 할 시 40,000rpm에 달하는 고속 회전에서 상 전압 floating 구간이 $250{\mu}s$밖에 안되기 때문에 샘플링이 부족하고 스위칭 노이즈도 강해 zero crossing에 대한 감지가 어려울 수 있다. 이 논문에서는 역 기전력에 대한 비선형 필터인 majority function을 응용함으로써 모터 상 역기전력의 zero crossing을 감지해내고 고속 BLDC 모터 센서리스 제어를 하는 방법을 소개한다.

Infection control among dental hygienists according to infection control education experiences (치과위생사의 감염관리 교육경험에 따른 감염관리 실태)

  • Objectives : The purpose of this study was to examine the relationship between the infection control education experiences of dental hygienists and the state of their infection control. Methods : The subjects in this study were the dental hygienists who worked in Seoul. The relationship of their infection control education experiences to their gender, marital status, academic credential, workplace, length of service, infection control implementation, experience of being exposed to infection, way of coping with it, hand washing, use of personal protection devices and equipment management was analyzed. Results : It is found that the variable to affect the state of infection control was educational experiences about handpiece water pipe management, ultrasonic scaler water pipe management and three-way syringe water pipe management. Conclusions : Dental hygienists who are one of major dental personnels should receive systematic education on infection control to acquire accurate knowledge to ensure the successful prevention of cross infection.

Development of Surgical Two-wavelegth Nd:YAG Laser (수술용 2파장 펄스형 Nd:YAG 레이저 개발)

  • The development of a compact two-wavelength Nd:YAG laser for dental and ENT applications is presented. The Nd:YAG resonator generates either 1.06$\mu$m or 1.32$\mu$m. The wavelength selection is made at the control panel. The Nd:YAG laser parameters at 1.06$\mu$m are ; the maximum pulse duration of 150$\mu$s, repetition rates of I-100Hz, and the maximum average power of 25W. At 1.32$\mu$m, the pulse duration is the same where the repetition rates and the maximum average power are I-30Hz and lOW respectively. High voltage power supply consists of a simmer module and two identical high voltage DC converters. In order to make a complete medical laser system, an optical fiber delivery unit, foot pedal and water spray handpiece are also developed. The wavelength selection is reliable since no movement of optical or mechanical components is required. The high voltage power supply is compact, easy to be maintained and applicable for other laser systems due to its modular design.
