Instrumental Shaving과 Manual Shaving을 병용한 액와부 액취증 및 다한증의 수술적 치료 후 장기 추적관찰 (Long Term Follow Up of Surgical Treatment of Axillary Osmidrosis and Hyperhidrosis by Instrumental Shaving and Manual Shaving)

  • Purpose: Axillary osmidrosis is caused by excessive apocrine gland secretion, which causes an unpleasant odor. Axillary osmidrosis causes difficulty in social activities and personal handicap. We studied the long term follow up results of our surgical procedures and sought to find a best surgical treatment methods. Methods: From January 1991 to December 2006, a total of 1864 patients(587 men, 1277 women) had been treated with this procedure for axillary osmidrosis and hyperhidrosis. Follow up periods varied from 10 to 15 years. We used the subdermal excision technique, using two small incisions over the axilary folds and using a Kawata dissector(instrumental shaving) and additional manual subdermal excision(manual shaving). Results: Among the total patients, 782 patients were followed up. Except a pure hyperhidrosis patients, 759 osmidrosis patients was included. 588 patients(77.4%) had a good results, 148 patients(19.5%) had a moderate results and 23 patients(3.1%) had a recurrence. Complication developed in 189 patients(10.1%): hematoma or seroma in 25 patients, wound dehiscence in 86 patients, partial skin necrosis in 45 patients, and infection in 28 patients. Conclusion: We conclude that our method has several advantages such as 1) short operation time, 2) minimal scaring, 3) lower complication rates, 4) high satisfactory rates.

정상 성인의 손목관절 위치가 파악력에 미치는 영향 (The Effect on Grip strength in Change of Wrist Position of Normal Adults)

  • Purpose: The purpose of this study was to investigate the effect of wrist position on grip strength. A Grip-Strength is measurement of muscle strength in hand. It is evaluated for the motor function and handicap of hand, projection of treatment plan. It is important because correct treatment. Methods: Total 80 college students were participated in this study, who consisted of 40 males and 40 females(age range: $20{\sim}26$). A Grip-Strength Dynamometer was used to measure the grip strength in three wrist position(flexion, extension, neutral position) and two elbow position(supination, pronation). The ANOVA was conducted to determine any significant difference in grip strength between the wrist position and the two elbow position. Results: The results were as follows: 1. The grip strength was affected by wrist position changes. 2. The difference of grip strength according to elbow position Was not significant difference (p>0.01). 3. The grip strength i1l neutral position was strongest among 3 position of wrist (p<0.01). Conclusion: A Grip-Strength was significant difference in wrist position. Pronation position was minimum grip-strength but not significant difference between supination and pronation.

사물탕합이진탕 가미방 투여한 편두통성 현훈을 치료한 치험 2례 (Korean Treatment with Samul-tang plus Yijin-tang gamibang in Patient for Migrainous Vertigo : 2 Cases Report)

  • This case study reports patients suffering from migraine and vertigo had treated with Samul-tang plus Yijin-tang Gamibang. Two patients had taken Samul-tang and Yijin-tang Gamibang three times a day during hospitalization in oriental hospital. One patient had been hospitalized for 2 weeks and other had been for 3weeks. We control dose of added herbs according to other symptoms like dyspepsia, sleep discomfort, flushing, heating sense and diarrhea for each patients. We measure migraine by migraine disability assessment (MIDAS) and dizziness by korean dizziness handicap inventory (KDHI). Also we measure overall improvement with Visual Analogue Scale(VAS). The main symptoms had been steadily decreased to VAS2. Patient complaing of dyspepsia, heating sense, flushing had been improved with reduction of dyspepsia and patient with diarrhea, abdominal discomofort had been improved following reduction of diarrhea. Two patients didn't agree with study because this paper was written following retrospective study but patients claimed no side effect. Samul-tang plus Yijin-tang Gamibang may be considered as a treatment of migrainous vertigo with digestive problems.

