본 연구는 뇌졸중 발병 후 지역사회 참여에 영향을 미치는 요인들을 알아봄으로서 지역사회 중심 재활에 중요한 자료를 제공하는 것을 그 목적으로 하였다. 연구 대상은 발병 기간이 6개월 이상인 뇌졸중 환자 100명이었다. 대상자의 사회 참여 정도를 파악하기 위해 한국판 크레이그 장애 평가(Craig Handicap Assessment and Reporting Technique: K-CHART)를 실시하였고, 일상생활활동의 독립 수준을 알아보기 위해 한국판 수정 바델 지수(Korean-Modified Barthel Index: K-MBI) 검사를 통해 대상자의 지역 사회 참여에 영향을 미치는 요인을 알아보았다. 결과, 뇌졸중 환자의 지역 사회 참여의 관련요인은 다음과 같이 나타났다. 일상생활활동, 연령, 고용의 유무보다는 뇌졸중 환자의 신체적 독립, 인지적 독립, 이동성, 작업, 사회 통합이 참여에 높은 설명력을 나타났다. 또한 신체적 독립, 인지적 독립, 이동성, 작업, 사회 통합이 참여에 영향을 미치는 요인들로 나타났다. 모든 항목에서 통계적으로 유의한 수준이었다(p<.01). 뇌졸중 환자의 사회참여를 증진하기 위한 작업치료의 초점에 대한 정보를 제공하며, 참여에 영향을 미치는 요인들에 대하여 적절한 평가와 중재가 참여를 증진하기 위해 필수적인 재활이 될 것이라 사료된다.
이 연구의 목적은 골프드라이버 샤프트의 가변성이 타구속도, 헤드스피드 및 비거리에 미치는 영향을 분석하고자 하였다. 이 연구에 참여한 피검자는 핸디캡이 0인 남자 프로골퍼 10명과 핸디캡이 18인 남자 아마추어 골퍼 10명으로 하였다. 클럽의 종류는 1번 드라이버로 한정하였고, 각기 다른 스팩의 24개 드라이버를 가지고 실시하였다. 종속변인으로는 타구속도, 비거리 및 헤드스피드로 하였다. 연구 결과 다음과 같은 결과를 얻었다. 첫째, CPM에 따라 유의한 차이가 나는 것으로 밝혀졌고, 사후검증 후 230< 이상일 때 타구속도, 비거리 및 헤드스피드에서 최적의 수행력을 보였다. 둘째, 샤프트길이에 따른 타구속도 및 비거리는 유의한 차이가 나는 것으로 나타났고, 사후검증 후 타구속도 및 헤드스피드는 46inch에서 비거리는 45inch에서 최적의 수행력을 보였다. 셋째, 샤프트 무게에 따른 변인 간 차이는 나지 않았고, 사후검증 후 샤프트 무게가 65g일 때 타구속도와 비거리에서 최적의 수행력을 보였고, 50g일 때 헤드스피드에서 최적을 보였다. 또한 프로와 아마추어 간에는 변인에서 유의한 차이가 나는 것으로 나타났다. 결론적으로 최적의 드라이버는 CPM이 230<, 샤프트길이 46inch, 샤프트 무게가 65g 샤프트 일 때 최고의 수행력을 발휘하는 것으로 판명되었다.
본 연구는 2008 베이징 장애인 올림픽에 참가하는 국가대표 휠체어 탁구 선수들 중 대표적인 절단장애 선수와 척수장애 선수의 스트로크를 대상으로 운동학적 비교분석을 실시하였다. 실재와 같은 경기상황을 설정하여 모든 경기 상황에서의 스트로크를 수집하였다. 그중 대표적인 포핸드 스트로크 세 개의 동작과 백핸드 스트로크 세 개의 동작을 추출하였다. 9대의 적외선 카메라를 이용하여 수집한 자료는 LabVIEW7.0 그래픽 프로그램을 이용한 운동학적 분석에 의하여 각운동으로 나타내었다. 그 결과 척수장애 선수의 포핸드 스트로크에서는 몸통의 회전이 하위분절인 어깨의 회전과 팔꿈치의 굴곡신전과 거의 동시에 이루어지고 있었다. 백핸드 스트로크는 몸통의 회전, 어깨의 굴곡-신전, 그리고 팔꿈치의 굴곡-신전이 날아오는 공의 연장선상에서 일정한 형태로 정렬되어 있었고 공을 치기위한 백스윙은 어깨의 회전을 이용해 만들고 있었다. 절단장애 선수의 포핸드 스트로크는 팔의 외측회전으로 백스윙과 내측회전으로 만들어지는 회전을 이용하여 스트로크 하고 있었다. 백핸드 스트로크는 어깨의 회전과 허리의 회전에 있어 내측회전이 백스윙을 만들고 백스윙 후 공을 향한 큰 폭의 외측회전으로 임팩트를 이루고 있었다.
Background and Objectives: Quality of voice may be deteriorated after thyroidectomy without any injury to recurrent laryngeal nerve. While there have been several studies showing the change of acoustic parameters after thyroidectomy, factors related to deteriorated voice quality have been rarely studies. In this study, we sought to analyze the factors associated to deteriorated voice quality after thyroidectomy. Materials and Methods: We made a retrospective review of 35 patients who underwent thyroidectomy for papillary thyroid carcinoma. Voice analysis including acoustic analysis, voice handicap index 10 (VHI-10), and GRBAS score was conducted before and 3 months after surgery. Patients were grouped according to the amount of increase in VHI-10 after surgery ; group A (${\Delta}VHI<7$) and group B (${\Delta}VHI{\geq}7$). Clinicopathologic factors associated to patients of group B were analyzed and changes of parameters from acoustic analysis in each group were evaluated. Results : Patients of group B were associated with age ${\geq}45$ years (p=0.025) and showed borderline association to total thyroidectomy (p=0.075) and tumor size ${\geq}1cm$ (p=0.086). Multivariate analysis demonstrated that those with age ${\geq}45$ years were independently associated to deteriorated quality of voice (p=0.014, HR=18.38). Patients of group B were also associated to significant deterioration of high pitch (p<0.001) and Grade score with borderline significance (p=0.054). Conclusion: Patients older than 45 years may have higher risk of deterioration of quality of voice after thyroidectomy based on increase of VHI-10 score (${\Delta}VHI{\geq}7$). Association with deterioration of high pitch should also be considered in these patients.
Background and Objectives : Voice disorders can cause problems in patients with benign vocal cord lesions emotionally, physically, economically and functionally. Neither subjective nor objective voice examinations can evaluate such factors adequately. The Voice Handicap Index (VHI) subjectively evaluates voice disorders in terms of physical, functional, emotional factors and measures the patient's perception of the impact of voice disorder. The purpose of this study is to evaluate the usefulness of VHI in the patients with benign vocal cord lesions. Materials and Method : The authors evaluated 37 patients who experienced laryngeal microsurgery for benign vocal cord lesions from september 2003 to August 2004. The VHI was used to measure the postoperative changes of the patient's perception and acoustic analysis and aerodynamic tests were also done. Statistical analysis was done using paired t-test and Pearson's correlation. Results : The VHI scores showed statistically significant reductions postoperatively. In acoustic analysis, jitter and shimmer had statistically significant reductions after surgery but noise-to-harmonics ratio did not. A statistically significant change in the average MFR and MPT perioperatively was found. The relationship between VHI and acoustic, aerodynamic analysis attained statistical significance. Conclusion : The VHI is a useful assessment tool to monitor the patient's self-perception of voice change after the surgery of benign vocal cord lesions. The VHI measurement, when combined with acoustic and aerodynamic analyses, will be helpful in comparing functional outcomes after voice surgery.
Background and Objectives : Vocal polyps are caused by inflammation induced by stress or irritation. Many patients with vocal polyps complain voice discomfort. For vocal polyps, surgery such as laryngeal microsurgery has been the mainstay of management. We analyzed the clinical features of vocal polyps, and how the size and location of vocal polyps affect the outcomes of surgery. Methods : We retrospectively reviewed 42 patients from March 2014 to December 2015, who were diagnosed as unilateral single vocal polyp. When we operated on a vocal polyp with laryngeal microscopy, we measured their size and location. The quality of voice was evaluated by GRABS scale, jitter, shimmer, NHR (noise to harmonic ratio), MPT (maximum phonation time), and VHI (voice handicap index) before operation and 4 weeks after operation. Results : When we divided the patients into large-sized vocal polyp group (the longest length >3 mm) and small-sized vocal polyp group (the longest length ${\leq}3mm$), all parameter differences tend to be greater at large sized vocal polyp. However, these differences were not statistically significant (p>0.05). When we divided into two groups depending on the volume of vocal polyp, no distinct tendency was found. When we compared the location (anterior, mid and posterior) of vocal polyp with the improvement of voice quality, more change was found at mid portion vocal polyp, except the difference of VHI. However, these differences were also not statistically significant (p>0.05). Conclusion : All parameter differences tend to be greater at large vocal polyp and polyp of the mid location.