유전 알고리즘을 이용한 삼목 게임 전략 분석 (Analysis of Tic-Tac-Toe Game Strategies using Genetic Algorithm)

  • 바둑은 단순한 규칙에도 불구하고 매우 복잡한 전략보드 게임이다. 몬테카를로 트리탐색을 이용하여 컴퓨터 바둑 프로그램들이 접바둑으로 프로기사를 제압해 왔다. 몬테카를로 트리탐색은 전략의 개념보다는 몬테카를로 시뮬레이션에 의해 계산된 승률에 근간을 한다. 반면에 적절한 적합도 함수로 된 유전 알고리즘은 게임 내 최적 해를 찾아낼 수 있다. 삼목 게임(또는 ${\bigcirc}{\times}$게임)은 가장 대중적인 게임 중의 하나이다. 저자는 삼목 게임에서의 최선의 전략을 찾고자 했다. 실험 결과로 유전 알고리즘은 효율적인 전략들을 찾을 수가 있으며, 바둑과 서양장기와 같은 여타 보드게임들에 적용할 수 있음을 보였다.

Clinical Problems in ML II and III: Extra-skeletal Manifestations

  • Mucolipidoses II and III alpha/beta (ML II and ML III) are lysosomal disorders in which the essential mannose-6-phosphate recognition marker is not synthesized onto lysosomal hydrolases and other glycoproteins. The disorders are caused by mutations in GNPTAB, which encodes two of three subunits of the heterohexameric enzyme, N-acetylglucosamine-1-phosphotransferase ML II, recognizable at birth, often causes intrauterine growth impairment and sometimes the prenatal "Pacman" dysplasia. The main postnatal manifestations of ML II include gradual coarsening of neonatally evident craniofacial features, early cessation of statural growth and neuromotor development, dysostosis multiplex and major morbidity by hardening of soft connective tissue about the joints and in the cardiac valves. Fatal outcome occurs often before or in early childhood. ML III with clinical onset rarely detectable before three years of age, progresses slowly with gradual coarsening of the facial features, growth deficiency, dysostosis multiplex, restriction of movement in all joints before or from adolescence, painful gait impairment by prominent hip disease. Cognitive handicap remains minor or absent even in the adult, often wheelchair-bound patient with variable though significantly reduced life expectancy. As yet, there is no cure for individuals affected by these diseases. So, clinical manifestations and conservative treatment is important. This review aimed to highlight the extra-skeletal clinical problems in ML II and III.

이명장애와 삶의 질의 관계 (Relationship between the Discomfort of Tinnitus and the Quality of Life in Tinnitus Patients)

  • Purpose: To investigate audiometery and characteristics related to tinnitus in tinnitus patients, and to understand the relationship between the discomfort of tinnitus and the quality of life. Methods: The subjects were 110 patients with tinnitus. The data were collected using tinnitus handicap questionnaire, quality of life, and pure tone audiometry, which measured hearing level in dB and frequency in Hz matching of tinnitus. Results: Majority of tinnitus patients (75.5%~81.8%) had normal auditory sensation. but 82.7% of patients indicated that they have heard tinnitus "frequently" or "always", and the major sounds of tinnitus were "wing, wung, wang". The matching sounds of tinnitus had various range in hearing level(dB) and frequency(Hz). Tinnitus patients reported moderate levels of the discomfort of tinnitus and the quality of life, and there were negative correlation between the discomfort of tinnitus and the quality of life(r=-.314, p<.001). Conclusion: These findings suggest that special attention is needed for tinnitus patients because the physical, psychological, and social impacts of tinnitus could be decrease their quality of life. Specialized nursing intervention considering specific conditions related to tinnitus is required to solve various health problems of tinnitus patients.