Background and Objectives : After thyroidectomy, many patients experience problems report such things as reduced voice range and vocal fatigue, swallowing problems without superior and recurrent laryngeal nerve injury. The purpose of this study was to evaluate voice and swallowing problems before and after thyroid surgery without laryngeal nerve injury. Materials and Methods : Ninety-three patients who underwent thyroidectomy without laryngeal nerve injury and completed the follow-up evaluations were studied between June 2013 and December 2015. Each evaluation was performed preoperatively, as well as 1 week, 1 month postoperatively. Analysis was performed including voice handicap index (VHI), dysphagia handicap index (DHI), and acoustic voice analysis. Results : Patients show significant variation of parameters in the fundamental frequency (F), maximal phonation time (MPT), shimmer, jitter and soft phonation index (SPI) early after operation, and most of them showed recovery of parameters after 1month of operation. Perceptive complaint of voice and swallowing also showed significant decreased after operation (p<0.005). After 1month of operation, MPT, highest frequency and frequency ranges still showed significant decreased parameters. Comparing acoustic and perceptive parameters of total thyroidectomy and lobectomy, there was no significant changes between them except highest frequency (p=0.042). Conclusion : The results from both subjective and objective evaluations show voice and swallowing disturbance after thyroidectomy even in the absence of laryngeal nerve and provide patients information about the recovery process after surgery. Highest frequency parameter showed most significant changes after operation.
This study is designed to understand the meaning and nature of raising children with cerebral palsy. It researches the experience of mothers of schoolchildren with cerebral palsy by the research method of hermeneutic phenomenology. The study was conducted from November 10, 1999 to December 20, 2000. When children with cerebral palsy usually show symptoms in the early stage of cerebral palsy, mothers do not take children to a doctor for diagnosis. And, most of mothers have a difficult time to accept the reality; they usually respond to the initial diagnosis with shock, reproach, and deny. When mothers start recognizing the reality, they consider that their children have cerebral palsy due to the their mismanagement during pregnancy, delivery, nursing, and initial treatment. They shelter their children from view and feel guilty that they cannot afford to try folk remedies for their children. As time passes, mothers face conflicts between families in diverse ways. Families put the blame on genetic effects. Mothers-in-law give their daughters-in-law a hard time, husbands shift the responsibility of raising children onto their wives, and trouble arises between families-in-law and mothers native families. When children grow up, it is physically difficult for mothers to take care their children. In addition, they suffer from all the troubles in family due to childrens handicap. Mothers try the diverse methods of bringing up children. However, they start getting tired of raising children as they experience failures and financial difficulties. Mothers feel collapsed recalling the ways of raising children. They feel anxiety, miserable, lonely, and worrying when they think how children would attend school, make friends, and live in the future. In this stage, mothers do their best to raise their children with hope. They tend to compare their children with others without handicap and spend money and time in attempting all the treatments. When mothers and children join the society at school, they find that the society does not understand disabled people, teachers show inconsiderate attitude, friends avoid them, and children hardly follow classes. Such experiences make mothers feel angry and frustrated. However, when children adapt to school, mothers see the possibility that children could accomplish schoolwork. They appreciate teachers help and others consideration. Mothers place appropriate expectations on their children and help them to prepare for the future. I would make following suggestions based on the results. 1. As a primary basic course of rehabilitation nursing intervention, solution-centered nursing intervention system should be developed. The intervention needs to be based on the understanding of mothers, who raise children with cerebral palsy, through in-depth interview. 2. Advance researches on the development of individual nursing intervention should be conducted. Individual nursing intervention needs to prevent and release actual pain focusing on mothers raising children with cerebral palsy. 3. Integrated curriculum that help children with cerebral palsy lead a normal school life with ordinary children should be developed. 4. Basic research on using of facilities and effective application of service volunteer to help children with cerebral palsy in school needs to be conducted.