월경 주기가 여성의 목소리에 미치는 영향 (Change of Voice during Menstrual Cycle)

  • Baekgroud and Objectives: The study was purposed to evaluate the relationship between the voice change and the menstrual cycle by measuring the variation of subjective and objective parameters. Materials and Methods: Prospective study of 13 healthy women during 2 mentrual cycles. Their voices were recorded at follicular phase and then luteal phase of the menstrual cycle. We used both single vowel /a/ and sentences for evaluate acoustic parameters. Aerodynamic parameters were also evaluated. Voice handicap index (VHI), and the presence of premenstrual syndromes (PMS) were checked at each period. We used Wilcoxon's signed rank test to compare the parameters of two periods. Results: VHI were 5.1 at both periods (p=0.146) and 92.3% of women were diagnosable with PMS. There were no significant differences in acoustic parameters and aerodynamic parameters between the two periods. Conclusion: This study shows that not only the subjective but also the objective changes of the voice parameters did not exist during the menstrual cycle in women.

실험 데이터를 이용한 클럽 페이스 움직임 분석 프로그램 개발 (Development of a Program That Computes the Position of the Club Face Based on the Experimental Data)

  • The moving trajectory of a golf ball is mainly determined by the angles of the clubface and the trajectory of the club shaft. This paper presents a computer program for analyzing the position and angles of the club while the club moves in a circular motion. For this purpose, a mathematical algorithm was developed and implemented on the experimental data(5 m and 10 m carries) using VC++ and OpenGL. A skilled female golfer(174 cm, 65 kg, 0 handicap) was participated in data collection for the short approach shots. An iron club(Titleist 52 degree, 91.5 cm length, 450 g mass), attached with five reflective markers(12 mm), was used to collect experimental data. However, exact 3D coordinates and angles of the clubface are not directly calculated from measured data. A reverse engineering platform(Minolta Vivid910 hardware and Rapidform software) was thus employed to acquire the scanned data of the clubface. The scanned data and measured data were first aligned by applying appropriate coordinate transformations, and then exact coordinates and angles of clubface could be obtained at each position during circular motion. The program(Club Motion Analysis 1.0) exports the open, heel, loft angles of the club.

측두하악관절 추나요법과 청궁혈 온침으로 호전된 만성 이명 환자 1례 보고 (The Effect of Temporomandibular Joint Chuna Manipulation and Warm Needle Acupuncture at SI19 on Chronic Tinnitus; a Case Report)

  • Objectives : The purpose of this study is to report the combination effect of Chuna manipulation and warm needle acupuncture for a case of obstinate chronic tinnitus. Methods : We applied Danmuji Anchu Tracton technique, lateral pterygoid Chuna and warm needle acupuncture at SI19 once a day for 2 weeks. Visual analogue scale(VAS) was used for the intensity of tinnitus and tinnitus handicap inventory(THI) was evaluated. Results : VAS was improved from 6 to 2 in right ear, 4 to 1 in left ear after the treatment. THI was also improved from 54 to 34 after the treatmemt. Conclusions : The combined therapy of Chuna manipulation and warm needle acupuncture can be used for chronic tinnitus.

정신지체아 어머니의 양육 스트레스, 자아존중감 및 사회적 지지와의 관계 (Relationship among Parenting Stress, Self-Esteem and Social Support on Mental Retardation Child Mother's)

  • Purpose. The study verified the relationship between the mental retardation child mother's parenting stress, self-esteem and social supports. Methods. The subjects were 250 mothers who had their children with mental retardation educated and who experienced to receive social services in U City. They were asked to answer the questionnaire and 134 subjects resent the questionnaire. The survey information was analyzed by SPSS 18.0. Results. Sociodemographic characteristic effected on the parenting stress; In accordance with the severity of handicap, the level of the stress was different, self-esteem are the mother's educational attainment. The social supports in functional perspective are the religion. Conclusions. The relationship between the parenting stress and social support, self-esteem and parenting stress the both have negative relationship, More information support, emotional support, economic support, evaluating support get the less the parenting stress, and self-esteem have. The parenting stress and self esteem are highly related.