뇌성마비인의 구강위생상태에 관한 조사를 위하여 $3{\sim}48$세의 뇌성마비인 총264명(남153명, 여111명)과 비교군으로 비슷한 연령대의 비뇌성마비인 220명(남125명, 여95명)을 대상으로 구강검사를 시행하고, 우식경험유치율(dft rate), 우식경험유치지수(dft index), 우식경험영구치율(DMFT rate), 우식경험영구치지수(DMFT index), 치태지수(plaque index)를 산출하여 비뇌성마비인과 비교하였으며, 지적, 행동, 언어장애의 정도, 유형 및 이환 부위, 거주형태, 보호자의 직업에 따라 분류, 비교하여 다음과 같은 결과를 얻었다. 1. DMFT rate, DMFT index, dft index에서 뇌성마비군과 비뇌성마비군의 유의한 차이가 없었고, dft rate는 유의한 차이로 뇌성마비군이 높게 나타났다. 2. 뇌성마비군의 성별, 유형 및 이환 부위별 비교에서 우식경험도의 유의한 차이를 보이지 않았다. 3. DMFT rate와 DMFT index는 각각의 장애의 정도가 심할수록 유의성 있는 차이로 감소하였으며, 거주형태에서는 수용군이 비수용군에 비해 낮았고, 보호자 직업에서는 자영업이 낮게 나타났다. 4. 치태지수에서 뇌성마비군이 비뇌성마비군보다 유의성 있게 높았고, 거주형태에서는 비수용군이, 보호자 직업에서는 스스로 벌어서 생활하는 경우가 유의성 있는 차이를 보이며 높게 나타났다. 5. 치태지수에서 뇌성마비군의 성별, 유형 및 이환 부위, 각각의 장애의 정도에 대해서는 유의한 차이를 보이고 있지 않았다.
언어는 사람의 의사전달에 중요한 역할을 할 뿐 아니라 행복한 일상생활을 영위해 나가는데 있어서 기본적이고도 필수적이며, 상호간의 문화교류면에서 중요한 수단이 된다함은 물론이 다. 이러한 언어의 정상발육은 사람의 정상적인 청각기능을 통해서 형성되며, 만일 성장도중에 어떠한 원인으로 청력을 상설하게 되면 언어습득에 장애를 입게됨은 물론. 정서 및 인격형성에도 장애를 초래함으로서 결국에는 주위환경이나 일상생활에 적응하기 어려운 경우를 맞게 된다. 더욱이 오늘날의 사회구조가 고도로 복잡해짐에 따라서 청력장애자들에 대한 치료와 특수교육문제 및 그들의 취업분야 선택이나 사회적인 보상문제, 혹은 이를 판정하기 위한 신체검사기준치등을 정하는데 있어서 청력장애도의 설정과 적절한 분류가 절실히 요구되고 있다. 청력장애에 대한 분류는 1940년에 Beasley가 3등급으로 분류한 것을 비롯하여, 1950년에 A.M.A. (American medical association)에서 음의 주파수에 따른 언어청취범위를 백분율로 조사한 바 있고, 1959년에 A.A.O.O. (American academy of ophthalmology and otolayngology)에서, Huzing (1959), Silverman(1963)등이 분류한 것을 기준으로 삼아오다가 그 후로 보청기의 성능 및 청력검사계기의 발달을 바탕으로, 1965년 (I.S.O 기준)에는 Goodman 씨가 좀 더 체계적으로 청력장애의 정도를 분류함과 동시에 그에 따른 치료 및 사회적 대책에 대해기재한 것을 사용하고 있으나 아직까지 우리나라에서는 이에 대한 연구보고가 없는 터에 본 교실에서는 최근 1년간 난청을 주소로 본 이비인후과 외래를 방문한 환자중 180명을 대상으로 하여 Goodman 씨 분류법에 따라 청력장애정도를 분류함과 동시에 우리 일상생활 가운데서 청력과 비교적 밀접한 관계가 있다고 생각되는 몇 가지 기준사항(대화, 방송, 학교 및 교회, 전화, 집단토의, 음원의 방향)을 지표로 하여 청력장애도와 각 사항의 응답재료를 검토한 바 있기에 보고하는 바이다.

② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
